Roles of $Ca^{2+}-Activated\;K^+$ Conductances on Spontaneous Firing Patterns of Isolated Rat Medial Vestibular Nucleus Neurons

  • To investigate the contributions of intrinsic membrane properties to the spontaneous activity of medial vestibular nucleus (MVN) neurons, we assessed the effects of blocking large and small calcium-activated potassium channels by means of patch clamp recordings. Almost all the MVN neurons recorded in neonatal $(P13{\sim}P17)$ rat were shown to have either a single deep after-hyperpolarization (AHP; type A cells), or an early fast and a delayed slow AHP (type B cells). Among the recorded MVN cells, immature action potential shapes were found. Immature type A cell showed single uniform AHP and immature B cell showed a lack of the early fast AHP, and the delayed AHP was separated from the repolarization phase of the spike by a period of isopotentiality. Application of apamin and charybdotoxin (CTX), which selectively block the small and large calcium-activated potassium channels, respectively, resulted in significant changes in spontaneous firings. In both type A and type B cells, CTX (20 nM) resulted in a significant increase in spike frequency but did not induce bursting activity. By contrast, apamin (300 nM) selectively abolished the delayed slow AHP and induced bursting activity in type B cells. Apamin had no effect on the spike frequency of type A cells. These data suggest that there are differential roles of apamin and CTX sensitive potassium conductances in spontaneous firing patterns of MVN neurons, and these conductances are important in regulating the intrinsic rhythmicity and excitability.

  • In general, dizziness is caused by various situations, but among them, symptoms due to dysfunction of the motor system belonging to the nervous system are the most severe, accompanied by nausea and vomiting. Treatment of these dizziness includes drug therapy, surgical therapy, and rehabilitation. Drug therapy and surgery are generally performed in vest rehabilitation training, which is a rehabilitation therapy because of the risk of aftereffects. The vestibular rehabilitation training includes eye training, posture stabilization training, and walking training. Among them, walking training is performed in a certain space under the supervision of a doctor or a professional therapist, so that the time and space burden is increased. In order to solve this problem, we implemented gait training contents which can be used for rehabilitation training by using the augmented reality technology. It is expected that it can be utilized as dizziness rehabilitative contents which can be used in medical environment through clinical tests for patients with dizziness.

Surgical and Electrical Anatomy of the Inter-Nodal and Intra-Atrial Conduction System in the Heart

  • An anatomical understanding of the atrial myocardium is crucial for surgeons and interventionists who treat atrial arrhythmias. We reviewed the anatomy of the inter-nodal and intra-atrial conduction systems. The anterior inter-nodal route (#1) arises from the sinus node and runs through the ventral wall of the atrial chambers. The major branch of route #1 approaches the atrioventricular node from the anterior aspect. Other branches of route #1 are Bachmann's bundle and a vestibular branch around the tricuspid valve. The middle inter-nodal route (#2) begins with a broad span of fibers at the sinus venarum and extends to the superior limbus of the oval fossa. The major branch of route #2 joins with the branch of route #1 at the anterior part of the atrioventricular node. The posterior inter-nodal route (#3) is at the terminal crest and gives rise to many branches at the pectinate muscles of the right atrium and then approaches the posterior atrioventricular node after joining with the vestibular branch of route #1. The branches of the left part of Bachmann's bundle and the branches of the second inter-nodal route form a thin myocardial network at the posterior wall of the left atrium. These anatomical structures could be categorized into major routes and side branches. There are 9 or more anatomical circles in the atrial chambers that could be structural sites for macro re-entry. The implications of normal and abnormal structures of the myocardium for the pathogenesis and treatment of atrial arrhythmias are discussed.

