Effects of Action-observational training Program on Gait and Balance of Patients with Hemiplegia (동작관찰 과제지향훈련이 만성 뇌졸중환자의 균형 및 보행능력에 미치는 효과)

  • Purpose : This study was conducted to evaluate the effects of an action-observational task oriented training on balance and gait ability of patients with chronic stroke. Method : The subjects of this study were 30 patients with chronic stroke who agreed to participate and were picked up. Participants were randomly divided into equal groups; namely, an experimental group that underwent action-observational task oriented training for at least 30 minutes/day for 6 weeks and a control group that underwent general task-oriented training. Patients' balance was assessed using the Sway Length(SL), Sway Area (SA) and Limit of Stability (LOS) test. In addition, gait ability was assessed using the 10 Meter Walking Test (10MWT). Gait time and speed taken to walk 10 metres were used to examine gait ability. Result : There were significant improvements in the subscales of the balance and gait ability test of those who participated in the action-observational task oriented training, while the control group showed no significant changes. Conclusion : Therefore, Action-observational training effectively improved the balance and gait abiltiy in patients with chronic stroke.

A CASE REPORT OF COMPLICATIONS DURING MANDIBULAR TRANSVERSE SYMPHYSIS WIDENING (하악 이부 확장술 시 나타나는 합병증의 치험례)

  • Orthodontists often treat cases which are difficult to treat with conventional orthodontics. In such cases, it could be treated with surgical procedures with the help of an oral surgeon. Especially, transverse deficiency of the mandible can be corrected by widening the transverse width of mandibular symphysis, using distraction osteogenesis. Transverse widening of mandibular sympysis is known as a safe treatment but still complications could occur during the treatment. We are reporting some complications of cases that mandibular symphysis transverse widening were applied. Some cases showed complications because of the inappropriate osteotomy line. Since straight vertical osteotomy line was inclined to the left, only the left bony segment was likely to expand. According to bio-mechanical considerations, it will be better to perform a step osteotomy, cutting the eccentric area of the alveolar crest and the centric area of the basal symphyseal area. Complications could also occur by the failure of the distraction device. The tooth borne distraction device was attached on the lingual side of the tooth with composite resin. During the distraction period, it was impossible to obtain appropriate distraction speed and rhythm because of frequent fall off of the distraction device. Therefore, distraction device should be attached firmly with orthodontic band or bone screw, etc. Tooth mobility increasement could also occur as a complication. 'Walking teeth phenomenon' was observed during the distraction period, showing severe teeth mobility and pain during mastication. These symptoms fade out during the consolidation period. Since the patient could feel insecure and uncomfortable, it should be notified to the patient before the procedure. Finally, alveolar crestal bone loss could occur. Alveolar crestal bone loss occurred because of lack of distraction device firmness and teeth trauma caused by lower lip biting habit. Therefore, adequate firmness of the distraction device and habit control will be needed.

Effect of Walking Speed on Lower Extremity Internal and External Rotation While Turning 90 Degrees (90도 회전 시 보행속도가 하지의 내외 회전에 미치는 효과)

  • 회전(turning)은 보행 중 방향을 바꾸는 운동 기술(motor skill)이고, 회전 전략(turning strategy)은 회전을 완수하는데 사용되는 일반적 행동 전형(generalized movement pattern)이다. 회전에 대한 보행속도의 영향은 분명하지 않다. 이 연구의 목적은 보행속도의 돌기 전략에 대한 영향을 분석하고 보행속도의 하지 내외 회전(internal and external rotation)에 대한 영향을 분석하는 것이다. 건강한 젊은 성인 15명이 이 연구에 자발적으로 참여하였다. 맥리플렉스 측정 장치(MacReflex measurement system)가 동작 분석(motion analysis)을 위해 사용되었다. 각각의 자원자들은 보행 중 90도 왼쪽으로 회전을 10회씩 완수하였다. 각각의 시도마다 보행속도를 다르게 하기 위해서 세 가지의 다른 요구들(slow, regular, fast)이 임의적으로 주어졌고 각각의 실제 보행속도가 자원자의 무게중심 변화에 따라 구해졌고 요구별 평균이 구해졌다. 회전 안쪽 발의 스핀(in side foot spin)은 보행속도가 증가함에 따라 증가했지만, 회전 바깥쪽 발의 스핀(out side foot spin)은 보행속도와 상관이 없었다. 하지의 내외 회전은 보행속도와는 상관이 없었지만, 같은쪽 발의 스핀과는 역관계가 있었다. 회전은 발 스핀이 있는 돌기와 발 스핀이 없는 돌기로 구분되는 것이 합당한 듯 하다. 제한된 시간과 공간 내에서 스핀은 보행속도가 빨라질수록 몸의 전방 운동량(forward momentum)에서 몸의 전방 운동량(forward momentum)으로의 전환이 스핀이 없는 회전 시보다 효율적이다. 고관절의 내외 회전 근육들은 회전전략에 상관없이 회전되는 동안 몸의 역학(body mechanics)을 조절하는데 중요한 역할을 맡고 있는 것으로 보인다. 앞으로 회전 시 몸의 생체 역학적 그리고 신경 근육적 기전들(biomechanical and neuromuscular mechanisms)을 밝히는 연구들이 필요하다.

