균형과 보행능력 향상을 위한 오타고 운동프로그램의 임상적 유용성: 만성 뇌졸중 환자를 대상으로, 무작위 배정 실험연구 (Clinical Feasibility of Otago Exercise Program to Improve the Balance and Gait Ability: In Chronic Stroke Patient, Randomized Controlled Trial)

  • PURPOSE: The aim of this study was to conduct an Otago exercise program with stroke patients in a clinical setting and ascertain its effects on balance and walking ability. METHODS: The participating subjects were 45 people who had been diagnosed with stroke. They were assigned to two groups (Otago exercise; OE, n=22; balance exercise; BE, n=23), and the exercises were conducted for three sessions per week for eight weeks. The main balance outcomes were evaluated using the timed up and go test (TUG) and the four step square test (FSST), while walking ability was evaluated using the 10 m walk test (10MWT), direction change ability was measured using the figure 8 of walk test (F8WT), and the decrease of fear was evaluated using the modified falls efficacy scale (MFES). RESULTS: In the analysis results, the Otago exercise group showed significant increases in TUG, FSST, 10MWT, and F8WT within the both groups. There were significant differences in all variables between the OE group and the BE group at the post-intervention evaluation, but there was no significant difference between the groups with respect to the 10MWT. This study showed that conducting Otago exercise in stroke patients increased the main outcomes for TUG, FSST, F8WT scores, but not for 10MWT. CONCLUSION: The results of this study demonstrated that Otago exercise would be useful to improve balance and gait for stroke patients who want to improve their abilities and activities of daily living.

Effects of High-Intensity Interval Training on Motor Skills Recovery in Sciatic Nerve Crush-Induced Rats

  • PURPOSE: This study examined the effects of mild-intensity exercise (MIE) and high-intensity interval exercise (HIIE) on the recovery of the motor function over time in sciatic nerve crush injury rats. METHODS: The MIE group ran on a treadmill at a speed of 8.3 m/min to perform low-intensity training with maximum oxygen uptakes ranging from 40 to 50%. The HIIE group ran on the treadmill at a speed of 25 m/min to perform high-intensity training with a maximum oxygen uptake of 80%. The interval training was performed based on a 1:1 work-to-rest ratio. The effects of each form of exercise on the rats' walking abilities following their recovery from the peripheral nerve injuries were evaluated based on the results of behavior tests performed at one and 14 days. RESULTS: According to the test results, the MIE group showed significant improvements in the rats' ankle angle in the initial stance phase, and in the ankle and knee angles in the toe-off phase (p<.05). The HIIE group exhibited significant improvements in the ankle and knee angles in the initial stance phase, SFI(p<.05). CONCLUSION: The state of such patients can be improved by applying the results of this study in that MIE and HIIE on a treadmill can contribute to the recovery of the peripheral nerve and motor skill. In particular, MIE is used as a walking functional training in the toe-off stance phase, while HIIE is suitable in the initial stance stage.

A Comparative Study on Clinical Gait Abilities of Stroke Patients According to Indoor and Outdoor Environments

  • Objective: This study aimed to compare gait ability through gait evaluations in indoor and outdoor environments according to the general characteristics and walking ability of stroke patients. Design: Crossed-sectional study. Methods: The subjects of this study were 57 hospitalized stroke patients.The study subjects were asked to select an indoor environment and an outdoor environment in random order, and the Timed Up and Go Test (TUG), 10-Meter Walk Test (10MWT), Figure-Eight Walk Test (F8WT) and the Functional Gait Assessment (FGA) were used to assess each environment. Results: The TUG, 10MWT, F8WT time and number of steps, and FGA showed a significant decrease in gait ability in the outdoor environment compared to the indoor environment (p<0.05). Although the TUG, 10MWT, and the time required for the F8WT were statistically higher in the outdoor compared to the indoor environment at points 2, 3, and 4, but not 5 of the functional ambulatory category (FAC), significant increases in the number of steps of the F8WT were found in the outdoor compared to the indoor environment for only points 2 and 3 of the FAC (p<0.05). In the FAC 3 and 4, there was a statistically significant decrease in the outdoor compared to the indoor environment only in the FGA (p<0.05). Conclusions: Therefore, it has been shown that the gait ability of stroke patients is reduced in the outdoor environment compared to the gait ability in the indoor environment.

