Clinical Application of Botulinum Toxin to Functional Dysphonia (기능성 음성장애에서 보툴리늄 독소의 임상적 적용)

  • Functional dysphonia (FD) is a disease entity which includes various voice disorders in the absence of structural or neurologic laryngeal pathology. Muscle tension dysphonia (MTD), psychogenic dysphonia are representative FD with completely different pathogenesis. Therefore there is no standard treatment modality for FD, the first step of treatment of FD is differentiating patient's voice symptoms from other organic voice disorders and other functional voice problems. MTD is a functional voice disorder caused by hyperfunction of intrinsic and extrinsic laryngeal musculature. Symptoms include increased vocal effort, roughness, fatigue and odynophonia. First line for MTD is indirect or direct voice therapy. Unfortunately, many patients with MTD improve with voice therapy alone. For these patients, various modalities tried; lidocaine application, surgical excision of the false vocal folds, and botulinum toxin injection, etc. Botulinum toxin injections are widely used in the field of otolaryngology, especially for spasmodic dysphonia. However, its use in FD or MTD has only been described in few case reports. The aim of this lecture is to evaluate the feasibility of botulinum toxin injection for FD, especially MTD.

The Effect of Voice Disorders on Quality of Life(QOL) in the Korean (한국인의 음성질환이 삶의 질에 미치는 영향)

  • Background and Objectives : Quality of life(QOL) is a construct representing physical, mental and social well-being. QOL has been used as a device for measuring the severity of health-related condition and treatment outcomes. As the social welfare system develops, the attention to QOL increases as well. The aims of this study was to examine whether the patients with voice disorder perceived significantly more the effects of voice disorder on QOL than nonpatient group did and if any, identify the sociodemographic risk factors influencing QOL of patients. Materials and Methods : This study asked 113 adults with voice disorders who were enrolled in Voice Clinic in the Department of Otolaryngology, Kangbuk Samsung Hospital between lune 1998 and January 1999 and 111 nonpatients to complete a questionnaire designed to elicit information about the effete of voice disorders on quality of lift. The questionnaire included items concerning sociodemographic areas, voice symptoms, job, effects of voice disorders on QOL domains(work, social, psychological, physical, and communication areas), potential risk factors to exposures, familial and medical history of voice disorders. Results : The sociodemographic characteristics of the patient group are as follows : (1) 75.2% of total patient group were female and the rest were male. (2) Age of total patient group ranged from 20 to 65 years. Hoarseness was the most commonly reported complaints, followed by complaints of high note difficulties during singing and voice fatigue. The patient group perceived effects of voice disorders on the areas of work, social, psychological, physical and communication more adversely than the comparison group did (p<0.05). QOL impairments were evaluated as a function of age, gender, education, and income, controlling other independent effects. The results were that (1) age was significantly associated with work problems and (2) gender and income were significantly associated with psychological problems. Conclusions : The findings indicated that the patients with voice disorders would perceive markedly adverse effect on all QOL domains, that is, work, social, psychological, physical, communicational areas. Therefore, the results of study suggest that lurker investigations about the nature of voice disorders, the prevention, treatment, and coping strategies are needed in the future.

  • Incomplete glottic closure of vocal cord atrophy is the common cause of dysphonia. Patients with vocal cord atrophy have complaints such as dysphonia, vocal fatigue, abnormal sensation in the throat, laryngeal pain, cough or sputum like functional voice disorders. Many investigators could not confirm the pathologic laryngeal structure because of their minute pathology. But recent advancements of laryngeal examinations made the many clinicians to detect minimal laryngeal pathology and to have mind the treatment for the vocal cord atrophy. But the results were less effective than their thoughts, the reasons of ineffectiveness were not known well. Authors have found the Hyperfunctional movement of the supraglottis during phonation before and after thyroplasty type I for vocal cord atrophy. Then we have applied the combined modality treatment with thyroplaty type I and voice therapy for relieve of hypefunctional movement of the supraglottis. These options have had more imporved results.

Study on Male Chronic Fatigue in Hyungsang Medicine (남자 만성피로의 형상의학적 고찰)

  • Chronic fatigue refers to the tiredness which disturbs one's daily activities and lasts more than six months without the sign of improvement by rest or sleep. Male chronic fatigue is included in internal injuries due to overstrain and exhaustion and closely related to river, spleen, stomach and kidney out of five Jang organs. Male chronic fatigue is based on the deficiency of Yang and aggravated by liquor, sex and emotional depression. Chronic fatigue has the following features in Hyungsang ; an inclination for lying: unwillingness to everything; drooping eyes and feeble voice; yellow pale complexion; withered ears; redness on the tip of nose; cheekbone, and lips; blood shot eyes; dryness and loss of hair. Male chronic fatigue usually attacks the persons of horse type and bird type because they are hyperactive compared with what they eat. The following medicines are commonly prescribed for the treatment of male chronic fatigue. Ssangwhatang, Sokunzungtang, and Yanghyulsamultang are commonly prescribed to horse type. Jaumkangwhatang, Chungsimyunzaum, Yookmijiwhangtang to bird type. Palmiwhan, Gozinumja. and Shinkihwan to fish type. Bozungikkitang, Galwhahaesungtang, Samchulgunbitang and Sipjundaebotang to turtle type.

