Clinical Characteristics and Prognostic Factors in Patients with Pulmonary Tuberculosis Admitted to Intensive Care Units (중환자실로 입원한 폐결핵 환자의 임상 양상과 예후 인자)

  • Background: Pulmonary tuberculosis (TB), requiring the intensive care unit (ICU) care, has been a high-mortality condition until now. In the present study, we aimed to investigate clinical features and parameters associated with TB mortality. Methods: From August 2003 to December 2008, patients with microbiologically or histologically confirmed pulmonary TB then admitted to the ICU, were retrospectively enrolled into the study. Upon enrollment, their medical records were reviewed. Results: Forty three patients (30 males, 13 females) were included and their mean age was 63.8 years (range: 17~87 years). Twelve patients died, an overall in-hospital mortality of 27.8%. The main reason for the ICU care was dyspnea or hypoxemia requiring mechanical ventilation (n=17). Other diagnoses for ICU care were hemoptysis, monitoring after procedures, neurologic dysfunction, shock, and gastrointestinal bleeding. On univariate analysis, the factors affecting the mortality were malnutrition-related parameters including low body mass index, hypoalbuminemia, lymphocytopenia, and hypocholersterolemia, as well as severity-related variables such as high acute physiology and chronic health evaluation (APACHE) score, number of involved lobes, and high C-reactive protein. In addition, respiratory failure requiring mechanical ventilation and acute respiratory distress syndrome contributed to patient fatality. It was shown on multivariate analysis that respiratory failure and hypoalbuminemia were significantly independent variables associated with the mortality. Conclusion: Acute respiratory failure is the most common reason for the ICU care and also the most important factor in predicting poor outcome. In addition, our data suggest that the parameters associated with malnutrition could be possible factors contributing to mortality.

  • Recently, the Well-dying Act was legislated in Korea, and it will come into effect in August 4, 2017. This Act allows to withdraw the life sustaining treatment from impending death patients and also provide the hospice and palliative treatment to terminal patients. In the Supreme Court's case so called "Madam Kim", medical condition of Madam Kim was a persistent vegetative status owing to brain damage and her family members wanted to remove the artificial ventilation. In 2009, the Supreme Court allowed to withdraw the artificial ventilation under the specific conditions. We applied this new Well-dying Act to the Madam Kim's case hypothetically in order to know this Act can reasonably solve the problem of life sustaining treatment for dying or terminal patients. For the impending patients, the Well-dying Act has the problem not to withdraw the futile treatment due to the advance directives of patients. Vice versa, the terminal patients have no chance to withdraw the life sustaining treatment due to the this Act impose the duty to provide the hospice and palliative treatment despite of advance directives. We need to ruke out the persistent vegetative patients from the terminal patients caused by the cancer, acquired immune deficiency syndrome, chronic obstructive lung disease and chronic liver cirrhosis, In addition, we have to discuss the effect of the advance directives of terminal patients in view of self determination right.

Effects of Shading Rate and Method of Inside Air Temperature Change in Greenhouse (차광율 및 차광방법이 온실내부의 온도변화에 미치는 영향)

  • This study was conducted to provide basic data for the design of shading facility of greenhouse. The proper distance between external shading screen and roof surface, transmissivity of shading materials, and shading effects of external and internal shadings were analyzed. About a distance of 10 cm between inclined external shading screen and roof surface was enough to guarantee the external shading effect in the greenhouse without roof vent. The inside temperature of greenhouse installed with 85% internal shading screen was lower the maximum of 4$^{\circ}C$ and mean of 2$^{\circ}C$ than that with 55% internal shading screen in both natural ventilation and no ventilation condition. The difference of soil temperature between shading and no shading greenhouse was great, but the difference by shading rate or shading method was small. The performance of external shading for controlling inside temperature down was superior to that of the internal shading. The externally inclined shading screen parallel to the roof surface of greenhouse was more effective than the externally horizontal shading screen in controlling inside temperature of greenhouse without roof vent.