The Influence of Auditory-Feedback Device Using Wearable Air-Pressure Insole on Spatiotemporal Gait Symmetry in Chronic Hemplegia

  • Objective: To investigate the effect of emphasized initial contact by using a wearable air-pressure insole to provide auditory-feedback with variations of maximum peak pressure (MPP) of the affected side on spatiotemporal gait parameters and gait symmetry of stroke patients Design: A cross-sectional study Methods: Eighteen stroke patients participated in this study. All subjects walked five trials using an air-pressure insole that provides auditory feedback with different thresholds set on the insole. First, subjects walked without any auditory feedback. Then, the MPP threshold on the affected side was set from 70% and increase threshold by 10% after each trial until 100%. They walked three times or more on the gait analyzer for each trial, and the average values were measured. Before starting the experiment, subjects measured body weight, initial gait abilities and affected side MPP without auditory feedback. Results: Temporal and spatial variables were significantly increased in trials with auditory feedback from air-pressure insole except for non-paralyzed single support time and spatial gait symmetry compared to trials without auditory feedback(p<0.05). Among the four different thresholds, the walking speed, unaffected side single support time, affected and unaffected side stride, and affected side step length were greatest at 80% threshold of maximum peak, while affected single support time, temporal gait symmetry, and unaffected step length were greatest at the maximum peak of 100% threshold. Conclusions: These results indicate that auditory feedback gait using air-pressure insoles can be an effective way to improve walking speed, single support time, step length, stride, and temporal gait symmetry in stroke patients.

스마트폰 사용이 정상인의 장애물 보행에 미치는 영향 (Effects of Smart Phone Use on the Gait Parameters When Healthy Young Subjects Negotiated an Obstacle)

  • 본 연구의 목적은 건강한 성인이 장애물 넘기 보행을 하는 동안 스마트폰 사용 유무에 따라 보행의 변화를 분석하고자 시행하였다. 74명의 건강한 성인이 본 연구에 참여하였고, 스마트폰 사용 군과 비사용 군에 무작위로 배정되었다. 스마트폰 사용 군은 스마트폰을 사용하면서 편안한 속도로 걷다가 장애물 넘기 보행을 수행하였고, 스마트폰 비사용 군은 스마트폰을 사용하지 않고 장애물 넘기 보행을 수행하였다. 각 군에 해당하는 대상자의 보행의 변화를 알아보기 위해 3차원 동작 분기를 이용하여 토우 클리어런스, 보행률, 보행 간격, 보행 넓이, 보폭, 그리고 보행 속도와 같은 보행 변수를 측정하였다. 본 연구 결과, 스마트폰 사용 군이 비사용 군보다 토우 클리어런스, 보행 넓이, 보행률, 그리고 보행 간격 변수에서 통계학적으로 유의하게 증가하였고, 보행 속도 변수에서는 유의하게 감소하였지만 보폭에서는 두 군간 유의한 차이가 없었다. 그러므로 장애물 넘기 보행을 하는 동안 정상인의 스마트폰 사용은 보행 능력을 저하시키며, 낙상의 위험을 증가시킬 것으로 사료된다.

