Human Parechovirus as an Important Cause of Central Nervous System Infection in Childhood (소아청소년기 중추신경 감염의 주요 원인으로서 Human Parechovirus의 의의)

  • Purpose: Human parechovirus (HPeV) is an increasingly recognized pathogenic cause of central nervous system (CNS) infection in neonates. However, HPeV infections have not been studied in older children. This study determined the prevalence and clinical features of HPeV CNS infection in children in Korea. Methods: Reverse transcription polymerase chain reaction assays were performed using HPeV-specific, 5' untranslated, region-targeted primers to detect HPeV in cerebrospinal fluid (CSF) samples from children presenting with fever or neurologic symptoms from January 1, 2013, to July 31, 2014. HPeV genotyping was performed by sequencing the viral protein 3/1 region. Clinical and laboratory data were retrospectively abstracted from medical records and compared with those of enterovirus (EV)-positive patients from the same period. Results: Of 102 CSF samples, six (5.9%) were positive for HPeV; two of 21 EV-positive samples were co-infected with HPeV. All samples were genotype HPeV3. Two HPeV-positive patients were <3 months of age and four others were over 1 year old. While HPeV-positive infants under 1 year of age presented with sepsis-like illness without definite neurologic abnormalities, HPeV-positive children over 1 year of age presented with fever and neurologic symptoms such as seizures, loss of consciousness, and gait disturbance. The CSF findings of HPeV-positive patients were mostly within the normal range, whereas most (73.7%) EV-positive patients had pleocytosis. Conclusions: Although HPeV is typically associated with disease in young infants, the results of this study suggest that HPeV is an emerging pathogen of CNS infection with neurologic symptoms in older childhood.

The Study of Affecting Image Quality according to forward Scattering Dose used Additional Filter in Diagnostic Imaging System (부가필터 사용 시 전방 산란선량에 따른 화질 영향에 대한 연구)

  • Recent clinical field utilizes the aluminium filter in order to reduce the low-energy photons. However, the usage of the filter can cause adverse effect on the image quality because of the scattered dose that is generated by X-ray hardening phenomenon. Further, usage of filter with improper thickness can be a reason of dose creep phenomenon where unnecessary exposure is generated towards the patient. In this study, the author evaluated the RMS and the RSD analysis in order to have a quantitative evaluation for the effect of forward scattering dose by the filter on the image. as a result of the study, the FSR and the RSD was increased together with the increasing of thickness of the filter. In this study the RSD means the standard deviation of the mean value is relatively size. It can be understood that the signal-to-noise ratio decreases when the average value is taken as a signal and the standard deviation is judged as a noise. The signal-to-noise ratio can understanding as index of resolution at image. Based on these findings, it was quantitatively verified that there is a correlation of the image quality with the FSR by using an additional filter. The results, a 2.5 mmAl which is as recommended by NCRP in the tube voltage of 70 kVp or more showed the 14.6% on the RSD when the filter was not in used. these results are considered able to be utilized as basic data for the study about the filter to improve the quality of the image.

Monitoring on Dose Index Analyzed in the Mammography (유방촬영검사에서 선량지표분석에 대한 모니터링)

  • This study is tried to determine whether the management of medical radiation is well handled by comparison the guidelines of KFDA(korea food & drug administration) with analysis of dose indicator in mammography. As a method, it is analysed that kVp, exposure time, mAs, compressed breast thickness, average glandular dose and body mass index that were classified in the examination of both breasts by CC(cranio-caudal) and MLO(medio-lateral oblique) with EMR(electronic medical record) and dose report that were sent to the PACS(picture archiving communication system). As a result, in the site inspection according to the age, Compressed breast thickness in CC and MLO were the thickest of 45.6 mm and 49.6 mm in the 50-59 year old respectively. In the overall average compressed breast thickness, CC were 44.2 mm and MLO were 48.9 mm. MLO has more thick by 4.7 mm. In average glandular dose, CC were 1.05 mGy and MLO were 1.14 mGy. MLO has higher by 0.09 mGy than CC. As the compressed breast thickness increases 10mm, CC and MLO increases 0.15 mGy and 0.17 mGy respectively. When it was compared with the average glandular dose of 1.16 mGy per 1 film presented by KFDA, CC was showed 1.05 mGy. However, the 60 mm or more was found to exceed a 1.30 mGy. Also, As the compressed breast thickness was higher, body mass index showed high score. And in the case of 25 or more in the obese body index according to body mass index, it was showed obesity in case of the compressed breast thickness was more than 50mm.

