전뇌조사의 체적변조회전치료 시 두피선량 감소에 관한 평가 (Evaluation of the reduced scalp dose at Volumetric Modulated Arc Therapy(VMAT))

  • 전이성 뇌종양의 증가에 따라 전뇌 방사선치료가 증가하게 되었다. 그리고 치료기법의 발전은 삶의 질 향상의 중요성을 증가시키게 되었다. 체적변조회전 전뇌조사는 용적별 선량을 차등시킬 수 있는 우수한 치료기법이다. 이에 두피선량 증가에 따른 탈모에 대하여 기존 전뇌조사와 체적변조회전 전뇌조사를 비교하고자 하였다. 두정부 인체모형을 이용하여 치료계획을 비교하였다. 뇌 실질의 경우 정합지수, 동질성지수, 범위의 질지수를 적용하였으며, 두피, 안구, 수정체, 경추의 경우 20%선량의 용적, 50%선량의 용적을 적용하여 비교하였다. 비교결과 뇌 실질은 기존 전뇌조사가 10%정도 우수하였지만, 두피, 안구, 수정체, 경추의 경우는 체적변조회전 전뇌조사가 1000%정도 우수하였다. 향후 체적변조회전 전뇌조사의 처방선량을 조정한다면 탈모를 방지하는 전이성 뇌종양 방사선치료로 선정될 것이다.

Dosimetric Comparison of Three-Dimensional Conformal, Intensity-Modulated Radiotherapy, Volumetric Modulated Arc Therapy, and Dynamic Conformal Arc Therapy Techniques in Prophylactic Cranial Irradiation

  • Purpose: This study aimed to dosimetrically compare the technique of three-dimensional conformal radiotherapy (3D CRT), which is a traditional prophylactic cranial irradiation method, and the intensity-modulated radiotherapy (IMRT) and volumetric modulated arc therapy (VMAT) techniques used in the last few decades with the dynamic conformal arc therapy (DCAT) technique. Methods: The 3D CRT, VMAT, IMRT, and DCAT plans were prepared with 25 Gy in 10 fractions in a Monaco planning system. The target volume and the critical organ doses were compared. A comparison of the body V2, V5, and V10 doses, monitor unit (MU), and beam on-time values was also performed. Results: In planned target volume of the brain (PTVBrain), the highest D99 dose value (P<0.001) and the most homogeneous (P=0.049) dose distribution according to the heterogeneity index were obtained using the VMAT technique. In contrast, the lowest values were obtained using the 3D CRT technique in the body V2, V5, and V10 doses. The MU values were the lowest when DCAT (P=0.001) was used. These values were 0.34% (P=0.256) lower with the 3D CRT technique, 66% (P=0.001) lower with IMRT, and 72% (P=0.001) lower with VMAT. The beam on-time values were the lowest with the 3D CRT planning (P<0.001), 3.8% (P=0.008) lower than DCAT, 65% (P=0.001) lower than VMAT planning, and 76% (P=0.001) lower than IMRT planning. Conclusions: Without sacrificing the homogeneous dose distribution and the critical organ doses in IMRTs, three to four times less treatment time, less low-dose volume, less leakage radiation, and less radiation scattering could be achieved when the DCAT technique is used similar to conventional methods. In short, DCAT, which is applicable in small target volumes, can also be successfully planned in large target volumes, such as the whole-brain.

손상위험장기에 인접한 불규칙한 모양의 타겟 치료 시, 용적변조회전 방사선치료와 비동일평면상의 빔을 이용한 세기변조 방사선치료의 유용성 평가 및 비교 (Comparison of Volumetric Modulated Arc Therapy and Non-coplanar Fixed-field Intensity Modulated Radiation Therapy for Irregular Target adjacent to Organ At Risk)

