A Study on the Effect of Aroma Massage Therapy on Normal Vaginal Delivery (아로마 마사지요법이 초산모의 정상분만에 미치는 영향)

  • Objective : As this is a quasi-experimental study by the post-test design for a nonequivalent control group, it was attempted to verify the effect of massage using aroma oil on whether or not the normal vaginal delivery of primiparous mothers. Methods : As for the experimental treatment, it applied the aroma oil massage to the experiment group when the cervical dilatation was $4{\sim}5cm$, and as for the data analysis, it conducted the significance test using the program of SPSS 12K for Windows, with Chi-square test $(X^2)$ and t-test. Results : The experiment group that carried out the aroma massage therapy, was high in the cases of normal vaginal delivery(P<0.05), compared to the control group that did not carry out the aroma massage therapy. Conclusion : The experiment group, which received the aroma massage therapy, had significant difference in the cases of normal vaginal delivery, compared to the control group that did not receive the aroma therapy, thus the aroma massage therapy is thought to be a nursing-intervention plan, which can be usefully applied as to the primiparous mothers aiming at normal vaginal delivery.

A Comparative Study on the Effects on Urinary Incontinence between Pelvic Floor Muscle Exercise and Magnetic Stimulation Therapy (지역사회 요실금 여성의 골반저근운동과 자기장자극 효과비교)

  • Purpose: To Perform a randomized comparative study investigating the effects of Pelvic Floor Muscle Exercise(PME) and Magnetic Stimulation Therapy(MST) and to identified the problems in each of PME & MST Method: Forty-nine patients with mild stress incontinence were randomly assigned to either of two treatment groups (24 patients in the PME group and 25 in the MST group). The PME group had a video exercising program for 40 times every day during 6 weeks. The MST group was treated with BioCon-2000TM, 2times/ week for six weeks. Pre-test and post-test were performed by Prineometer, 1-hour pad test. and Jackson's BFLUTS questionnaire of Jackson. Collected data were analysed using SAS 9.1 by frequency, Kolmogorov-Smirnov Z. Chi Square-test, t-test, Fisher Exact probability test, Paired t-test, and Wilcoxon's rank sum test. Results: In comparison between before and after PME and MST, statistically significant difference was observed in maximal vaginal pressure, duration of vaginal contraction, amount of urine, and symptom of urination. In the comparison of the effectiveness between PME & MST, only the maximal of vaginal pressure (Z=2.58, p= .010) was significantly different. Conclusion: The factor more effective in the MST group than in the PME group was high compliance.

A Comparison of Medical Care Services by Type of Medical Care Facility -In cases of normal spontaneous vaginal delivery and acute appendicitis- (의료기관 종류별 진료내역 비교 -정상분만과 급성 충수염을 중심으로-)

  • To find out the differences in medical practice according to the environment of patient care and managerial situation of the medical care institutions, normal spontaneous vaginal delivery and acute appendicitis were selected, which is suitable for comparison because of their high comparability. A total of 473 cases of normal spontaneous vaginal delivery and 408 cases of acute appendicitis was sampled from the claims to Korea Medical Insurance Cooperations during January to June 1984. Complicated cases were excluded from population and sampling was restricted from 40 to 60 percentile for total charges by the type of medical care facility in order to rule out the influence originating from case mix. Important items representing type and quantity of medical care service were compared by type of facility. Major findings are as follows : 1. University hospital shows the highest in charges per case and decrease in order of general hospital, and clinic. 2. In case of normal spontaneous vaginal delivery, average length of stay shows statistically significant difference by type of facility. 3. Charge amount for each service item affected by practice pattern shows statistically significant difference mostly by type of facility. It is suggested that medical practice pattern is different by type of facility for medical services. 4. Difference in total medical expenditure by type of facility is affected more with charges for materials, consumables and drugs than with fee for service activity. 5. In administering drugs to patients, hospital and clinic show higher injection rate than university and general hospital. 6. Clinical Laboratory tests were common in order of uninalysis, hemoglobin, hematocrit, white blood cell count, urine microscopic examination in cases of normal spontaneous vaginal delivery; white blood cell count, urinalysis, hemoglobin, hematocrit, urine microscopic examination, white blood cell differential count, in cases of acute appendicitis. 7. The result for Laboratory test and Radiologic study shows extreme difference by type of facility. Test rate is lowest in clinic and increase hospital, general hospital, and university hospital in order, both in type and frequency.

