A Review on 'cunkou(寸口)' in Maijing(脈經) ("맥경(脈經)"중(中) '촌구(寸口)'에 대한 고찰(考察))

  • Objective : There are several terms on palpation such as cunkou(寸口), qikou(氣口), maikou(脈口). In traditional medicine dictionary or even WHO international standard terminologies on traditional medicine in the western pacific region, there are no discriminations about them. However, there are some differences in usages of cunkou, qikou, maikou at Maijing. Method : First, we searched usages of term, cunkou, qikou, maikou in Huangdineijing(黃帝內經) and Maijing and compared the meanings. Result : Meanings of cunkou, qikou, maikou in Huangdineijing are pair with renying(人迎) and inch-bar-cubit on wrist. Meanings of cunkou in Maijing are pair with renying, inch-bar-cubit on wrist and only inch on wrist. Meanings of qikou, maikou in Maijing are inch-bar-cubit on wrist. Conclusion : Cunkou, qikou, maikou are known as same terms. However, scholars have to be cautious when they use cunkou, qikou, maikou term and comprehend those in book.

Effects of TENS & Auricular of Somatic on Experimental Pain Threshold (외이경혈자극과 경피신경전기자극이 체성감각에 미치는 영향)

  • The purpose of this study were to examine the effect of high intensity, low frequency transcutaneous electrical nerve stimulation at auricular acupuncture points on experimental pain threshold measured at the wrist and to determine the changes in effect over time. Forty-two healthy adult men and women were assigned randomly to one of three treatment groups. Group 1(n=15) received TENS to appropriate auricular points for wrist, Group 2(n=12) received TENS to wrist, and Group 3(n=15) received no TENS. We measured experimental pain threshold at the wrist after an electrical stimulus during one pre-treatment and three post-treatment time periods. Group 1 and 2 showed stastically significant increase(p<0.05) in pain threshold after treatment whereas the Group 3 did not. Group 1 showed a significant increase in pain threshold than Group 2. These results suggest that high intensity, low frequency TENS applied to appropriate auricular acupuncture points can increase pain threshold.

  • In this paper, Artificial Neural Network(ANN) based motion classification algorithm is proposed to classify wrist motions using surface electromyograms(sEMG). surface EMGs are obtained from two electrodes placed on the flexor carpi ulnaris muscle and extensor carpi ulnaris muscle of 26 subjects under no strain condition during wrist motions and used to recognize wrist motions such as up, down, left, right, and rest. Feature is extracted from obtained EMG signals in time domain for fast processing and used to classify wrist motions using ANN. DAMV, DASDV, MAV, and RMS were used as features and accuracies of motion classification based on ANN were 98.03% for DAMV, 97.97% for DASDV, 96.95% for MAV, 96.82% for RMS.

Segmental Sensory Nerve Conduction Study in Vibration Exposed Subjects

  • The present study was performed to assess peripheral neural involvement by exposure to hand-arm vibration. Segmental sensory nerve conduction in the median and ulnar nerves were measured in shipyard workers exposed to vibration. The subjects were 47 male adults exposed to hand-arm vibration and 7 healthy male controls. The subjects underwent an extensive bilateral neurophysiological examination. Sensory compound nerve action potential (SNAP) of the median and ulnar nerves in palm-finger and wrist-palm segments were measured by antidromic method. And SNAP of the median and ulnar nerves in wrist-proximal finger and wrist-distal finger segments were measured by orthodromic method. Result of sensory nerve conduction study was abnormal in 31 patients $(66\%)$ and normal in 16 patients $(34\%)$ of subjects. The pathological pattern in the hand-arm vibration exposed group was 13 patients $(28\%)$ of carpal tunnel syndrome, 18 patients $(38\%)$ of distal sensory neuropathy, 7 patients $(15\%)$ of multifocal and 1 patient $(2\%)$ of Guyon syndrome. The present study indicates that vibration-induced nerve impairments exist both in the finger-palm and palm-wrist segment of median and ulnar sensory nerves. The results suggest that segmental sensory nerve conduction study would be useful as objective indication of peripheral nerve impairment induced by the hand-arm vibration.

  • Recently, the development of practical wearable robot interfaces has resulted in the emergence of wearable robots such as arm prosthetics or lower-limb exoskeletons. In this paper, we propose a novel method of wrist movement intention estimation based on a regression technique using electromyography of human bio-signals. In daily life, changes in user arm position changes cause decreases in performance by modulating EMG signals. Therefore, we propose an estimation method for robust wrist movement intention for arm position changes, combining several movement intention models based on the regression technique trained by different arm positions. In our experimental results, our method estimates wrist movement intention more accurately than previous methods.

