Purpose: To analyze the effects of customized home visiting health services on the health and health behaviors of clients with hypertension (n=107) and diabetes mellitus (DM: n=67). Methods: A one group pre and post-test research design was used. The subjects were registered in a customized 8-week, interventional, home visiting health services available in Daegu. Data was collected from November 17, 2008 to January 23, 2009. Analyses involved descriptive statistics, $x^2$ test and paired t-test. Result: Hypertension control rate was improved 25.2% and DM control rate was improved 3.0%. There were significantly beneficial hypertension-related differences in BP, health belief, health knowledge and health behaviors including performance of 10 min of moderate exercise, diet, BP monitoring and medication. Significantly beneficial DM-related changes included glucose, health belief, health knowledge and health behaviors including performance of 10 min of moderate exercise and glucose monitoring. But there were no significant hypertension-related differences in health belief (barrier) and health behavior including drinking and exercise length/frequency. Also, no significant DM-related differences were evident in health belief (barrier) and health behaviors including drinking, smoking, exercise length/frequency, diet and medication. Conclusion: Customized home visiting health service can provide effective, but not complete. Whether these benefits are maintained in the longer term is unknown.
Purpose: This study aimed to investigate possible ways to expand the services of home-visit nursing through a review of the progress, achievements, and obstacles of home-visit nursing; a pilot project of an integrated home-service; the application of the Omaha System; as well as a case analysis of providing home-visit nursing services. Method: An integrated review was conducted using various source materials, including laws, previous studies, and a case analysis. Results: In case analysis of providing visiting nursing service, rehabilitation nursing, end-of-life nursing, and dementia care showed high nursing needs. It was necessary that the various home visit nursing services in the intervention area of the Omaha System, administrative services, case management, and center operation activities were all included in the payment systems of long-term care insurance. Conclusion: In the future, home visit nursing services of long-term care insurance should be reborn in the form of a center for integrated case management in the community, which would set long-term goals until the time of a client's death and encompass the realm of human rights for health, quality of daily life, and a dignity of life.
Purpose: The purpose of the study was to analyze the ADL, IADL and cognitive function of low-income elderly who are receiving the visiting nurse service in the community. Method: Study participant were 2,413 community-dwelling elderly who live in S City. The data were collected from 5th Jan. to 28th Feb. 2006. The cross-sectional descriptive survey was done using a structured questionnaire through interviews by visiting nurses. Result: The average scores of ADL and IADL was respectively high, which indicates a relatively independent everyday life. However, the score of cognitive function was 21.87(normal range is over 23). There was a significantly positive correlation among ADL, IADL and cognitive function with the pearson correlation coefficients. Conclusion: It is concluded that elder subjects who are women, old aged, and live alone should be watched carefully for the cognitive impairment. In addition, the government should apply early detection and management system for cognitive impaired people who live in the community.
Purpose: This study examined the definition of care coordination, and the activities and roles of nurse care coordinators. The study also proposed suggestions for establishing and expanding the role of nurse care coordinators in community health practice. Methods: This study derived its conclusions by conducting a literature review. Results: The definition of care coordination is a comprehensive concept that includes case management and transitional care, and can be summarized as organizer and integrator of care. According to the literature review, 12 activities and 6 roles of nurse care coordinator were identified such as a collaborator, an individualized care planer, an educator/counselor, a direct nursing care providers, a population care coordinator, and a program evaluator. Training for nurse care coordinators is currently addressed in the Primary Medical Chronic Disease Management Program. Visiting nurses working at the Eupyeong-dong visiting health and welfare service are expected to act as nurse care coordinators, and the role of nurse care coordinators will, in the future, gradually increase in various nursing facilities. Conclusion: In addition to developing competencies to act as a nurse care coordinator, there is need for approaches to health policy that develop both independent role and population focused role as care coordinators.
Today, home health care services needs a linkage plan of the customized home visiting health service in public health center, the medical institute home health care service, and home visiting nursing service based on long term care insurance for the elderly program which acknowledges the independence and professionalism of the home health care services while minimizing overlap through linking the projects. So, this study was performed by applying the Delphi technique, which draws agreement from professional opinion, to determine a method to link home health care services in Korea. The results of this study are as follows. Specialists agreed on 24 important items within the two domains of institutional linkage and medical linkage. And the significance of this study is as follows. The 24 items deduced for the approved nursing service linkage plan are expected to improve the home health care service business system, enhance the quality of home health care service, and bring increased satisfaction for service recipients. Also, seeking ways to minimize overlap in service can increase the effectiveness of health care and public health management at a national level. In addition, it is considered that this will ultimately reduce public medical costs as well as improve home health care service.
