Storm-Water CSOs for Reservoir System Designs in Urban Area (도시유역 저류형 시스템 설계를 위한 CSOs 산정)

  • Combined sewer overflows(CSOs) are themselves a significant source of water pollution. Therefore, the control of urban drainage for CSOs reduction and receiving water quality protection is needed. Examples in combined sewer systems include downstream storage facilities that detain runoff during periods of high flow and allow the detained water to be conveyed by an interceptor sewer to a centralized treatment plant during periods of low flow. The design of such facilities as stormwater detention storage is highly dependant on the temporal variability of storage capacity available(which is influenced by the duration of interevent dry periods) as well as the infiltration capacity of soil and recovery of depression storage. As a result, a contiunous approach is required to adequately size such facilities. This study for the continuous long-term analysis of urban dranage system used analytical Probabilistic model based on derived probability distribution theory. As an alternative to the modeling of urban drainage system for planning or screening level analysis of runoff control alternatives, this model have evolved that offer much ease and flexibility in terms of computation while considering long-term meteorology. This study presented rainfall and runoff characteristics or the subject area using analytical Probabilistic model. Runoff characteristics manifasted the unique characteristics of the subject area with the infiltration capacity of soil and recovery of depression storage and was examined appropriately by sensitivity analysis. This study presented the average annual COSs and number of COSs when the interceptor capacity is in the range 3xDWF(dry weather flow). Also, calculated the average annual mass of pollutant lost in CSOs using Event Mean Concentration. Finally, this study presented a dicision of storage volume for CSOs reduction and water quality protection.

Intervention Analysis of Urbanization Effect on Rainfall Data at the Seoul Rain Gauge Station (서울지점 강우자료에 나타난 도시화의 간섭 분석)

  • This study estimated the urbanization effect of Seoul, the largest city in Korea, on its rainfall. For a comparative analysis, two different data sets are used: One is the precipitation data at the Jeonju rain gauge station, which has a relatively long record length but least urbanization effect, and the other at the Ichon rain gauge station, which has a short record length but located very near to Seoul with least urbanization effect. Also, the difference of the rainfall between Seoul and Jeonju rain gauge stations, as an indicator of urbanization effect, is quantified by use of the intervention model. As a result, it was found that the maximum rainfall intensity of the annual maximum rainfall events shows the increasing trend, its duration the decreasing trend, and the mean intensity the decreasing trend especially after 1960. Also, the quantification of urbanization effect using the intervention model shows that the increasing trend of rainfall intensity and total volume is still on going.

Risk Factors Associated with Frequent Hospital Readmissions for Exacerbation of COPD

  • Background: Chronic obstructive pulmonary disease (COPD) is one of the leading causes of disability and mortality worldwide. The aim of this study was to evaluate the risk factors associated with recurrent hospital admissions for exacerbation of COPD in Korea. Methods: A retrospective study of 77 consecutive patients hospitalized for exacerbation of COPD at Pusan National University Hospital during the time period January 2005 to May 2008 was performed. The information was collected from the hospitalization period: clinical information, spirometric measures, and laboratory variables. In addition, socioeconomic characteristics, co-morbidity, anxiety, and depression were reviewed. Frequent readmission was defined as 2 or more hospitalizations in the year following discharge. Results: During the 1-year period after discharge, 42 patients (54.6%) reported one hospital admission and 35 patients (45.4%) reported 2 or more hospital readmissions. Among the 35 frequent readmission patients, 4 had more than 10 readmissions. Univariate analysis showed that a body mass index (BMI) <$18.5kg/m^2$, duration >36 months, forced expiratory volume in 1 second ($FEV_1$) <50% predicted, arterial $CO_2$ partial pressure ($PaCO_2$) >40 mm Hg, and arterial oxygen saturation ($SaO_2$) <95% at discharge were associated significantly with frequent readmissions. The multivariate analysis revealed that BMI <$18.5kg/m^2$, $PaCO_2$ >40 mm Hg at discharge were independently associated with frequent readmissions for exacerbation of COPD. Conclusion: Frequent readmissions for exacerbation of COPD were associated with low BMI and hypercapnia at discharge.

  • Since most of soil slopes are in an unsaturated state, it is necessary to consider the unsaturated characteristics of soil slopes, in order to obtain more reasonable results. Therefore in this study we supplemented a slope stability analysis program to consider them, based on the concept of limit equilibrium. We also applied an optimization technique to search for a failure surface. Besides, we carried out experiments to obtain the unsaturated soil properties required in the analysis with weathered granite soils. We formulated a nonlinear apparent cohesion relationship with the matrix suction to be able to apply the unsaturated shear strength characteristics to the stability analysis. In addition, we intended to obtain more accurate soil water characteristic curves(SWCC) by measuring the change in volume of the specimen in the SWCC tests. As a result, we could appropriately assess the change of the safety factor according to the rainfall intensity and duration, by considering the variation of suction, permeability, and shear strength caused by the infiltration of rainfall into slopes.

