Purpose: The aim of this study was to investigate the efficacy and validity of subgingival bacterial sampling using a retraction cord, and to evaluate how well this sampling method reflected changes in periodontal conditions after periodontal therapy. Methods: Based on clinical examinations, 87 subjects were divided into a healthy group (n=40) and a periodontitis group (n=47). Clinical measurements were obtained from all subjects including periodontal probing depth (PD), bleeding on probing (BOP), clinical attachment loss (CAL), and the plaque index. Saliva and gingival crevicular fluid (GCF) as a subgingival bacterial sample were sampled before and 3 months after periodontal therapy. The salivary and subgingival bacterial samples were analyzed by reverse-transcription polymerase chain reaction to quantify the following 11 periodontal pathogens: Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Tannerella forsythus (Tf), Treponema denticola (Td), Prevotella intermedia (Pi), Fusobacterium nucleatum (Fn), Pavimonas micra (Pm), Campylobacter rectus (Cr), Prevotella nigrescens (Pn), Eikenella corrodens (Ec), and Eubacterium nodatum (En). Results: Non-surgical periodontal therapy resulted in significant decreases in PD (P<0.01), CAL (P<0.01), and BOP (P<0.05) after 3 months. Four species (Pg, Tf, Pi, and Pm) were significantly more abundant in both types of samples in the periodontitis group than in the healthy group. After periodontal therapy, Cr was the only bacterium that showed a statistically significant decrease in saliva, whereas statistically significant decreases in Cr, Pg, and Pn were found in GCF. Conclusions: Salivary and subgingival bacterial sampling with a gingival retraction cord were found to be equivalent in terms of their accuracy for differentiating periodontitis, but GCF reflected changes in bacterial abundance after periodontal therapy more sensitively than saliva.
Purposes: The objectives of this study present the direction of the criteria for the separately reimbursement of therapeutic medical device. Methodology: We summarized experts' opinion using Delphi survey and Analytic Hierarchy Process(AHP). 48 experts were gathered from Medical Insurance Review Nurses Association, medical device industry, academy and association, Medical Device Expert Evaluation Committee. Descriptive statistics, consistency index, content validity ratio were analyzed. Findings: Clinical utility, patient safety, infection control, cost-homogeneity, cost-effectiveness showed high feasibility and importance, but market contribution and functional utility showed low feasibility and importance in a relative sense. The results of functional utility differed between clinical and non-clinical experts. Measurability was low across the whole area. Among the criteria for the separately reimbursement of therapeutic medical device. Patient safety/infection control and clinical utility showed the highest relative importance values, analyzed using AHP. Practical Implications: Patient safety and infection control are needed to be considered as one of Value Assessment Criteria. It is important to find out how to improve the measurability of therapeutic medical device.
본 연구는 인체 골격의 좌표형 임상용어체계 표준을 개발하기 위한 방법론적 연구이다. 문헌고찰과 자료 수집을 통해 연구 계획을 수립하여 예비 표준(안)을 만들고 전문가 세미나와 자문을 통해 수정 표준(안)을 만든 후 내용타당도 검증 후 최종(안)을 제시하는 4단계를 거쳐 표준을 개발하였다. 좌표형 임상용어체계 표준은 인체 이미지를 2D는 x, y축, 3D는 x,y,z축으로 좌표화하고, 좌표의 개념과 정의는 SNOMED CT의 FSN, Synonym, Preferred name으로 선조합하고 후조합은 개발 된 18개의 Relationship을 통해 정의되고, 개발된 Relationship 표준의 내용타당도 지수는 평균 4.01점이었다. 본 연구를 통해 인체 골격 이외의 뇌, 장기, 조직 등의 인체의 다른 부분에 대한 후속 표준 개발을 제언하고 임상에서 활용성을 높이기 위한 방법 연구를 제언한다.