Reducing Dose in SPECT/CT Using Adaptive Statistical Iterative Reconstruction Technique (Adaptive Statistical Iterative Reconstruction 기법을 이용한 Bone SPECT/CT 검사에서 피폭량 감소 방안)

  • Purpose: Adaptive statistical iterative reconstruction (ASIR) technique is a reconstruction method of CT image using statistical noise modeling which is known to reduce image noise and to preserve image quality despite reducing radiation dose. The aim of this study is to evaluate images using ASIR on bone SPECT/CT which is primarily performed in our hospital. Materials and Methods: We compared the images of applied ASIR (ASIR level: 20-80%) and none ASIR by changing the mA based on 120 kVp, 100 mA using Discovery NM/CT 670 (GE, U.S.A). First, we evaluated attenuation correction in SPECT image by changing the ASIR level using Anthropomorphic phantom. Second, we compared the contrast to noise ratio (CNR), image noise and spatial resolution in CT image using ACR phantom. Third, after selecting the ASIR level applicable patient using lower torso phantom, we examined 2 patients who followed up bone SPECT/CT and we performed blind test. Results: The degree of attenuation correction in SPECT image showed no significant difference between applied ASIR and none ASIR (P>0.05). When applied ASIR, the noise of CT image were reduced at least 17 up to 52% by changing the mA. The CNR of image with ASIR was maintained more than 0.8 at 40 mA (ASIR 60%) while those without ASIR showed 0.42 at standard 40 mA. In comparison of the high contrast object, we distinguished 12 line pairs/cm at 40 mA regardless of appling ASIR. Comparison of the patients image applied ASIR level 60% (40 mA) which found out by spine image of lower torso phantom showed no signigicant difference between applied ASIR and none ASIR in blind test. The CTDIvol and DLP for applied ASIR 60% showed decreased by 60%, 60% on average than using standard mA. Conclusion: The study show that the radiation dose in SPECT/CT using ASIR can be reduced despite degradation of SPECT and CT images. In addition, higher ASIR level could be possibly applied characteristics of SPECT/CT that region of interest is limited to bone.

Invivo Dosimetry for Mammography with and without Lead Apron Using the Glass Dosimeters (유방촬영술에서 유리선량계를 이용한 납치마의 선량차폐 효과 측정)

  • The purpose of this study is to see the usefulness of lead apron for critical organs near the breast under examining. For clinical experiment, 30 female volunteers who agreed to their participation in the experiments, were chosen and divided into two groups, 15 in group A and 15 in group B respectively. group A is to see whether each side of breast under mammography affects to other side glandular on the critical organs is same, because it is not allowed to scan the both breast for same person or to scan repeatedly. Group B is to see the effectiveness of lead apron during the mammography of right breast. Glass dosimeters were placed on the thyroid, the contralateral breast, and lower abdomen where near the breast during examining. The average glandular doses on the surface in mammography of the thyroid gland, the contralateral breast, the lower abdomen were 0.0692 mGy, 0.6790 mGy, and 0.0122 mGy, respectively, which was an extremely low level of glandular dose. In group B, as to the thyroid gland, average dose was decreased from 0.0922 mGy to 0.0158 mGy. The average dose of contralateral breast was decreased from 0.8575 mGy to 0.0286 mGy. The average doses of lower abdomen was decrease 0.0150 mGy to 0.0173 mGy. As to the lower abdomen, dose decreased from 0.0150 mGy before the use of an apron down to 0.0173 mGy after the use. As p-value was under 0.05, statistically significant difference was observed between the two groups. Wearing an apron can have the protective effects on the thyroid gland up to 20 times lower than not wearing one. Besides, it is also necessary to protect the other breast during the examination by wearing one.

Evaluation of Image Quality According to Presence or Absence of Upper limbs in Scan Field of View During CT Examinations (Including LUNG MAN) (CT 검사 시 스캔 범위 내 상지 유무에 따른 영상의 질 평가(LUNG MAN 포함))