The review about ultra long subsea tunnel design under high water pressure (고수압 초장대 해저터널에 관한 연구)

  • Subsea tunnel needs to be built over 50 km long to connect between nations and continents. However there are only 19 tunnels longer than 5 km until recently. And there is no history of constructing and operating tunnel longer than 50 km. In Korea, subsea tunnels with a length of more than 50 km are being planned, such as Korea~Japan, Korea~China, Honam~Jeju subsea tunnels. Because of the geographical conditions of Korea, most of these tunnels are inter-contry tunnels. So technology preemption for the subsea tunnel construction is getting more and more important. Most of these subsea tunnels are ultra-long tunnels under high water pressure conditions. So new technologies are required such as ventilation and disaster prevention of high-speed tunnels, securing of structural stability under high pressure conditions, and pressure reduction in high-speed conditions. These technologies are different from those of ground tunnels. Therefore, this paper describes the ultra-long subsea tunnel design under high water pressure of maximum 16 bars through the Honam (land) - Jeju (island) virtual subsea project. We proposed a reasonable solution to various problems such as securing structural stability in high pressure condition and ventilation disaster prevention system of ultra long-tunnel.

The Study on Risk Factors Analysis and Improvement of VDT Syndrome in Nuclear Medicine (핵의학과 Video Display Terminals Syndrome 유해 요인 조사 및 개선에 관한 연구)

  • Purpose: Recently, Department of Nuclear Medicine have an interest in Video Display Terminals (VDT) syndrome including musculoskeletal disorders, ophthalmologic disorders, trouble of electromagnetic waves and stress disorders occur to VDT workers as the growing number of users and rapid pace of service period supply in large amount. This study research on the actual condition for VDT syndrome in Nuclear Medicine, Seoul National University Hospital (SNUH), discover the problem and draw a plan of upcoming improvement. The aim of this study establish awareness about VDT syndrome and is to prevent for it in the long run. Materials and Methods: Department of Nuclear Medicine, SNUH is composed Principle part, Pediatric part and PET center. We estimated risk factors visit in each part directly. Estimation method use "Check list for VDT work" of Wonjin working environment health laboratory and check list is condition of VDT work, condition of work tables, condition of chairs, condition of keyboards, condition of monitors, working position, character of health management and other working environment. Analysis result is verified in Department of Occupational and Environment, Hallym University Sacred Heard Hospital. Results: As a result of analysis, VDT condition of Department of Nuclear Medicine, SNUH is rule good. In case of work tables, recent of things are suitable to users upon the ergonomical planning, but 15% of existing work tables are below the standard value. In case of chairs are suitable, but 5% of theirs lost optimum capacity become superannuated. The keyboards are suitable for 98% of standard value. In case of monitors, angle control of screen is possible of all, but positioning control is impossible for 38%. In case of working position, 10% is fixed positioning for long time and some of the items researched unsuitable things for standard. At health management point, needed capable of improvement. Also, other working condition as lighting, temperature, noise and ventilation, discovered the problem, but is sufficient to advice value. Conclusion: VDT syndrome is occurrences of possibility continuously, come economical expensive about improvement, is inherent in various causes and originate without your knowledge. So, there is need systematic management system. In Nuclear Medicine, VDT syndrome make it better that constant interest and effort as improvement of ergonomical working environment, improvement of working procedure, regular exercise and steady stretching, and can be prevented fairly. This promote physical and mental condition of worker in top form in comfortable working environment, so this is judged by enlargement of operations efficiency and rising of satisfaction ratings of the inside client.