The Evaluation of 10 week urinary Tract Function Promotion Program for the Elderly Women in the Community (노인 여성의 요로기능증진 10주 프로그램 개발 및 평가)

  • Purpose: This study was accomplished to apply and to evaluate the 10-week Urinary tract Function Promotion for the Elderly Women (UFPE), and to suggest guideline and to be utilized on the community level. Method: Subjects were 30 persons of 16 healthy elderly and 14 hemiplegic elderly. This UFPE was composed of diagnosis of incontinence & evaluation of physical condition, understanding of urogenital system & urinary incontinence management. UFPE was evaluated by subjective urinary condition, intra-vaginal contraction power, continence self-efficacy (CSE), incontinence stress, geriatric depression. The collected data was analyzed using Wilcoxon Matched Signed-Ranks test by SPSS/WIN program. Result: 5 subjects (31.2%) in healthy group and 2 subjects(14.3%) in hemiplegic group have a daytime frequency, 14 subjects(87.5%) in healthy- group and 14 subjects(100%) have a nocturia. After program, subjective urinary condition was increased just only healthy elderly (Z=-2.545, p<0.05), while intra-vaginal contraction power and CSE were increased significantly only in the hemiplegic elderly (Z=-2.57, P<0.05: Z=-2.29, p<0.05). There were barriers not to comply program guidance such as inadequate perception of pelvic floor muscle, forget to do exercise, fatigue. Conclusion: UFPE was effective in increasing subjective urinary condition for healthy elderly and in increasing intra-vaginal contraction power and continence self-efficacy for the hemiplegic elderly women. I suggest that this UFPE be utilized at the health center and elderly center, and public health nurse counsel and guide the elderly's work through phone service.

Stress in pregnant women and the effect of cesarean delivery on anxiety and subjective anxiety statuses (임부의 스트레스 정도와 응급제왕절개분만시 상태불안과 주관적 불안에 관한 연구)

  • Purpose: This study was a descriptive survey research that aimed to investigate the stress level of pregnant women and subsequent effect of emergency cesarean delivery on anxiety and subjective anxiety statuses. Methods: The study samples were 233, including 109 emergency cesarean delivery and 124 normal vaginal deliveries between May 1, 2014, and August 26, 2014, in the Gwangju Metropolitan City. Results: None of the results showed any significant statistical difference in psychological stress between emergency cesarean delivery during mid-pregnancy and normal vaginal delivery (t = 1.784, p = .076). Emergency cesarean delivery has a significantly high level of anxiety (t = 10.849, p < .001) and subjective anxiety statuses (t = 13.294, p < .001) compared with normal vaginal delivery. Conclusion: A prenatal education program for stress and anxiety from emergency cesarean delivery needs to be developed for more effective stress management.

Maternal Factors Affecting Delivery Mode of the Previous Cesarean Delivery Mothers (선행 제왕절개분만 산모의 제왕절개 후 질식분만과 반복 제왕절개분만 관련 요인)

  • Purpose: To examine delivery type of mother who have had a previous cesarean and identify maternal factors related to type of delivery. Methods: The study sample included 60,504 mothers who had delivered through cesarean section. Related variables were categorized as sociodemographic factors (age, residence, health insurance type, income level) and clinical characteristics (14 maternal factor, 4 fetal factor and pre-term). For data analysis, $x^2$ and multivariate logistic regression were conducted. Results: Among the 60,504mothers, 3,075 were delivered through Vaginal Birth After C-Section (VBAC) and the VBAC rate was 5.1%. Underage 34, the VBAC rate increased according to age increases up to 3%. Mothers residing in urban areas had VBAC more frequently than mothers in rural area. Mothers in the high and middle income levels had a greater possibility of having VBAC than mothers in lower income levels. A greater likelihood of increase in repeated cesarean section were found in mothers with maternal and fetal factors. Conclusion: Evidence based nursing practice guidelines and education programs for previous cesarean section mothers and health policy are needed to increase VBAC.

Personal Hygiene Practices related to Genito-urinary Tract and Menstrual Hygiene Management in Female Adolescents (여자 청소년의 비뇨생식기 관련 개인위생과 월경기 위생 관리)

  • Purpose: The study was to survey personal hygiene practice related to genito-urinary tract and menstrual hygiene management in female adolescents in order to obtain basic information for health education. Methods: With a descriptive survey design, 389 adolescents were recruited via convenience sampling in Korea. Survey instrument was the feminine and menstrual hygiene practice and perception of vaginal douching. Data were collected from a self-administered structured questionnaire. Results: Mean age of adolescents was 16.09 and menarche was at 13.21. While washing hands after urination/defecation was highly performed, wipe front to back and wash with soap and water were reported as being not well done. Twenty-eight percent reported douching habit. More positive beliefs about douching were reported by adolescents who practiced douching. Menstrual hygiene management was very appropriate with changing sanitary pads regularly with hand washing; but less performed for limiting bathing activity during menstrual periods and washing hands after activity of genito-urinary area. Conclusion: Some adolescents practiced inadequate hygiene practices especially for body cleansing during menstrual period and vaginal douching. It is important to develop and implement school health education programs on feminine and personal hygiene for adolescents to help them perform adequate health behaviors.

