동맥경화도와 생리학적 변수들 간의 상관관계 (Correlation between Arterial Stiffness and Physiological Parameters)

  • Arterial stiffness(AS) is an important pathologic state of vascular injury. This study was carried out to elucidate the effect of physiological variables on brachial-ankle pulse wave velocity(BAPWV), index of AS. Four hundred adults(volunteers) participated in this study. Body indices, biochemical, cardiac and inflammatory markers, and right(Rt)- and left(Lt)-BAPWV were measured. Body mass index(BMI), Rt- and Lt-BAPWV, glucose, triglyceride, alkaline phosphatase(ALP), gamma-glutamyl transferase(GGT), creatinine, uric acid, troponin-I(TNI), NT-proBNP and high sensitivity C-reactive protein(hs-CRP) levels were higher than the reference value of each variable. Rt- and Lt-BAPWV were directly correlated with age, body weight, BMI, glucose, ketone, aspartate aminotransferase, alanine aminotransferase, ALP, GGT, total cholesterol, low density lipoprotein, lipoprotein(a), apolipoprotein-B, blood urea nitrogen, heart rate, TNI, creatine kinase, CK-MB, lactic dehydrogenase, myoglobin, hs-CRP, lipase, reumatoid factor, fibrinogen and D-dimer (P<0.05, P<0.01, P<0.001 or P<0.000, respectively), but inversely associated with total bilirubin, uric acid, apolipoprotein-A1 and GFR (P<0.05). These observations suggest that a variety of physiological variables may influence BAPWV, resulting in increased risk or prevention of cardiovascular and/or cerebrovascular attacks. Therefore, physiological variables affecting BAPWV should be regularly controlled.

The Association of Body Fat and Arterial Stiffness Using the Brachial-Ankle Pulse Wave Velocity

  • Background: BMI alone may not serve as an index of obesity because it does not reflect body composition. The present study aimed to compare arterial stiffness as assessed by the brachial-ankle pulse wave velocity (ba-PWV) among groups defined by body fat percentage (pBF) and BMI. Methods: This cross-sectional study was based on 1,700 participants (1,044 men and 656 women) who completed a health screening examination at a national hospital between January 2011 and February 2016. Participants were divided into four groups according to BMI and pBF: normal fat and normal weight (NFNW); excessive fat and normal weight (EFNW); normal fat and obese (NFO); and excessive fat and obese (EFO). The ba-PWV and other cardiometabolic factors were compared among the four groups in men and women separately. Results: For both sexes, the NFNW group had a lower metabolic risk compared to that in the other groups (EFNW, NFO, and EFO). After adjusting for multiple variables, the NFO males had a significantly lower ba-PWV compared to those in the other groups, including NFNW males. The NFO group had significantly more skeletal muscle mass and muscle mass compared the other groups (P<0.05). Among women, the NFNW group had a significantly lower ba-PWV compared the other groups, even after adjusting for multiple variables. Conclusion: Lower pBF in obese men may be associated with improved cardiovascular risk.

노화에 따른 혈관벽의 피로파괴가 혈관 내의 압력변화에 미치는 영향 (The Simulation of Fatigue Fracture Effect of Arterial Wall on Blood Pressure)

  • The conventional studies of the aging effect on cardio-vascular system are mostly obtained from human experiences and can only provide local information. In this study, the effect of fatigue fracture of arterial wall on the blood pressure was investigated by hemodynamic analysis. The aging process was simulated with three stages. The pulse wave velocity increased with age. The systolic blood pressure increases consistently but diastolic blood pressure decreases with age. The simulated results show in good accordance with the clinical ones.

