Septated Extradural Arachnoid Cyst in Thoracolumbar Spine Causing Myelopathy

  • Spinal extradural arachnoid cyst is uncommon and rarely cause neural compression. We report a rare case of severe cord compression due to septated spinal extradural arachnoid cyst. A 35-year-old woman has developed back pain 3 months prior to her visit, but recently motor weakness and urinary incontinence occurred. Magnetic resonance images showed an extradural cyst posterior to the cord, which was flattened and displaced from T12 to L2. Urgent decompressive laminectomy and cyst removal was performed. Histopathological examination confirmed that cyst wall was formed by nonspecific fibrous connective tissue without a single-cell layer of inner arachnoid lining. Motor weakness and voiding difficulty were recovered completely after operation.

  • The present review report introduces the existing analysis codes and physical modeling of two-phase flow associated with initiating event of HCDA in Liquid Metal Reactors for the effective study in the future, because the related research has not been systematically carried out in Korea compared with other areas. The description in this report is specifically addressed to the results yielded from careful review of the technical concepts on the two-phase flow modeling in the SAS2A code which was developed in ANL. The report is prepared in 2 parts based on the definite physical phenomena. The liquid slug and gas behavior models are main representations in the part (I) and (II), respectively. In this regard, it is expected that this report provide a fundamental knowledge on the two-phase flow model in LMR and, thus, contribute to establishment of the necessary HCDA analysis technology concerned with the LMR development in Korea.

  • In this paper, a nonlinear controller based on adaptive sliding-mode method which has a sliding surface vector including new boundary function is proposed and applied to a two-wheeled voiding mobile robot (WMR). This controller makes the welding point of WMR achieve tracking a reference point which is moving on a smooth curved welding path with a desired constant velocity. The mobile robot is considered in view of a kinematic model and a dynamic model in Cartesian coordinates. The proposed controller can overcome uncertainties and external disturbances by adaptive sliding-mode technique. To design the controller, the tracking error vector is defined, and then the new sliding is proposed to guarantee that the error vector converges to zero asymptotically. The stability of the dynamic system will be shown through the Lyapunov method. The simulations is shown to prove the effectiveness of the proposed controller.

  • Urinary disturbance is deficit in volitional control of voiding. Stroke can cause temporary or permanent urinary disturbance. The purpose of this study was to confirm the effect of Jasin-hwan on urinary disturbance as heat syndrome. This study was carried out on 3 patients with urinary disturbance caused by stroke. We measured components of IPSS (international prostate symptom score) at pre-treatment and post-treatment. After prescribing medication to the patients, symptoms improved rapidly within three weeks. These results support that Jasin-hwan is effective in reduction of urinary disturbance as heat syndrome.

  • Transsacral neurolytic block with 2.5ml of phenol in glycerine or bupivacaine was performed in 6 patients with malignant diseases and a patient with sphincter spasm of bladder due to spinal cord injury. Pain relief was satisfactory in all patints except one patient with very low pain threshold. In one patient, second transsacral neurolytic block alone was not sufficient because of widespread pain along distant metastasis of the malignant disease, although the first block was satisfactory. The complications include transient motor weakness(4), voiding difficulty(1), subarachnoid puncture(1), and epidural venous puncture(1), but they were all spontaneously recovered within a sbort period of time and did not give any limitation to the block.

Pressure Wave Propagation in the Discharge Piping with Water Pool

  • Pressure wave propagation in the discharge piping with a sparger submerged in a water pool, following the opening of a safety relief valve, is analyzed. To predict the pressure transient behavior, a RELAP5/MOD3 code is used. The applicability of the RELAP5 code and the adequacy of the present modeling scheme are confirmed by simulating the applicable experiment on a water hammer with voiding. As a base case, the modeling scheme was used to calculate the wave propagation inside a vertical pipe with sparger holes and submerged within a water pool. In addition, the effects on wave propagation of geometric factors, such as the loss coefficient, the pipe configuration, and the subdivision of sparger pipe, are investigated. The effects of inflow conditions, such as water slug inflow and the slow opening of a safety relief valve are also examined.

  • The safety level of Sodium Fast Reactors is directly related with the sodium void reactivity. A low-void effect design has been proposed within the Horizon2020 ESFR-SMART project thanks to the introduction of a sodium plenum above the active core. In order to assess the impact of this core conception on transient analysis, a map with the spatial distribution of sodium void worth can be computed and fed into a point-kinetics-based transient code. Due to the spatial correlations between neighboring zones, the global effect of voiding two different axial or radial regions is not necessarily the sum of both individual contributions. Neglecting those correlations in the void worth map and consequently in the transient analysis may lead to an unrealistic prediction of the transient sequences. In this work, a method based on sensitivity analysis and similarity assessment is proposed for predicting those correlations. The method proved to be able to establish correlations between axial slices of a sub-assembly and was checked against realistic sodium void propagation patterns.

  • The aim of this study was to implement the system that could evaluate the function of urinary tract with noninvasive and comfort methods. There for in this study hardware device and computer software have been developed that enables the analysis of the urine flow rate and vesical pressure signal measured during voiding. These signals were recorded simultaneously and transmitted to the PC. For the measurement system evaluation, the model system for the lower urinary system of men was designed. From the evaluation of the model system, vesical pressure was correlated with the occlusion degree. In a pilot study on 5 male subjects, means of standard deviation was 1.06, average error rate was 2.09 and coefficient variation was 2.09.

  • A case of primary hyperparathyroidism due to parathyroid adenoma is presented. A 14 year-old male was admitted to the hospital comptaining of voiding difficulty. The intravenous pyelogram demonstrated a stone in the proximal one third of the left ureter and marked hydronephrosis of the left kidney. The Tc-99m sestamibi nuclear scan demonstrated a hot spot below the lower pole of the left lobe of the thyroid. Laboratory study demonstrated hypercalcemia (12.4 mg/dL) and elevated parathyroid hormone (143.67 pg/mL). A parathyroid gland located below the lower pole of the left lobe of the thyroid was excised. A parathyroid adenoma, consisting of mainly chief cells was found on pathologic examination. Postoperatively the patient had transient hypocalcemic symptoms, which resolved with administration of calcium preparation and vitamin D.