UV/H2O2 Advanced Oxidation of Photo Processing Chemicals in a UV-free Reflecting Reactor (사진현상폐수의 UV-자유반사 반응조에서의 UV/H2O2 고급산화처리)

  • In this study, UV-catalyzed $H_2O_2$ oxidation and $H_2O_2$ oxidation to remove contaminants from photo processing chemicals were investigated at various conditions. Photo processing chemicals contains high concentrations of organic compounds and has very low biodegradability. Hydrogen peroxide is subjected to gradual decomposition as metastable substance. In the process, short-lived and highly reactive hydroxyl radicals are formed. The decomposition can be significantly accelerated by use of appropriate catalyst, such as ultraviolet radiation. The experiments were conducted in a UV-free reflecting reactor in batch and a high-pressure mercury lamp was used as UV source. Mixing, cooling and ventilation of the reactor were operated during experiments. In $UV/H_2O_2$ oxidation and $H_2O_2$ oxidation, the removal efficiencies of $COD_{Cr}$, TOC and chromaticity increased with the increase of $H_2O_2$ dosage and were higher in the controlled pH condition of 3 than in original pH condition of 8. In $UV/H_2O_2$ oxidation under the optimum condition of pH 8 and 1.3 stoichiometric $H_2O_2$ dosage, the removal efficiencies of $COD_{Cr}$, TOC and chromaticity were 47.5%, 75.0% and 91.5% respectively and $BOD/COD_{Cr}$ ratio was significantly increased from 0.04 to 0.21.

Occupational Exposure Assessment for Benzene Using Exposure Models (ECETOC TRA and Stoffenmanager) and Applicability Evaluation of Exposure Models in K-REACH (노출 모델의 화평법 적용성: ECETOC TRA와 Stoffenmanager Tier 1 노출 모델을 활용한 벤젠의 작업자 노출 평가)

  • Objectives: The objectives of this study are to estimate the inhalation exposure level of benzene for workers using Tier 1 exposure models ECETOC TRA (European Center for Ecotoxicology and Toxicology of Chemicals Target Risk Assessment) and Stoffenmanager, and to investigate their reliability for exposure assessment in K-REACH. Methods: Two exposure scenarios, 'manufacture of benzene' and 'use as solvents,' were developed for assessment of workers' exposure to benzene. The Process Category (PROC) for ECETOC TRA was collected from the European Chemical Agency (ECHA) registration dossier, and the Activity for Stoffenmanager was converted from PROC using translation of exposure models (TREXMO). The information related to exposure, such as working duration, Respiratory Protective Equipment (RPE), Local Exhaust Ventilation (LEV), and Risk Management Measure (RMM) were classified into high, medium, and low exposure conditions. The risk was determined by the ratio of the estimated exposure and occupational exposure limits of benzene. Results: Under high exposure conditions, the worker exposure level calculated from all PROCs and Activities exceeded the risk level, with the exception of PROC 1 and Activity 1. In the medium exposure condition, PROC 8a, 8b, and 9 and Activity 3, 7, and 8 all exceeded the risk, whereas in the low condition, all PROCs and Activities were determined to be safe. As a result, action corresponding with the low exposure condition is required to reduce the risk of exposure among workers in workplaces where benzene is manufactured or used as a solvent. In addition, the predicted exposure levels derived from the exposure models were lower than measured levels. The exposure levels estimated from Stoffenmanager were more conservative than those from ECETOC TRA. Conclusions: This study demonstrates the feasibility of exposure models for exposure assessment through the example of occupational inhalation exposure assessment for benzene. For more active utilization of exposure models in K-REACH, the exact application of collected information and accurate interpretation of obtained results are necessary.

Influence Factors on Health of Dental Hygienist by Dental Office's Indoor Air Quality (치과위생사의 병원실내공기질에 의한 건강영향요인)

  • This survey was compared and analyzed about the primary factor that dental office's working environment effect on physical subjective symptom and based on self-filling survey, 656 dental hygienists on July through August 2006, and analyzed using descriptive statics, ANOVA and Multiple Regression Analysis. The result of satisfaction degree of hospital working environment was pretty low about office air condition. Most people have complained that office's air quality makes it difficult to their work and mentioned that they were sore and dull all over the back, shoulder, and neck. The odor is major factor to be satisfied with office environment. Proper ventilates the way open the window more often was essential to maintain fresh indoor air quality and keep the extraction materials by separator and sealing tightly for remove the odor. Other factors were temperature, lighting fixture, ventilation facilities, and freshness of air. Dental hygienist was unsatisfied with hospital air condition and this polluted air condition was the cause of physical subjective symptom in work place. Furthermore, this research would be applied for improvement of working environment by decreasing of indoor air pollution.