  • The purpose of this study was to evaluate whether or not there was artifact when the upper limb could not be lifted to the top of the head during multi-detector computed tomography(MDCT) scans of the chest and abdomen. Contrast radiography of the human and chest phantom was performed with 128channal MDCT. Under the same conditions(120 kVp, 110 mAs, standard algorithm)both hands lifted up and put down each time in the human experiment. In the chest phantom experiment, the radiography was carried out when the upper limb phantom was adjusted at a certain distance(0, 3, 7 cm) from the chest phantom. Subsequently, the values of Noise, CT number, SNR, and CNR were measured in the field of concern. The noise value of fat, rib, and muscle increased when the arm was lifted in humans(0.79, 47.8, 27%). Furthermore, when the upper limb was lowered, the noise value of muscle and lung increased in the phantom(31.2, 9.4%). In addition, the noise value of the muscles and lung decreased by 5, 25.12% and 5.6, 15.35% as the upper limb moved about 0,3,7cm away from the chest. When the chest and abdominal radiography were performed, in the case of the presence of other parts outside the inspection area, the probability of artifact was minimal while the distance was more than 3cm away from the upper limb to the chest and abdomen.

Effects of Changes in Collimation Size and the sub ROI on Exposure Index of Hand Radiography (손 방사선검사에서 조사야 크기와 보조관심영역 변화가 노출지수 값에 미치는 영향)

  • The purpose of this study is to investigate the effect of changes in collimation size and sub ROI on exposure index(EI) in hand radiography, present collimation size and EI suitable for average hand size of Koreans, and present the effect of changes in sub ROI on EI. The subjects of this study were hand-wrist phantom, and the exposure conditions were set to 55 kVp, 125, mA, and 6.25 mAs, and source to image receptor distance was applied to 110 cm. Based on the vendor recommended sub-ROI (18.7" × 18.7", 8" × 10", 8" × 7.4", 6" × 7.4")and the textbook's recommended sub-ROI 8" × 10", each obtaining 30 images, and comparing the EI shown in the equipment. The EI according to the change in the size of the collimation were 1663.7±4.52, 8"×10" is 1489.1±4.49, 8"×7.4" is 1716.9±3.00, 6"×7.4" is 168.7±3.66 for each EI, and the average value of each value was statistically significant. The average EI according to the sub ROI change was 1489.1±4.49 for SS, LS was 1694.8±5.19 for AEC, 2052.9±5.96, VR was 1548.3±3.20, and HR was 1663.2±4.33. The appropriate field size considering the hand size of Koreans was found to be 8"×7.4". In addition, when the field size increases based on the generally known field size (8"×10") during hand radiography, the EI value changes from a maximum of 15% to a minimum of 11%, and the sub ROI shape based on sub ROI 'SS' Depending on the change, the EI value increased from a maximum of 37% to a minimum of 3%.

Effects of The Sub ROI Changes on Exposure Index (Sub ROI 변화가 노출지수에 미치는 영향)

  • This study aims to investigate the effect of changes in the Sub ROI on exposure index(EI) and to present indicators of changes in EI values that may occur when changing Sub ROI in clinical practice. This study was conducted on a subject of 20 cm of acrylic for a setting similar to abdominal radiography, and the specifications of one acrylic sheet is 20 × 20 × 5 cm. The survey conditions were the same as 80 kVp , 320 mA, 25 ms, SID 110 cm and the Sub ROI obtained 30 images for each type using five types provided by the equipment company. The EI value provided by the equipment and entrance skin exposure(ESE) were compared and analyzed. The mean value of EI according to the change in Sub ROI was 101.18±0.27 for LS, 106.57±0.31 for AEC, 107.74±0.39 for VR, 107.90±0.38 for HR, and 109.72±0.32 for SS (p<0.01). The average value of ESE by sub ROI type (LS, AEC, VR, HR, SS) was measured to be 476.45±1.71 μGy, 476.92±1.48 μGy, 476.14±2.30 μGy, 475.61±1.96 μGy, and 477.14±1.46 μGy, with statistically significant differences (p<0.01). As a result of this study, the EI according to the sub ROI type is based on LS(109.72), which represents the minimum value. AEC increased 5.3%, VR increased 6.4%, HR increased 6.6%, SS increased 8.4%, and overall, increased by about 5.3%. As for the average value of ESE, HR(475.61 μGy)type showed the minimum value, and based on this, AEC increased 0.27%, VR increased 0.11%, LS increased 0.17%, SS increased 0.32%, and overall, increased by about 0.17%.