BACKGROUND/OBJECTIVES: This study was designed to provide scientific evidence on the effectiveness of worksite-based dietary intervention to reduce obesity among overweight/obese employees. MATERIALS/METHODS: Electronic search was performed using Ovid Medline, Embase, Cochrane Library, and CINAHL databases. The keywords used were "obesity," "nutrition therapy," and "worksite." The internal validity of the randomized controlled trials (RCTs) was assessed using the Cochrane's Risk of Bias. Meta-analysis of selected studies was performed using Review Manager 5.3. RESULTS: A total of seven RCTs with 2,854 participants were identified. The effectiveness of dietary interventions was analyzed in terms of changes in weight, body mass index (BMI), total cholesterol, and blood pressure. The results showed that weight decreased with weighted mean difference (WMD) of -4.37 (95% confidence interval (CI): -6.54 to -2.20), but the effectiveness was statistically significant only in short-term programs < 6 months (P = 0.001). BMI also decreased with WMD of -1.26 (95% CI: -1.98 to -0.55), but the effectiveness was statistically significant only in short-term programs < 6 months (P = 0.001). Total cholesterol decreased with WMD of -5.57 (95% CI: -9.07 to -2.07) mg/dL, demonstrating significant effectiveness (P = 0.002). Both systolic (WMD: -4.90 mmHg) and diastolic (WMD: -2.88 mmHg) blood pressure decreased, demonstrating effectiveness, but with no statistical significance. CONCLUSIONS: The worksite-based dietary interventions for overweight/obese employees showed modest short-term effects. These interventions can be considered successful because weight loss was below approximately 5-10 kg of the initial body weight, which is the threshold for the management of obesity recommended by the Scottish Intercollegiate Guideline Network (SIGN).
The power quality of Single Phase Grid-Connected Inverters (GCIs) has received much attention with the increasing number of Distributed Generation (DG) systems. However, the performance of single phase GCIs get degraded due to several factors such as the grid voltage harmonics, the dead time effect, and the turn ON/OFF of the switches, which causes the harmonics at the output of GCIs. Therefore, it is not easy to satisfy the harmonic standards such as IEEE 519 and P1547 without the help of harmonic compensator. To meet the harmonic standards a certain kind of harmonic controller needs to be added to the current control loop to effectively mitigate the low order harmonics. In this paper, the harmonic compensation is performed using a novel robust harmonic compensation method based on Digital Lock-in Amplifier (DLA). In the proposed technique, DLAs are used to extract the amplitude and phase information of the harmonics from the output current and compensate it by using a simple PI controller in the feedforward manner. In order to show the superior performance of the proposed harmonic compensation technique, it is compared with those of conventional harmonic compensation methods in terms of the effectiveness of harmonic elimination, complexity, and implementation. The validity of the proposed harmonic compensation techniques for the single phase GCIs is verified through the experimental results with a 5kW single phase GCI. Objectives This study is aimed to develop a standard instrument of diagnosis and assessment for the original symptom of Taeeumin. Methods 1. Based on the clinical practice guidelines, the works of Dong-mu, and the expert clinical judgment of the research team, the items to be used in the instrument of diagnosis and assessment for the original symptom of Taeeumin were extracted, and translation was proceeded in plain Korean for easy application and feasible use in daily clinical practice. As a result, candidate items for the diagnosis and assessment tool were derived. 2. An online questionnaire survey was conducted on the candidate items derived through the above process to the expert advisory group for (1) inclusion/exclusion as diagnosis and assessment index (2) evaluation of importance and (3) the validity of translation. Results & Conclusions Through survey of expert advisory group, the number of candidate items were reduced and the preliminary weights were assigned. And based on this, was developed. There were a total of 9 items for Exterior Cold[Wiwanhan] original symptom diagnosis and assessment tool, 21 items for Interior Heat[Ganyeol] original symptom, 15 items for Exterior Disease Unfavorable Patterns[Wiwanhan-paejo] original symptom, and 17 items for Interior Disease Unfavorable Patterns[Ganyeol-paejo] original symptom, and the weights were reflected in each response score so that the final total score could be calculated.
Objective: This clinical study describes the effect of Korean medicine on a patient with Delayed Neuropsychiatric Sequelae after Carbon Monoxide Poisoning. Case presentation: A patient with the delayed neuropsychiatric effects of carbon monoxide exposure was treated with herbal medicine, acupuncture, moxibustion, and cupping. Clinical symptoms were measured with the Korean Mini Mental State Examination (K-MMSE), Functional Independence Measure (FIM), and the Modified Barthel Index (MBI). After 32 days of treatment, the patient's K-MMSE score increased from 11 to 21 points, the FIM score from 52 to 94 points, and the MBI score from 46 to 84 points. There were no side effects. Conclusion: According to this case study, Korean medicine may be considered an effective treatment for delayed neuropsychiatric sequelae, although more studies are needed to confirm its validity.
