안모유형에 따른 교정치료 (ORTHODONTIC TREATMENT RELATED TO FACIAL PATTERNS)

  • Certain malocclusion are associated with specific "facial type," and it is important for the clinician to classify the common facial characteristic of each patient. Because the reaction to treatment mechanics and the stability of the denture is depended upon the analysis of the facial pattern. Basically, there are 3 district facial types or patterns under which almost all malocclusion can be classified. 1. mesofacial is the most average growth. 2. brachyfacial which is a horizontal growth pattern has a week muscle, with dental arch, deep bite. 3. dolichofacial which is a vertical growth pattern has a strong muscle, narrow dental arch, open bite. Brachyfacial pattern show a resistant to mandibular rotation during treatment can accept a more protrusive denture and are prominantly nonextraction, whereas dolichofacial patterns tend to open during treatment require a more retracted denture in order to assure post-treatment stability. Brachyfacial pattern would better treat to use extrusive force system, whereas dolichofacial pattern treat to use intrusive force system with head gear and intermaxillary elastics.

Relationship between the maxillofacial skeletal pattern and the morphology of the mandibular symphysis: Structural equation modeling

  • Objective: The purpose of this study was to investigate the relationship between the facial skeletal patterns and the shape of the mandibular symphysis in adults with malocclusion by using a structural equation model (SEM). Methods: Ninety adults who had malocclusion and had records of facial skeletal measurements performed using cone-beam computed tomography were selected for this study. The skeletal measurements were classified into three groups (vertical, anteroposterior, and transverse). Cross-sectional images of the mandibular symphysis were analyzed using generalized Procrustes and principal component (PC) analyses. A SEM was constructed after the factors were extracted via factor analysis. Results: Two factors were extracted from the transverse, vertical, and anteroposterior skeletal measurements. Latent variables were extracted for each factor. PC1, PC2, and PC3 were selected to analyze the variations of the mandibular symphyseal shape. The SEM was constructed using the skeletal variables, PCs, and latent variables. The SEM showed that the vertical latent variable exerted the most influence on the mandibular symphyseal shape. Conclusions: The relationship between the skeletal pattern and the mandibular symphysis was analyzed using a SEM, which showed that the vertical facial skeletal pattern had the highest effect on the shape of the mandibular symphysis.

악안면 두개골격의 수직${\cdot}$수평적 부조화에 관한 두부방사선 계측학적 연구 (A CEPHALOMETRIC STUDY ON THE VERTICAL AND ANTEROPOSTERIOR DYSPLASIA OF THE CRANIOFACIAL SKELETON)

  • The purposes of the present study were to describe the vortical and anteroposterior facial dysplasia, and to identify morphologic differences associated with various facial patterns. Anteroposterior dysplasia was classified by traditional Angle's malocclusion classification and according to vertical relationships based on the SN-MP angle, facial pattern was subclassified to 3 vertical groups in each malocclusion group. Each vertical group composed of 20 samples and total 180 samples aged from 9 to 14 years. The results were as follows; 1. The skeletal differences that lead to disportionate lower facial height in vortical and anteroposterior facial dysplasia were closely related to mandibular morphology. 2. Hyperdivergent group, compared with hypodivergent group, demonstrated decrease of SNA and SNB and linguoversion of maxillary and mandibular central incisors in all malocclusion groups. 3. Irrespective of rotation of the jaws, Wits appraisal was a useful measurement in evaluation of relative anteroposterior relationship of maxilla and mandible. 4. As SN-MP angle increased, anterior facial height, especially lower anterior facial height, demonstrated significant increase and intermaxillary space also tended to increase. 5. The correlation coefficients of SN-MP angle and PTFH/ATFH demonstrated the highest value in all malocclusion groups.

하악골 전돌증을 동반한 안모비대칭의 유형 분석 (FACIAL ASYMMETRY WITH MANDIBULAR PROGNATHISM - A NEW TRIAL OF CLASSIFICATION AND INTERPRETATION -)

  • Object : Patients with facial asymmetry accompanying mandibular prognathism have various causes and clinical features. So, it is difficult to find a satisfactory treatment method functionally and esthetically. Every traditional classification and interpretation to find etiopathogenesis and/or to establish ideal surgical modality has many limitations because it can't be applied simply to various conditions of patients with facial asymmetry accompanying mandibular prognathism. Therefore, we employ a new classification to interpret more details of the morphologic change of mandible and the spatial change of mandible and maxilla. Materials and Methods : Using panoramic X-ray films, PA cephalograms and submentovertex films of 126 patients diagnosed with facial asymmetry accompanying mandibular prognathism as resources, the following results were gathered after analyzing each characteristics through distributing the patterns according to the morphological mandibular asymmetry and mandibular and maxillary spatial asymmetry. Results : Almost frequency of morphological mandibular asymmetry was shown. In case of condyle-ramus elongation and body elongation group, it's frequency was the highest. Higher frequency of compensating vertical growth was shown on the side of over growing maxilla in case of vertical length difference between left and right condyle-ramus. On the other hand, higher frequency of no compensating vertical growth difference between left and right side was shown in case of no vertical length difference in condyle-ramus. Spatial mandibular asymmetry generally occurred when there was no morphological mandibular asymmetry. Correlation between condyle length difference and condyle-ramus length difference between left and right side was very high, but correlation between condyle length difference and body length difference, and correlation between condyle length difference and body vertical length difference was low. Conclusion : In case of patients with facial asymmetry accompanying mandibular prognathism, it is suggested that various pattern of facial asymmetry is occurred by the independent growth of each unit rather than dependent growth of other unit by major growth unit abnormality. Due to the untypical pattern and the various asymmetry occurring according to the changes of each mandibular growth unit, it is considered that an appropriate surgical method should be searched based on the accurate recognition of the each pattern for patients with facial asymmetry accompanying mandibular prognathism.

