A Case Report on Post-stroke Epilepsy Treated by Korean Medicine with Uwhangchungsimwon and Jingansikpungtang-gagambang (뇌졸중 후 발생한 뇌전증 환자에 우황청심원과 진간식풍탕가감방을 활용한 한의 치료 증례 보고 1례)

  • Post-stroke epilepsy (PSE) slows the recovery process and reduces the quality of life of stroke survivors. Antiepileptic drugs are empirically prescribed to prevent PSE. However, the long-term use of antiepileptic drugs increases the risk of atherosclerosis, and up to 25% of patients have drug-resistant epilepsy. Herein, We report a patient with PSE who was treated with Korean medicine including Jingansikpungtang-gagambang and Uwhangchungsimwon. A 51-year-old patient had a past medical history of cerebral infarction that occurred in 2014. His first seizure occurred in January 2020 and he was diagnosed with PSE through a brain magnetic resonance imaging. The patient had a partial seizure with secondary generalization. After the initiation of taking Korean medicine, both the rate of progression to generalized seizures and the frequency of seizures was progressively and significantly reduced. This case report suggests that Korean medicine-based treatment may be safe and effective for PSE.

Effects of Uwhangchungsimwon(牛黃淸心元) on Cerebral Blood Flow and Systemic Blood Pressure in Humans (우황청심원이 정상인의 뇌혈류 및 혈압에 미치는 영향)

  • Uwhangchungsimwon(UC) has been used in the treatment of a wide variety of conditions including stroke, hypertension, arteriosclerosis, autonomic imbalance, mental instablity, etc in Korean traditional hospitals, In particular it is often initialy chosen for emergency care of acute stroke. The aim of this study was to evaluate the effect of UC on cerebral hemodynamics. Using transcranial Doppler ultrasound, we studied changes of mean flow velocity and pulsatility index(PI) of middle cerebral arteries (MCAs) from 11 health young volunteers who were administrated with 1 pill UC and 11 health controls who were not. We obtained hypercapnia with breath-holding and evaluated cerebrovascular reactivity with breath-holding index(BHI). Systolic blood pressure, diastolic blood pressure, and heart rate were measured using ambulatory blood pressure monitoring(ABPM). In UC administration group, the evaluation was performed during basal condition. and repeated at 20, 40, and 60 min after administration. In controls, the evaluation was performed at corresponding time intervals. Mean flow velocity in middle cerebral artery, systolic blood pressure, diastolic blood pressure, and heart rate did not change during the observation period and were not different between these two groups. However, administration of UC was associated with decreases in PI by $3.6{\sim}12.4%$ in BHI by $17.9{\sim}24.8%$ compared with pre-administration period. Decreases in PI and BHI with UC were significantly different compared with control group (p<0.05). These results indicate that UC decreases PI and BHI in cerebral artery, which is due to a dilation of cerebral resistance vessels.

Effects of Uwhangchungsimwon on Arterial Stiffness (우황청심원이 정상인의 동맥경직도에 미치는 영향)

  • Objectives : Uwhangchungsimwon (UC) is one of the famous herbal medicine frequently used for stroke, hypertension, and atherosclerosis, etc. Therefore, we intended to examine if UC could have therapeutic effect on arterial stiffness by assessing Cardio-Ankle Vascular lndex (CAVI). Methods : We enrolled 38 healthy male subjects (28±4.41 years old) and divided them into two groups (UC and control group). The UC group (N=22) were treated with 1 pill of UC and monitored by CAVI every hour for 6 hours. The control group(N=16) did not receive any medicine and were also monitored by CAVI the same manner as the UC group. In both group, we also assessed each subject's Yin-Yang pattern score by the diagnostic scoring system. In analysis, we compared the difference of CAVIs between the UC and the control group considering subject's Yin-Yang pattern. Results : There was no significant difference in the change of CAVI between the UC and the control group. After excluding Yang-pattern subjects in both group, however, we could observe that CAVI decreased significantly 5 hours after UC administration compared with the corresponding CAVI in the control group (P=0.025). In the repeated measure analysis, the between-subjects effect had statistical significance (P=0.035). Conclusion : According to these results, we suggest that UC have therapeutic effect on arterial stiffness especially after taking oriental medical diagnosis into consideration.

Korean Medicine Treatments for Depression Induced by Plica Syndrome Operation and Worsen by Traffic Accident: A Case Report (추벽증후군 수술 후 발생하여 교통사고 후 심화된 우울증에 대한 한의학적 치료 1례)

  • Objectives: The purpose of this study is to report the efficacy of using Korean medicine treatment for depression induced by Plica syndrome operation and worsen by a traffic accident. Methods: We treated a patient's depression with Korean medicine treatments (Uwhangchungsimwon, acupuncture, pharmacoacupuncture, cupping, moxibustion, interferential current therapy). To evaluate the results of this treatment, we measured Beck Depression Inventory (BDI), EuroQol-Five Dimensions (EQ-5D), Numeric Rating Scale (NRS), Oswestry Disability Index (ODI), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Results: Improvements of BDI, EQ-5D, NRS, ODI and WOMAC were observed after the treatments. Conclusions: This study suggests that Korean medicine treatment for depression may be effective.
