Accuracy of Maxillary Segmental Osteotomy using 3D Simulation: A Case Report

  • To overcome limitations of conventional diagnosis and planning for orthognathic surgery, surgeons have begun to use 3-dimensional (3D) virtual simulation to plan complex orthognathic surgery. In many literatures, it has shown that better surgical outcome achieved with 3D virtual simulation than that with conventional methods. But, there is still lack of data about accuracy of maxillary segmental surgery with 3D virtual simulation. The purpose of this paper was to report the case of maxillary segmental orthognathic surgery with 3D virtual simulation and to assess the actual surgical outcome. Though the result was clinically acceptable, discrepancy between 3D simulation and actual surgery was not superior compared with conventional method. The accuracy of 3D simulation surgery and intermediate wafer fabrication for maxillary segmental surgery needs to be improved. Advancement in 3D software program and careful surgical technique will make it more precise and reliable method.

Computer-assisted Virtual Surgery and Splint Fabrication for Paediatric Mandible Fracture

  • Closed reduction using acrylic splints with circummandibular fixation has been known to be useful techniques in pediatric mandibular fractures. However, this technique has some shortcomings, including needs for impression taking or additional laboratory process, which can increase the exposure time of general anesthesia or make an additional sedation visit. Recently, the advancement of computer-aided maxillofacial surgery offers to clinicians to expansion of its application. This case report represents a technique of computer-assisted virtual reconstruction and computer-aided designed splint fabrication in a 2-year-old boy with mandibular body fracture.

  • Le fort 1 osteotomy surgery is one of the most popular surgical methods for the treatment of patients with facial bone deformities. An intermediate wafer splint is used to fix the bone segment to the planned position, but there are many steps that can cause errors. To reduce these errors, we propose a method of using a surgical guide made with virtual surgical simulation.

  • Fibrous dysplasia(FD) is a disorder in which normal bone is replaced with pathologic tissue. When occurring in craniofacial regions, the zygomaticomaxillary complex is most commonly affected and this pathologic lesion results in facial asymmetry. and By using computer-assisted virtual simulation, precise maxillofacial contouring was achieved for harmonious facial morphology and the surgical procedure was simplified and the surgery brought satisfactory results in terms of both esthetics and functionality.

  • 가상 의료 시뮬레이션을 통한 실습 교육은 학습 효과를 높이는 장점이 있기 때문에 최근 실제 의료 환경에 도입이 확대되고 있다. 특히 최소 침습적 수술 분야에서 가상 수술 의료 시뮬레이션의 필요성이 증가하고 있다. 하지만, 가상 수술 의료 시뮬레이션 환경에서 자주 발생하는 출혈 특수 효과를 현실감이 있게 표현하기 위한 출혈 애니메이션 기술은 아직까지 제안되지 않고 있다. 본 논문에서는 사실적인 실시간 출혈 애니메이션 기법을 제안한다. 제안 기법은 출혈 시뮬레이션을 위해서 주 출혈 방향 벡터 방향을 기준으로 주 출혈 영역과 유효 출혈 영역을 계산하여 자연스러운 출혈 효과의 표현이 가능하다. 또한, 출혈 렌더링을 위해서 정점들의 불투명도 값을 시그모이드 함수를 이용한 가중치 부여로 부드러운 불투명도 변화를 통하여 사실적이고, 자연스러운 출혈 애니메이션이 가능하다. 본 논문에서 제안한 기법은 가상 수술 의료 시뮬레이션의 현실감, 몰입감을 증진시켜 이러한 시뮬레이션을 사용하는 의사 혹은 의대생들에 대한 교육의 효과를 증진시킬 수 있다.

Application of 3D Simulation Surgery to Orthognathic Aurgery : A Preliminary Case Study

  • The aim of this report is to evaluate accuracy using3D surgical simulationand digitally printedwafer in orthognathic surgery. 22-year-old female was diagnosed with mandibular prognathism and apertognathia based on 3D diagnosis using CT. Digital dentition images were taken by laser scanning from dental cast, and each STL images were integrated into one virtual skull using simulation software. Digitalized intermediate wafer was manufactured using CAD/CAM software and 3D printer, and used to move maxillary segment in real patient. Constructed virtual skull from 1 month postoperative CT scan was superimposedinto simulated virtual model to reveal accuracy. Almost maxillo-mandibular landmarks were placed in simulated position within 1 mm differences except right coronoid process. Thus 3D diagnosis, surgical simulation, and digitalized wafer could be useful method to orthognathic surgery in terms of accuracy.