Consistent Comparison for The Linearity Air Kerma of IEC Standards and Commercial Load in Diagnosing DR Generators (진단용 DR 발생장치에서 IEC 표준규격과 상용부하의 공기커마 직선성에 대한 일관성 비교)

  • In this study, based on IEC 60601-2-54 standard load conditions presented in the limited interval over the air kerma at the absolute linearity closely evaluated by measuring the X-ray results were as follows: 10 units targeted all Diagnostic X-ray generating device (DR) presented in the IEC 60601-2-54 standard linearity of air kerma emerged as inappropriate, the general evaluation of the dose linearity from four in the top 50% and 80 kVp % of the two measurement series were as irrelevant all the rest from six of the top tube voltage of 50% and 80% of the two measurement series, appeared in all suitable. Presented in IEC 60601-2-54 standard dose linearity testing and conventional linearity tests showed many differences. IEC 60601-2-54 standard linearity in the proposed international standards of air kerma is the recommendation of the existing dose linearity considerably more feasible, and to quantify the amount of radiation as the standard suggested by the standard IEC 60601-2-54 air kerma of a diagnostic X-ray imaging device linearity performance management is considered key elements in the critical appraisal.

DC/RF 중첩형 마그네트론 스퍼터링법으로 증착한 ITO 초박막의 SnO2 함량에 따른 전기적 ,광학적 특성 및 미세구조 변화

  • 차세대 디스플레이에서 3차원 감성 터치 또는 플렉시블 기판 등에 사용되고 있는 ITO(Tin-doped Indium Oxide) 박막은 고 해상도 및 소자 효율 향상을 위해 전 가시광 영역에서 높은 투과율이 요구되고 있다. 일반적으로 ITO 박막은 두께 감소에 따라 빛의 두께 산란 없이 전 가시광 영역에서 높은 투과율을 가지는 반면, 두께가 감소할수록 박막 성장 시 비정질 기판의 영향을 크게 받아 박막 결정성 감소와 더불어 전기전도성이 감소되는 경향을 보인다. 특히, 매우 얇은 두께에서의 ITO 박막 물성은 초기 박막 핵 생성 및 성장과 증착 공정 중에 발생하는 고 에너지 입자(산소 음이온, 반사 중성 아르곤 등)의 박막 손상에 대한 영향을 크게 받을 뿐만 아니라 ITO 박막 내의 SnO2 도핑함량에도 매우 의존한다. 따라서, 매우 얇은 두께에서 높은 투과율과 뛰어난 전기전도성을 동시에 가지는 고품질 ITO 초박막 제조를 위해서는 박막 초기 핵 성장 제어기술 및 SnO2 함량에 따른 ITO 초박막의 전기적, 광학적 거동에 관한 연구가 필요하다. 본 연구에서는 다양한 SnO2 함량에서 고품질의 ITO 초박막을 DC/RF 중첩형 마그네트론 스퍼터링법을 이용하여 박막 증착 중에 발생하는 고에너지 입자의 기판충격으로 인한 박막손상을 최소화하여 증착된 박막의 전기적, 광학적 특성 및 미세구조를 관찰하였다. 그리고 전체파워에서 RF/(RF+DC) 비율을 제어하여 증착한 ITO 초박막의 물성을 최적화 하였으며, 상온 및 결정화 온도 이상에서 다양한 SnO2 함량을 가진 ITO 박막을 두께(150 nm, 25 nm)에서 각각 증착하여 전기적, 광학적 거동 및 XRD를 통한 박막의 미세구조 변화를 비교 분석하였다. 그리고 증착된 모든 ITO 초박막에서 가시광 투과율은 빛의 두께 산란 없는 높은 투과율(>85 %) 을 보이는 것을 확인 할 수 있었다. 증착된 ITO 박막의 전기적 특성 및 미세구조는 RF/(DC+RF)비율 50%에서 최적임을 확인하였다. 이는 RF/(DC+RF) 비율 증가에 따른 캐소드 전압 최적화로 박막의 초기 핵 성장 과정에서 기판상의 고에너지 입자로 인한 박막 손상의 감소 및 리스퍼터 되는 산소량을 최적화 시키고, 이는 박막의 결정성 향상으로 이어져, 박막내의 결함 밀도 감소 및 SnO2 고용 효율을 증가시켜 전기전도성 향상에 기인하였다고 판단된다. 또한, 증착된 ITO 초박막은 SnO2 함량 변화에 따라 박막의 결정성 및 전기적 특성에서 미세한 변화를 보였다. 이러한 ITO 박막의 물성변화는 박막 두께 감소에 따른 결정성 감소와 함께 SnO2의 고용 한계 변화로 인한 것으로 판단된다. 또한, RF/(DC+RF) 비율의 증가에 따른 ITO 초박막의 전기적, 광학적 및 미세구조는 Vp-Vf의 변화와 관련하여 설명되어 진다.