The Effects of Smooth Pursuit Eye Movement and Saccadic Eye Movement on Balance and Muscle Activation during One Leg Standing (한 다리 서기 동안 매끈눈따라보기와 신속눈운동이 균형과 근활성도에 미치는 영향)

  • PURPOSE: This study is examined the effects of vestibular stimulation through eye movement on balance and muscle activity. METHODS: In 42 healthy adults, no eye exercise was applied to both feet and one foot. The speed of smooth pursuit eye movement (.2 Hz, .3 Hz, and .5 Hz) and saccadic eye movement (.5 Hz and 1.1 Hz) were randomized. The measurements were taken three times for 30 seconds while standing on two feet and measured three times for 10 seconds while standing on one foot. The muscle activity measurement equipment was used to measure the electromyogram signals of the tibialis anterior, peroneus longus, gastrocnemius medialis, vastus medialis, vastus lateralis, biceps femoris, abdominal internal oblique, and erector spinae muscle. RESULTS: As a result of this study, when applying smooth pursuit eye movement on one leg, the pressure center movement increased, the muscle activity of the lower extremity increased, in the saccadic eye movement, and the center of pressure decreased. CONCLUSION: Accordingly, the smooth pursuit of eye movement, the intervention of this study, affects balance. Through this, the balance can be improved by applying eye movement to the target who needs to improve the balance ability.

Clinical and patient-reported outcomes after recession coverage using modified vestibular incision subperiosteal tunnel access with a volume-stable collagen matrix as compared to a coronally advanced flap with a subepithelial connective tissue graft

  • Purpose: Coronally advanced split-or full-thickness (CAST or CAFT) flaps in combination with subepithelial connective tissue grafts (SCTGs) are commonly used in root-coverage procedures despite postoperative pain and bleeding from the graft donor site. Therefore, the modified vestibular incision subperiosteal tunnel access procedure (VISTAX) uses a novel collagen matrix (VCMX) instead of autogenous tissue to address the limitations associated with autogenous tissue grafting. This retrospective study compared the clinical outcomes of VISTAX to the results obtained after using a CAST or CAFT flap in combination with SCTG for root coverage. Methods: Patients with single or multiple adjacent recession I/II defects were included, with 10 subjects each in the VISTAX, CAFT, and CAST groups. Defect coverage, keratinized tissue width, esthetic scores, and patients' perceived pain and dentinal hypersensitivity (visual analogue scale [VAS]) were assessed at baseline, 3 months, and 6 months. Results: All surgical techniques significantly reduced gingival recession (P<0.0001). Defect coverage, esthetic appearance, and the reduction in dentinal hypersensitivity were comparable. However, the VAS scores for pain were significantly lower in the VISTAX group than in the CAFT and CAST groups, which had similar scores (P<0.05). Furthermore, the clinical results of VISTAX and CAFT/CAST generally remained stable at 6 months. Conclusions: The clinical outcomes of VISTAX, CAFT, and CAST were comparable. However, patients perceived significantly less pain after VISTAX, indicating a potentially higher patient acceptance of the procedure. A prospective trial with a longer follow-up period and a larger sample size should therefore evaluate VISTAX further.

Treatment of Multiple Gingival Recessions Using Vestibular Incision Subperiosteal Tunnel Access with Platelet-rich Fibrin: Two Cases Reports

  • Treatment of multiple gingival recession defects is usually more challenging than that of single gingival recession. Various techniques for the treatment of multiple gingival recession have been established. Recently, vestibular incision subperiosteal tunnel access (VISTA) technique has been considered to exhibit high predictive ability. Connective tissue graft (CTG) has also been considered a gold standard technique owing to its high predictability of root coverage. However, this technique requires a suitable donor site and has clinical disadvantages, such as additional pain. Thus, in this case presentation, platelet-rich fibrin (PRF) was used as an alternative material for CTG along with VISTA. We herein report cases of two patients with Miller's class I and III multiple gingival recession defects, respectively. These patients underwent VISTA along with the use of a PRF membrane. They were followed up for 12 months postoperatively, and their clinical parameters, including probing depth, depth of gingival recession, clinical attachment level, and width of attached gingiva at baseline and at 2, 6, and 12 months postoperatively, were assessed. The patient with class 1 recession defects exhibited a significant amount of root coverage, which remained stable during the follow-up period. Whereas the patient with class 3 recession defects had lesser amount of coverage compared to class 1 patient. The partial coverage observed may be attributed to not only anatomical factors but also the technique-sensitive nature of the procedure. Considering these results, the use of VISTA along with PRF is a viable option for treating gingival recession, as it does not cause discomfort to patients. However, various factors need to be considered during the surgical procedure.

