Spontaneously Regressed Rathke's Cleft Cyst

  • We report two rare cases of spontaneously regressed Rathke's cleft cyst (RCC). A 52-year-old woman presented with headache. A pituitary hormone study was normal. Brain magnetic resonance imaging (MRI) showed a $0.45-cm^3$ cystic sellar lesion. The cyst was hyperintense on T1-weighed imaging and hypointense on T2-weighted imaging without rim enhancement, comparable to a RCC. Six months later, brain MRI showed no change in the cyst size. Without any medical treatments, brain MRI 1 year later revealed a spontaneous decrease in cyst volume to $0.05cm^3$. A 34-year-old woman presented with headache and galactorrhea lasting 1 week. At the time of the visit, the patient's headache had disappeared. Her initial serum prolactin level was 81.1 ng/mL, and after 1 week without the cold medicine, the serum prolactin level normalized to 11.28 ng/mL. Brain MRI showed a RCC measuring $0.71cm^3$. Without further treatments, brain computed tomography 6 months later showed a spontaneous decrease in cyst volume to $0.07cm^3$. Another 6 months later, brain MRI revealed that the cyst had remained the same size. Neither patient experienced neurological symptoms, such as headache or visual disturbance, during the period of cyst reduction. The RCCs in both patients underwent spontaneous regression without any medical treatment during a period of 6 months to 1 year. Although spontaneous regression of a RCC is rare, it is still possible and a sufficient follow-up period should be considered.

A Case Report of Unspecified Arthritis Patient with Fever of Unknown Origin Treated by Korean Medicine (원인 불명 열을 동반한 상세불명의 관절염 환자 한방 치험 1례)

  • Objective: This study evaluates the effect of Korean medicine in a patient who suffered from unspecified arthritis with fever of unknown origin. Methods: A patient was treated with herbal medicine for 12 weeks. The clinical improvements were evaluated by body temperature, blood test (CBC-diffcount, CRP, ESR, etc.) and Visual Analogue Scale (VAS). Results: Improvements in body temperature, blood tests and clinical symptoms were observed after Korean medicine treatments. Conclusions: Korean medicine treatment may be an effective treatment for unspecified arthritis with fever of unknown origin.

Three Cases of Nummular Eczema Patients Treated with Cheongshimyeonja-eum Gagambang and Soyeom Pharmacopuncture (청심연자음가감방(淸心蓮子飮加減方)과 소염 약침으로 호전된 화폐상 습진 한방 치험 3례)

  • Objectives : The purpose of this study is to report the effectiveness of Cheongshimyeonja-eum gagambang and Soyeom pharmacopuncture for nummular eczema patients. Methods : This study was conducted with three patients treated with nummular eczema at Cheonan Oriental Medical hospital of Daejeon university. All three patients were treated with Cheongshimyeonja-eum gagambang and Soyeom pharmacopuncture. For the evaluation of the results, we used Visual Analogue Scale(VAS) as an assessment methods and took photographs of the lesions for comparison. Results : After the medication, eczema lesions of the three patients were remarkably improved. The lesions were examined by the photographs and the VAS of pruritus and relating symptoms, Conclusions : This study suggests that Cheongshimyeonja-eum gagambang and Soyeom pharmacopuncture could show effectiveness on nummular eczema.

Tourette Syndrome Treated with Hoeyeoksan Based on Shanghanlun Provisin 318: A Case Report (『상한론(傷寒論)』 소음병(少陰病) 318조(條)로 진단하여 회역산(回逆散) 투여 후 호전된 뚜렛장애 환자 1례 보고)

  • Objective : The purpose of this study is to report the improvement in a patient who had Tourette syndrome treated with Hoeyeoksan based on Shanghanlun provisions. Methods: The symptoms of the patient were checked on days 14, 34, 62, 82, 103, and 119 after administration of Hoeyeoksan. Tourette syndrome was evaluated using Yale Global Tic Severity Scale (YGTSS), and oversleep and constipation were evaluated using visual analog scale (VAS). In addition, new clinical implications for the interpretation of Shanghanlun provision 318 were reviewed. Results: Daily tics occurred ten times a day for 14 days after the initial diagnosis but decreased to eight times a day after 34 days, six times a day after 62 days, five times a day after 82 days, twice a day after 103 days, and once a day after 119 days after administration of Hoeyeoksan. Oversleep and constipation improved together with Tourette syndrome. Conclusions: In this study, we observed that Hoeyeoksan had a therapeutic effect by improving the chief complaint of the patient. Hence, the interpretation of Shanghanlun provision 318 must include a patient's behavioral and sleep patterns as predisposing factors of Tourette syndrome.

A Case Study of the Patient with Glossodynia Treated by Korean Medicine Treatment with Postural Yinyang Correction of Temporomandibular Joint (구강내 통증 환자에 대해 턱관절 자세 음양교정술을 병행한 한방치료 치험 1례)

  • Objectives: This study was designed to report the clinical improvement of the patient with glossodynia. Methods: During the treatment period (from Jul. 7th, 2021 to Aug. 3rd), a patient was treated using acupuncture, Korean Medicine, pharmacopuncture and FCST (Functional Cerebrospinal Therapy). Symptoms were evaluated by Visual Analogue Scale (VAS), the Oral Health Impact Profile-14 (OHIP-14). Results: After the treatments for 28 days, the OHIP-14 score decreased from 53 to 7. Also, the patient's VAS score decreased from 6 to 0. Conclusions: Korean medicine treatment with FCST may be valuable for glossodynia patients. Nevertheless further studies are needed.

