Association between Caregiver's Awareness of Human Rights and Quality of Service: Focused on Human Right Education (요양보호사의 노인인권의식과 서비스 질에 대한 인식 수준의 관련성: 인권교육 조절효과 중심으로)

  • Background: Long-term care insurance for the elderly has been stably established along with the quantitative expansion of long-term care facilities. Indeed, the need for a paradigm about human rights-based service approach is being raised throughout society from a service perspective. Therefore, this study aimed to analyze the association between elderly human rights awareness and quality of service by considering human rights education as a moderate variable. Methods: This study conducted surveys with 138 caregivers working in long-term care facilities located in Seoul and Gangwon. General characteristics, awareness of human rights, and the level of service quality were examined using descriptive statistics, frequency analysis, and correlation analysis. And multi-variable linear regression with a hierarchical framework was employed. These analyses were performed using IBM SPSS ver. 25.0. Results: Of the 138 caregivers, 97.1% were female, 87.7% were more than 50 years old, and most of their education level was high-school graduates. Their length of employment ranged from more than 5 years to less than 10 years. The level of awareness regarding elderly human rights of the elderly was below normal (mean=2.21), but the quality of service was high (mean=4.21), and the need for human rights education was also high (mean=4.28). Among the general characteristics, the length of employment was significantly associated with awareness of elderly human rights. Moreover, political rights awareness, included as sub-domains of human rights, was positively associated with quality of service. However, the moderating variable, human rights education, was not significantly associated with the quality of service. Conclusion: In this study, human rights education, as a moderating variable, did not have a statistically significant effect on caregivers' human rights awareness in relation to service quality. This finding is inconsistent with previous research results. These results can be explained by the fact that the frequency of education in long-term care facilities was a significant factor in the practice of protecting the human rights of the elderly. Therefore ongoing encouragement for the frequency of current human rights education and improvements in the educational approach appear to be necessary. In addition, these findings reveal the need for strength of education policies and effective in-depth research about human rights and quality of service to respect the human rights of the elderly.

Validation of the Effectiveness of Education for Obtaining Consent in Clinical Study (임상연구 동의서 교육 시행의 효용성 검증)

  • Purpose: To validate the effectiveness of obtaining consent education on errors in the consent process and to develop the education program for researchers. Methods: From February 2019 to February 2022, a 30-minute, 1:1 face-to-face consent education developed using the ADDIE model was conducted on 78 nurses as principal investigators. An informed consent audit tool, which includes 6 items developed by Asan Medical Center Human Research Protection Center, was used to analyze errors in obtaining informed consent process. Data analysis was performed using the SPSS ver. 25.0, and the Mann-Whitney U-test and χ2-test were utilized to verify the difference in errors between the experimental and control groups. Results: The participants consisted of 42 in the experimental group and 36 in the control group, with no statistically significant difference between the 2 groups. Both 2 groups showed the highest frequency of documentation errors, followed by format errors, errors related to a suitability of investigator, participant, or participant's legally acceptable representative, witness and confidentiality issues. After education, there was a significant decrease in both format errors (p=0.002) and documentation errors (p<0.001) in the experimental group. The proportion of participants without any errors in all items was higher in the experimental group (35.7%) compared to the control group (5.6%), and this difference was statistically significant (p=0.001). Conclusion: The obtaining consent education program was found to be effective in reducing informed consent errors. This study emphasizes the importance of education, suggesting the need for its expansion and accessibility, as well as the necessity for all researchers conducting clinical studies to receive the obtaining consent education.

Necessity of oral health education for students major in early childhood education (유아교육 관련 학과 학생의 구강보건교육 필요도에 관한 연구)