This study was conducted to find qualitative approaches to occupational reference to group health practices. In-depth interviewing was done on 8 subjects health monitor members, owners and occupational nurses, respectively). The major findings were as follows; 1. Visiting health management Useful services were 'health counseling', 'medical examination', 'providing informations about managing diseases', 'agency business in relation to Labour Ministry' and 'giving a recognition about occupational health service to owners'. Insufficiencies were 'lack of treatment service after medical examination' and 'lack of follow up services constantly'. 2. Occupational nursing service Useful services were 'providing health information' and 'counselling about health'. Major contents of occupational nursing services were 'management of occupational and adult diseases' and 'explanation of the results after medical examination'. Insufficiencies were 'deficiency of the place where group health education could be performed', 'lack of additional or closer examinations needed in counselling' and 'discontinuous selection of additional or more exact examinations'. 3. Health monitor members Health monitor members in industries were classified into two. Some were selected by owners and the others were selected simply by considering their administrative abilities such as proficient management of documents. Their major tasks were to connect workers with occupational health management agencies. This study suggests that programs should be developed which enable health monitor members to cooperate with occupational nurses.
Purpose: Through a thorough examination of the CCSC (Community Comprehensive Support Center) system in Japan, this study suggests a scheme to provide community-based preventive health care services for the elderly in Korea. Methods: The study inquired into the applicability of the Japanese model by reviewing the data related to the CCSC project, aided by both in-depth interviews with staff in the field and consultations with specialists. Results: Rearrangement of the Visiting Health Management Project system is needed to manage the collective or individual visiting care management for frailty prevention of the elderly in communities. The delegated service system for preventive care in the community, including direct management by one of the public health centers, also needs to be reviewed and the application of stricter standards for the selection of the agency or corporation to run the delegated service is necessary. Long-Term Care Insurance, along with national and local grants, is to be considered as a financial resource for the community-based preventive health care model for the elderly. By making active use of education rooms at district offices, senior citizen centers in neighborhoods for the elderly with easy access can be created. The project needs to raise active supports from communities, develop programs which can be absorbed into particular local cultures, and promote the understanding of the preventive project in local communities. The preventive program should focus on first solving the problems of depression, seclusion, and lack of mobility of the elderly. Second, the program should instruct physical self-management for exercise-nutrition-dental maintenance, and third, the program should strengthen the cognitive abilities of the elderly. In addition, it is necessary to systematize and implement counter-plans of the family and community to protect the elderly who has mental and cognitive problems. Finally, by establishing a network of public health welfare resources based upon research on a community level, assessment and planning for the health of the elderly should be one with their family, and comprehensive consultation and recommendations should be provided to the family. Conclusion: Taking into consideration the experience Japan has had with respect to a similar project, it is appropriate to develop and implement a service system which would combine the Visiting Health Management Project system which has already been established and a preventive health care model for the elderly on a community level.
Purpose: This research was carried out to present an ideal cooperative model between the public and private health sectors for the management of community dementia patients. Method: In this study a public-private cooperative council was formed, basic data for dementia patients and their families were collected, and a dementia service program was carried out in cooperation between the two sectors. Results: The survey data shows the majority of the registered patients were undergoing a chronic diseases which would make the dementia health service inefficient. The cooperative public-private council adopted the reinforcement of medical service to the public enrolled dementia patients. The intensive medical service program showed effects on the health status of the dementia patients. Conclusion: The results of this study pointed out that change of the health insurance program supportive to the private sectors to be made; a referral system for the public health sector to the private sector should be established; and expanding the capacity of the visiting health program in the public health sector is needed.
Purpose: This study aims to investigate factors related to long-term length of stay (LOS) of patients with chronic diseases in Korean veterans hospitals. Methods: The subjects were 196 elderly patients with chronic disease staying in the hospital for more than 10 days, Data were collected by the survey of patients with structured questionnaires and medical records review by nurses from July 15 to August 10, 2019. Collected data were analyzed using t-test, ANOVA, Pearson's correlation coefficient and stepwise multiple regression. Results: The present and desired LOS were 37.78±32.66 days and 60.87±45.95 days, respectively. Factors affecting hospital LOS were found to be main disease (genitourinary) (p<.001), assistance in activities of daily living (p<.001), area of hospital (p<.001), payment of medical fees (p=.026), hospital satisfaction (p=.036) and the explanatory power of these variables was 26.4%. The most common health problems that need to be solved after discharge were symptom alleviation and health promotion. These problems can be solved using community-based facility services or visiting medical-welfare services (especially home care nursing). Conclusion: In order to reduce hospital LOS, the following measures are required: personalized self-management education, provision of transportation services for dialysis therapy of inactive patients, linking patients with visiting medical-welfare services including home care nursing and mobile healthcare services, operation of the case management system including the notice of the discharge date at admission, interim check of patient status, and connecting the patient with community resources or transferring the patient to long-term care facilities at discharge.
Objectives : This study aimed to investigate factors affecting multicultural acceptability of adolescents at individual and environmental levels. Methods : Participants were 2,032 first-year high school students who participated in the 4th-year Korean Children and Youth Panel Survey in 2013. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation, and multilevel analysis. Results : Individual factors affecting multicultural acceptability of adolescents were health status, ego-resilience, and experience of visiting foreign countries. Learning activity, school regulation, relationship with teachers, and community spirit were significant factors at environmental level. Conclusions : Findings suggest that it is necessary to consider health status, improve positive resilience, activate a multicultural experience program, establish friendly relations with teachers, adhere to social norms, and strengthen community spirit for multicultural acceptability.