Correlation among Serum and Urinary Calcium, Zinc, Magnesium and Other Factors in Non-Insulin Dependent Diabetic Women (인슐린 비의존형 당뇨병 여성환자의 혈청과 뇨중 칼슘, 아연 및 마그네슘 함량과 관련인자들과의 상관관계)

  • Nineteen non-insulin-dependent diabetic(NIDD) and healthy control women were investigated to study the relationship between glycemic control and the level of calcium, zinc, and magnesium in the serum and urine. Urinary calcium, zinc and magnesium levels in the NIDD women were significantly higher(p<0.01) than those of the control women. There were no difference in serum magnesium and zinc levels between the two groups, but se겨m calcium level was lower(p<0.01) in the NIDD women compared to that of the control group. In the NIDD women, serum magnesium was negatively related to fasting blood glucose(r=-0.533 : p<0.05), urinary glucose(r=-0.767 ; P<0.001), urinary protein(r=-0.476 : p<0.05), and urine volume(r=-0.571 : p<0.05). The levels of zinc in both serum (r=0.515, p<0.05) and urine(r=0.623 : p<0.01) were related to urinary protein but only urinary zinc level(r=0.570 : p<0.01) was related to serum albumin. Urinary magnesium, not calcium was correlated with the urinary glucose(r=0.563 : p<0.05) and urinary protein(r=0.568 ; p<0.05). Fasting blood glucose was positively correlated with duration of diabetes, as well as dietary fat and calorie intake. The results of this study suggest that NIDD alters all magnesium, zinc, and calcium utilization, particularly magnesium is involved in glycemic control in this condition.

Comparison of Percutaneous Endoscopic Lumbar Discectomy and Open Lumbar Microdiscectomy for Recurrent Disc Herniation

  • Objective : The purpose of this study was to compare clinical and radiological outcomes of percutaneous endoscopic lumbar discectomy (PELD) and open lumbar microdiscectomy (OLM) for recurrent disc herniation. Methods : Fifty-four patients, who underwent surgery, either PELD (25 patients) or repeated OLM (29 patients), due to recurrent disc herniation at L4-5 level, were divided into two groups according to the surgical methods. Excluded were patients with sequestrated disc, calcified disc, severe neurological deficit, or instability. Clinical outcomes were assessed using Visual Analogue Scale (VAS) score and Oswestry Disability Index (ODI). Radiological variables were assessed using plain radiography and/or magnetic resonance imaging. Results : Mean operating time and hospital stay were significantly shorter in PELD group (45.8 minutes and 0.9 day, respectively) than OLM group (73.8 minutes and 3.8 days, respectively) (p < 0.001). Complications occurred in 4% in PELD group and 10.3% in OLM group in the perioperative period. At a mean follow-up duration of 34.2 months, the mean improvements of back pain, leg pain, and functional improvement were 4.0, 5.5, and 40.9% for PELD group and 2.3, 5.1, and 45.0% for OLM group, respectively. Second recurrence occurred in 4% after PELD and 10.3% after OLM. Disc height did not change after PELD, but significantly decreased after OLM (p = 0.0001). Neither sagittal rotation angle nor volume of multifidus muscle changed significantly in both groups. Conclusion : Both PELD and repeated OLM showed favorable outcomes for recurrent disc herniation, but PELD had advantages in terms of shorter operating time, hospital stay, and disc height preservation.

Surgical Outcomes of Pneumatic Compression Using Carbon Dioxide Gas in Thoracoscopic Diaphragmatic Plication

  • Background: Surgical correction needs to be considered when diaphragm eventration leads to impaired ventilation and respiratory muscle fatigue. Plication to sufficiently tense the diaphragm by VATS is not as easy to achieve as plication by open surgery. We used pneumatic compression with carbon dioxide ($CO_2$) gas in thoracoscopic diaphragmatic plication and evaluated feasibility and efficacy. Methods: Eleven patients underwent thoracoscopic diaphragmatic plication between January 2008 and December 2013 in Pusan National University Hospital. Medical records were retrospectively reviewed, and compared between the group using $CO_2$ gas and group without using $CO_2$ gas, for operative time, plication technique, duration of hospital stay, postoperative chest tube drainage, pulmonary spirometry, dyspnea score pre- and postoperation, and postoperative recurrence. Results: The improvement of forced expiratory volume at 1 second in the group using $CO_2$ gas and the group not using $CO_2$ gas was $22.46{\pm}11.27$ and $21.08{\pm}5.39$ (p=0.84). The improvement of forced vital capacity 3 months after surgery was $16.74{\pm}10.18$ (with $CO_2$) and $15.6{\pm}0.89$ (without $CO_2$) (p=0.03). During follow-up ($17{\pm}17$ months), there was no dehiscence in plication site and relapse. No complications or hospital mortalities occurred. Conclusion: Thoracoscopic plication under single lung ventilation using $CO_2$ insufflation could be an effective, safe option to flatten the diaphragm.

