Convergence analysis of pain changes on brain wave and autonomic nervous system after intervention for delayed onset muscle soreness (지연성근육통 중재 후 통증의 변화가 뇌파와 자율신경계에 미치는 융합적 분석)

  • This study aimed to investigate the effect of changes in pain on the autonomic nervous system and brain waves after inducing delayed-onset muscle soreness(DOMS). Based on voluntary participation, 28 participants with induced-DOMS were randomly divided into control(non-treatment, n=14) and experiment groups(transcutaneous electrical nerve stimulation (TENS) and kinesio taping, n=14). Intervention was performed from first day to fifth days after the onset of DOMS. Measurements were performed using the Visual Analogue Scale(VAS), Heart Rate Variability(HRV), and brain wave before DOMS induction, 24 hours after, fifth day after, and eighth day after. According to the study results, when DOMS occurred, the activity of the sympathetic nervous system was increased or the activity of the parasympathetic nervous system was suppressed, and reduction of pain due to interventions showed the opposite activity. A decreased in alpha was seen during pain, but was not significant. These results will help develop and study pain management and treatment strategies.

The effects of deep neck flexor exercise on pain and neck disability index of the patients with chronic neck pain (심부목굽힘근 운동이 만성 목통증 환자의 통증과 기능에 미치는 영향)

  • This study was to investigate the effectiveness of deep neck flexor exercise with using modalities in pain reduction and functional improvement to those who have chronic neck pain. The subjects were instructed the patients with chronic neck pain (37 people). Randomized study design of two groups was used: Only using modalities group and deep neck flexor exercise group with using modalities, and each group was taken by three times per week for 12 weeks. To evaluate the effects of therapies was to research the questionnaire about VAS(visual analog scale), NDI(neck disability index), and endurance of deep neck flexor before experiment, after 6 weeks and after 12 weeks. In visual analog scale(VAS) and neck disability index(NDI), there were significant decreased in using modalities group and deep neck flexor exercise group after 6 weeks more than before experiments but were only significant decrease in deep neck flexor exercise group after 12 weeks. The endurance of deep neck flexor was significantly increased in deep neck flexor exercise group after 6 weeks and 12 weeks more than before experiments. Thorough these results deep neck flexor exercise has the effectiveness to the therapies of chronic neck pain. Using the deep neck flexor exercise would be high effects on pain reduction and functional improvement and also considered in improving the living qualities of those who have chronic neck pain.

The Effect of Horse-Riding Exercise on Pain and Body Flexibility for the Patient with Chronic Low Back Pain (승마 운동이 만성요통환자의 통증 및 체간 유연성에 미치는 영향)

  • Purpose : The purpose of this study was to investigate the effect of horse riding exercise for pain and body flexibility in the patient with chronic low back pain. Method : 26 subjects in H-equestrian were randomly divided two group, ball exercise(BE) group and horse-riding exercise(HE) group. Each group carried out 40 minutes exercise three times a week for 8 weeks. VAS were measured for pain, and evaluation of body flexibility in position with forward and backward. Result : The results were as follows, the pain scales of VAS(visual analog scale) between ball exercise and horse-riding exercise groups in post-test, were significantly different in measures(p<.05). And there were significant in two group after exercise(p<.05). The flexibility scales of body between ball exercise and horse-riding exercise groups in post-test, were significantly different in measures(p<.05). And there were significant in two group after exercise(p<.05). Conclusion : These finding revealed that horse-riding exercise was effective on pain and body flexibility of patient with chronic low back pain so that these exercise can be new altematives exercise for pain and body flexibility in the patient with chronic low back pain.

The Effect of Rectus Abdominis Functional Massage on Forward head posture and Pain in Patients with Chronic Neck Pain (복직근 기능적 마사지가 만성 목통증 환자의 머리전방자세와 통증에 미치는 영향)

  • Background: The purpose of this study was to determine the effects of cervical deep muscle flexion exercise (CCFE) on craniovertebral angle, pain, and neck disability for patients with chronic neck pain Methods: The subjects of this study were randomly divided into three groups of 30 patients with chronic neck pain: rectus abodominis functional massage (n=10), cervical deep muscle flexion exercises group (n=10), and the control group(n=10). To assess visual analog scale (VAS) was used to test the neck pain, To assess neck posture was used to craniovertebral angle, VAS was used to test the neck pain, neck disability index (NDI) was used to test the neck dysfunction. All measurements were performed before and after each intervention was applied 3 times a week for 4 weeks. Results: In the results of all measurements, 2 groups except for the control group showed a significant change in the recovery of posture, neck pain, neck disability index (p<.05). Conclusions: Our results of this study showed that applying cervical deep muscle flexion exercise and rectus abodominis functional massage to patients with chronic neck pain improved cervical posture, neck pain, neck disability.

