Factors Affecting Blood Loss During Thoracoscopic Esophagectomy for Esophageal Carcinoma

  • Background: Major intraoperative hemorrhage reportedly predicts unfavorable survival outcomes following surgical resection for esophageal carcinoma (EC). However, the factors predicting the amount of blood lost during thoracoscopic esophagectomy have yet to be sufficiently studied. We sought to identify risk factors for excessive blood loss during video-assisted thoracoscopic surgery (VATS) for EC. Methods: Using simple and multiple linear regression models, we performed retrospective analyses of the associations between clinicopathological/surgical factors and estimated hemorrhagic volume in 168 consecutive patients who underwent VATS-type esophagectomy for EC. Results: The median blood loss amount was 225 mL (interquartile range, 126-380 mL). Abdominal laparotomy (p<0.001), thoracic duct resection (p=0.014), and division of the azygos arch (p<0.001) were significantly related to high volumes of blood loss. Body mass index and operative duration, as continuous variables, were also correlated positively with blood loss volume in simple linear regression. The multiple linear regression analysis identified prolonged operative duration (p<0.001), open laparotomy approach (p=0.003), azygos arch division (p=0.005), and high body mass index (p=0.014) as independent predictors of higher hemorrhage amounts during VATS esophagectomy. Conclusion: As well as body mass index, operation-related factors such as operative duration, open laparotomy, and division of the azygos arch were independently predictive of estimated blood loss during VATS esophagectomy for EC. Laparoscopic abdominal procedures and azygos arch preservation might be minimally invasive options that would potentially reduce intraoperative hemorrhage, although oncological radicality remains an important consideration.

무가압함침법에 의한 $Al_2O_{3p}$/AC8A 복합재료의 제조 및 특성 (Fabrication and Characteristics of $Al_2O_{3p}$/AC8A Composites by Pressureless Infiltration Process)

  • 무가압함침법에 의한 $Al_2O_{3p}$/AC8A 복합재료의 제조와, 제조법과 관련하여 부가적인 Mg의 첨가와 강화상의 부피분율이 $Al_2O_{3p}$/AC8A복합재료의 기계적 성질과 마모저항에 미치는 영향을 조사하였다. 강화상 입자와 기지재료의 일부를 분말로 조합한 혼합분말 속으로 기지금속을 자발적으로 침투시켜 부피분율이 20~40%인 $Al_2O_{3p}$/AC8A 복합재료를 제조할 수 있었다. 그러나 강화상의 부피분율이 40%인 복합재료의 경우 기공율의 상승으로 복합재료의 강도는 저하하였다. Mg의 첨가량이 5~7wt% 일 때 가장 높은 강도를 나타냈으며, 경도는 Mg 첨가량의 증가에 따라 점진적으로 상승하였다. $Al_2O_{3p}$/AC8A복합재료는 저속에서 기지재료에 비해 내마모성이 저하하였으나, 고속에서는 AC8A합금에 비해 약 5.5배의 우수한 내마모성을 나타냈다. 마모기구의 관찰에 의해 부피분율 20% $Al_2O_{3p}$/AC8A복합재료의 경우 연삭마모가 주된 마모기구임을 알 수 있었으며, 부피분율 40% $Al_2O_{3p}$/AC8A복합재료는 높은 기공율로 인한 마모 가중으로 저속에서도 경미한 응착마모가 관찰됐고 마찰 속도가 증가함에 따라 격심한 마모로 진행되었다.

지하 열저장 공동의 종횡비와 저장용량에 따른 열성층화 및 열손실 (Thermal Stratification and Heat Loss in Underground Thermal Storage Caverns with Different Aspect Ratios and Storage Volumes)

  • 열저장소 내 열성층화는 에너지저장 시스템의 효율을 향상시키고 수요 발생시 더 많은 유효에너지를 공급하기 위해 필수적인 기술이다. 일반적으로 저장소의 종횡비(폭에 대한 높이의 비)와 크기에 따라 열성층도가 달라지는 것으로 알려져 있다. 본 논문은 열수 저장을 위한 암반공동의 종횡비와 저장용량이 저장공동 내 열성층화와 외부로의 열손실에 미치는 영향을 조사하는 데 연구 목적이 있다. 이를 위해 전산유체역학 코드인 FLUENT를 이용하여 암반공동의 종횡비와 저장용량에 따른 열전달 시뮬레이션을 수행하였다. 성층도 정량화 지수를 이용하여 시간경과에 따른 열성층화의 변화를 분석하였으며, 저장공동 외부로의 열손실을 평가하였다. 분석 결과, 종횡비가 증가함에 따라 공동 내 열성층화가 향상되는 경향을 보였으나, 종횡비 3-4 이상부터는 이러한 영향이 크지 않은 것으로 분석되었다. 저장용량이 작은 암반공동에 비해 용량이 큰 암반공동에서 상대적으로 긴 시간 동안 열성층화가 높게 유지되는 것으로 분석되었으나, 종횡비 증가에 따라 저장용량이 다른 공동들간의 성층화 차이가 줄어드는 경향을 나타냈다. 암반공동의 종횡비가 커질수록 공동의 표면적이 늘어나 종횡비의 증가에 따라 주변 암반으로의 열손실이 증가하는 경향을 보였으며, 단위 저장용량을 줄여 소규모 다중공동을 적용하는 경우, 총 저장용량이 동일한 단일공동에 비해 전체 열손실량이 증가하는 것으로 분석되었다.

