Aggravated Concealed Penis Resulting from Circumcision (포경수술에 의해 악화된 매복음경)

  • Purpose: To report a rare case of concealed penis that was aggravated by circumcision and simple way of treatment using skin flap. The concealed penis is a primarily pediatric disease and is usually treated by urologists. The plastic surgeons tend to see these patients when it become secondary deformation or in conjoinment with other specialties. In such situation, the plastic surgeons do not have many experiences. Methods: A 3-year-old boy was admitted with aggravated concealed penis after circumcision in congenital webbed penis. Patient had been operated with scar revision and skin flap and peri operative antibiotics. Results: The patient recovered well without any complication such as infection, hematoma and necrosis of flap. Patient was discharged POD#7 with remaining sutures. Patient was followed up for 7 months and he has no sign of erectile dysfunction, voiding dysfunction and evidence of inflammation of the urinary system. Conclusion: We experienced a case of concealed penis that was aggravated from aggressive circumcision in congenital webbed penis.

Effects of Ginseng Saponins on the Induction of Differentiation in Mammary Epithelial Cells and Mammary tumor Cells (홍삼 사포닌에 의한 유선상피 및 유선암세포의 분화 유도 효과 연구)

  • Using Ginseng saponins (crude saponin and total saponin) and ginsenoside Rbl Rb2, Rc, Rd, Re, Rhl, and Rh2 in this study, we have examined the effects of the compounds on the induction of differentiation in normal rat mammary epithelial cells and 7,12-dimethylbenz[a]anthracene (DMBA)-induced mammary tumor cells in culture. When normal rat mammary organoids were cultured in 100-mm culture plates in the presence or absence of ginseng saponins, there were four different cell colonies after two weeks in culture: cobble stone, spindle, honey comb, and senescence type colonies. Ginseng saponins showed different effects on the development of each colonies. Scrape-loading dye transfer tech-nique was performed to measure the effects of total saponin, Rhl, and Rh2 on intercellular junctional communication. Intercellular communication was not observed at short cultilral time, e.g., four or seven days, but when it cultured it up to two weeks, cell to cell communication was observed in saponin-treated cells. Reconstituted basement membrane, Matrigel, supported the growth and development several different multicellular structures from normal mammary organoids (e.g., ductal, webbed, stellate, and squamous colonies) or DMBA-induced mammary tumor (e.g., alveolar unit, foamy alveolar unit, squamous metaplasia, lobule-ductal, stellate, and webbed colony). In ginseng saponin-treated groups, webbed colonies were more and squamous colonies were less than control group. Moreover, the ductal colonies, marker tructure of well-differentiate mammary epithelial cells, were developed more in saponin-treated group than in control group. In conclusion, ginseng saponins affected on the differentiation of normal rat mammary epithelial cells and DMBA-induced mammary tumor cells in culture.

Effects of Kimchi Extract on the Development of Multicellular Structures from Rat Mammary Organoids Cultured in Matrigel

  • The effect of methanol soluble fraction(MSF) of kimchi on the proliferating and differentiating activity of normal rat mammary epithelial cells or organoids in culture were studied. Reconstituted basement membrane, Matrigel, supported the growth and development several different multicellular structures from mammary organoids. The five type colonies of multicellular structures, stellate, ductal, webbed, squamous, and lobulo-ductal colonies, were observed in Matrigel culture. In methanol extract groups, webbed colonies were more and squamous colonies were less than control group. and the lobulo-ductal colonies which is known that it formed in well differentiated mammary epoithelial cells were developed more in MSF treated group than control group. These results showed that methanol extract of kimchi affected on the proliferation and differentiation of normal rat mammary epithelial cells cultured in serum free medium condition.

A Study on the Tactical Aspect of Mathematical Internal Connections (수학 내적 연결성에 관한 형식적 측면 연구)

  • When planning lessons and developing materials about mathematical teaching and learning, we should condignly change and reconstruct contents and orders in light of ranks and connections between subject materials. Moreover teachers should teach mathematical concepts so that students might understand then not only independently and disjunctively but also relationally and reflectively. For this, teachers have to prepare thoroughly. By analyzing advanced research for mathematical connections, this study categorizes them according to two conditions: internal-external and content-formality. Through this, tactical aspect similarity and indistinguishability between mathematical external connections and mathematical internal connections have been identified. Based upon this fact, this study proposed the principles and the examples of tactical aspect on mathematical Internal Connetions.

A Case Report of Noonan Syndrome with Mental Retardation and Attention-Deficit Hyperactivity Disorder (정신지체와 주의력결핍 과잉행동장애를 보이는 Noonan 증후군 1예)

  • Noonan syndrome is characterized by short stature, typical facial dysmorphology, and congenital heart defects. The main facial features of Noonan syndrome are hypertelorism with down-slanting palpebral fissures, ptosis, and low-set posteriorly-rotated ears with a thickened helix. The cardiovascular defects most commonly associated with this condition are pulmonary stenosis and hypertrophic cardiomyopathy. Other associated features are webbed neck, chest deformity, mild intellectual deficit, cryptorchidism, poor feeding in infancy, bleeding tendency, and lymphatic dysplasias. The patient is a 10-year-old boy. He had experienced repeated febrile convulsions. He had typical facial features, a short stature, chest deformity, cryptorchidism, vesicoureteral reflux, and mental retardation. His language and motor development were delayed. When he went to school, it was difficult for him to pay attention, follow directions, and organize tasks. He also displayed behavior such as squirming, leaving his seat in class, and running around inappropriately. Clinical observation is important for the diagnosis, so we report a patient who was diagnosed with Noonan syndrome, mental retardation, and attention-deficit hyperactivity disorder.

