Calcific tendinitis of the shoulder in the Korean population: demographics and its relation with coexisting rotator cuff tear

  • Background: To evaluate the demographics, clinical and radiographic features of calcific tendinitis of the shoulder in the Korean population, specifically focusing on the incidence of coexisting rotator cuff tear. Methods: Between October 2014 and January 2015, we performed a prospective multicenter study with 506 patients from 11 training hospitals in Korea. We collected data of demographics and radiographic analysis based on simple radiographs, clinical assessments based on visual analog scale (VAS) and the American Shoulder Elbow Surgeons (ASES) score, and treatment modalities that are used currently. We also evaluated coexisting rotator cuff tear by ultrasonography (US) or magnetic resonance imaging (MRI) images. Results: There were 402 female patients (79%) with mean age of 55 years (range, 31-87 years). Mean duration of symptoms was 16 months. Mean size of calcific materials was 11.4 mm (range, 0-35 mm). Mean value of VAS and ASES scores were 6.5 (range, 1-10) and 47 (range, 8-95), respectively. Of 383 patients (76%), 59 (15%) had rotator cuff tear including 15 full-thickness tears on US or MRI. Patients with rotator cuff tears were significantly associated with older age, recurrent symptoms, menstrual disorders in females, and having undergone calcification removal surgery and rotator cuff repair (all p<0.05). Conclusions: This study reported demographic, radiographic, and clinical features of calcific tendinitis of the shoulder in Korean population, which were not different from those of Western population. Coexisting rotator cuff tear was found with 15% incidence in this large series, suggesting that further radiographic study to evaluate rotator cuff tear might be needed in some calcific tendinitis patients of older age and presenting with recurrent symptoms.

  • This study aims to establish a foundation for autonomous driving on campus and communication of abundant university information in the HCI environment in a VR environment where college freshmen can freely travel around campus facilities. The purpose of this study is to develop a three-dimensional VR-style campus tour system to establish a media environment to provide abundant university information guidance services to freshmen in non-face-to-face situations. This study designed a three-dimensional virtual reality campus tour system to solve the problem of discontinuity in which VR360 filming space does not lead to space like reality, and to solve many problems of expertise in VR technology by constructing an integrated production environment of tour system. We aim to solve the problem of inefficiency, which requires a large amount of momentum in virtual space, by constructing a GUI that utilizes the motion of the field of view focus. The campus environment was designed as a three-dimensional virtual reality using a three-dimensional graphic design. In non-face-to-face situations, college freshmen freely transformed the HMD VR device, smartphone, FPS operation mode of the gyroscope sensor. The design elements of the three-dimensional virtual reality campus tour system were classified as ①Visualization of factual experiences, ②Continuity of space movement, ③Operation, automatic operation mode, ④Natural landscape animation, ⑤Animation according to wind direction, ⑥Actual space movement mode, ⑦Informatization of spatial understanding, ⑧GUI by experience environment, ⑨Text GUI by building, ⑩VR360, 3D360 Studio Environment, ⑪Three-dimensional virtual space coupling block module, ⑫3D360-3D Virtual Space Transmedia Zone, ⑬Transformable GUI(VR Device Dual Viewer-Gyro Sensor Full Viewer-FPS Operation Viewer) and an integrated production environment was established with each element. It is launched online (http://vautu.com/u1) by constructing a GUI for free driving mode and college information screens to adapt to college life for freshmen, and designing an environment that can be used simultaneously by current media such as PCs, Android, and iPads. Therefore, it conducted user research, held a development presentation, a forum on excellence in university innovation support projects, and applied it as a system on the website of a particular university. College freshmen will be able to experience university information directly from the web and app to the virtual reality campus environment.