  • Objectives: This study examined a clinical case of a paralytic ileus patient diagnosed with Soeumin. Methods: The patient was treated with herbal medicine (Hyangsayangwi-tang, Baekhaogunja-tang, Palmulgunja-tang), infusion solution treatments, acupuncture, moxibustion, and rehabilitative therapy. Their effects were evaluated using the visual analog scale (VAS). Results: After receiving Korean medical treatment for 38 days, the VAS score for dyspepsia was reduced from 8 to 2, the VAS score for nausea and vomiting was reduced from 8 to 2, and the VAS score for abdominal pain was reduced from 7 to less than 1. Conclusions: Korean medicine could be effective in relieving the symptoms of paralytic Ileus.

A Case Report on the Therapeutic Effect of Integrated Cancer Therapy for Glioblastoma Multiforme (교모세포종에 대한 통합암치료의 치료 효과 증례보고)

  • Objective: The purpose of this report is to present the effects of integrative cancer treatment (ICT) on a patient diagnosed with glioblastoma multiforme (GBM). Methods: A 71-year-old male GBM patient received ICT from May 14 to October 12, 2021 and concurrently received temozolomide and radiotherapy. The effect on symptoms was evaluated using a visual analog scale (VAS), and changes in tumor size were assessed using magnetic resonance imaging. Results: After treatment, the VAS score for nausea decreased from 5 to 1, and the tumor size also reduced. Conclusion: ICT could be effective in treating GBM patients by reducing the size of the tumor as well as alleviating the side effects.

Spleen Daoyin Method (脾腸導引法) for Functional Dyspepsia Patients: A Case Report (비장도인법(脾腸導引法)을 적용한 복부 팽만감 주소의 기능성소화불량 증례보고)

  • A 42-year-old patient with functional dyspepsia had suffered from early satiation and postprandial fullness for 1 year despite Western medicine treatment. We treated her with the Spleen Daoyin method, along with acupuncture, moxibustion, and herbal medicine and scored her symptoms using a 4-point Functional Dyspepsia-related Quality of Life (4p-FD-QoL) questionnaire, the Gastrointestinal Symptom Rating Scale (GSRS), and a visual analogue scale (VAS). A 7-day Spleen Daoyin treatment decreased her 4p-FD-QoL score from 61 to 51, with a marked decrease in the emotional part (18 to 10). Her GSRS score did not change, but her bloating score decreased from 3 to 1. Her VAS score decreased from 6.6 to 4.3 for upper abdomen discomfort, but lower abdomen discomfort was unchanged. The patient showed high compliance and satisfaction with exercise therapy and reported no adverse events. Spleen Dayoin may be a useful therapy for functional dyspepsia, especially when accompanied by psychological problems.

Insomnia Treated with Baekho-tang Based on the Shanghanlun Disease Pattern Identification Diagnostic System (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 백호탕(白虎湯) 투여 후 호전된 불면증 증례 1례)

  • Objective: This study aimed to report the improvement of a patient with chronic insomnia who was treated with Baekho-tang based on the Shanghanlun disease pattern identification diagnostic system (DPIDS). Methods: The patient's symptoms were checked on days 19, 37, and 52 after taking Baekho-tang. Chronic insomnia was evaluated by the degree of insomnia, hours spent asleep, and the number of times the patient woke up during the night, which were all assessed by a visual analog scale (VAS) with the Leeds Sleep Evaluation Questionnaire. In addition, new clinical implications for the interpretation of the 350th Shanghanlun provision were reviewed. Results: After administering Baekho-tang for 52 days, based on the 350th Shanghanlun provision according to the DPIDS, the VAS score decreased from 10 to 1. Conclusions: The patient recovered from chronic insomnia. This case report suggests that the words "滑," "厥," and "裏" in the 350th provision of Shanghanlun mean physical and psychological causes of insomnia. In addition, Baekho-tang had a therapeutic effect in improving the chief complaint of the patient. Therefore, the interpretation of the 350th Shanghanlun provision requires a new definition that includes the patient's lifestyle, family conflict, and past history of the patient as a cause of chronic insomnia.

Three Cases of Sudden Hearing Loss Improved after East-West Medical Combined Treatment through Cooperation in a Hospital (동일기관 내 협진을 통한 돌발성 난청의 한·양방 병용치험 3례)

  • Objectives : The purpose of this study is to report the effect of east-west medical combined treatment of sudden sensorineural hearing loss(SSNHL) through cooperation in a hospital. Methods : We treated three patients diagnosed as SSNHL by combination of herbal medicine, acupuncture, pharmoacupuncture, moxibustion, cupping, laser therapy and conventional medications. We evaluated the results of this treatment with pure tone audiometry, word recognition score(WRS), the changes in distortion product otoacoustic emissions(DPOAE) and visual analogue scale. Results : One of them showed a meaningful improvement in the hearing level of the low frequency region. The others showed 'Complete recovery' in pure tone audiometry and WRS. The subjective symptoms including tinnitus and ear fullness improved in three patients. Conclusion : This study suggests that east-west medical combined treatment is effective on SSNHL patients.