  • 목 적: 본 연구는 모양이 불규칙하고 손상위험장기(Organ At Risk, OAR)에 매우 인접한 타겟(Target)의 방사선 치료 시, 비동일평면상(Non-coplanar)의 빔(Beam)을 이용한 Fixed-field 세기변조 방사선치료(Intensity Modulated Radiation Therapy, IMRT)와 용적변조회전 방사선치료(Volumetric Modulated Arc Therapy, VMAT) 치료계획의 유용성을 평가 및 비교해 보고자 하였다. 대상 및 방법: 본원에서 True Beam STX(Varian Medical Systems, USA)를 이용하여 전체 두피(Whole Scalp), 부분적 두피(Partial Scalp), 그리고 전체 뇌실(Whole Ventricle)에 방사선 치료를 받은 환자 중 각 부위별로 2명 씩, 총 6명을 대상으로 하였다. VMAT 치료계획 시, Beamlet에 포함되는 OAR의 용적을 최소화하기 위해 Coplanar 또는 Non-coplanar 빔을 이용하였고, Fixed-field IMRT는 6명 모두 2개 이상의 카우치(Couch) 각도를 이용한 Non-coplanar IMRT(이하 ncIMRT)로 치료계획 하였다. 결 과: 양측 수정체, 양측 시신경, 시신경 교차, 그리고 뇌 줄기의 최대선량과 양측 안구와 해마의 평균선량을 측정하였고, 6명 모두 9개의 OAR 중 6개 이상에서 VMAT 치료계획이 ncIMRT보다 1.1배에서 8.2배가량 높은 선량 값을 나타내었다. 전체 두피와 부분적 두피의 치료 시, 20 Gy가 조사되는 뇌의 용적은 VMAT이 ncIMRT의 2배 이상이었고, 전체 뇌실의 치료 시에는 두 치료계획이 큰 차이를 보이지 않았다. 타겟 Coverage는 두 치료계획 모두 $PTV_{100%}=95%$를 만족시켰고, 타겟 내 최대선량과 치료 시 필요한 총 Monitor Unit(MU)은 ncIMRT가 VMAT보다 높았으며 두 치료계획 모두 임상적용을 위한 Gamma test 시행 결과, 2 mm/2 % 조건을 통과하였다. 결 론: 본 연구 결과, ncIMRT는 VMAT에 비해 치료 시 필요한 MU가 높아 치료시간과 장비의 로딩(Loading)을 증가시키므로 치료의 효율성은 다소 떨어지지만, VMAT과 비슷한 타겟 Coverage를 유지하면서, OAR의 선량은 훨씬 감소시키는 것을 알 수 있었다. 따라서 모양이 불규칙하고 OAR에 매우 인접해 있는 타겟의 방사선 치료 시에는 ncIMRT 치료계획을 고려해 볼 필요가 있다고 사료된다.

Volumetric modulated arc therapy for carotid sparing in the management of early glottic cancer

  • Purpose: Radiotherapy of the neck is known to cause carotid artery stenosis. We compared the carotid artery dose received between volumetric modulated arc therapy (VMAT) and conventional fixed-field intensity-modulated radiotherapy (IMRT) plans in patients with early glottic cancer. Materials and Methods: Twenty-one early glottic cancer patients who previously underwent definitive radiotherapy were selected for this study. For each patient, double arc VMAT, 8-field IMRT, 3-dimensional conformal radiotherapy (3DCRT), and lateral parallel-opposed photon field radiotherapy (LPRT) plans were created. The 3DCRT plan was generated using lateral parallel-opposed photon fields plus an anterior photon field. VMAT and IMRT treatment plan optimization was performed under standardized conditions to obtain adequate target volume coverage and spare the carotid artery. Dose-volume specifications for the VMAT, IMRT, 3DCRT, and LPRT plans were calculated with radiotherapy planning system. Monitor units (MUs) and delivery time were measured to evaluate treatment efficiency. Results: Target volume coverage and homogeneity results were comparable between VMAT and IMRT; however, VMAT was superior to IMRT for carotid artery dose sparing. The mean dose to the carotid arteries in double arc VMAT was reduced by 6.8% compared to fixed-field IMRT (p < 0.001). The MUs for VMAT and IMRT were not significantly different (p = 0.089). VMAT allowed an approximately two-fold reduction in treatment delivery time in comparison to IMRT (3 to 5 minutes vs. 5 to 10 minutes). Conclusion: VMAT resulted in a lower carotid artery dose compared to conventional fixed-field IMRT, and maintained good target coverage in patients with early glottic cancer.

Dosimetric Evaluation of Low-Dose Spillage Volumes for Head and Neck Cancer Using Intensity-Modulated Radiation Therapy and Volumetric Modulated Arc Therapy Treatment Techniques

  • Purpose: This study was designed to investigate the dosimetric difference between intensity-modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT) in head and neck cancer (HNC). The study primarily focuses on low-dose spillage evaluation between these two techniques. Methods: This retrospective study involved 45 patients with HNC. The treatment plans were generated using the IMRT and VMAT techniques for all patients. Dosimetric comparisons were performed in terms of target coverage, organ-at-risk (OAR) sparing, and various parameters, including conformity index, uniformity index, homogeneity index, conformation number, low-dose volumes, and normal tissue integral dose (NTID). Results: No significant (P>0.05) difference in planning target volume coverage (D95%) was observed between IMRT and VMAT plans for supraglottic larynx, hard palate, and tongue cancers. A decrease in dose volumes ranging from 1 Gy to 30 Gy was observed for VMAT plans compared with those for IMRT plans, except for V1Gy and V30Gy for supraglottic larynx cancer and V1Gy for tongue cancer. Moreover, decreases (P<0.05) in NTID were observed for VMAT plans compared with that for IMRT plans in supraglottic larynx (4.50%), hard palate (12.80%), and tongue (7.76%) cancers. In contrast, a slight increase in monitor units for VMAT compared with those for IMRT in supraglottic larynx (0.46%), hard palate (2.54%), and tongue (7.56%) cancers. Conclusions: For advanced-stage HNC, both IMRT and VMAT offer satisfactory clinical plans. VMAT offers a conformal and homogeneous dose distribution with comparable OAR sparing and higher dose falloff outside the target volume than IMRT, which provides an edge to reduce the risk of secondary malignancies for HNC over IMRT.