Efficacy of glycine powder air-polishing in supportive periodontal therapy: a systematic review and meta-analysis

  • Purpose: This systematic review and meta-analysis was conducted to assess the effects of glycine powder air-polishing (GPAP) in patients during supportive periodontal therapy (SPT) compared to hand instrumentation and ultrasonic scaling. Methods: The authors searched for randomized clinical trials in 8 electronic databases for relevant studies through November 15, 2019. The eligibility criteria were as follows: population, patients with chronic periodontitis undergoing SPT; intervention and comparison, patients treated by GPAP with a standard/nozzle type jet or mechanical instrumentation; and outcomes, bleeding on probing (BOP), patient discomfort/pain (assessed by a visual analogue scale [VAS]), probing depth (PD), gingival recession (Rec), plaque index (PI), clinical attachment level (CAL), gingival epithelium score, and subgingival bacteria count. After extracting the data and assessing the risk of bias, the authors performed the meta-analysis. Results: In total, 17 studies were included in this study. The difference of means for BOP in patients who received GPAP was lower (difference of means: -8.02%; 95% confidence interval [CI], -12.10% to -3.95%; P<0.00001; I2=10%) than that in patients treated with hand instrumentation. The results of patient discomfort/pain measured by a VAS (difference of means: -1.48, 95% CI, -1.90 to -1.06; P<0.001; I2=83%) indicated that treatment with GPAP might be less painful than ultrasonic scaling. The results of PD, Rec, PI, and CAL showed that GPAP had no advantage over hand instrumentation or ultrasonic scaling. Conclusions: The findings of this study suggest that GPAP may alleviate gingival inflammation more effectively and be less painful than traditional methods, which makes it a promising alternative for dental clinical use. With regards to PD, Rec, PI, and CAL, there was insufficient evidence to support a difference among GPAP, hand instrumentation, and ultrasonic scaling. Higher-quality studies are still needed to assess the effects of GPAP.

보육교직원의 젠더감수성 교육에 대한 태도 연구 (A Study on the Childcare Teachers' Attitudes Toward Gender Sensitivity Education)

  • 본 연구는 유아를 대상으로 하는 젠더감수성 교육에 대한 보육교직원의 태도를 질적방법으로 탐색하였다. 이를 위해, 수도권 어린이집에 근무하는 보육교사와 원장을 연구참여자로 선정하고, 총 227명을 모집하였다. 자료수집은 시각방법론에 근거하여 젠더감수성 교육과 관련된 그림책을 이용한 프로토콜서술로 실시되었다. 수집된 자료의 분석은 중심주제 분석에 따라 이루어졌다. 젠더감수성 교육은 성역할 유연성과 성지향 개방성에 대한 내용으로 나눌 수 있다. 연구결과에 따르면, 연구참여자 대부분이 성역할 유연성 교육에 대해서는 긍정적 태도를 나타냈다. 연구참여자들은 성역할 유연성이란 주제가 이미 사회적으로 포용되었고, 유아의 발달에 적합하며, 보육현장에서 실행하기에 용이하다고 인식하였다. 교육의 효과도 아동의 개별성 존중에 긍정적일 것으로 기대하였다. 한편, 성지향 개방성 교육에 대해서는 연구참여자의 53.3%가 부정적 태도를 보였다. 성지향 개방성에 대한 쟁점이 아직 사회적으로 합의되지 않았고, 발달적으로 유아보다는 청소년기 이상의 연령에 적합하며, 실제로 보육현장에 적용하기에는 관련 정보가 부족하다는 점이 지적되었다. 하지만, 연구참여자들의 40.5%가 향후 성지향 개방성에 대한 사회적 포용이 이루어질 것으로 예상하며, 보육현장에 적합한 교재와 교수방법이 개발되면 결국 유아를 대상으로 하는 성지향 개방성 교육이 이루어질 것이라는 긍정적 태도를 보였다. 본 연구는 한국 사회의 쟁점으로 부각된 젠더감수성 교육에 대한 이론적 고찰과 함께 유아를 대상으로 하는 젠더감수성 교육에 대한 보육교직원들의 태도를 비판적으로 검토하여 향후 유아 젠더감수성 교육의 실천적 담론화에 이바지할 수 있을 것으로 사료된다.