Prevalence of MSDs and Postural Risk Assessment in Floor Mopping Activity Through Subjective and Objective Measures

  • Background: Residential and commercial cleaning is a part of our daily routine to maintain sanitation around the environment. Health care of professionals involved in such cleaning activities has become a major concern all over the world. The present study investigates the risk of musculoskeletal disorders in professional cleaners involved in floor mopping tasks. Methods: A cross-sectional study was performed on 132 mopping professionals using a modified Nordic questionnaire. The Pearson correlation test was implemented to study the association of perceived pain with work experience. The muscle strain and postural risk were evaluated by means of three-channel electromyography and real-time motion capture respectively of 15 professionals during floor mopping. Results: Regarding musculoskeletal injuries, risk was reported majorly in the right hand, lower back, left wrist, right shoulder, left biceps, and right wrist of the workers. Work experience had a low negative association with MSDs in the left wrist, right wrist, right elbow, lower back, and right lower arm (p < 0.01). Surface EMG showed occurrence of higher muscle activity in upper trapezius and biceps brachii (BB) muscles of the dominant hand and flexor carpi radialis and BB muscles of the nondominant hand positioned at the upper and lower portion of the mop rod, respectively. Conclusion: Ergonomic mediations should be executed to lessen the observed risk of musculoskeletal injuries in this professional group of workers.

A Study on Wrist Band Type Vital Sign Acquisition Device (손목형 생체신호수집 장치에 대한 연구)

  • In this study, we proposed a new method that can be measure ECG (Electrocardiography) and PPG (Photoplethysmography) in realtime on the site of the wrist for check the state of health in daily life. For convenience measurement of ECG the lead I method was used on the wrist, and omit the reference junction ECG I was measured in the right hand and the left hand of the potential difference. Then the measured electrocardiogram was amplified by the differential amplifier and the signals were passed HPF, LPF, and BPF filters. For removing the PPG's noise from the Motion artifact and temperature, we apply the reflective photoelectric volume pulse wave measurement method using green LED as a light source. The circuits was designed to be able to check the waveform using higher active amplification method at weak signals. For the validation of our device, the measured signals were compared with E2-KIT on same time. The results shows that the error does not exceed the maximum one, most of the data is confirmed to be issued Peak inspection of the same number.

A Comparative Study of Blood Pressure According to Cuff Size and Measurement Site (커프크기와 측정부위에 따른 혈압측정치 비교 연구)

  • Purpose: The purpose of this study was to identify differences in blood pressure according to cuff size and measurement sites of the participants. Method: The participants consisted of 50 women and 50 men whose upper arm circumference was $26\sim30cm$. They had no chronic illness and gave consent to participate. Blood pressure of the wrist was measured in the sitting position, the upper arm with a standard cuff, large and small cuffs were used for measurement in supine position and the thigh in prone position. The data were analyzed with paired t-test using SPSS 12.0 program. Result: The data for the upper arm showed a difference in systolic and diastolic blood pressure depending on the site of measurement. There was a significant difference between measurements with a standard cuff and measurements with large and small cuffs. The systolic blood pressure of the wrist and the thigh were significantly lower than that of the upper arm. Conclusion: These results suggest that the selection of an appropriate cuff is an essential element in ensuring accuracy when measuring blood pressure and differences in systolic blood pressure for the upper arm, wrist and thigh indicate the need to record the measuring site when measuring blood pressure.

  • Ganglionic cyst is the most common benign tumor in the wrist and hand, and easily diagnosed with ultrasonography. Ganglionic cyst in sonography usually appears as hypoechoic or anechoic with well-demarcated cystic mass. We report a case of the hyperechoic mass at the volar aspect of the wrist, which confirmed as ganglionic cyst by operative and histological findings.

The Effect of Grip Strength in Change of Wrist Position according to Elbow Flexion (주관절 굴곡에 따른 손목관절 위치의 파악력에 미치는 영향)

  • Purpose:The purpose of this study are analysis characteristic of grasping power with each different elbow flexion degree and grasping power with each different elbow each different hand position and announcement. Methods:Measuring about 10cm wide open position with both feet for each elbow in the line positions of the $0^{\circ}$, $45^{\circ}$, $90^{\circ}$, $135^{\circ}$, and forearm position of supination, mid position, pronation is of the order. Results:The averge position of maximal grip strength was $0^{\circ}$ of elbow flexion with mid position in male and $90^{\circ}$ of elbow flexion with mid position in female. Grip strength in change of wrist position according to elbow flexion was significant difference(P<0.05). Grip strength in change of wrist position according to elbow flexion by sex was significant difference(P<0.05) Conclusion:When the elbow has flexion with $0^{\circ}$, $45^{\circ}$, $90^{\circ}$, $135^{\circ}$ then grasping power has a intimate relation with forearm position. Grasping power with forearm supination, pronation and mid position also has a intimate relation with elbow flexion.