  • This research is intended to develop oral health education program that can improve quality of oral health of infants by investigating the actual condition of oral health education provided to students major in early childhood education and contents and method of oral health education that they needed. A questionnaire survey was conducted for 427 students enrolled in related departments such as the Early Childhood Education Division and the Early Childhood Education Department at five universities in Gyeongsangnam-do. Questionnaires consisted of general characteristics, awareness of oral health, presence of experience in oral health education, necessity of oral health education, preference for oral health education method, oral health education contents. The collected data were analyzed by SPSS(Statistical Package for the Social Science) Ver 20.0. 1. Presence of experience in oral health education based on the general characteristics showed statistically significant differences only concerning the 'school system' and the 'school year' (p<0.05), and subjective awareness of oral health based on the presence of oral health education experiences showed statistically significant differences concerning the 'interest in oral health' and the 'importance of oral health' (p<0.05). 2. Necessity of oral health education based on the subjective awareness of oral health showed statistically significant differences concerning the 'interest in oral health' and the 'importance of oral health' (p<0.05). 3. Necessity of oral health education based on the preference for oral health education method showed statistically significant differences concerning the 'intention to participate in oral health education' and the 'oral health education cycle' (p<0.05). 4. The most necessary information for oral health education is proper toothbrushing method 4.24, cause of tooth decay and prevention method 4.13, helpful food and poor food for tooth 3.97, toothbrush selection and storage method 3.85. Fluoride application and fissure sealant were lowest 3.38. As a result of this research, necessity of oral health education was large regardless of general characteristics, experience in oral health education, subjective awareness of oral health, and preference for oral health education. Also the more the 'interest in oral health' in 'subjective awareness of oral health', the more the 'necessity of oral health education' and 'intention to participate in oral health education'. Therefore it is necessary to develop systematic and repetitive oral health education for students major in early childhood education.

Correlation of Mental State with Resilience of Stroke Patients during Rehabilitation (뇌졸중 환자의 재활치료 중 정서 상태와 회복 탄력도와의 관련성 연구)

  • Objectives: To investigate the relationship between rehabilitation treatment, mental state and resilience of stroke patients undergoing rehabilitation by examining the correlation between The Core Seven Emotions Inventory-Short Form (CSEI-s) and the Korean version of the Connor-Davidson Resilience Scale (K-CD-RISC). Methods: All 104 participants (44 diagnosed with stroke who were receiving rehabilitation and 60 without stroke or psychiatric history) completed the CSEI-s, K-CD-RISC, and Questionnaire for stroke symptoms. All data were analyzed using by Statistical Package for the Social Sciences (SPSS) ver. 27.0. Descriptive statistics, chi-square test, t-test, Mann-Whitney U test, Kruskal-Wallis H test, and Pearson correlation coefficient were used for data analysis. Results: As a result of the CSEI-s, compared to the control group, the stroke group showed significantly lower Joy (喜) scores and significantly higher scores for Depression (憂) and Sorrow (悲). With a morbidity period of 12 months or less, the Thought (思) score was significantly higher. The Fear (恐) score was significantly higher when the rehabilitation was initiation more than 4~8 weeks after than that when the treatment was started immediately. Meanwhile, the K-CD-RISC score was significantly higher when rehabilitation was started immediately. In the stroke group, the K-CD-RISC score was positively correlated with Joy (喜) but negatively correlated with Depression (憂) and Fear (恐). In the control group, K-CD-RSIC showed a positive correlation with Joy (喜) but negative correlations with Depression (憂), Sorrow (悲), and Fear (恐). Conclusions: In addition to early rehabilitation treatment, mental approach through Korean medicine psychotherapy is crucial for enhancing the resilience of stroke patients.

Study of 68Ga Labelled PET/CT Scan Parameters Optimization (68Ga 표지 PET/CT 검사의 최적화된 매개변수에 대한 연구)