Asymmetrical Volume Loss in Hippocampal Subfield During the Early Stages of Alzheimer Disease: A Cross Sectional Study

  • Hippocampal atrophy is a well-established imaging biomarker of Alzheimer disease (AD). However, hippocampus is a non-homogenous structure with cytoarchitecturally and functionally distinct sub-regions or subfield, with each region performing distinct functions. Certain regions of the subfield have shown selective vulnerability to AD. Here, we are interested in studying the effects of normal aging and mild cognitive impairment on these sub-regional volumes. With a reliable automated segmentation technique, we segmented these subregions of the hippocampus in 101 cognitively normal (CN), 135 early mild cognitive impairment (EMCI), 67 late mild cognitive impairment (LMCI) and 48 AD subjects. Thereby, dividing the hippocampus into hippocampal tail (tail), subiculum (SUB), cornu ammonis 1 (CA1), hippocampal fissure (fissure), presubiculum (PSUB), parasubiculum (ParaSUB), molecular layer (ML), granule cells/molecular layer/dentate gyrus (GCMLDG), cornu ammonis 3(CA3), cornu ammonis 4(CA4), fimbria and hippocampal-amygdala transition area (HATA). In this cross sectional study of 351 ADNI subjects, no differences in terms of age, gender, and years of education were observed among the groups. Though, the groups had statistically significant differences (p < 0.05 after the multiple comparison correction) in the Mini-Mental State Examination (MMSE) scores. There was asymmetrical volume loss in the early stages of AD with the left hemisphere showing volume loss in regions that were unaffected in the right hemisphere. Bilateral parasubiculum, right cornu ammonis 1, 3 and 4, right fimbria and right HATA regions did not show any volume loss till the late MCI stages. Our findings suggest that the hippocampal subfield regions are selectively vulnerable to AD and also that these vulnerabilities are asymmetrical especially during the early stages of AD.

쉴드 TBM 터널의 gap parameter와 뒤채움재를 고려한 수치모델링 방법에 대한 연구 (A study on numerical modeling method considering gap parameter and backfill grouting of the shield TBM tunnel)

  • 쉴드 TBM 터널에 대한 2차원 수치해석 시 기존 연구에서는 뒤채움재 및 실제적인 내공변위 분포를 적절히 고려하지 않았다. 따라서 본 연구에서는 이를 보완하고자 gap parameter 적용 시 굴착면에서 발생하는 실제적인 내공변위분포를 고려하고, 적정한 뒤채움재 물성치를 적용한 모델링 방법을 제시하였다. 이를 위해 단일층 풍화토 지반을 대상으로 gap parameter와 토피고가 굴착면의 지반손실량과 지표손실량으로 구성되는 체적손실량에 미치는 영향을 분석하였다. 그 결과 대부분의 지표침하가 굴착 직후에 발생하였으며, 토피고 및 gap parameter가 증가할수록 수치해석을 통해 얻은 지표침하곡선의 영향범위가 제안된 이론식보다 넓게 나타났다. 따라서 2차원 수치해석 시 쉴드 TBM 터널의 거동을 정확히 평가하기 위해서는 하중분담률을 이용하여 gap parameter를 적용하고, 뒤채움재의 물성치도 적절히 고려해야 한다고 판단된다.

삽입재 크기에 따른 코니컬 커터의 현장 내구성 평가 연구 (Durability evaluation depending on the insert size of conical Picks by the field test)

  • 본 연구는 로터리드럼커터에 적용된 두 종류의 코니컬 픽커터에 대한 현장시험을 통해 시험 전과 후의 육안조사와 중량감소 및 마모부피를 측정하여 코니컬 픽커터의 내구성을 비교 분석하였다. 육안조사를 통해 슬림 타입 코니컬 픽커터에서만 총 9개의 삽입재 손실이 나타남을 확인하였고 삽입재 보호를 위해 두부의 두께가 중요함을 알 수 있었다. 시험 전과 후의 픽커터의 중량손실과 마모부피는 헤비타입이 슬림타입에 비해 반이하로 작게 나타나 경암반에서 헤비타입이 더 유용함을 확인할 수 있었다. 픽커터의 마모손실을 판단할 때, 중량측정과 마모부피 측정결과의 상호 비교는 재료의 단위중량과 굴착에 의한 스폴링에 의해 차이가 나타날 수 있으므로 주의해야 할 것으로 판단된다.