상완-발목 맥파 전달 속도와 뇌동맥 혈관 협착과의 유의성 평가 (Evaluation of Significance on the Brachial-ankle arterial pulse wave velocity And Cerebral Artery Vascular Stenosis)

  • 뇌혈관 질환은 우리나라의 주요 10대 사망 원인 중 3대 사인에 해당하며, 이러한 질환은 동맥경화와 관련이 있으므로 동맥경화를 예측할 수 있는 인자의 진단 방향의 제시가 필요하다. 본 논문에서는 연령대별 상완-발목동맥 맥파전달속도와 뇌동맥 혈관 협착 및 혈압, 비만도, 복부비만과의 유의성을 평가하였으며, 뇌동맥 혈관 협착과 연령대별 유의성 평가는 50대의 연령대에서만 유의한 결과를 나타내었으며, 동맥경화의 위험인자인 혈압, 비만, 복부비만의 경우 전체 연령대에서 유의한 결과를 나타내었다. 그리고 상완-발목동맥 맥파전달속도가 상승하였을 경우, 뇌동맥 혈관 협착은 전체 1,376명중 788명으로 57.2%의 분포를 나타내었으며, 위험인자인 혈압의 경우, 전체 7,557명중 3,255명으로 43%, 비만도는 2,708로 35.8%, 복부비만은 2,941명으로 38.9%의 분포를 나타내었다. 이러한 결과를 통하여 상완-발목동맥 맥파전달속도 검사 시, 동맥경화가 의심된다면 우선적으로 뇌동맥 혈관 협착을 의심하고 뇌동맥 혈관 검사를 시행할 것을 권한다. 동맥경화의 위험인자인 혈압, 비만, 복부비만과의 유의성 평가를 통하여 향 후 유사 연구 시, 기초자료로 활용될 것이라고 사료된다.

전달함수를 이용한 대동맥 맥파 추정 및 증강점 검출 알고리즘 개선에 관한 연구 (Estimation of the Central Aortic Pulse using Transfer Function and Improvement of an Augmentation Point Detection Algorithm)

  • 대동맥 증강지수는 심실의 부하뿐만 아니라 대동맥의 탄력성을 직접적으로 나타낼 수 있는 장점 때문에 동맥의 경직도를 평가하는 지표로 주목받고 있다. 하지만, 정확한 대동맥 증강지수를 계산하기 위해서는 직접 카테터를 피험자에 삽입하여 측정해야 하기 때문에 임상에 적용하기에는 한계가 존재한다. 이러한 문제점 때문에 전달함수를 이용하여 요골 동맥 맥파로부터 대동맥 맥파를 간접적으로 추정하는 방법이 이용되고 있다. 본 논문에서는 전달함수를 구하기 위하여 Millar 카테터를 이용한대동맥 맥파와 토노메트릭 방식의 압력센서를 이용하여 요골동맥 맥파를 측정하였다. 또한, 기존의 증강점 검출 알고리즘 대신단계적으로 미분 차수를 증가시키면서 증강점을 검출하는 새로운 알고리즘을 제안하였다. 10차 ARX 모델을 이용하여 전달함수를 구현하였으며, 잔차 분석을 통하여 모델을 검증하였다. 증강점 검출 알고리즘 검증을 위하여 네 가지 종류의 합성파를 만들어 제안된 알고리즘이 기존 알고리즘 보다 더 정확한 결과를 나타내는 것을 확인할 수 있었다. 본 연구는 쉽게 측정할 수 있는 요골동맥 맥파를 이용하여 대동맥의 경직도를 평가할 수 있는 방법을 제시하였으며 이를 통하여 다양한 심혈관 질환의 조기 진단에 기여할 수 있을 것이다.