  • Purpose: Gallium-68 (68Ga) is increasingly used in nuclear medicine imaging for various conditions such as lymphoma and neuroendocrine tumors by labeling tracers like Prostate Specific Membrane Antigen (PSMA) and DOTA-TOC. However, compared to Fluorine-18 (18F) used in conventional nuclear medicine imaging, 68Ga has lower spatial resolution and relatively higher Signal to Background Ratio (SBR). Therefore, this study aimed to investigate the optimized parameters and reconstruction methods for PET/CT imaging using the 68Ga radiotracer through model-based image evaluation. Materials and Methods: Based on clinical images of 68Ga-PSMA PET/CT, a NEMA/IEC 2008 PET phantom model was prepared with a Hot vs Background (H/B) ratio of 10:1. Images were acquired for 9 minutes in list mode using DMIDR (GE, Milwaukee WI, USA). Subsequently, reconstructions were performed for 1 to 8 minutes using OS-EM (Ordered Subset Expectation Maximization) + TOF (Time of Flight) + Sharp IR (VPFX-S), and BSREM (Block Sequential Regularized Expectation Maximization) + TOF + Sharp IR (QCFX-S-400), followed by comparative evaluation. Based on the previous experimental results, images were reconstructed for BSREM + TOF + Sharp IR / 2 minutes (QCFX-S-2min) with varying β-strength values from 100 to 700. The image quality was evaluated using AMIDE (freeware, Ver.1.0.1) and Advanced Workstation (GE, USA). Results: Images reconstructed with QCFX-S-400 showed relatively higher values for SNR (Signal to Noise Ratio), CNR (Contrast to Noise Ratio), count, RC (Recovery Coefficient), and SUV (Standardized Uptake Value) compared to VPFX-S. SNR, CNR, and SUV exhibited the highest values at 2 minutes/bed acquisition time. RC showed the highest values for a 10 mm sphere at 2 minutes/bed acquisition time. For small spheres of 10 mm and 13 mm, an inverse relationship between β-strength increase and count was observed. SNR and CNR peaked at β-strength 400 and then decreased, while SUV and RC exhibited a normal distribution based on sphere size for β-strength values of 400 and above. Conclusion: Based on the experiments, PET/CT imaging using the 68Ga radiotracer yielded the most favorable quantitative and qualitative results with a 2 minutes/bed acquisition time and BSREM reconstruction, particularly when applying β-strength 400. The application of BSREM can enhance accurate quantification and image quality in 68Ga PET/CT imaging, and an optimization process tailored to each institution's imaging objectives appears necessary.

Image Comparison of Heavily T2 FLAIR and DWI Method in Brain Magnetic Resonance Image (뇌 자기공명영상에서 Heavily T2 FLAIR와 DWI 기법의 영상비교)

  • The purpose of this study is to obtain brain MRI images through Heavenly T2 FLAIR and DWI techniques to find out strengths and weaknesses of each image. Data were analyzed on 13 normal people and 17 brain tumor patients. Philips Ingenia 3.0TCX was used as the equipment used for the inspection, and 32 Channel Head Coil was used to acquire data. Using Image J and Infinity PACS Data, 3mm2 of gray matter, white matter, cerebellum, basal ganglia, and tumor areas were set and measured. Quantitative analysis measured SNR and CNR as an analysis method, and qualitative analysis evaluated overall image quality, lesion conspicuity, image distortion, susceptibility artifact and ghost artifact on a 5-point scale. The statistical significance of data analysis was that Wilcox-on Signed Rank Test and Paired t-test were executed, and the statistical program used was SPSS ver.22.0 and the p value was less than 0.05. In quantitative analysis, the SNR of gray matter, white matter, cerebellum, basal ganglia, and tumor of Heavily T2 FLAIR is 41.45±0.13, 40.52±0.45, 41.44±0.51, 40.96±0.09, 35.28±0.46 and the CNR is 15.24±0.13, 16.75±0.23, 16.28±0.41, 15.83±0.17, 16.63±0.51. In DWI, SNR is 32.58±0.22, 36.75±0.17, 30.21±0.19, 35.83±0.11, 43.29±0.08, and CNR is 13.14±0.63, 14.21±0.31, 12.95±0.32, 11.73±0.09, 17.56±0.52. In normal tissues, Heavenly T2 FLAIR obtained high results, but in disease evaluation, high results were obtained at DWI, b=1000 (p<0.05). In addition, in the qualitative analysis, overall image quality, lesion conspicuity, image distortion, susceptibility artifact and ghost artifact aspects of the Heavily T2 FLAIR were evaluated, and 3.75±0.28, 2.29±0.24, 3.86±0.23, 4.08±0.21, 3.79±0.22 values were found, respectively, and 2.53±0.39, 4.13±0.29, 1.90±0.20, 1.81±0.21, 1.52±0.45 in DWI. As a result of qualitative analysis, overall image quality, image distortion, susceptibility artifact and ghost artifact were rated higher than DWI. However, DWI was evaluated higher in lesion conspicuity (p<0.05). In normal tissues, the level of Heavenly T2 FLAIR was higher, but the DWI technique was higher in the evaluation of the disease (tumor). The two results were necessary techniques depending on the normal site and the location of the disease. In conclusion, statistically significant results were obtained from the two techniques. In quantitative and qualitative analysis, the two techniques had advantages and disadvantages, and in normal and disease evaluation, the two techniques produced useful results. These results are believed to be educational data for clinical basic evaluation and MRI in the future.