Analysis of pile group behaviour to adjacent tunnelling considering ground reinforcement conditions with assessment of stability of superstructures

  • Tunnel construction activity, conducted mainly in mountains and within urban centres, causes soil settlement, thus requiring the relevant management of slopes and structures as well as evaluations of risk and stability. Accordingly, in this study we performed a three-dimensional finite element analysis to examine the behaviour of piles and pile cap stability when a tunnel passes near the bottom of the foundation of a pile group connected by a pile cap. We examined the results via numerical analysis considering different conditions for reinforcement of the ground between the tunnel and the pile foundation. The numerical analysis assessed the angular distortion of the pile cap, pile settlement, axial force, shear stress, relative displacement, and volume loss due to tunnel excavation, and pile cap stability was evaluated based on Son and Cording's evaluation criterion for damage to adjacent structures. The pile located closest to the tunnel under the condition of no ground reinforcement exhibited pile head settlement approximately 70% greater than that of the pile located farthest from the tunnel under the condition of greatest ground reinforcement. Additionally, pile head settlement was greatest when the largest volume loss occurred, being approximately 18% greater than pile head settlement under the condition having the smallest volume loss. This paper closely examines the main factors influencing the behaviour of a pile group connected by a pile cap for three ground reinforcement conditions and presents an evaluation of pile cap stability.

준설토의 유실율 평가방법에 관한 연구 (A Study on Estimation of Loss Rate of Hydraulic Fills)

  • Recently, the hydraulic fill method is commonly used in many reclamation projects due to lack of fill materials. The method of hydraulic fill in reclamation is executed by transporting the mixture of water-soil particles into a reclaimed land through dredging pipes, then the dredged soil particles settle down in the water or flow over an out flow weir with the water. In the present study, practice each three method in order to suggest method of determining the loss rate of the dredged fills. The first sieve and hydrometer analysis were performed with the soil samples obtained before and after dredging and then apply theory of particle breakage, the second compare with the volume of dredged soil between at the dredging area and the target pond and the last compare with weight of dredged soil between before and after dredging at the dredging area and in the target pond for estimating the amount of soil particles residual at the reclaimed area and the loss of soil particles passed through the weir. In addition to compare with the loss ratio between as using Marsal's modified theory of particle breakage and measured weight and volume in the field.

동충하초 첨가식빵의 저장기간에 따른 이화학적. 텍스쳐 특성 (Physicochemical and Texture of Bread added Paecilomyces japonica according to Storage Period)

  • For a period of 5 days, the quality of breads with Paecilomyces japonica powder were investigated using chemical, sensory, and mechanical evaluations at $25^{\circ}C$. The result of analysis of chemical properties revealed that the pH value of dough was increased as the amount of Paecilomyces japonica powder increased, but volume of dough and baking loss rate were decreased. Breads with 1% and 2% Paecilomyces japonica powder showed a good overall preference in sensory evaluation. As storing time passed. moisture content, lightness, and redness were decreased in all breads, but yellowness increased. Hardness and gumminess of texture analysis were increased as storing time passed, but springiness decreased. Springiness of the mechanical properties was negatively correlated with pH value of dough and volume of bread, while was positively correlated with volume of dough and baking loss rate. In the analysis of correlation between sensory and mechanical properties showed that hardness of mechanical properties had negative correlation with softness, moistness, and springiness. Cohesiveness of mechanical properties had positive correlation with acceptability.

홍삼 분말을 첨가한 식빵의 품질특성 (Quality Characteristics of White pan bread with Led Ginseng powder)

  • This study investigated properties of bread utilizing extracts of ginseng powder. Ginseng powder bread was baked. Addition of 1~3% of ginseng powder extract to wheat flour was carried out. Rheological properties, dough pH, dough volume, bread volume, water absorption capacity, baking loss, bread color, bread texture, and sensory evaluation were tested to examine properties of bread baked with extracts of ginseng powder. The results are as follows. The dough pH decreased gradually with increasing ginseng powder extract concentration. The bread volume, baking loss, and bread weight increased with increasing use of ginseng powder extract, and springiness and cohesiveness increased as red ginseng powder additive concentration increased. Lightness of the L decreased while a and b increased. In the sensory evaluation, ginseng powder with 1% ginseng powder extract was evaluated as the best for taste, texture, flavor, and overall acceptability.
