일측 성대에 동시 발생한 다른 두 종류의 성대 낭종 1례 (Simultaneously Presented Two Histopathologically Different Cysts in Unilateral Vocal Fold : A Case Report)

  • A 66-year-old woman described a 6-month history of hoarseness after upper respiratory infection. She was a house wife and referred from an outside clinic under a diagnosis of vocal fold nodules. Strobovideolaryngoscopy revealed bilateral vocal fold lesions, and decreased mucosal wave of both vocal folds. She was brought to the operating room for microsuspension laryngoscopy. Under general anesthesia, dual intracordal cysts on left vocal fold were completely resected with microflap technique. The lesion on the right vocal fold turned out to be a reactive fibrous mass, which was also resected. Dual intracordal cysts were confirmed histopathologically. The one was an epidermoid cyst lined with squamous epithelium, and the other was a mucus retention cyst lined with cuboidal epithelium. Postoperative voice was acceptable by the patient and the mucosal vibration has much improved after the surgery.

연축성 발성장애와 음성 진전 환자의 감별 (Differences in Patient Characteristics between Spasmodic Dysphonia and Vocal Tremor)

  • Spasmodic dysphonia, essential tremor, and vocal tremor related with Parkinson's disease are different disorders showing fairly similar symptoms such as difficulty in the speech onset, and tremble in the voice. However, the cause and the resulting treatment of these diseases are different. Spasmodic dysphonia is a vocal disorder characterized by spasms of the laryngeal muscles during a speech, invoking broken, tense, forced, and strangled voice patterns. Such difficult-to-treat dysphonia disease is classified as central-origin-focal dystonia, of a yet unknown etiology. Its symptoms arise because of intermittent and involuntary muscle contractions during speech. Essential tremor, on the other hand, is characterized by a rhythmic laryngeal movement, resulting in alterations of rhythmic pitch and loudness during speech or even at rest. Severe cases of tremor may cause speech breaks like those of adductor spasmodic dysphonia. In the case of hyper-functional tension of vocal folds and accompanying tremors, it is necessary to distinguish these disorders from muscular dysfunction. A diversified assessment through the performance of specific speech tasks and a thorough understanding for the identification of the disorder is necessary for accurate diagnosis and effective treatment of patients with vocal tremors.

양성성대질환의 웃음 음성치료(SKMVTT) (Efficacy of laughing voice treatment (SKMVTT) in benign vocal fold lesions)

  • The purpose of this study was to evaluate the efficacy of a multiple voice therapy technique ($SKMVTT^{(R)}$) using laughter for the treatment of various benign vocal fold lesions. To achieve this, 23 female patients diagnosed with vocal nodules, vocal polyp, and muscle tension dysphonia through videostroboscopy were enrolled in vocal hygiene and $SKMVTT^{(R)}$. All of the patients were treated once a week for 4 to 12 sessions. The GRBAS scale was used to confirm the changes in voice quality before and after the treatment. Acoustic analysis was performed to evaluate jitter, shimmer, NHR, fundamental frequency variation, amplitude variation, PFR, and dB range. Videostroboscopy was performed to confirm the changes in the laryngeal features before and after the treatment. After the $SKMVTT^{(R)}$, the results of the perceptual evaluation demonstrated that the G, R, and B scales significantly improved. An acoustic evaluation also demonstrated that jitter, shimmer, NHR, vAm, vFo, PFR, and dB range also significantly improved after the $SKMVTT^{(R)}$. In comparison to the videostroboscopic findings, the size of the vocal nodules and vocal polyp decreased or disappeared after the treatment. In addition, the size of the cuneiform tubercles decreased, the length of the aryepiglottic folds became longer, and the laryngeal findings of the supraglottic compressions improved after the $SKMVTT^{(R)}$. These results suggest that the $SKMVTT^{(R)}$ is effective in improving the vocal quality of patients with benign vocal fold lesions. In conclusion, it seems that laughter and inspiratory phonation suppressed abnormal laryngeal elevation and lowered laryngeal height, which seems to have the effect of improving hyperfunctional phonation.

외래에서 시행하는 경윤상갑상막 접근 성대주입술 중 주입 위치를 정확하게 파악하는 방법에 관한 기술적 고찰 (Technical Review of How to Determine the Exact Location of Needle Tip During Office-Based Injection Augmentation of the Vocal Folds Via Cricothyroid Approach)

  • The office-based, un-sedated vocal fold injection laryngoplasty has re-emerged in the past decade as an appealing alternative to microsuspension laryngoscopic procedure which is conducted under general anesthesia. The trend toward vocal fold injection laryngoplasty in an in-office setting was possible due to technological evolution for visualization and new injection materials. However, invisibility of the injection needle has been the main shortcomings of cricothyroid approach. The purpose of this review is to provide an up-to-date review of office-based, trans-cricothyroid membrane approach injection laryngoplasty technique under local anesthesia and efforts made to increase the preciseness of amount and location of the injected materials in the management of glottic insufficiency. A review of variable efforts undertaken to maximize the result of cricothyroid approach by technically increasing accuracy of the location of needle tip was done. With the proper patient selection and utilization of the new technologies, office-based and un-sedated vocal fold injection laryngoplasty via cricothyroid approach can be redeem as a main-stay in the management of glottic insufficiency, such as vocal fold paralysis, vocal fold paresis, vocal fold atrophy and vocal fold scar.