An Epidemiological Study for Child Pedestrian Traffic Injuries that Occurred in School-zone (어린이 보호구역 내에서 발생한 6-14세 어린이들의 보행 중 교통사고에 대한 역학적 조사)

  • Objectives: Pedestrian traffic injuries have been an important cause of childhood mortality and morbidity for decades. The aim of this study was to describe the epidemiological characteristics of child pedestrian traffic injuries that occurred during 2000 in one metropolitan city and its school-zones, and to determine the factors associated with those accidents. Methods: A cross-sectional study was performed in 2001. Police records were used to identify the cases of pedestrian injury. Children aged between 6 and 15 years, injured during road walking, were included in this study. A direct survey of the environmental factors within the school-zones in study area (n=116) was also performed. Self-administered questionnaires, via mail and telephone surveys, were used to assess the safety education programmes. The schools were divided into two groups according to the occurrence of pedestrian traffic injuries in their school-zone. Results: Pedestrian injuries (n=597) were found to account for 3.2% of all traffic injuries in the subject area. The epidemiological characteristics were not significantly different between genders. There were some significant risk factors within the environmental factors, such as local road (OR: 2.3, 95% CI=1.05-5.35), heavy traffic volume (OR: 2.2, 95% CI=1.00-5.04), poor visibility of speed-limit signs (OR: 2.8, 95% CI=1.25-6.42), no separation of pedestrian routes from cars (OR: 2.6, 95% CI=1.02-6.75) and barriers on the pedestrian routes (OR: 2.2, 95% CI=1.01-5.08). Only one factor, that of education in a safety-park (OR: 0.3, 95% CI=0.09-0.96), was significantly associated in the traffic and pedestrian safety education factors. Conclusion: Significant associations with pedestrian injury risk were identified in some of the modifiable environmental factors than in the educational factors.

Comparisons of the gait characteristics depended on Unilateral Trans-Femoral or Trans-Tibial Prostheses (편측대퇴의지와 편측하퇴의지의 보행특성 비교)

  • The aim of this study is to present the basic reference data of age and specipic gait parameters for comparisons of the gait characteristics depended on Unilateral Trans-Femoral or Trans-Tibial Prostheses. The basic gait parameters were extracted from 10 Adult, 10 above knee(A/K) patients and 10 below knee(B/K) patients, 50 to 60 years of age using VICON 512 Motion Analyzer. The results were as follows; 1) The mean Cadence of the above knee(A/K) patients and below knee(B/K) patients were $87.77{\pm}8.64$ steps/min, to $99.84{\pm}11.14$ steps/min.(p<0.05) 2) The mean Walking Speed of the above knee(A/K) patients and below knee(B/K) patients were $0.84{\pm}0.15$ m/s, to $0.96{\pm}0.25$ m/s.(p>0.05) 3) The mean Stride Length of the above knee(A/K) patients and below knee(B/K) patients were $1.14{\pm}0.14$ m, to $1.14{\pm}0.22$m.(p>0.05) 4) The mean maximal angles of joint on the hip flexion motion for different above knee(A/K) patients and below knee(B/K) patients were $34.75{\pm}10.18_{\circ}$, to $32.32{\pm}6.34_{\circ}$ .(p>0.05) 5) The mean maximal angles of joint on the knee flexion motion for different above knee(A/K) patients and below knee(B/K) patients were $66.97{\pm}15.08_{\circ}$, to $52.65{\pm}9.21_{\circ}$ .(p<0.05) 6) The mean maximal angles of joint on the ankle dorsiflexion motion for different above knee(A/K) patients and below knee(B/K) patients were $14.41{\pm}4.82_{\circ}$, to $10.04{\pm}3.49_{\circ}$ .(p>0.05) 7) The mean maximal angles of joint on the ankle plantarflexion motion for different above knee(A/K) patients and below knee(B/K) patients were $5.77{\pm}3.17_{\circ}$, to $2.75{\pm}4.49_{\circ}$ .(p>0.05)

Correlations Among the Berg Balance Scale, Gait Parameters, and Falling in the Elderly (노인에서 Berg 균형 척도, 보행 변수, 그리고 넘어짐과의 관계)