정맥 문합 만을 위한 교차-하지 유리 피판술을 이용한 하지의 재건 (Lower Extremity Reconstruction with Cross-Leg Free Flap Only for Vein Anastomosis)

  • This study was designed to introduce the cross-leg free flap only for vein anastomosis as an alternative salvage method for the reconstruction of severe soft tissue defects in vascular-compromised lower extremities. Four cross-leg free flap reconstructions were performed using the latissimus dorsi muscle to reconstruct soft tissue defects in the lower extremity. The recipient artery was confined to the ipsilateral side and the venous anastomosis was carried out in the contralateral side. Both legs were immobilized together with an external fixator. All patients were males, and had a mean age of 31 years. The mean time of pedicle division was 8.8 days range of 7 to 10 days. The mean size of the flap was 186.5 cm2. All flaps survived after pedicle division without venous congestion. There was no complication in joint stiffness, nor donor site morbidity except for a linear scar. The cross-leg free flap only for vein anastomosis is a refinement of a salvage procedure used for the reconstruction of severe soft tissue defects in vascular-compromised lower extremities.

복합운동이 고령여성의 혈중 염증인자, DHEA-s 및 동맥경직도에 미치는 영향 (Effects of combined exercise on the blood inflammatory factors, DHEA-s and arterial stiffness of elderly women)

  • 본 연구는 만 70~85세의 고령여성을 대상으로 주 1회 아쿠아로빅과 주 2회 맨손근력운동을 실시하는 복합운동프로그램을 12주간 적용하여 혈중 염증인자, DHEA-s 및 동맥경직도에 미치는 영향을 구명하기 위하여 운동군 21명, 대조군 21명으로 구분하여 실시하였다. 1회 운동지속시간은 60분으로 4주 간격으로 운동강도를 점진적으로 증가시켜 실시하였다. 복합운동 전과 후에 측정된 자료의 상호작용 효과 검증을 위해 이원반복측정 분산분석을 실시하였고, T검정을 사용하여 사후분석을 하였으며, 유의수준은 .05로 설정하여 분석하였다. 그 결과 12주간의 복합운동은 고령여성의 염증반응을 감소 시켰으며, DHEA-s가 증가함에 따라 노화호르몬에도 긍정적인 영향을 미치는 것으로 나타났다. 동맥경직도는 중심동맥혈압(수축기, 이완기)이 감소하였고, 맥압 및 파형증가지수의 증가를 완화하였으며, 맥파전파속도를 감소시키는 것으로 나타났다. 이상의 결과 고령여성의 건강한 노화와 장수를 위해서 항염증, 노화호르몬의 개선 및 혈관건강의 역할로 규칙적이고 지속적인 복합운동이 권장 될 수 있을 것으로 사료된다.

The effect of oral glucose tolerance testing on changes in arterial stiffness and blood pressure in elderly women with hypertension and relationships between the stage of diabetes and physical fitness levels

  • [Purpose] The purpose of this study was to assess changes in blood glucose level, blood pressure, and arterial stiffness after a 75 g oral glucose tolerance test (OGTT) in elderly women aged over 65 years with hypertension and either normal glycemic control, impaired fasting glucose tolerance, or diabetes mellitus. We also wished to investigate the relationship between stages of diabetes and physical fitness. [Methods] A total of 24 elderly women with hypertension were assigned to a control group (CON; n=7), impaired fasting glucose group (IFG; n=9), and diabetes mellitus group (DM; n=8). In each group, blood glucose level, brachial ankle pulse wave velocity (PWV), and blood pressure were measured at baseline as well as 60 and 120 minutes after a 75 g OGTT. Physical fitness factors such as hand grip strength, balance test, 4 m gait speed test, chair stand test, short physical performance battery, and 6-minute walking test were subsequently assessed. [Results] In all three groups, blood glucose levels were significantly increased at 60 and 120 minutes after a 75 g OGTT. In the DM group, blood glucose levels were significantly higher before and after a 75 g OGTT than in the CON group. In the CON group, PWV was significantly increased at 60 minutes after a 75 g OGTT; however, there were no changes in other groups after glucose ingestion. In the CON group, systolic and diastolic blood pressures were significantly decreased at 60 and 120 minutes after a 75 g OGTT compared to baseline. However, there was no change in blood pressure after ingestion in the DM group. The IFG group had greater grip strength than the CON group; however, there were no differences in other variables between the groups. [Conclusion] After a 75 g OGTT, elderly women with hypertension and diabetes maintain higher blood glucose levels compared to those with hypertension alone. Unlike elderly women with hypertension alone, those with hypertension and diabetes did not show changes in arterial stiffness and blood pressure after a 75 g OGTT. Therefore, elderly women with hypertension and diabetes may not be able to control their blood vessels following a 75 g OGTT due to impaired vascular endothelial function. Moreover, there was no association between diabetes stage and physical fitness in elderly women with hypertension.