  • This study aimed to improve the service quality of the ski rental industry and provide basic data to satisfy customers' needs by investigating how the service quality of ski rental shops affects consumers' perceived value and purchasing behavior. Therefore, a survey was conducted on users visiting rental shops at six ski resorts in Korea (High1 Resort, Yongpyeong Resort, Jisan Resort, Gonjiam Resort, Phoenix Park, and Welli Hilli). The collected data was conducted by using SPSS (B. 25.0) program, visual factors analysis, and multivariate distributed analysis, simple, simple and multivariate variance analysis, simple and multivariate analysis. The results derived from this study are as follows. First, it was found that the service quality of ski resort rental shops had a significant effect on perceived value in the order of convenience, empathy, service, and facility environment. Second, it was found that the perceived value of purchasing behavior of purchase behavior and recommendations. Third, it was found that the service quality of ski resort rental shops had a significant effect on purchase intention and recommendation intention of purchase behavior in the order of empathy, facility environment, convenience, and service.

Evaluation of the Usefulness of MapPHAN for the Verification of Volumetric Modulated Arc Therapy Planning (용적세기조절회전치료 치료계획 확인에 사용되는 MapPHAN의 유용성 평가)

  • Purpose: Latest linear accelerator and the introduction of new measurement equipment to the agency that the introduction of this equipment in the future, by analyzing the process of confirming the usefulness of the preparation process for applying it in the clinical causes some problems, should be helpful. Materials and Methods: All measurements TrueBEAM STX (Varian, USA) was used, and a file specific to each energy, irradiation conditions, the dose distribution was calculated using a computerized treatment planning equipment (Eclipse ver 10.0.39, Varian, USA). Measuring performance and cause errors in MapCHECK 2 were analyzed and measured against. In order to verify the performance of the MapCHECK 2, 6X, 6X-FFF, 10X, 10X-FFF, 15X field size $10{\times}10$ cm, gantry $0^{\circ}$, $180^{\circ}$ direction was measured by the energy. Comparison of One-day and Two-day Protocol of $^{11}C$-Acetate and $^{18}F$-FDG Scan in Hepatoma (간암환자에 있어서 $^{11}C$-Acetate와 $^{18}F$-FDG PET/CT 검사의 당일 검사법과 양일 검사법의 비교)

  • Purpose: $^{11}C$-Acetate PET/CT is useful in detecting lesions that are related to livers in the human body and leads to a sensitivity of 87.3%. On the other hand, $^{18}F$-FDG PET/CT has a sensitivity of 47.3% and it has been reported that if both $^{18}F$-FDG and $^{11}C$-Acetate PET/CT are carried out together, their cumulative sensitivity is around 100%. However, the normal intake of the pancreas and the spleen in $^{11}C$-Acetate PET/CT can influence the $^{18}F$-FDG PET/CT leading to an inaccurate diagnosis. This research was aimed at the verification of the usefulness of how much influence these two radioactive medical supplies can cause on the medical images through comparative analysis between the one-day and two-day protocol. Materials and Methods: This research was carried out based on 46 patients who were diagnosed with liver cancer and have gone through the PET/CT (35 male, 11 female participants, average age: $54{\pm}10.6$ years, age range: 29-69 years). The equipment used for this test was the Biograph TruePoint40 PET/CT (Siemens Medical Systems, USA) and 21 participants who went through the one-day protocol test were first given the $^{11}C$-Acetate PET/CT and the $^{18}F$-FDG PET/CT, the latter exactly after one hour. The other 25 participants who went through the two-day protocol test were given the $^{11}C$-Acetate PET/CT on the first day and the $^{18}F$-FDG PET/CT on the next day. These two groups were then graded comparatively by assigning identical areas of interest of the pancreas and the spleen in the $^{18}F$-FDG images and by measuring the Standard Uptake Value (SUV). SPSS Ver.17 (SPSS Inc., USA) was used for statistical analysis, where statistical significance was found through the unpaired t-test. Results: After analyzing the participants' medical images from each of the two different protocol types, the average${\pm}$standard deviation of the SUV of the pancreas carried out under the two-day protocol were as follows: head $1.62{\pm}0.32$ g/mL, body $1.57{\pm}0.37$ g/mL, tail $1.49{\pm}0.33$ g/mL and the spleen $1.53{\pm}0.28$ g/mL. Whereas, the results for participants carried out under the one-day protocol were as follows: head $1.65{\pm}0.35$ g/mL, body $1.58{\pm}0.27$ g/mL, tail $1.49{\pm}0.28$ g/mL and the spleen $1.66{\pm}0.29$ g/mL. Conclusion: It was found that no statistical significant difference existed between the one-day and two-day protocol SUV in the pancreas and the spleen (p<0.05), and nothing which could be misconceived as false positive were found from the PET/CT medical image analysis. From this research, it was also found that no overestimation of the SUV occurred from the influence of $^{11}C$-Acetate on the $^{18}F$-FDG medical images where those two tests were carried out for one day. This result was supported by the statistical significance of the SUV of measurement. If $^{11}C$-Acetate becomes commercialized in the future, the diagnostic ability of liver diseases can be improved by $^{18}F$-FDG and one-day protocol. It is from this result where tests can be accomplished in one day without the interference phenomenon of the two radioactive medical supplies and furthermore, could reduce the waiting time improving customer satisfaction.