Effect of Oriental Postpartum Management on the Hematology and Blood Chemistry in Puerperium Maternity (한방산후조리가 산욕기 산모의 혈액 및 생화학검사에 미치는 영향)

  • Objectives : As participation of women in public affairs increases, they are more concerned about postpartum management. However, objectivity of its effect tends to be insufficient. This study was done to investigate the clinical applications of postpartum management in Oriental Medicine. Subjects and methods : This clinical study was done on 74 patients who were treated by postpartum management in the Oriental Medical Hospital of Woosuk University from November 1999 to August 2000. We analyzed the changes of hematology and blood chemistry. Then, we examined the differences between two groups : one group had undergone vaginal duct delivery and the other cesarean section in mode of delivery. Results : I) The ratio of vaginal duct delivery to cesarean section was 39:35. Two major age groups were 25-29 and 30-34, respectively 43% and 35%. 2) According to the hematology, hemoglobin, hematocrit, REC and platelet count significantly increased, while WBC and ESR level significantly decreased. 3) To the hematology by mode of delivery, WBC level decreased in both groups. WBC level of the vaginal duct delivery group was significantly lower than that of cesarean section. RBC level increased in both groups, the cesarean section group were statistically significant. Hemoglobin, hematocrit and platelet count increased but they were not significant in either group. 4) To the blood chemistry, Triglyceride (TG), protein, albumin and sodium levels were significantly increased but cholesterol, ALT, BUN, creatinine, potassium and chloride levels were not statistically significant. 5) According to the blood chemistry by mode of delivery, TG level of the vaginal duct delivery group was significantly reduced but the others were not significant. Conclusious : The effective results were shown that postpartum management by Oriental Medicine assisted postpartum health care as well as postpartum anemia.

Malignant Tumors of the Female Reproductive System

  • This review summarizes the epidemiology of cancer of the female reproductive system and associated lifestyle factors. It also assesses the available evidence for occupational factors associated with these cancers. Cervical, endometrial, and ovarian cancers are relatively common, and cause significant cancer morbidity and mortality worldwide, whereas vulvar, vaginal, fallopian tube cancers, and choriocarcinomas are very rare. As several lifestyle factors are known to play a major role in the etiology of these cancers, very few published studies have investigated possible relationships with occupational factors. Some occupational exposures have been associated with increased risks of these cancers, but apart from the available evidence on the relationships between asbestos fibers and ovarian cancer, and tetrachloroethylene and cervical cancer, the data is rather scarce. Given the multifactorial nature of cancers of the female reproductive system, it is of the utmost importance to conduct occupational studies that will gather detailed data on potential individual confounding factors, in particular reproductive history and other factors that influence the body's hormonal environment, together with information on socio-economic status and lifestyle factors, including physical activity from multiple sources. Studies on the mechanisms of carcinogenesis in the female reproductive organs are also needed in order to elucidate the possible role of chemical exposures in the development of these cancers.

Effect of Lactobacilli Oral Supplement on the Vaginal Microflora of Antibiotic Treated Patients: Randomized, Placebo-Controlled Study

  • Many antibiotic monographs cite the induction of vaginal infections as a possible side effect. Invariably, this is believed to be due to Candide albicans, and empirical therapy is given. However, recent studies raise the question of the extent to which yeast do infect the host after antibiotic use. A double-blind, randomized, placebo-controlled study was undertaken on female patients to determine how many yeast infections occurred following 10 days antibiotic use. In addition, the study was designed to examine whether oval use of probiotic lactobacilli can reduce the risk of vaginal infection. Twenty four patients diagnosed with respiratory, oval or throat infections received one of several types of antibiotic for 10 days, and two capsules containing 10$^{9}$ dried Lactobacillus rhamnosus GR-1 and L. fermentum RC-14 from the day of commencement of antibiotic therapy for 21 days. The most commonly prescribed antibiotic was biaxin (clarithromycin). All but one patient had lactobacilli in the vagina upon entry to the study, and none developed yeast vaginitis or diarrhea during treatment or 20 days after completion of antibiotics. The mean Nugent score was higher in the placebo than the lactobacilli group (4.1 versus 2.4), and three cases of bacterial vaginosis arose (25 % incidence compared to 0% in the lactobacilli group) in the placebo group (2 receiving cefuroxime, 1 on biaxin). The study suggested that current antibiotic use is not necessarily associated with either diarrhea or yeast infection, as is often surmised. Nevertheless, daily use of probiotics was safe and could potentially reduce the risk of patients developing bacterial vaginosis after antibiotic use.