Segmental Analysis Trial of Volumetric Modulated Arc Therapy for Quality Assurance of Linear Accelerator

  • Purpose: Segmental analysis of volumetric modulated arc therapy (VMAT) is not clinically used for compositional error source evaluation. Instead, dose verification is routinely used for plan-specific quality assurance (QA). While this approach identifies the resultant error, it does not specify which machine parameter was responsible for the error. In this research study, we adopted an approach for the segmental analysis of VMAT as a part of machine QA of linear accelerator (LINAC). Methods: Two portal dose QA plans were generated for VMAT QA: a) for full arc and b) for the arc, which was segmented in 12 subsegments. We investigated the multileaf collimator (MLC) position and dosimetric accuracy in the full and segmented arc delivery schemes. A MATLAB program was used to calculate the MLC position error from the data in the dynalog file. The Gamma passing rate (GPR) and the measured to planned dose difference (DD) in each pixel of the electronic portal imaging device was the measurement for dosimetric accuracy. The eclipse treatment planning system and a MATLAB program were used to calculate the dosimetric accuracy. Results: The maximum root-mean-square error of the MLC positions were <1 mm. The GPR was within the range of 98%-99.7% and was similar in both types of VMAT delivery. In general, the DD was <5 calibration units in both full arcs. A similar DD distribution was found for continuous arc and segmented arcs sums. Exceedingly high DD were not observed in any of the arc segment delivery schemes. The LINAC performance was acceptable regarding the execution of the VMAT QA plan. Conclusions: The segmental analysis proposed in this study is expected to be useful for the prediction of the delivery of the VMAT in relation to the gantry angle. We thus recommend the use of segmental analysis of VMAT as part of the regular QA.

하지 연부조직육종을 위한 방사선치료기술 별 선량평가 연구 (A dosimetric evaluation of volumetric modulated arc therapy, intensity modulated radiation therapy, and three-dimensional conformal radiation therapy for the lower extremity soft tissue sarcoma)

  • 본원에서 수술 후 방사선치료를 수행한 하지 연부조직육종 환자에 대해 용적세기조절회전치료(VMAT), 세기조절방사선치료(IMRT), 3차원입체조영방사선치료(3D-CRT)의 세 가지 치료계획을 수립하여 치료계획용적(PTV)과 대퇴골에 전달되는 방사선량을 각각 비교하였다. 세 치료계획방법 모두 치료계획용적에 전달되는 방사선량은 큰 차이를 보이지 않았으나 용적세기조절회전치료와 세기조절방사선치료의 경우 3차원입체조영방사선치료보다 대퇴골에 전달되는 방사선량을 줄일 수 있었다. 또한 용적세기조절회전치료의 경우 세기조절방사선치료 보다 치료에 필요한 monitor unit이 적어 치료시간 감소의 효과가 나타났다. 따라서 하지 연부조직육종 방사선치료 시 용적세기조절회전치료의 사용은 효과적인 방사선치료를 수행할 수 있을 것이라 사료된다.

VMAT 기법을 활용한 HA-WBRT에서 빔 배열 변화에 따른 선량학적 인자 비교: 팬텀 연구 (Comparison of Dosimetric Parameters Based on Beam Arrangement Changes in HA-WBRT by Using VMAT Technique: A Phantom Study)

  • This study aimed to evaluate the impact of changes in beam arrangement and arc counts on dosimetric factors in volumetric modulated arc therapy (VMAT) inverse radiation therapy planning for hippocampal-avoidance whole brain radiation therapy (HA-WBRT) by using the Elekta Monaco radiation therapy planning system (RTPs). For coplanar VMAT, both the arc per beam (APB) method and the beam determined arc(BDA) method, which is determined by the number of beams, were applied. For non-coplanar VMAT, the BDA method was utilized, and a total of 9 treatment plans were established by varying the arc counts. All radiation therapy plans met the radiation oncology group (RTOG) 0933 protocol standards, and 14 dosimetric factors were compared and analyzed. The results showed that the BDA-NC VMAT method demonstrated superior performance in terms of planning target volume (PTV) coverage and protection of normal organs, while APB-VMAT was advantageous in terms of hippocampal protection, monitor unit and delivery time. This study is expected to contribute to the efficient establishment of HA-WBRT plans considering the changes in beam arrangement and rotation arc numbers in Monaco RTPs.

