균혈증을 동반한 비피막형 인플루엔자균에 의한 접형동 부비동염 소아 1례 (Isolated Sphenoid Sinusitis with Nontypeable Haemophilus influenzae Bacteremia in a Healthy Child)

  • 급성 세균성 부비동염은 소아에서 흔하게 발생하는 질환 중 하나로, 바이러스성 상기도 감염 후 5-13%에서 동반하는 것으로 알려져 있다. 접형동에 국한된 부비동염이 발생할 확률은 모든 부비동염의 3% 미만이나 접형동의 구조적 특성상 주위 조직으로 감염이 전파되면 치명적일 수 있다. 비피막형 인플루엔자균(non-typeable Haemophilus influenzae[NTHi])은 급성 세균성 부비동염의 흔한 원인균이나 정상면역인 환아에서 균혈증으로 파급되는 경우는 매우 드물다. 저자들은 특별한 기저질환이 없는 소아가 발열과 두통으로 내원하여 균혈증을 동반한 NTHi에 의한 편측 접형동에 국한된 부비동염이 확인되어 항생제 및 수술적 치료로 호전된 예를 경험하여 보고하는 바이다.

Three Different Viruses Isolated from Typical Weed Plants that Grown Adjacent to Common Crop Fields

  • Weeds are widely grown in the field and are infected by many viruses. A survey was conducted to identify viruses infecting weeds in Korea. Virus-infected weed samples including Rorippa indica (L.) Hiern, R. islandica (Oed.) Bord, Crepidiastrum denticulatum (Houtt.) Pak & Kawanno, Achyranthes japonica (Miq.) Nakai, and Chrysanthemum boreale (Makino) Makino were collected in Kyonggi Province. These weeds were grown in the greenhouse and were isolated on 10 test plants. Several virus isolates were isolated fron infected tissues and were further studied by host range assay, serological test, electron microscopy (EM), reverse transcription-polymerase chain reaction (RT-PCR) and sequencing. Each isolated virus strain was mechanically transmitted to weeds and various hosts including Nicotiana spp., Brassica spp., Vigna unguiculata, Capsicum annuum, and Cucumis sativus and showed systemic mosaic, vein clearing, necrosis, mottle, malformation, chlorosis, and/or death of host plants in some cases. Each virus was then purified using infected leaves and observed by EM. From these results three viruses were isolated and identified as Turnip mosaic virus (TuMV), Broad bean wilt virus (BBWV), and Cucumber mosaic virus (CMV). RT-PCR using virus-specific oligonucleotide primers and the cloning were conducted to determine the nucleotide sequences of coat proteins of the three viruses their amino acid sequence were deduced. The amino acid sequence homologies were about 92.7 to 99.7%, 96.2 to 97.7%, and 93.9 to 98.6% to other reported TuMV, BBWV, and CMV strains, respectively. These results suggest that many weeds may serve as primary inoculum source of diseases caused by TuMV, BBWV, CMV and that the management of these viral diseases can be achieved through weed control.

전북지역 토마토와 박과류 무농약재배지의 주요 병해 발생 현황 (Occurrence of major diseases in pesticide-free cultivated tomato and cucurbit in Jeollabuk-do, South Korea)

  • 토마토와 박과류채소의 유기농산물을 생산하기 위하여 무농약으로 재배하는 시설하우스에서 최근 문제되는 병해를 조사한 결과 토마토 잎곰팡이병과 토마토반점위조바이러스병, 토마토퇴록바이러스병, 박과류 쥬키니황화모자이크바이러스병, 박과류 흰가루병 등의 피해가 심한 것으로 조사되었다. 농약을 사용하여 일반재배를 하는 시설하우스에 비해 무농약재배 하우스에서 재배되는 작물에서 병 발생 시기가 더 빠르고 병 발생도 심한 것으로 나타났으며, 총채벌레와 진딧물 등에 의해 매개되는 바이러스병 피해가 심한 것으로 조사되었다. 따라서 우수한 유기농산물을 생산하기 위해서는 병해충방제를 효율적으로 하는 것이 중요하며, 병 발생 피해를 최소화기 위해서는 포장위생과 물리적 차단, 유기농업자재나 미생물제 등을 종합적으로 활용하여 예방적으로 방제하는 것이 가장 중요하다.

  • Here we report the first fatality caused by H1N1 influenza virus infection with acute respiratory distress syndrome in Korea. A 55-year-old man presented at our emergency department with dyspnea, fever, diffuse myalgia and malaise. Bilateral lung air-space consolidation was detected on his initial chest radiograph combined with severe hypoxemia. He was supported by mechanical ventilation and treated with antibiotics. A nasopharyngeal aspirate was positive for influenza A rapid antigen and oseltamivir was started on day 3 of admission. The nasal swab sample was positive for influenza H1N1 virus by real-time reverse-transcriptase polymerase chain reaction. Despite aggressive treatment, he had refractory hypoxemia and uncontrolled septic shock. On day 5 of admission he went into cardiac arrest and expired.