The Effect of Aquatic Task Training on Gait and Balance Ability in Stroke Patients

  • Purpose: The purpose of study was to measure stroke patients' ability to balance and their degrees of clinical function and to examine the effect of the aquatic exercise method using tasks related to these features. Methods: Twenty stroke patients were randomly assigned to an aquatic task exercise group and a land task exercise group. Both groups used the same exercise method for 60 minutes each session, three times a week for 12 weeks at the same time point and with the same amount of exercise. Results: Before and after the exercise, static balance was measured using balance measuring instruments locomotive faculties, muscular strength, and dynamic balance were assessed through the Berg balance and 10 m gait tests. Finally, gait abilities were measured, and the data obtained were analyzed to generate the results. Conclusion: Both groups showed significant improvement, but the aquatic exercise group showed slightly more significant results in static balance, Berg balance, and upright walking tests. It is thought that the improvement of stroke patients' balance and gait ability can be triggered through the application of aquatic exercise programs in the future.

경기지역 노인의 건강과 식생활관리 II - 75세 미만의 젊은 노인과 75세 이상 고령 노인 비교 - (The Elderly Health and Dietary Management in Gyeonggi Province II - Comparison with Younger Old and Older Old -)

  • The purpose of this study is to provide the information of the aged olds for which basic data are almost no available. The subjects were divided on the base of age 75. The elderly under 75 were named 'the young olds' and over 75 were 'the aged olds'. The aim of this research is to promote health and to improve nutrition, and the survey was made for health promotion behaviors, habits against health risk, dietary management status and diet intake. And it was conducted by 24 hr-recall method and analyzed by nutrients intake. 242 subjects were collected in 12 cities or counties in Gyeonggi Province and 20 elderly people (10 male and 10 female) were selected out of 1 village in each district. Questionnaire about health behavior and dietary management was carried out by interview method through regional home extension workers. Statistical analyses were made by SAS (version 8.1) and Chi-square tests and General Linear Models were used. Out of the subjects the aged olds over 75 was 31.9%, elementary school educated (93.5%), with spouse (40.3%), with adult children(28.6%), monthly living cost of 500-1,000 thousand won(40.3%). Mean age was 78.82 years compared with 69.75 years of the young olds. 46.8% of the aged olds used monthly pocket money over 1000 won and it was lower than 63.3% of the young olds. Only half of the aged olds had regular exercise of walking (77.8%) or with athletic equipment (17.8%). However, the young olds did more frequent walking (82.1 %) and less exercise with athletic equipment (4.8 %), which was significantly different. Kinds of disease were different with the young or the aged olds, as more proportion of cardiovascular disease(37.9%) for the young olds and joint lumbago neuralgia(41.6%) for the aged olds. Dietary management was good (3 meals per day: 93.4%, fixed mealtime: 72.4%, and regular amount: 79.9%). But there was significant difference in side dish varieties and kinds of snacks; for the aged olds only 8% had over 5 sorts (compared with 18.8% of the young olds) and the kinds of snacks were cookie, candy, juice, carbonated beverage for the aged olds (compared with noodle, milk, soybean-milk for young olds). The ratio of nutrients intake (energy, riboflavin and niacin) with RDA was significantly higher for the aged olds than that of the young olds. The surveyed subjects had no difficulties in Activities of Daily Living (ADL), but some of the aged olds had difficulties in Instrumental Activities of Daily Living (IADL) like housekeeping, using transportation, going shopping and making phone calls. These results suggest that low quality of life is linked with low economic status of the rural elderly and congregate meal at village hall would be required because of the lack of side dishes variety for the aged olds. And nutrition education program about good snacks and exercise practice would be needed for the aged olds. By operating nutrition education program the aged olds would enjoy better quality life maintaining or ameliorating IADL abilities.

Position Control of Mobile Robot for Human-Following in Intelligent Space with Distributed Sensors