Patient's Satisfaction with Medical Care Services in Hospital (병원 이용자의 의료서비스 만족도 조사)

  • The purpose of this study was to determine factors influencing patient satisfaction with medical services in hospital, which is classified into environmental aspect, human services and procedural services. Based on the results of literature review, the study focused on effects of social-demographical factors on patient satisfaction. The environmental aspect of medical care services included medical equipment and facilities, hygiene, ventilation, heating and air-conditioning, waiting and resting space, ward space and parking facilities. Procedural service included registration process, bill payment, waiting time after registration, examination and prescription as well as appointment process. Human services consisted of physicians listening to stories of patients, examination duration, physicians' explanation and physicians' service. As for nurses, explanation about disease, examination procedure and results, kindness and nursing care were evaluated. Services provided by other staff members were also evaluated. Patient satisfaction, defined as individual attitude toward medical service as a whole, was measured using a questionnaire. A total of 700 in-or out-patients were surveyed in 6 hospitals with more than 300 beds in North Gyeongbuk Province. 1. The level of patient satisfaction varied with characteristics of patients. Male patients and those in their 30s had a low level of satisfaction. Dissatisfaction level was positively related to education level but negatively related to economic condition. 2. As for patient satisfaction with medical service providers and other employees in hospital, satisfaction level with physician's explanation about treatment was higher. But dissatisfaction levels with treatment duration and the lack of explanation about examination procedures were high, calling for improvement. Dissatisfaction level with nursing care was high, calling for training of nurses for better service. Given the low level of satisfaction with human services, hospital employees need to be trained to improve their service. 3. It Was found that administrative service was also a significant factor influencing patient satisfaction in addition to medical service. It is therefore important for hospitals to provide patients with prompt and convenient procedural service. 4. Environmental factors such as medical equipment and amenity facilities also affected patient satisfaction. Thus environmental condition, procedural service and human service are all important to improve medical service in hospital. In summary, procedural service was the most significant factor for patient satisfaction. The level of satisfaction in patients was also affected by human service and environmental condition. It is therefore necessary to take patient-oriented approach in providing medical service in an effort to improve patient satisfaction. The finding of a lower level of satisfaction with human service signifies the need for training of healthcare providers and other hospital employees for better services. The introduction of advanced management programs is also needed to improve procedures that patients go through in hospitals.

Fog Nozzle-Greenhouse Cooling System Analysis (포그노즐을 이용한 온실냉방시스템 분석)

  • Among the various vegetables eggplant and gourd family can stand against high temperature environmental condition, about 35$^{\circ}C$. However, most of greenhouse farmers are giving up crop cultivation during hot summer season due to extreme temperature, 4$0^{\circ}C$ or above, condition of greenhouse interior. To improve this inferior crop growth condition, for nozzle system was installed in the pet greenhouse and the effect of fog system was investigated in order to determine fog water amount and the required fog nozzle numbers according to house volumes. MEE fog nozzle was selected for this Investigation which can produce water particle size of 27${\mu}{\textrm}{m}$ with water amount of 100$m\ell$ at pumping pressure of 70kg/$\textrm{cm}^2$. House cooling test was conducted in the pet greenhouse with one minute fogging and one minute air ventilation without stopping. It maintained 32$^{\circ}C$ at the house interior when the atmosphere and the house temperature were 35 and 4$0^{\circ}C$, respectively. And, an experimental equation was developed through calculating the changes of relative humidity and temperature with psychrometric equation which revealed the moisture transfer pattern between the house air and fog system. It showed that the required water fogging amounts to reduce 1$0^{\circ}C$, 40 to 3$0^{\circ}C$, needs 80.7$\ell$ for 1-2W(8,350㎥) and 99.9$\ell$ for 3-2G-3S(10,330㎥) type greenhouse with particle size of 27${\mu}{\textrm}{m}$.