야외지질답사 및 모델링 기반 순환 학습에서 학생들이 그린 그림의 목적과 기능에 대한 이해 (Understanding Purposes and Functions of Students' Drawing while on Geological Field Trips and during Modeling-Based Learning Cycle)

  • 이 연구의 목적은 학생들이 그린 그림이 야외지질답사와 모델링 기반 순환 학습에서 어떤 의미를 갖는지 질적으로 탐색하는 것이다. 서울의 한 대학 부설 영재교육원에 재학 중인 10명의 학생이 참여하였다. 한탄강 형성과정이라는 것을 주제로 야외지질답사와 3차시 모델링 3차시 수업을 진행하였다. 각 차시별 학생들이 작성했던 모든 기록장(글, 그림), 연구자 필드노트, 학생들이 참여한 모든 영상 자료 및 음성 녹음, 전사한 인터뷰 자료 등을 연구진과 공유하였다. Hatisaru (2020) 그림 표상화를 야외지질학습의 맥락에 맞게 수정하여 그림의 유형을 분류하였다. 학생들의 글(text, memo)을 포함한 그림의 특징을 분석하기 위해 연연적 내용 분석(deductive content analysis)을 사용하였다. 또한, 그림이 모델링 기반 순환 과정(자료 수집 관찰, 모델 생성, 모델 발달, 자연현상의 구체화) 속에서 어떤 역할을 하는지 분석하였다. 그 결과 학생들의 그림 유형은 지질학적인 개념을 포함한 상징적 이미지, 지형학적으로 외형을 묘사한 외형적 이미지, 학생들의 심리적인 영역을 표현한 정의적 이미지가 있었다. 특징은 설명, 생산화, 정교화, 증거, 일치, 심상(心狀)으로 분류하였다. 그림의 유형과 특징은 모델링 기반 순환 학습 과정에서 연속적으로 나타나며 학생들의 모델 발달 과정 속에서 학생들의 인지적인 영역에 관한 특성과 학업에 대한 긍정적인 태도와 감정을 반영하였다. 학생들이 그린 그림은 야외지질답사와 모델링 과정 모두에 있어서 학생들의 사고와 의사표현을 반영할 수 있는 도구로써 의미를 있음을 밝힘으로써 과학교육 관계자들에게 학생들의 그림 그리기 활동의 중요성을 역설하였다.

딥러닝을 활용한 이미지 기반 교량 구성요소 자동분류 네트워크 개발 (Image-Based Automatic Bridge Component Classification Using Deep Learning)

  • 우리나라의 교량은 대부분이 건설된 지 20년 이상이 지나 현재 노후화로 인하여 많은 문제점이 제기되고 있으며, 교량의 안전점검은 대부분 전문 인력의 주관적인 평가로 이루어지고 있다. 최근 교량 안전점검의 데이터의 체계적인 관리를 위해 BIM 등을 활용한 데이터 기반의 유지관리 기술들이 개발되고 있지만, BIM과 구조물의 유지관리 데이터를 연동을 위해서 영상정보를 직접 라벨링하는 수작업을 필요로한다. 따라서 본 논문에서는 이미지 기반의 자동 교량 구성요소 분류 네트워크를 개발하고자 한다. 본 연구에서 제안한 방법은 두 개의 CNN 네트워크로 구성되었다. 첫 번째 네트워크에서 특정 교량 이미지에 대하여 교량의 형식을 자동으로 분류한 뒤, 두 번째 네트워크에서 교량의 형식별로 구성요소를 분류함으로써 정확도와 효율성을 향상시키고자 한다. 본 연구에서 개발한 시스템을 검증한 결과, 847개의 교량 이미지에 대해서 98.1 %의 정확도로 교량의 구성요소를 자동으로 분류 할 수 있었다. 본 연구에서 개발한 교량의 구성요소 자동분류 기술은 향후 교량의 유지관리에 기여를 할 수 있을 것으로 기대된다.

Does dexmedetomidine combined with levobupivacaine in inferior alveolar nerve blocks among patients undergoing impacted third molar surgery control postoperative morbidity?