  • Acute transverse myelitis(ATM) is defined as an acute intramedullary dysfunction of the spinal cord, ascendng or static involving both halves of the cord and appearing without any history of previous neurological diseases due to traumatic accident, tumor of all kind, encephalitis and of course excluding all possible viral, bacterial and fungal infection. It is mainly characterized by acute motor disorder of both limbs in respect to which spinal segments are affected as well as sensory disorder and dysuria & dyschezia. The exact cause is unknown, however it is recently suggested that immunological factors are highly involved. It has been reported by several reliable sources that it is often accompanied by immunological diseases such as systemic lupus erythematosus(SLE). As treatments non steroid anti-inflammatory drugs(NSAIDS) are primarily recommended as to steroids, limited doses are injected only with the proper prescription from the physician. Operative methods are not options as traumatic accidents and tumors are excluded as factors. To enhance muscle strength and prevent articular contracture physical therapy and passive exercise is imperative. The following patient whose chief complaints were mainly about hypoesthesia of Rt. lower limb and stiffness of phalanges of both fingers as well as to weakness of lower extermity. Therefore it has been diagnosed as arthalgia syndrome. In oriental medicine factors such as wind evil heat-evil, dampness-heat evil, cold evil cause the arthalgia syndrome. In this case the patient was diagnosed as dampness-heat evil and herbal medicine Chunglijagam-Tang and Dong-Qi acupuncture was applied to treat bladder disorder.

광주(光州)·전남지역내(全南地域內) 소의 바이러스성 질병(疾病)에 관한 혈청학적(血淸學的) 연구(硏究) (Seroepidemiological studies on virus-borne diseases of cattle in Kwangju and Chonnam area)

  • Sera from 304 Holsteins or Korean native cattle were collected from slaughterhouse in Kwangju area to study the infection of major virus-borne diseases. Serum antibody titers against infectious bovine rhinotracheitis virus(IBRV), bovine viral diarrhea virus(BVDV), parainfluenza type-3 virus(PI-3V), bovine ephemeral fever virus(BEFV), bovine Ibaraki virus(BIV), bovine Akabane virus(BAKV), bovine rotavirus(BRV), bovine coronavirus(BCV) were measured by serum neutralization tests. Results which obtained were as follows. Sera from 280 cattle(92.1%) contained antibodies against BRV which rate was the highest among the 8 viruses, and serum antibodies against BCV, BVDV, BIV, BAKV, BEFV, IBRV and PI-3V were detected from 266(87.5%), 149(49%), 108(35.5%), 94(30.9%), 80(26.3%), 32(10.5%) and 24(7.9%) cattle, respectively. Prevalence of seropositives to BVDV, BIV, BAKV, BEFV were higher among Holsteins than among the Korean native cattle(P<0.05). Prevalence of antibody titers against BVDV, BIV and BEFV in Korean native cattle were higher among females than males, while more males contained antibodies to BAKV, IBRV and PI-3V than females in their blood(P<0.05). Seropositives to BVDV, BIV, BAKV, BEFV and IBRV in Holsteins were higher among females than males(P<0.05). In Korean native cattle, serum antibody titers against IBRV and PI-3V ranged from 1:2~1:32 and 1:2~1:64, respectively, while serum antibody titers against the rest 6 viruses ranged from $1:2{\sim}1:{\geq}256$. In Holsteins, serum antibody titers against IBRV and PI-3V ranged from 1:2~1:64 and 1:2~1:32, respectively, while serum antibody titers against the rest 6 viruses ranged from $1:2{\sim}1:{\geq}256$.

  • Subsequent to an allogenic stem cell transplantation(ASCT) on patients with hematologic malignancy(AML, ALL, CML, multiple myeloma, lymphoma etc.), chronic GVHD(graft versus host disease), which is an immunological reaction, occurs. With treatment results from patients who were diagnosed with ALL(acute lymphocytic leukemia), undergone BMT(bone marrow transplantation) and showed oral and skin lesions due to GVHD, treatment of oral manifestations of leukemia and its general management were studied. 90% of patients with chronic GVHD show change in the oral mucosa causing oral manifestations such as leukoplakia, lichenoid change of the oral mucosa, mucosal atrophy, erythema, ulceration and xerostomia. In treating GVHD, extensive systemic immunosuppression cause bacterial, viral, fungal infection that are fatal, and even if the treatment is successful, the patient is already in a severe immunosuppressed state. Therefore, localized target therapy is preferred. In another words, topical application(rinse, cream, ointment etc.) of cyclosporin and steroid in treating oral chronic GVHD is highly recommended, and the use of PUVA(Psoralen Ultraviolet A) and thalidomide is reported to be effective. In treating such diseases, dental treatment to control pain and prevent secondary infection of oral manifestations is very important. To those patients with systemic diseases who show limited effect by general dental treatment, non-invasive treatment such as the dental laser, in addition to the use of drugs, may be necessary to actively treat pain and help the healing process. For greater results, new effective methods are to be developed for treatment.