Background: The vulnerability of international migrant workers is on the rise, affecting the frequency of occupational accidents at workplaces worldwide. If migrant workers are managed in the same way as native workers, the consequences on safety assurance and risk management will be significant. This study aimed to develop the vulnerability factor model for migrant workers in seafood processing industries because of significant risk-laden labor of Thailand, which could be a solution to control the risk effectively. Methods: A total of 569 migrant workers were surveyed (432 Burmese and 137 Cambodian), beginning with 40 initial vulnerability factors identified in the questionnaire established from experts. The data were analyzed through descriptive analysis; exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were used to ascertain the model. Results: The result of content validity >0.67 and the Cronbach's alpha of 0.957 specified the high reliability of 40 factors. The EFA indicated a total variance of 65.49%. The final CFA validated the model and had an empirical fitting; chi-square = 85.34, Adjust Goodness-of-Fit Index = 0.96, and root mean square error of approximation = 0.016. The structure concluded with three dimensions and 18 factors. Dimension 1 of the structure, "multicultural safety operation," contained 12 factors; Dimension 2, "wellbeing," contained four factors; and Dimension 3, "communication technology," contained two factors. Conclusion: The vulnerability factor structure developed in this study included three dimensions and 18 factors that were significantly empirical. The knowledge enhanced safety management in the context of vulnerability factor structure for migrant workers at the workplace.
주파수 및 시간영역 유도분극 탐사는 지하 매질의 분광 정보를 포함하고 있다. 분광 특성의 분석은 주로 주파수영역 유도분극 탐사에서 연구되어 왔으나, 근래에 시간영역 유도분극에서도 연구가 활발하게 이루어지고 있다. 이 연구에서는 반무한 균질 공간을 가정하고 측정된 2차 전위 및 전기비저항으로부터 Cole-Cole 변수를 추정하는 역산법을 개발하였다. 충전성, 완화시간 및 주파수 승수로 구성되는 Cole-Cole 변수들은 비독립적이기 때문에 통상적인 비선형 역산을 적용할 경우 느린 수렴속도, 적정 초기 모델 설정의 어려움, 지역 극소점, 발산 위험 등의 다양한 문제점이 발생한다. 이 연구에서는 격자 탐색법을 도입하여 참 모델에 근접한 초기 모델을 설정하는 효과적인 역산법을 개발하였다. 마지막으로 다양한 역산 실험을 통하여 개발된 역산법의 타당성을 검증하였다.
이 연구는 작은도서관의 활성화와 내실화를 위해 현행 작은도서관 실태조사 및 평가 결과 달성률 분석과 표적집단 인터뷰를 통하여 현행 평가지표의 유효성을 검증하였다. 이를 위해 현행 작은도서관 평가지표 문항을 검토하고, 시간 흐름에 따른 평가 점수와 항목의 차이를 살펴보기 위하여 2018년부터 2020년까지 총 19,476개관의 실태조사 데이터를 조사하여 평가 영역별 달성률을 분석하였다. 또한, 작은도서관 관련 단체와 담당자를 전문가로 선정하고 표적집단 인터뷰를 진행하여 현행 평가지표의 문제점과 개선사항에 대한 논의와 의견을 수렴하였다. 이를 통해 도출한 시사점은 다음과 같다. 첫째, 작은도서관 평가는 외적인 환경 평가보다는 내적인 질적 서비스 위주의 평가가 선행되어야 한다. 둘째, 체계적인 증빙이 가능한 지표를 설계하고, 용어의 모호성을 해소하기 위한 명확한 설명을 첨부해야 한다. 셋째, 공립작은도서관과 사립작은도서관의 평가 문항의 구분과 지방자체단체의 직접적인 평가가 필요하다. 넷째, 이용자들의 만족도를 측정할 수 있는 정성 평가 요소가 추가되어야 한다.