Elevation of heart-femoral pulse wave velocity by short-term low sodium diet followed by high sodium diet in hypertensive patients with sodium sensitivity

  • BACKGROUND/OBJECTIVES: We compared changes in heart-femoral pulse wave velocity (hfPWV) in response to low sodium and high sodium diet between individuals with sodium sensitivity (SS) and resistance (SR) to evaluate the influence of sodium intake on arterial stiffness. SUBJECTS/METHODS: Thirty-one hypertensive and 70 normotensive individuals were given 7 days of low sodium dietary approach to stop hypertension (DASH) diet (LSD, 100 mmol NaCl/day) followed by 7 days of high sodium DASH diet (HSD, 300 mmol NaCl/day) during 2 weeks of hospitalization. The hfPWV was measured and compared after the LSD and HSD. RESULTS: The hfPWV was significantly elevated from LSD to HSD in individuals with SS (P = 0.001) independently of changes in mean arterial pressure (P = 0.037). Conversely, there was no significant elevation of hfPWV from LSD to HSD in individuals with SR. The percent change in hfPWV from the LSD to the HSD in individuals with SS was higher than that in individuals with SR. Subgroup analysis revealed that individuals with both SS and hypertension showed significant elevation of hfPWV from LSD to HSD upon adjusted analysis using changes of the means arterial pressure (P = 0.040). However, there was no significant elevation of hfPWV in individuals with SS and normotension. CONCLUSION: High sodium intake elevated hfPWV in hypertensive individuals with SS, suggesting that high sodium intake increases aortic stiffness, and may contribute to enhanced cardiovascular risk in hypertensive individuals with SS.

The Correlation of Serum Osteoprotegerin with Non-Traditional Cardiovascular Risk Factors and Arterial Stiffness in Patients with Pre-Dialysis Chronic Kidney Disease: Results from the KNOW-CKD Study

  • Background: Osteoprotegerin (OPG) plays protective roles against the development of vascular calcification (VC) which greatly contributes to the increased cardiovascular events in patients with chronic kidney disease (CKD). The present study aimed to find the non-traditional, kidney-related cardiovascular risk factors correlated to serum OPG and the effect of serum OPG on the arterial stiffness measured by brachial ankle pulse wave velocity (baPWV) in patients with the pre-dialysis CKD. Methods: We cross-sectionally analyzed the data from the patients in whom baPWV and the serum OPG were measured at the time of enrollment in a prospective pre-dialysis CKD cohort study in Korea. Results: Along with traditional cardiovascular risk factors such as age, diabetes mellitus, pulse pressure, and baPWV, non-traditional, kidney-related factors such as albuminuria, plasma level of hemoglobin, total $CO_2$ content, alkaline phosphatase, and corrected calcium were independent variables for serum OPG in multivariate linear regression. Reciprocally, the serum OPG was positively associated with baPWV in multivariate linear regression. The baPWV in the 3rd and 4th quartile groups of serum OPG were higher than that in the 1st quartile group after adjustments by age, sex and other significant factors for baPWV in linear mixed model. Conclusion: Non-traditional, kidney-related cardiovascular risk factors in addition to traditional cardiovascular risk factors were related to serum level of OPG in CKD. Serum OPG level was significantly related to baPWV. Our study suggests that kidney-related factors involved in CKD-specific pathways for VC play a role in the increased secretion of OPG into circulation in patients with CKD.