Reliability and Validity of Objective Diagnosis Tools According to the Emotional State of Companion Canine (반려견 정서상태에 따른 객관적 진단 도구의 신뢰도와 타당도 연구)

  • This study aimed to verify the results of emotional analysis with respect to canine behavior as reported in the existing animal behavior field based on the dog vestibular emotional reflex (VER) principle, and to evaluate humans and animals based on empathy gained through true communication rather than human-centered interpretation. A total of 200 canines were divided into non-daily and daily situation groups (n=100 each). To assess the psychological and emotional state of each group, these changes were applied to the principle of VER, and six measurement values were selected: positive emotion (balance and energy), negative emotion (tension/anxiety and stress), and body condition (inhibition and neuroticism). The results showed a statistically significant difference (p<0.05) in positive emotions, negative emotions, and body conditions between the two groups. In addition, balance, energy, tension/anxiety, inhibition, and neuroticism were higher in the non-daily situation group than in the daily situation group, with an exception for stress. Compared with the canine daily situation groups, canine in the non-daily situation groups instinctively observed their surroundings to cope with possible threats, had decreased concentration, and elevated vitals due to high-intensity anxiety. This can be perceived as a state of immediate reaction to possible situations/threats. In conclusion, it was confirmed through Vibra Image technology that canine instability mainly affects three factors: positive emotion, negative emotion, and physical condition.

  • Vestibuloplasty are following categories : Mucosal advancement(submucous), secondary epithlization(reepithelization) and grafting vestibuloplasty. Although certain procedures are indicated for alveolar bone loss and sulcus shortening, relapse can occur. Every efforts to minimize or compensate for it is controversy. O'Steen(1970) reported the mucous graft methods that none of vestibular shrinkage and graft contracture. 15patients in mucous graft vestibuloplasty with fibrin adhesive system(Beriplast) were taken in cases of alveolar bone resorption and mucosal shortening due to traumatized alveolar bone defects, senile atrophic alveolar bone, postoperative cyst or tumor resection, edentulous alveolar bone loss, and others. A technique in the use of small piece of palatal mucosa$(1{\times}20mm)$ from the lateral aspect of the palate with adhesive system provided to secure the skin grafts, avoid stent fixation, postoperative patient's comfort and less time-consuming than the standard technique, especially excellent bleeding control.

Comparison of Japanese Sensory Inventory-2 Scores of Children With and Without Developmental Disabilities - A Pilot Study (일본감각력개정판(JSI-2)에 의한 발달장애아동과 정상아동의 감각처리능력 비교에 관한 예비연구)

  • Objective : To describe the sensory-based behaviors of children with developmental disorders as reported on the JSI-2. Methods : The scores of children with disability were compared with those of children without disability. JSI-2 was completed by parents of 38 children with disability 3 through 6 years of age and 36 children without disability 3 through 6 years of age. Results : The scores of children with disability were significantly different from that of children without disability for 6 of 8 factors, including vestibular, tactile, auditory, visual and other. There is no significant differences of age or sex. Conclusion : From this study, it is found that children with disability have deficit in a variety of sensory processing abilities as measured by JSI-2. Further research is needed to replicate and develop these findings.

  • 저자들은 Vestibular fornix의 연장, 노출된 치근의 치근회복 또는 부착치근의 폭경을 증가시켜주기 위해서 유리치근 자가이식수술을 6례는 하악전치부에, 2례는 하악구치부에 시행하여 성공하였다. 수술후 3개월 6개월에 완전치유결과를 나타냈으며 수술후의 후유증으로 이식부위의 조직의 색조가 인접조직과 비교해서 약간 차이가 있을 뿐이다. 본 수술은 비교적 간단하고 또한 치유가 빠르며 예후가 양호하므로 치주영역에 추천할만한 수술법이다.