말단 비대증 환자에서 발생한 양측성대마비 1예 (A Case of Bilateral Vocal Fold Paralysis from Acromegaly)

  • Acromegalic patients can develop mild upper airway obstruction. However, the limitation of both vocal folds mobility developing dyspnea is rare. We report a case with bilateral vocal cord paralysis associated with acromegaly. The patient visited our clinic presenting dyspnea showing bilateral vocal cord hypomobility in laryngoscopy. The patient underwent a tracheostomy and a transsphenoidal resection of the pituitary adenoma. Thereafter, laser cordotomy with medial arytenoidectomy was done for the permanent treatment of glottal obstruction. The tracheotomy canula was successfully removed one month after the surgery.

노인애성환자에 대한 임상적연구 (Clinical Study of Aged Patients with Hoarseness)

  • The voice of aged persons is known generally to be somewhat different from that of other adults, suggesting that laryngeal change occurs with advancing age. However, because knowledge of the voice characteristics of aged persons is limited, it is difficult to judge whether their voices arc normal. Chart review and laryngoscopic examination from ninety-one patients with hoarseness over the age of 60(1st group) and one hundred sixteen patients with hoarseness below the age of 50(2nd group) were done to define aging related voice disorders. The following results were obtained. 1) Associated diseases related to laryngeal disease were hypertension(12%), pulmonary disease(4.4%), thyroid disease(1.1%) in 1st group and hypertension(9.5%), thyroid disease(1.7%) in 2nd group. 2) The underlying diseases causing hoarseness in order of frequency were benign vocal fold lesion(37.7%), inflammatory disease(36.8%), functional dysphonia(17%) in 1st group and benign vocal fold lesion(43.6%), functional dysphonia(26.3%), inflammatory disease(16.5%) in 2nd group. 3) In stroboscopic findings, atrophy and sulcus of vocal cords are more prevalent in males than in females and edema of vocal cords is more common in females. Generally the voice characteristics of aged persons depend on the mass of the vocal folds which may be decreased through atrophy or be increased by edema. However, other factors such as systemic diseases, drug side effects and compensatory mechanism to presbylaryngis must be taken into account in diagnosing and treating voice disorders in aged persons.

Prediction of Closed Quotient During Vocal Phonation using GRU-type Neural Network with Audio Signals

  • Closed quotient (CQ) represents the time ratio for which the vocal folds remain in contact during voice production. Because analyzing CQ values serves as an important reference point in vocal training for professional singers, these values have been measured mechanically or electrically by either inverse filtering of airflows captured by a circumferentially vented mask or post-processing of electroglottography waveforms. In this study, we introduced a novel algorithm to predict the CQ values only from audio signals. This has eliminated the need for mechanical or electrical measurement techniques. Our algorithm is based on a gated recurrent unit (GRU)-type neural network. To enhance the efficiency, we pre-processed an audio signal using the pitch feature extraction algorithm. Then, GRU-type neural networks were employed to extract the features. This was followed by a dense layer for the final prediction. The Results section reports the mean square error between the predicted and real CQ. It shows the capability of the proposed algorithm to predict CQ values.

노인성 음성에 대한 최신 연구동향 (Current Researches on Vocal Fold Aging)

  • Aging causes a variety of changes in the structure of the vocal fold (VF), resulting in aging-induced dysphonia (presbyphonia). Several studies have investigated the structure of the VF of elderly people from autopsy as well as animal studies. There is an increasing evidence on correlation of structural changes of VF with deteriorated voice in elderly. Although the cellular mechanisms of aging VF have only partially been elucidated, there are many recent advances in biological treatment on aging VF using bioactive molecules such as growth factors. In this study, I'd like to address aging-related structural, biological, and physiological changes in previous literature about human and rodent aging VFs, to provide further insight into the mechanisms responsible for presbyphonia and to translate the basic researches for future clinical trials.

성대에 발생한 신경섬유종 1례 (A Case of Neurofibroma of the Vocal Cord)

  • Solitary neurofibroma of the glottis is extremely rare and accounts for only 0.1-1.5% of benign laryngeal tumors. Aryepiglottic fold is the most frequent involved site followed by arytenoids and ventricular folds. There have been few reports of neurofibroma of the true vocal cord. We report a case of neurofibroma which was deeply embedded in the vocal cord and misdiagnosed as muscle tension dysphonia with a review of literatures.