식도암의 방사선치료에서 부분 각도에 의한 회전 치료를 이용한 조사체적의 감소 (Decrease of Irradiated Volume using Rotational Treatment by Avoidance Sector in Radiation Therapy for Esophageal Cancer)

  • 세기변조방사선치료와 입체적세기조절회전치료 시 조사체적과 선량 퍼짐 현상을 줄이기 위한 방법으로 조사 각도를 제한하는 부분 각도에 의한 회전치료 기능을 적용하여 표적체적과 주변 정상장기의 선량에 대해 입체조형방사선치료와 비교 분석하였다. 치료계획에 따른 표적체적의 선량분포는 통계적으로 유의한 차이를 확인할 수 없었으며, 폐의 5 Gy(V5) 체적에서 입체조형방사선치료 56.53%, 세기변조방사선치료 52.03%, 입체적세기조절회전치료 47.84%를 나타내어 유의한 차이를 나타내었다(CRT-IMRT p=0.035, CRT-VMAT p<0.001, IMRT-VMAT p<0.001). 10 Gy 체적(V10)에서는 입체조형방사선치료 35.12%, 세기변조방사선치료 34.04%, 입체적세기조절회전치료 33.28%를 보여, 입체조형방사선치료와 세기변조방사선치료(p=0.018), 입체적세기조절회전치료(p=0.035)에서 유의한 차이를 나타내었으나 20 Gy 체적(V20)에서는 유의한 선량 차이를 확인할 수 없었다. 심장의 평균선량과 20 Gy 체적은 치료계획에 따라 유의한 차이를 확인할 수 없었으나, 30, 40 Gy 체적은 입체적세기조절회전치료에서 37.16%, 22.46%를 나타내어 입체조형방사선치료와 비교에서 유의한 차이(p=0.028)를 보였다. 이와 같이 조사체적 감소에 따른 폐의 저 선량 체적(V5, V10)의 감소를 확인할 수 있었으며, 세기변조방사선치료와 입체적세기조절회전치료 시 조사 각도를 일부 제한함으로써 표적체적의 선량분포는 동일하게 유지함과 동시에 조사체적을 줄일 수 있었다. 이로 인해 폐의 선량 퍼짐 현상의 감소로부터 폐의 독성을 낮추는데 기여할 수 있을 것으로 사료된다.

Gamma Evaluation with Portal Dosimetry for Volumetric Modulated Arc Therapy and Intensity-Modulated Radiation Therapy

  • The aim of this study is to investigate the characteristics of portal dosimetry in comparison with the MapCHECK2 measurments. In this study, a total of 65 treatment plans including both volumetric modulated arc therapy (VMAT) and intensity-modulated radiation therapy (IMRT) were retrospectively selected and analyzed (45 VMAT plans and 20 IMRT plans). A total of 4 types of linac models (VitalBeam, Trilogy, Clinac 21EXS, and Clianc iX) were used for the comparison between portal dosimetry and the MapCHECK2 measurements. The VMAT plans were delivered with two VitalBeam linacs (VitalBeam1 and VitalBeam2) and one Trilogy while the IMRT plans were delivered with one Clinac 21EXS and one Clinacl iX. The global gamma passing rates of portal dosimetry and the MapCHECK2 measurements were analyzed with a gamma criterion of 3%/3 mm for IMRT while those were analyzed with a gamma criterion of 2%/2 mm for VMAT. Spearman's correlation coefficients (r) were calculated between the gamma passing rates of portal dosimetry and those of the MapCHECK2 measurements. For VMAT, the gamma passing rates of portal dosimetry with the VitalBeam1, VitalBeam2, and Trilogy were $97.3%{\pm}3.5%$, $97.1%{\pm}3.4%$, and $97.5%{\pm}1.9%$, respectively. Those of the MapCHECK2 measurements were $96.8%{\pm}2.5%$, $96.3%{\pm}2.7%$, and $97.4%{\pm}1.3%$, respectively. For IMRT, the gamma passing rates of portal dosimetry with Clinac 21EXS and Clinac iX were $99.7%{\pm}0.3%$ and $99.8%{\pm}0.2%$, respectively. Those of the MapCHECK2 measurements were $96.5%{\pm}3.3%$ and $97.7%{\pm}3.2%$, respectively. Except for the result with the Trilogy, no correlations were observed between the gamma passing rates of portal dosimetry and those of the MapCHECK2 measurements. Therefore, both the MapCHECK2 measurements and portal dosimetry can be used as an alternative to each other for patient-specific QA for both IMRT and VMAT.