  • Background: Postoperative analgesia (POA) is an important determinant of successful treatment. Dexmedetomidine (DEX) has recently gained attention as a promising adjuvant to local anesthetics (LA). The present study aimed to evaluate the efficacy and safety of levobupivacaine (LB) as an adjuvant during inferior alveolar nerve block (IANB) in the extraction of lower impacted third molars (LITM). Methods: A prospective, randomized, placebo-controlled, triple-blind, parallel-arm, and clinical study was performed on 50 systemically healthy participants who required removal of an asymptomatic LITM. Using a 1:1 distribution, the participants were randomized into two groups (n = 25). Group L (control group) received 1.8 mL of 0.5% LB and 0.2 mL normal saline (placebo) and Group D (study group) received a blend of 1.8 mL of 0.5% LB and 0.2 mL (20 ㎍) DEX. The primary outcome variable was the duration of POA and hemodynamic stability, and the secondary variable was the total number of analgesics required postoperatively for up to 72 h. The participants were requested to record the time of rescue analgesic use and the total number of rescue analgesics taken. The area under the curve was plotted for the total number of analgesics administered. The pain was evaluated using the visual analog scale. Data analysis was performed using paired students and unpaired t-test, Mann-Whitney U test, Chi-square test, and receiver operating characteristic analysis. Statistical significance was set at P < 0.05. Results: The latency, profoundness of anesthesia, and duration of POA were statistically significant (P < 0.05). The differences between mean pain scores at 6, 12, 24, 48, and 72 h were found to be significant (each P = 0.0001). Fewer analgesics were required by participants in group D (2.12 ± 0.33) than in L (4.04 ± 0.67), with a significant difference (P = 0.0001). Conclusion: Perineurally administered LA with DEX is a safe, effective, and therapeutic approach for improving latency, providing profound POA, and reducing the need for postoperative analgesia.

Change in the Alignment and Distal Junctional Kyphosis Development after Posterior Cervical Spinal Fusion Surgery for Cervical Spondylotic Myelopathy - Risk Factor Analysis

  • Objective : This study analyzed the risk factors in patients who developed distal junctional kyphosis (DJK) after posterior cervical fusion. Methods : We retrospectively analyzed the clinical and radiographic outcomes of 64 patients, aged ≥18 years (51 and 13 male and female patients, respectively), who underwent single-staged multilevel (3-6 levels) posterior cervical fusion surgery due to multiple cervical spondylotic myelopathy. The surgeries were performed by a single spinal surgeon between January 2012 and December 2017. Demographic data, clinical outcomes, and radiological results were collected. We divided the patients into a DJK group and a non-DJK group according to the presence of DJK and investigated the risk factors by comparing the differences between the two groups. Results : Of the 64 patients, 13 developed DJK. No significant differences in clinical results were observed between the two groups before and immediately after the surgery. At the final follow-up, a higher visual analog score for neck pain was observed in the DJK group compared to the non-DJK group (p<0.01). The DJK group had a significantly lower T1 slope and a significantly higher C2-7 sagittal vertical axis (SVA) before surgery compared to the non-DJK group (p=0.03 and p<0.01, respectively). Immediately after surgery, the difference between the two groups decreased and no significant difference was observed. However, at the last follow-up, a significantly higher C2-7 SVA was observed in the DJK group (p<0.01). At the last follow up, there is no discrepancy in T1S-CL. In multiple logistic regression analysis, preoperative higher C2-7 SVA and preoperative lower T1 slope were identified as independent risk factors (p=0.03 and p<0.01, respectively). As a result, it was confirmed that DJK occurred along the process of returning to preoperative values. Conclusion : DJK can be considered to be caused by cervical misalignment due to excessive change in the surgical site in patients with low T1 slope and high C2-7 SVA before surgery. This also affects the clinical outcome after surgery. It is recommended to refrain from excessive segmental lordosis changes during multilevel cervical post fusion surgery, especially in patients with a small preoperative T1 slope and a large SVA value.