  • This study examined the correlations among the Berg Balance Scale, which is a clinical tool used to evaluate balance ability, spatiotemporal parameters of gait, and falling; determined the parameters most closely related to falling; and identified a discriminatory parameter and its predictability. Thirty-four subjects aged 72 to 92 years participated in this study. Following a questionnaire survey about falling, the Berg Balance Scale and spatiotemporal parameters of gait were measured. The results revealed that the incidence of falls increased with aging and an accompanying reduction in the flexion range of motion of the hip joint. The gait characteristics of elderly people who fell easily included a slower walking speed, shorter stride, and longer stance time than other elderly. When the cutoff score was set at 45, the Berg Balance Scale was able to identify correctly those individuals who truly have experience of falling than when the cutoff score was set at 39. But when the cutoff score was set at 39, the scale's specificity identifying correctly those individuals who truly have not experience of falling was higher than at the cutoff score of 45. Therefore, the Berg Balance Scale is an appropriate screening method in a clinical setting for the early detection of elderly people at risk of falling. In conclusion, elderly people with a Berg Balance Scale score. below 45 are the most likely to fall owing to their decreased balance ability.

  • To measure the athletic information of exercisers, the applications of smartphone are programmed based on the sensing data from GPS device. These applications provide exercisers for running or walking distance, exercising time, calorie consumption, average speed, and so on. Among them, the exercising distance should measure accurately because it directly affects the other athletic information for exercisers. However, the existing methods for measuring the exercising distance makes errors because they are worked on the simple sphere or ellipse earth models. Actually, the surface of real earth is composed of inclined ground like hills and valleys. In this paper, a new exercising distance measuring algorithm is proposed to compensate the errors of existing method. It considers the altitude of slopes in exercising routes. To evaluate exercising distance measuring algorithms, we implement the athletic life-guide system based on the smartphone platform. In experiments, the proposed method shows that it provides more accurate distance measurement.

A Study of Clothing Design in the Digital Age (디지털 시대의 의상 디자인 개발에 관한 연구)

  • This study shows that clothes to be just the same as the real thing can be Produced through the third dimension computer graphics, and then presents that not only the area of fashion design can be expanded in the virtual reality field by doing the simulation of the fashion show, but also the information can be made the real time public ownership and the communication can be fulfilled smoothly. In this study, analyzing the third dimension computer graphic programs to be used much at present, Alias Wavefront Company's Maya software which was the most effective in the clothes simulation and the clothes CAD SGS OptiTex 8.7 which went well substitutive for it were used of them. The conclusions of this study that got through the work manufacture are as follows: The first, if the file manufacturing in the clothes CAD by using the computer was stored, the pattern used 3D simulation was available because it could be summoned in 3D software. The second, if the data of DXF form in Maya program was summoned, they could not be applied by Maya Cloth supported in Nurbs only because they were recognized as the DXF_layer. So the curve along the outer lines of the pattern was drawn and Maya Cloth was applied to be possible to get the natural silhouette of clothes. The third, when the clothes were manufactured by 3D, if the draping character was applied according to the textile special quality, not only the control of textile's thickness, weight, quality feeling, and silhouette was available, but also the clothes were available to graft the special textile materials. The fourth, the natural motion of model was produced by capturing the actual model's walking action In order to produce the fashion show motion and also the dynamic fashion show was available by the angle of camera, the establishment of lighting, and etc. in the final rendering. The clothes manufactured by 3D are available to change the design by changing the materials, or by adding the details, or by utilizing the special materials on clothes. Therefore, the trial and error following at the clothes manufacture can be reduced. But the elevation of the rendering speed, the price down, the strengthening of personal security, and etc. are required.

Validation on the Application of Bluetooth-based Inertial Measurement Unit for Wireless Gait Analysis (무선 보행 분석을 위한 블루투스 기반 관성 측정 장치의 활용 타당성 분석)

  • The purpose of this paper is to review the validation on the application of low frequency IMU(Inertial Measurement Unit) sensors by replacing high frequency motion analysis systems. Using an infrared-based 3D motion analysis system and IMU sensors (22 Hz) simultaneously, the gait cycle and knee flexion angle were measured. And the accuracy of each gait parameter was compared according to the statistical analysis method. The Bland-Altman plot analysis method was used to verify whether proper accuracy can be obtained when extracting gait parameters with low frequency sensors. As a result of the study, the use of the new gait assessment system was able to identify adequate accuracy in the measurement of cadence and stance phase. In addition, if the number of gait cycles is increased and the results of body anthropometric measurements are reflected in the gait analysis algorithm, is expected to improve accuracy in step length, walking speed, and range of motion measurements. The suggested gait assessment system is expected to make gait analysis more convenient. Furthermore, it will provide patients more accurate assessment and customized rehabilitation program through the quantitative data driven results.

A Study on Gait Analysis of Normal Adult and Hemiplegia Patients (정상 성인과 편마비 환자의 보행분석 연구)

  • The aim of this study is to present the basic reference data of age and specipic gait parameters for Hemiplegia Patients.