  • Latest advances in hardware technology and state of the art of mobile robot and artificial intelligence research can be employed to develop autonomous and distributed monitoring systems. And mobile service robot requires the perception of its present position to coexist with humans and support humans effectively in populated environments. To realize these abilities, robot needs to keep track of relevant changes in the environment. This paper proposes a localization of mobile robot using the images by distributed intelligent networked devices (DINDs) in intelligent space (ISpace) is used in order to achieve these goals. This scheme combines data from the observed position using dead-reckoning sensors and the estimated position using images of moving object, such as those of a walking human, used to determine the moving location of a mobile robot. The moving object is assumed to be a point-object and projected onto an image plane to form a geometrical constraint equation that provides position data of the object based on the kinematics of the intelligent space. Using the a priori known path of a moving object and a perspective camera model, the geometric constraint equations that represent the relation between image frame coordinates of a moving object and the estimated position of the robot are derived. The proposed method utilizes the error between the observed and estimated image coordinates to localize the mobile robot, and the Kalman filtering scheme is used to estimate the location of moving robot. The proposed approach is applied for a mobile robot in ISpace to show the reduction of uncertainty in the determining of the location of the mobile robot. Its performance is verified by computer simulation and experiment.

The Effects of Robot-Assisted Gait Training with Visual Feedback on Gait, Balance and Balance Confidence in Chronic Stroke Patients

  • Purpose: This study was conducted to assess the effects of robot-assisted gait training with visual feedback on gait, balance, and balance confidence in patients with chronic stroke. Methods: Thirty subjects with chronic stroke were randomly assigned to two groups: the experimental group (n=15) and the control group (n=15). The experimental group performed robot-assisted gait training for 30 minutes and the control group performed gait training with assisted devices training for 30 minutes after both groups performed conventional physical therapy for 30 minutes. Both groups performed the therapeutic interventions for 5 days per week, for a period of 4 weeks. For assessment of the 10 m walking test (10 MWT), Figure of 8 on the walk test (F8WT), Timed-Up and Go test (TUG), and Berg Balance Scale (BBS) were used to test the gait and balance, and the Korean version of the Activities-specific Balance Confidence Scale was used to test the balance confidence. Results: The experimental group showed significant improvement in the 10 MWT and the K-ABC (p<0.05), and the control group showed significant improvement in the BBS and the TUG (p<0.05). In four measurements, there were significant differences between the two groups (p<0.05), and the control group showed significant improvement in the F8WT at pre and post intervention (p<0.05). Conclusion: This study demonstrated that Robot-assisted gait training with visual feedback is an effective intervention for improving straight gait abilities and balance confidence, while the control group showed some improvement in curve gait and balance. Thus, we suggest both Robot-assisted gait training with visual feedback and gait training with assisted devices training exercise as a therapeutic intervention in chronic stroke rehabilitation.

고령화 사회에 대비한 중년층의 노후 주거선호 - 광주지역의 대학생 자녀를 둔 학부형을 대상으로 - (The Preference of Housing for the Elderly among the Middle-aged Households for Aging Society)

  • This study was conducted as a survey to find out middle-aged persons's housing preferences for a future elderly housing and to grasp background characteristics of persons influencing the preferences and the effect of elderly's aging situations on the changes of their preferences. A sample of 300 middle-aged people was selected purposely from parents of university students living in Gwang-ju city. Questionnaires were administered to parents by students and analysed with SPSS Windows 10 program. Middle-aged persons's housing preferences were differed according to elderly's aging situations, in the case of healthy situation, they tended to prefer to stay present house without their children in irrespective to couple or single, whereas in the case of unhealthy situation, they tended to prefer to live with their children. and couples showed preferences for planned housing for the elderly and singles showed preferences for elderly housing facilities with care services. The most important characteristics of elderly housing among middle-aged people was a places where they can enjoy leisure and provide home-help services, located in the suburbs. Also middle-aged persons were likely to prefer to a green spaces such as garden, path for walking, and madang for the elderly housing. Needs for community care services (NCCS) were required above the average. Among the items of NCCS, a visiting medical examination was the most demanding, and a visiting nursing services, a emergency calling bell, and home repairs were sequently followed in the level of needs. The characteristics of the elderly housing which preferred to and needs for community care services were influenced by middle-aged persons's age, gender, subjective evaluation of economic abilities, tenure status, the cost of living, and the size of housing. This findings suggests that it must to be considered to develop various types of the elderly housing depending on socioeconomic status.