Effect of Different Packaging Atmosphere on Microbiological Shelf Life, Physicochemical Attributes, and Sensory Characteristics of Chilled Poultry Fillets

  • This trial was conducted to evaluate the effect of overwrap, vacuum, and modified atmosphere packaging (MAP) on poultry breast fillets' microbiological, biochemical shelf life and sensory attributes. The fillets were divided into 4 groups, and each of the treatments was replicated 3 times with 60 breast fillets. The first group was a control group with overwrap packaging; the second group was vacuum packed (VP); the third and fourth groups were MAP-1: 0% O2, 40% CO2, 60% N2, and MAP-2: 20% O2, 40% CO2, 40% N2. The microbiological and biochemical analyses were performed for the total viable count, coliform count, Pseudomonas count, Salmonella count, total volatile basic nitrogen (TVB-N), pH, cooking loss, color, lipid oxidation, tenderness, and sensory analysis. The data were analysed through two-way ANOVA by Minitab (Minitab 17.3.1). Meat treated with understudy MAP compositions and vacuum packaging reduced total viable count, Pseudomonas count, and total coliform count than control (p<0.05). TVB-N remained below the recommended limit throughout storage except aerobic packaging (p<0.05). Cooking loss (%) was lowered and showed non-significant results (p>0.05) between vacuum packaging and both MAP concentrations. The meat stored in MAP-2 was characterised by higher (p<0.05) visual scores. Whilst MAP-1 showed higher (p<0.05) L* values and overall acceptability. Sample packaged under aerobic packaging showed significant (p<0.05) results for b* and thiobarbituric acid reactive substances (TBARS). Meat stored in aerobic packaging showed higher (p<0.05) shear force values. The outcome of this trial may help to promote the application of understudy MAP compositions and rapid detection of microbes by biochemical analysis under local conditions.

Full-Endoscopic versus Minimally Invasive Lumbar Interbody Fusion for Lumbar Degenerative Diseases : A Systematic Review and Meta-Analysis

  • Objective : Although full-endoscopic lumbar interbody fusion (Endo-LIF) has been tried as the latest alternative technique to minimally invasive transforaminal lumbar interobody fusion (MIS-TLIF) since mid-2010, the evidence is still lacking. We compared the clinical outcome and safety of Endo-LIF to MIS-TLIF for lumbar degenerative disease. Methods : We systematically searched electronic databases, including PubMed, EMBASE, and Cochrane Library to find literature comparing Endo-LIF to MIS-TLIF. The results retrieved were last updated on December 11, 2020. The perioperative outcome included the operation time, blood loss, complication, and hospital stay. The clinical outcomes included Visual analog scale (VAS) of low back pain and leg pain and Oswestry disability index (ODI), and the radiological outcome included pseudoarthosis rate with 12-month minimum follow-up. Results : Four retrospective observational studies and one prospective observational study comprising 423 patients (183 Endo-LIF and 241 MIS-TLIF) were included, and the pooled data analysis revealed low heterogeneity between studies in our review. Baseline characteristics including age and sex were not different between the two groups. Operation time was significantly longer in Endo-LIF (mean difference [MD], 23.220 minutes; 95% confidence interval [CI], 10.669-35.771; p=0.001). However, Endo-LIF resulted in less perioperative blood loss (MD, -144.710 mL; 95% CI, 247.941-41.478; p=0.023). Although VAS back pain at final (MD, -0.120; p=0.586), leg pain within 2 weeks (MD, 0.005; p=0.293), VAS leg pain at final (MD, 0.099; p=0.099), ODI at final (MD, 0.141; p=0.093) were not different, VAS back pain within 2 weeks was more favorable in the Endo-LIF (MD, -1.538; 95% CI, -2.044 to -1.032; p<0.001). On the other hand, no statistically significant group difference in complication rate (relative risk [RR], 0.709; p=0.774), hospital stay (MD, -2.399; p=0.151), and pseudoarthrosis rate (RR, 1.284; p=0.736) were found. Conclusion : Relative to MIS-TLIF, immediate outcomes were favorable in Endo-LIF in terms of blood loss and immediate VAS back pain, although complication rate, mid-term clinical outcomes, and fusion rate were not different. However, the challenges for Endo-LIF include longer operation time which means a difficult learning curve and limited surgical indication which means patient selection bias. Larger-scale, well-designed study with long-term follow-up and randomized controlled trials are needed to confirm and update the results of this systematic review.