Feasibility and Safety of Single-Port Video-Assisted Thoracic Surgery for Primary Lung Cancer

  • Background: The feasibility of single-port video-assisted thoracic surgery (SPVATS) for primary lung cancer is not well understood. In this study, we compared SP and multi-port (MP) VATS for the surgical treatment of patients with primary lung cancer. Methods: Surgical treatment was performed in 181 patients with primary lung cancer at Inje University Haeundae Paik Hospital between June 2012 and December 2015. A propensity-matched analysis was used to compare the postoperative outcomes and to evaluate the comparative feasibility and safety of SPVATS and MPVATS. Results: There were 37 patients in the SPVATS group and 67 patients in the MPVATS group. Propensity matching produced 32 pairs. The operation time (210 minutes versus 200 minutes, p=0.11), volume of the estimated blood loss (170 mL versus 160 mL, p=0.19), duration of chest tube drainage (5 days versus 6 days, p=0.66), and length of hospital stay (9 days versus 10 days, p=0.89) were similar between the 2 groups. Conclusion: In our study, SPVATS for primary lung cancer was safe and feasible in well selected patients. A prospective, randomized study with a large group and long-term follow-up is necessary to evaluate the clinical feasibility and the advantages of SPVATS for primary lung cancer.

Food Habits, Eating Behaviors and Food Frequency by Gender and among Seoul and Other Regions in Upper-Grade Elementary School Children (서울 및 지방에 거주하는 초등학교 고학년 남녀 아동들의 식습관, 식행동 및 식품섭취빈도)

  • This study was conducted to investigate the differences in food habits, eating behaviors and food frequency by gender and regions in upper grade elementary school children in Korea. Subjects of this study were 1,211 children, 645 boys and 566 girls, attending 6 elementary schools (4th, 5th and 6th graders). 389 children lived in Seoul and 822 children lived in other regions. Mean height, weight and BMI of subjects were 142.4 cm, 38.7 kg and $18.8\;kg/m^2$ respectively. 70.5% of the subjects had breakfast every day and 12.4% of the subjects skipped breakfast. There were significant differences between boys and girls in causes of skipping breakfast, overeating, meal volume, and duration of meals. Girls showed more desirable eating behavior than boys (p < 0.001). Scores for specific eating behaviors such as unbalanced diet and intake of meats and vegetables were better in girls (p < 0.001), and boys ate meals faster than girls (p > 0.001). There were significant differences between Seoul and other regions in eating behavior. Eating behavior scores of children in other regions were higher than in Seoul (p < 0.01). Children in Seoul had a higher frequency of watching television with meals. Frequency of fruits (p < 0.001), milk products (p < 0.01), and meats (p < 0.05) of children in Seoul were higher than other regions. Otherwise the frequency of simple sugar products of children in other regions was higher than Seoul (p < 0.001). Frequency of green vegetables was higher for girls than boys, and frequency of fast foods was higher for boys than girls. These results showed that in nutrition education programs designed to improve the nutritional health of elementary school children, we need to focus more on differences in the children's food habits and eating behaviors by gender and regions. In consideration of these differences, such educational programs need to have a different emphasis for each gender and regions to serve its specific needs.

Iodine Supplementation of Leucaena leucocephala Diet for Goats. II. Effects on Blood Metabolites and Thyroid Hormones

  • Twelve adult male goats, comprising of six castrated and six intacts, (2.5-3 years; $24.4{\pm}0.62kg$) were randomly but evenly divided into two groups ($I_0$ and $I_{100}$) and fed conventional concentrate mixture along with Leucaena leucocephala leaf meal (100 g/head approx.), the latter to supply 50 per cent of the crude protein (CP) requirements. The $I_{100}$ group was provided with supplemental iodine as potassium iodide solution at 0.1 mg/day/animal. Wheat straw was provided ad libitum as sole source of roughage during the experimental period of 105 d. Blood samples were collected at the begining (0 d) and thereafter at 30, 60 and 90 d of experimental feeding. The study revealed that the serum glucose level was significantly higher (p<0.01) in $I_{100}$ group as compared to $I_0$. Haemoglobin, packed cell volume and serum concentrations of total protein, albumin, globulin, calcium, inorganic phosphorus and alkaline phosphatase did not show significant differences as a result of iodine supplementation. Though the serum levels of triiodothyronine ($T_3$) were comparable between the two groups, that of thyroxine ($T_4$) increased significantly (p<0.001) in the $I_{100}$ group. The $T_3:T_4$ ratio was also similar between both the groups. The study indicated that the adverse effect of Leucaena feeding on thyroid gland could possibly be alleviated by provision of extra iodine. However, this needs further confirmation using long duration studies.