Damping characteristics of CFRP strengthened castellated beams

  • In recent years, Carbon Fibre Reinforced Plastic (CFRP) strengthening is found to be one of the best methods to strengthen steel structures. The fibrous bond can also influence the vibration characteristics of the strengthened element apart from its static strength enhancement property. The main objective of this study is to understand the influence of CFRP strengthening on the dynamic Behaviour of Thin-Webbed Castellated Beams (TWCBs). A detailed experimental investigation was carried out on five sets of beams with varying parameters such as domination of shear (Shear Dominant, Moment Dominant and Moment and Shear Dominant), sectional classification (Plastic and Semi-compact) and perforation geometries (ho/dwratio 0.65 and e/ho ratio 0.3). Free vibration analysis was carried out by exciting the simply supported TWCBs with an impact force generated by a ball dropped from a specific height. Logarithmic decrement method was used to obtain the damping ratio and natural frequencies of vibration were found by Fast Fourier Transform (FFT). Natural frequency showed an increase in a range of 10.5 - 55% for the different sets of castellated beams. An increase of 62.30% was noted in the damping ratio of TWCBs after strengthening which is an indication of improvement in the vibration characteristics of the beam.

A Clinico-Horomonal and Cytogenetic Studies in Patients with Gonadal Dysgenesis (성선 발육 부전 환자에 대한 임상 및 세포 유전학적 연구)

  • As the cytogenetic developed, cytogenetic study has also developed progressively. This study is a systematical cytogenetic and clinico-hormonal analysis of 20 cases Wp.ere gonadal dysgenesis was diagnosed and deferred to the Dept. of obstetrics and Gynecology, Yonsei University, Medical School from Jan. 1974 to Aug. 1983. Twenty patients with the diagnosis of gonada dysgenesis have been assesed as to possible correlations between clinical, homonal and cytogenic findings. The desults were as follows; l. Gonadal dysgenesis were found in 20 cases, consisting of 15 cases (75%) of turnurs syndrome, 4 case of pure gonadal dysgenesis (20%), 46. XX and 1 case of mixed gonadal dysgenesis, 45,XO/46,XY. 2. Patients with XO karyotype, turner's ryndorme, have a resonably constant clinical picture of sexual infantilism with streak gonads, short status and webbed neck. 3. 17 cases were found primary amenorhea and two cases were noted with 2 ndary amenorrhea. one case has been presented with menstruation. 4. The rudimentary streak gonads were found in 7 cases of 8 cases and one case has a rudimentary streak gonad on one side and a testis on the contralateral side. 5. The study showed that potients with gonadal dysgenesis had an average of about 4-8 times higher basal FSH and about 3-7 times higher basal LH than that of the early follicular phase of normal menstrual cycle. 6. Two cases of three gonadal dysgenesis patieats, who performed LH-RH challage test, showed that the serum FSH levels reached the maximal level at 30 min after injection of CHRH and the serum LH level reached the maximal level at 60 min ofter injection of LHRH one case showed no significant response to LH-RH injection. Thus, bu studying simultoneously the clinical, cytogenic, hormonal aspects and visualization of gonads, we have gained some practical insight into the requirements for proper disgnosis and treatment.

Corrective Surgery of Congenital Cardiac Anomalies in the Noonan syndrome - Report of two cases - (Noonan 증후군에 동반된 심기형의 수술적 교정 - 2례 보고 -)

  • Noonan syndrome is characterized by a Turner-like phenotype and a normal karyotype associated with congenital abnormalities, such as short stature, variable mental retardation, hypertelorism, webbed neck, low posterior hair line, skeletal malformation and congenital cardiovascular defect. Two third of Noonan syndrome have cardiac anormalies, half with pulmonary stenosis. We have experienced two cases of pulmonary stenosis associated with other cardiac anomalies in Noonan syndrome. The first 31-year-old male patient had characteristic appearance of Noonan syndrome with severe infundibular pulmonic stenosis and patent foramen ovate. The second 28-year-old male patient had valvular and subvalvular Pulmonary stenosis with typical Noonan\`s face and stature. Pulmonary valvotomy and hypertrophied muscle bundles in the right ventricular cavity were resected in both cases. Patent foramen ovate was closed directly in the first case. Postoperative follow-up examinations revealed no symptoms and successful outcome.

  • Noonan syndrome (NS) is a developmental disorder characterized by dysmorphic facial features in association with short stature, mental retardation and congenital heart disease. NS may be sporadic or inherited as an autosomal dominant or recessive trait. The children with NS usually have ocular hypertelorism, downslanting palpebral fissures, low-set ears and a webbed neck, chest deformity. In addition, oral features include micrognathia, high arched palate, dental malocclusion, dental anomalies and rarely, cleft palate. The phenotype of NS bears similarities to that of Turner syndrome. However, NS occurs in both males and females with a normal sex chromosome 46, XX and 46, XY constitution. This case presents the intermittent treatment of an 8-year-old girl who was referred from a local clinic for the extraction of supernumerary teeth and treatment of dental caries. The focus of this case report is the oral aspects on NS and particularities of the dental treatment in subjects affected by this genetic disease.