A Study on Low Back Pain Prevalence Rate and Related Factors among Emergency Medical Technicians Working at Fire Stations (소방서에 근무하는 응급의료종사자의 요통 유병 실태 및 관련 요인 분석)

  • Purpose: The purpose of the study was to identify low pain prevalence rate and factors affecting low back pain in emergency medical technicians. Method: A descriptive relationship research design was utilized. The participants were 74 emergency medical technicians who worked at fire stations in W and J city and agreed to participate in this study. Data were collected between April and August 2008 using a questionnaire, which included demographic factors, severity of low back pain using a visual analog scale, factors related to low back pain and health behavior, and BMI. Descriptive statistics and multiple regression were performed for data analysis. Results: 85.1% of emergency medical technicians experienced low back pain. The mean score of low back pain was relatively low (M=4.0, SD=2.33). Significant factors affecting low back pain were found to be age, marital status, exercise, hours of standing work, and BMI. These variables explained 35.1% of variance in low back pain (F=8.45, p<.001). Conclusion: These results suggest that further research is needed to develop interventions for prevention and management of low back pain among emergency medical technicians.

Analgesic Effects of Intramuscular Stimulation Therapy on Pain in the Elderly (근육내 전기자극에 의한 노인성 통증조절 효과)

  • The purpose of this study was to determine the effect of intramuscular stimulation (IMS) therapy in older persons with musculoskeletal pain. The subjects were 181 older persons (54 males, 127 females) with musculoskeletal pain. Intramuscular stimulation unit with needles (size $.3{\times}30$ mm) was applied for the treatment. The analgesic effects were measured by visual analog scale (VAS). Results showed that the post-treatment VAS score was significantly decreased after IMS therapy for fifteen minutes compared to pre-treatment score. In addition, the post-treatment VAS score was significantly decreased in patients with chronic pain (pain duration of one year after onset) compared to the post-treatment VAS score in patients with subacute pain (pain duration less than three months after onset). There was no significant difference in analgesic effects according to gender and age groups. It is determined from this study that IMS therapy can be beneficial for patients with chronic musculoskeletal pain in clinical setting. Further study is needed to identify whether the IMS therapy can change the pain threshold in patients with neurologic pain.

Effects on Knee Joint Pain by Phonophoresis of Piroxicam Gel (피록시캄 겔의 음파영동이 슬관절 통증에 미치는 효과)

  • This study was to compared the effectiveness of piroxicam gel phonophoresis with conventional ultrasound therapy in knee osteoarthritis. Eighteen patients with a means age of $55.7{\pm}5$ years were randomly assigned to phonophoresis or ultrasound groups. Continuous ultrasonic waves of 1 MHz frequency and $1\;W/cm^2\;SATA_i$ were applied for 5 minutes to the target knee joint. This study indicated their pain level by marking on a visual analog scale(VAS) at the start of treatment and at the end of 3day. Osteoarthritis VAS scores, pain on rest state, non weight bearing range of motion, 20 meters walking, and walking step by the patients were evaluated before and after therapy. Both therapeutic modalities were found to be effective and generally well tolerated after 3 therapy sessions. But piroxicam phonophoresis was not superior to conventional ultrasound in patients with knee osteoarthritis.

  • Purpose : This study was aimed to examine the Influence of PNF indirect treatment for frozen shoulder patient Methods : Frozen shoulder patient was measured range of motion test of shoulder joint, visual analog scale(VAS) for pain and clean up the mirror activity at pre intervention and post-intervention in 2 weeks. Results : The ROM, activities of shoulder were increased and decreased VAS point for pain after treatments. Conclusion : We found that PNF indirect treatment could improve ROM and functions of shoulder and pain for frozen shoulder patient.

  • Background: The objective of this study was to describe the extent of pain relief after an epidural steroid injection in the patients suffering from chronic low back pain from herniated disks or lumbar spinal stenosis. Methods: The study was prospectively designed for patients suffering with lumbar spinal stenosis (LSS) and herniated disks (HD) who were referred to a hospital-based pain clinic for epidural steroid injections (ESI). The pain was assessed with using a visual analog scale at baseline, two weeks after the first ESI and two weeks after the third ESI. Results: Eighty patients were enrolled, and all of them provided pain ratings before and after the injections. The LSS patients seemed to improve less than did the HD patients. The results showed no significant differences in the triamcinolone dosage and the frequency of injections for determining the efficacy of ESI. Conclusions: The LSS patients tended to have a less effective respond to ESIs than did the HD patients. The unsatisfactory response to ESI by the LSS patients underscores the need for randomized controlled trials of performing ESI in this population.

Reliability and Validity of Korean Version Northwick Park Neck Pain Questionnaire in Neck Pain Patients (경통 환자들을 위한 한국어판 Northwick Park Neck Pain Questionnaire의 신뢰도와 타당도)

  • The purpose of this study was to establish reliability and validity of the Northwick Park Neck Pain Questionnaire (NPQ) translated into Korean for neck pain patients. Sixty-two subjects (35 males, 27 females) with neck pain enrolled in the study. They completed a standardized self-administered questionnaire that included pain intensity, sleeping, sensory at night, duration of symptoms, carrying, reading and watching television, working, social activities, and driving. Reliability was determined by intraclass correlation coefficient (ICC) and Cronbach's alpha by internal consistency. Validity was examined by correlating the NPQ scores to the Visual Analog Scale (VAS) score. Test-retest reliability of the translated versions of the NPQ was good ICC(2,1)=.83 (95%CI=.85~.95). Cronbach's alpha value for NPQ was found to be .87 and this was statistically significant (p<.05). The criterion-related validity coefficients was .75 (p<.01). We concluded that the Korean version NPQ was shown to be a reliable and valid instrument for the assessment of neck pain.