비폐쇄를 보이는 III급 부정교합아동의 기도 공간 형태와 안모 골격 형태와의 상관관계 연구 (A study on the correlation between airway space and facial morphology in Class III malocclusion children with nasal obstruction)

  • 본 연구는 비폐쇄를 보이는 III급 부정교합아동에서, 기도 공간의 형태와 안모 골격 형태 사이의 상관관계를 평가하였다. III급 부정교합을 보이며, 비폐쇄 소견을 보여 이비인후과로 의뢰된 환아 100명의 초진 측모 두부 규격 방사선 사진에서 상 인두기도 공간(upper PAS), 하 인두기도 공간(lower PAS), 편도의 크기, 구개-혀 공간을 측정하여 기도 공간 형태를 분석하였으며, 통상적인 계측점을 사용하여 안면 골격 분석을 시행하여 기도 공간 형태 계측항목과 안면 골격분성 항목간의 상관관계를 연구하여 다음과 같은 결과를 얻었다. Upper PAS는 ramal height, SNA, SNB, PFH, FHR, facial plane angle 항목과 양의 상관관계를 보였으며, saddle angle, articular angle, gonial angle의 sum, SN-GoGn, Y-axis to SN, FMA 항목과 음의 상관관계를 보였다. Lower PAS는 genial angle, FMA와 양의 상관관계를 보였으며, articular angle, facial depth, PFH, FHR와 음의 상관관계를 보였다. 편도의 크기는 PCBL, ramal height, Mn. body length, Mn. body length to ACBL, facial depth, facial length, PFH, AFH와 양의 상관관계를 보였다. 구개-혀 공간은 saddle angle, articular angle, genial angle의 합, facial length, AFH, FMA, LFH와 양의 상관관계를 보였으며, IMPA, overbite와 음의 상관관계를 보였다.

수직적 안모형태와 치아성숙도의 상관관계에 대한 연구 (INTERRELATIONSHIP BETWEEN VERTICAL FACIAL TYPE AND DENTAL MATURATION)

  • 본 연구는 수직적 안모형태와 치아성숙도의 관계를 평가하고 이러한 연관성에 대한 사춘기 성장의 영향을 평가하기 위한 것이다. 이번 연구를 위해 7세에서 9세, 13세에서 15세 환자 1306명을 Ricketts의 VERT index를 이용하여 수직적 안모 형태를 분류하였고 환자들의 치령을 Demirjian법으로 평가하여 다음과 같은 결과를 얻었다. 1. 모든 군에서 치령이 역령과 비교 시 통계적으로 유의한 수준에서 크게 측정되었다. 2. 사춘기 연령군에서 brachyfacial type의 경우가 dolichofacial type 보다 치아성숙도가 크게 나타나는 경향을 보였다. 여성의 경우는 통계적으로 유의성있게 차이가 나타났으며, 남성의 경우는 통계적 유의성은 보이지 않았다. 3. Brachyfacial type의 경우 사춘기전 군들과 비교 시 사춘기 군들에서 치아성숙도가 유의성있게 높게 나타났으며, dolichofacial type의 경우는 연령층간 치아성숙도의 차이를 보이지 않았다.

연령에 따른 편측성 순구개열자의 안모형태 변화에 관한 연구 (A STUDY ON THE FACIAL MORPHOLOGY AND GROWTH CHANGES IN UNILATERAL CLEFT LIP AND PALATE PATIENTS ACCORDING TO THE AGES)

  • Orthodontic treatment of cleft patients is difficult as the growth is different from that of normal ones. So it is very important to know the characteristic features of the craniofacial morphology and growth pattern in unilateral cleft lip and palate patients. The materials for this study consisted of 55 normal males and 50 unilateral cleft lip and palate ones who received cheiloplasty and palatoplasty previously. The cleft subjects were divided into 4 groups according to their ages kto find out the growth pattern of hard and soft tissue, and to compare the features with those of normal ones. Each cephalogram analysed by McNamara method and others. The obtained results were as follows 1. In the unilateral cleft lip and palate subjects, forward growth of the maxilla was smaller than that of normal ones from 9 years old. So the maxilla was retruded. The maxillary incisors were severely retruded in all age groups. 2. The mandibular overall length and its anteroposterior position did not show any significant differences between two groups. But the height of ramus was very short and the mandible had vertical growth tendency to compensate for undergrowth of the maxilla in cleft subjects after 12 years of age. 3. Horizontal growth of the soft tissue in middle face was smaller than that of any other facial region from 9 years old. The vertical growth rate of upper lip was decreased as growing old. 4. In cleft subjects, the upper and lower facial component angle and the facial convexity angle were large. So their facial profile changed to straight or concave as growing old.