  • Accidents or disease can lead to acquired voice dysphonia. In this case, we propose a new input interface based on eye movements to facilitate communication for patients. Unlike the existing method that presents the English alphabet as it is, we reorganized the layout of the alphabet to support the Korean alphabet and designed it so that patients can enter words by themselves using only eye movements, gaze, and blinking. The proposed interface not only reduces fatigue by minimizing eye movements, but also allows for easy and quick input through an intuitive arrangement. For natural communication, we also implemented a system that allows patients who are unable to speak to communicate with their own voice. The system works by tracking eye movements to record what the patient is trying to say, then using Glow-TTS and Multi-band MelGAN to reconstruct their own voice using the learned voice to output sound.

Mobile Voice Web Browser for the Low Vision (저시력자를 위한 모바일 보이스 웹 브라우저 개발)

  • The web has become indispensable in all of our daily lives. We communicate, study and get information with others through the web. This behavior also continues in the smart phone environment. The biggest problem is that the small display screen of a smart phone degrades the accuracy in selecting or manipulating content for people with low vision. To compensate for this, voice guidance services that combine touch and voice, such as VoiceOver and Talkback, are currently provided to smart phone devices. However, restrictions arise in GUI, TTS control problems, and content expansion and selection. In addition, unnecessary content is also output by voice, which causes fatigue for low vision people to use. In this study, we propose a mobile web browser interface that selects and enlarges a desired area from web browsers and contents, or outputs it as a voice so that people with low vision can easily use the mobile web browser. In this paper, we propose a context selective focusing function that enables selection for each element of web content. In addition, we intend to develop a mobile voice web browser that can enlarge the selected content or output it by voice.

Acoustic Characteristics of the Vocal Major Students with Vocal Nodule (성대결절이 있는 성악도에서의 음성분석학적 특징)

  • Objectives : The purpose of this study was to systematically analyze and compare the acoustic characteristics of vocal major students with vocal nodule when singing. Materials and Methods : Ten sopranos with vocal nodule, who have never been treated due to voice problems, were the subject group. Twenty healthy sopranos major students were the control group for this study. The vocal nodule was confirmed by stroboscopy and the acoustic and electroglottographic analysis was conducted on be groups. Additionally, an inquiry on usual voice problems during speaking and singing were performed on each of the groups. Results : The vowel /a/ was spoken and sung by sopranos of each group. There were no significant differences in Fo, jitter, shimmer, NHR at speaking and singing in either group. We did not register any significant differences of closed quotient and speed quotient in electroglottography for both groups at singing, but speed quotient was increased in subject group at speaking(p<0.05). And the sopranos with nodule demonstrated voice fatigue(70%), strained and tired voice after long period of performance and conversation(60%) on inquiry. Conclusions : Although they had a vocal nodule, this group, who had a long period of voice training, did not present any differences in acoustic and electroglottographic problems during short performance, but, the vocalists noticed delicate voice problems after a long performance. We conclude that the voice training teacher and laryngologist must initiate voice management and speech therapy for vocal nodule, even if there is no specific abnormality in acoustic analysis.

  • Users who using computer may experience fatigue or sickness on their wrists if they use the keyboard and mouse for a long time. People with physical disabilities will find it difficult to work with the keyboard and mouse. There is a problem in that the substitute product for solving this is limited in function or expensive. In this paper, we development a system for controlling a personal computer with voice commands using the Amazon Echo and Amazon Web Services lambda functions. The implemented system processes the user's voice commands from the Amazon web server and sends them to the personal computer. The personal computer processes the received command and uses it to operate the application program.

Design of Metaverse for Two-Way Video Conferencing Platform Based on Virtual Reality

  • As non-face-to-face activities have become commonplace, online video conferencing platforms have become popular collaboration tools. However, existing video conferencing platforms have a structure in which one side unilaterally exchanges information, potentially increase the fatigue of meeting participants. In this study, we designed a video conferencing platform utilizing virtual reality (VR), a metaverse technology, to enable various interactions. A virtual conferencing space and realistic VR video conferencing content authoring tool support system were designed using Meta's Oculus Quest 2 hardware, the Unity engine, and 3D Max software. With the Photon software development kit, voice recognition was designed to perform automatic text translation with the Watson application programming interface, allowing the online video conferencing participants to communicate smoothly even if using different languages. It is expected that the proposed video conferencing platform will enable conference participants to interact and improve their work efficiency.

  • Voice is a physical phenomenon, generated by vocal fold and expiratory airflow. Dysphonia should come from abnormal vocal fold and airflow. Occassionally larynx looks normal in show, but it is actually not. There should be undetected structural or functional abnormalities. So when ENT doctors face dysphonia patients who looks normal larynx, should make a diagnosis through close observation. In this review article we present some dysphonia diseases which looks normal larynx. For example vocal fatigue, vocal fold paresis, posterior glottic diastasis, muscle tension dysphonia and psychogenic dysphonia.