  • Traditionally 2D cephalometric analysis has been used for diagnosis and treatment of maxillofacial deformities. However, 2D has some limitations in diagnosis and treatment planning especially facial asymmetry cases. The most weakness of 2D is overlapping and unpredictability. Today 3D treatment tools are used by many maxillofacial surgeons. 3D treatment tools can show ungarbled facial anatomy and do virtual surgery. The aim of this report is to present usefulness of using 3D analysis and virtual orthognathic surgery for severe facial asymmetry patients.

가상 수술 시뮬레이션과 3차원 프린팅 절골술 가이드를 이용한 교정 수술: 증례 보고 (Corrective Surgery Using Virtual Surgical Simulation and a Three-Dimensional Printed Osteotomy Guide: A Case Report)

  • A 74-year-old female patient, who underwent surgery for a left distal tibiofibular fracture 40 years earlier, visited the hospital with an ankle varus deformity due to malunion. The patient complained of discomfort while walking due to the ankle and hindfoot varus deformity but did not complain of ankle pain. Therefore, correction using supramalleolar osteotomy was planned, and through virtual surgical simulation, it was predicted that a correction angle of 24° and an osteotomy gap open of 12 mm would be necessary. An osteotomy guide and an osteotomy gap block were made using three-dimensional (3D) printing to perform the osteotomy and correct the deformity according to the predicted goal. One year after surgery, it was observed that the ankle varus was corrected according to the surgical simulation, and the patient was able to walk comfortably. Thus, for correction of deformity, virtual surgical simulation and a 3D-printed osteotomy guide can be used to predict the target value for correction. This is useful for increasing the accuracy of correction of the deformity.

가상 수술 의료 시뮬레이션을 위한 소작 효과 애니메이션 기법 (Cauterizing Effect Animation for Virtual Surgery Medical Simulation)

  • 최근 의료 환경에 도입되고 있는 가상 의료 시뮬레이션을 통한 실습 교육은 학습 효과를 최대화한다. 특히 최소 침습적 수술 분야에서 가상 수술 의료 시뮬레이션의 필요성이 매우 증가하고 있다. 최소 침습적 수술 환경에서 많이 발생하는 소작 효과는 단순 출혈 효과로 표현되고 있어 아직까지 현실감이 있는 소작 효과 애니메이션 기술은 제안되지 않고 있다. 본 논문에서는 사실적인 실시간 소작 애니메이션 기법을 제안한 다. 제안 기법은 메쉬의 정점에 대한 칼라 값을 구성하는 각 요소 색상을 개별적으로 변화시키고, sigmoid 함수를 이용한 가중치 부여로 유효 영역 메쉬 내부의 부드러운 색상 변화를 통하여 사실적이고, 자연스러운 소작 효과의 애니메이션이 가능하였다. 또한, 소작 칼라 버퍼를 제안하여 중첩된 소작 효과 애니메이션도 자연스럽게 표현하였다. 본 논문에서 제안한 기법은 가상 수술 의료 시뮬레이션의 현실감, 몰입감을 크게 증진시켜 이러한 시뮬레이션을 사용하는 의사 혹은 의대생들에 대한 교육의 효과를 극대화시킬 수 있다.

3D Character Animation: A Brief Review

  • Virtual Reality makes a virtual environment more realistic and, furthermore, it provides a variety of experiences which we cannot have in reality. A drastic growth of GPU performance and increase of computing capability make virtual environment more realistic than ever. One important element of constructing virtual environment is to animate 3D characters. Many researchers have been studying 3D characters animating and a myriad of methods have been proposed to make them more realistic. In this paper, we discuss the technologies and characteristics of 3D character animation. We believe that realistic characters in Virtual Reality will be applied to various fields: education, film and game industry, business and, particularly, medical area such as telemedicine, virtual surgery, etc.