수직적 안모유헝에 따른 치료 후 교합평면 안정성에 관한 연구 (Post-treatment stability of the occlusal plane according to different vertical facial patterns)

  • 본 연구는 수직적 안모유형에 따른 치료 후 교합평면 변화양상을 조사하여 향후 적절한 치료계획과 기전의 설정에 도움이 되고자 시행하였다. 골격성 I급 부정교합으로 진단되어 비발치로 치료받은 성민 60명(남자 28명, 여자 32명)을 대상으로 하였으며, Ricketts법의 facial axis, facial depth, mandibular plane angle, lower face height, mandibular arc의 5개 항목을 이용하여 한국 성인의 정상교합자 통계치의 기준에 따라 short face type (1군), average face type (2군), long face type (3군)으로 분류하였다. 각 군의 치료 전, 치료 종료, 종료 후 1년의 측모두부 방사선사진 계측치를 비교 분석하였다. 1군은 치료 종료 시와 비교하여 유지기간에 일반적 교합평면각, 기능적 교합 평면각, L6/L1, MP-L6 항목이 유의하게 감소하였고(p < 0.01) L1-FOP 항목은 유의하게 증가하였다(p < 0.001). 2군은 유지기간에 유의한 변화를 보이지 않았으며 3군은 유지기간에 기능적 교합평면각이 유의하게 증가하였다(p < 0.05). 기능적 교합평면각의 치료 후 변화량은 각 군 간에 유의한 차이를 보였으며 특히, 1군과 3군 간에 매우 큰 유의성을 보였다. 따라서 치아의 압하, 정출 및 치료 후 전치부 피개 등에 있어 유지기간 중 교합평면각 변화의 고려가 필요하다고 생각된다.

안모의 성장유형에 따른 경부견인 헤드기어의 효과에 대한 두부방사선계측학적 연구 (Cephalometric study of the effect of cervical pull headgear based on facial growth patterns)

  • 이 연구의 목적은 II급 부정교합의 치료를 위해 경부견인 헤드기어(cervical pull headgear)를 사용할 때, 치료에 불리한 요인으로 작용한다고 언급되는 효과들이 실제로 발현되는지의 여부와 안모의 성장유형에 따라 그 차이가 존재하는 지를 알아 보고자 함에 있다. 이를 위해서 삼성서울병원 치과교정과에서 II급 부정교합으로 진단되어 경부견인 헤드기어로 치료받은 환자중, 성장기에 있으면서 비발치 치료를 받은 환자를 연구대상으로 선별하였다. 그리고 연구대상을 "후안면/전안면 고경비" 에 따라 수직성장군과 수평성장군으로 분류하였다. 수직성장군 15명(남자 5명, 여자 10명)과 수평성장군 11명(남자 4명, 여자 7명)의 치료전, 후의 두부방사선규격사진상에서 SN-PP angle, SN-GoGn angle, SN-MP angle, PFH/AFH ratio, SN to 6, PP to 6 등을 계측하고, 비교분석하여 다음의 결과를 얻었다. 1)수직성장군에서 구개평면이 전하방으로 경사되었다. 2)수평성장군에서 후안면/전안면 고경비가 증가하였다. 3)수직성장군과 수평성장군 모두에서 하악평면각은 변화하지 않았다. 4)상악대구치의 정출양에 있어서 수직성장군과 수평성장군의 차이는 없었다.

한국인 아동의 하악골 성장유형에 따른 안모변화에 관한 누년적 연구 (A LONGITUDINAL STUDY OF KOREAN CHILDREN'S PROFILE CHANGE IN RELATION WITH MANDIBULAR GROWTH PATTERN)

  • Vertical and horizontal growth occur in the craniofacial complex which ensues continuous changes in facial morphology, until the end of active growth period. Longitudinal study for individual is essential, in the research on growth and development, however, the difficulties in obtaining long term subjects in Korea, the research has been limited. The author analyzed the cephalometric roentgenogrems of 43 boys and 47 girls taken from the ages 6 to 10. The subjects were divided into 3 groups according to SN-MP angle and 2 groups according to gonial angle. In this longitudinal study, 21 variables were measure 4. The obtained results were as follows: 1. SN-MP angle and genial angle had no significant changes in each group with age. 2. With age, facial convexity of hard tissue decreased in all groups, facial angle of hard tissue increased in low SN-MP angle group, but facial convexity of soft tissue had no significant changes in all groups with age. 3. In comparison of high SN-MP angle group and low SN-MP angle group, the former had greater facial convexity and smaller facial angle than the latter. 4. SN-MP angle and the ratio of posterior dental height to anterior dental height had reverse correlation in all groups. 5. High genial angle group revealed larger SN-MP angle, anterior dental height facial convexity, but smaller mandibular length, and the ratio of posterior dental height to anterior dental height compared with low genial angle group.