사상체질별 간기능 검사 및 초음파상의 간질환에 대한 연구 (Study on Liver Function Test and Liver Disorders under the Ultrasonography for each of the Sasang Constitutional Types)

  • Objectives This study aimed to examine differences in the hepatic function disorders and prevalence rates of liver diseases in accordance with the Sasang constitutional type, and to analyze whether Sasang constitution is a significant risk factor for fatty liver. Methods A total of 1211 patients who underwent health check-up at the Ilsan Hospital of Dongguk University and had their physical constitutions diagnosed by a specialist in Sasang constitutional medicine from October 31, 2011 to September 28, 2012 were used as the subjects of this study. Presence of hepatobiliary diseases was analyzed from the results of the hepatic function test, lipid test and viral hepatitis infection marker test in the biochemistry tests, and abdominal ultrasonography. Results & Conclusion Subjects of the study were composed of 691 males and 520 females and, in terms of Sasang constitution distribution, 550 Taeeumin, 343 Soeumin and 318 Soyangin. As the results of analysis of prevalence rates in accordance with Sasang constitutions, significant differences were observed in the prevalence rates of abnormal groups in the cases of AST, ALT, GGT, Triglyceride, HDL-cholesterol and LDL-cholesterol, and all of their prevalence rates were in the descending order of Taeeumin, Soyangin and Soeumin. Soeumin displayed significantly higher positive rate for HBs Ab in comparison to Taeeumin and Soyangin. In the results of abdominal ultrasonography, the prevalence rates of fatty liver displayed significant differences and were in the descending order of Taeeumin, Soyangin and Soeumin. And as the result of logistic regression analysis in order to find the risk factors of fatty liver, the Sasang constitution was found to be a significant risk factor for fatty liver. The odds ratio of Taeeumin in fatty liver was found to be 1.634 higher than Soeumin and 1.773 higher than Soyangin

Efficacy and Safety of Azithromycin for the Treatment of COVID-19: A Systematic Review and Meta-analysis

  • Background: The lack of effective medications for coronavirus disease 2019 (COVID-19) has led to a trend of drug repurposing such as the case of azithromycin which shows immunomodulatory and anti-viral effect. Several clinical trials have shown conflicting results. It is currently unclear whether the available evidence is in favor or against the use of azithromycin in COVID-19 patients. Thus, the aim of this study was to investigate the efficacy and safety of azithromycin in COVID-19 patients. Methods: Four independent reviewers selected relevant studies from PubMed, ScienceDirect, EBSCO, and ProQuest published prior to March 2021. The protocol used in this study has been registered in PROSPERO (CRD42020224967). Results: We included 17 studies and found that the mortality rate (odds ratio [OR], 0.95; 95% confidence interval [CI], 0.76-1.19), need of respiratory support (OR, 1.30; 95% CI, 0.98-1.73), hospitalization rate (standardized mean difference, 0.12; 95% CI, -0.02 to 0.27), and intensive care unit transfer (OR, 1.21; 95% CI, 0.79-1.86) of azithromycin-treated group did not differ significantly (p>0.05) from those of the control group. Azithromycin treatment did not significantly increase the risk of getting secondary infection (OR, 1.23; 95% CI, 0.83-1.82), hypoglycemia (OR, 0.73; 95% CI, 0.38-1.40), gastrointestinal problems (OR, 1.03; 95% CI, 0.73-1.45) or electrocardiogram abnormalities (OR, 1.16; 95% CI, 0.94-1.42). The overall quality of evidence ranged from low to very low. Conclusion: Azithromycin did not result in a superior clinical improvement in COVID-19 patients, although it was well-tolerated and safe to use.

Factors Associated with Worsening Oxygenation in Patients with Non-severe COVID-19 Pneumonia

  • Background: This study aimed to determine the parameters for worsening oxygenation in non-severe coronavirus disease 2019 (COVID-19) pneumonia. Methods: This retrospective cohort study included cases of confirmed COVID-19 pneumonia in a public hospital in South Korea. The worsening oxygenation group was defined as that with SpO2 ≤94% or received oxygen or mechanical ventilation (MV) throughout the clinical course versus the non-worsening oxygenation group that did not experience any respiratory event. Parameters were compared, and the extent of viral pneumonia from an initial chest computed tomography (CT) was calculated using artificial intelligence (AI) and measured visually by a radiologist. Results: We included 136 patients, with 32 (23.5%) patients in the worsening oxygenation group; of whom, two needed MV and one died. Initial vital signs and duration of symptoms showed no difference between the two groups; however, univariate logistic regression analysis revealed that a variety of parameters on admission were associated with an increased risk of a desaturation event. A subset of patients was studied to eliminate potential bias, that ferritin ≥280 ㎍/L (p=0.029), lactate dehydrogenase ≥240 U/L (p=0.029), pneumonia volume (p=0.021), and extent (p=0.030) by AI, and visual severity scores (p=0.042) were the predictive parameters for worsening oxygenation in a sex-, age-, and comorbid illness-matched case-control study using propensity score (n=52). Conclusion: Our study suggests that initial CT evaluated by AI or visual severity scoring as well as serum markers of inflammation on admission are significantly associated with worsening oxygenation in this COVID-19 pneumonia cohort.