Usefulness of Abdominal Compressor Using Stereotactic Body Radiotherapy with Hepatocellular Carcinoma Patients (토모테라피를 이용한 간암환자의 정위적 방사선치료시 복부압박장치의 유용성 평가)

  • Purpose: We evaluated usefulness of abdominal compressor for stereotactic body radiotherapy (SBRT) with unresectable hepatocellular carcinoma (HCC) patients and hepato-biliary cancer and metastatic liver cancer patients. Materials and Methods: From November 2011 to March 2012, we selected HCC patients who gained reduction of diaphragm movement >1 cm through abdominal compressor (diaphragm control, elekta, sweden) for HT (Hi-Art Tomotherapy, USA). We got planning computed tomography (CT) images and 4 dimensional (4D) images through 4D CT (somatom sensation, siemens, germany). The gross tumor volume (GTV) included a gross tumor and margins considering tumor movement. The planning target volume (PTV) included a 5 to 7 mm safety margin around GTV. We classified patients into two groups according to distance between tumor and organs at risk (OAR, stomach, duodenum, bowel). Patients with the distance more than 1 cm are classified as the 1st group and they received SBRT of 4 or 5 fractions. Patients with the distance less than 1 cm are classified as the 2nd group and they received tomotherapy of 20 fractions. Megavoltage computed tomography (MVCT) were performed 4 or 10 fractions. When we verify a MVCT fusion considering priority to liver than bone-technique. We sent MVCT images to Mim_vista (Mimsoftware, ver .5.4. USA) and we re-delineated stomach, duodenum and bowel to bowel_organ and delineated liver. First, we analyzed MVCT images to check the setup variation. Second we compared dose difference between tumor and OAR based on adaptive dose through adaptive planning station and Mim_vista. Results: Average setup variation from MVCT was $-0.66{\pm}1.53$ mm (left-right) $0.39{\pm}4.17$ mm (superior-inferior), $0.71{\pm}1.74$ mm (anterior-posterior), $-0.18{\pm}0.30$ degrees (roll). 1st group ($d{\geq}1$) and 2nd group (d<1) were similar to setup variation. 1st group ($d{\geq}1$) of $V_{diff3%}$ (volume of 3% difference of dose) of GTV through adaptive planing station was $0.78{\pm}0.05%$, PTV was $9.97{\pm}3.62%$, $V_{diff5%}$ was GTV 0.0%, PTV was $2.9{\pm}0.95%$, maximum dose difference rate of bowel_organ was $-6.85{\pm}1.11%$. 2nd Group (d<1) GTV of $V_{diff3%}$ was $1.62{\pm}0.55%$, PTV was $8.61{\pm}2.01%$, $V_{diff5%}$ of GTV was 0.0%, PTV was $5.33{\pm}2.32%$, maximum dose difference rate of bowel_organ was $28.33{\pm}24.41%$. Conclusion: Despite we saw diaphragm movement more than 5 mm with flouroscopy after use an abdominal compressor, average setup_variation from MVCT was less than 5 mm. Therefore, we could estimate the range of setup_error within a 5 mm. Target's dose difference rate of 1st group ($d{\geq}1$) and 2nd group (d<1) were similar, while 1st group ($d{\geq}1$) and 2nd group (d<1)'s bowel_organ's maximum dose difference rate's maximum difference was more than 35%, 1st group ($d{\geq}1$)'s bowel_organ's maximum dose difference rate was smaller than 2nd group (d<1). When applicating SBRT to HCC, abdominal compressor is useful to control diaphragm movement in selected patients with more than 1 cm bowel_organ distance.
