Reproducibility of Trunk Control Assessment and the Clinical Utility of the Distinguishing Barthel Index in Chronic Stroke Patients (만성 뇌졸중 환자들의 체간 조절 평가의 재현성과 Barthel Index구분을 위한 임상 유용성)

  • PURPOSE: This study examined the test-retest reliability and clinical utility of the Modified Trunk Impairment Scale (mTIS), Trunk Control Test (TCT), and Postural Assessment Scale for Stroke - Trunk Control (PASS-TC) in patients with chronic stroke. METHODS: Thirty-eight stroke patients were reassessed using the mTIS, TCT, and PASS-TC with a seven-day interval between assessments. The test-retest reliability was evaluated using the intraclass correlation coefficient (ICC2,1), the standard error of measurement (SEM), the minimal detectable change (MDC), and MDC%, as well as Bland-Altman analysis. The relationship between the mTIS, TCT, PASS-TC scores, and the Barthel Index (BI) was also investigated. RESULTS: The test-retest reliability for the mTIS, TCT, and PASS-TC was high, with ICC values ranging from .91 to .94 (95% confidence interval: .83-.97). The MDCs for the mTIS and TCT were 2.35 and 13.9, respectively, while the MDC for the P ASS-TC was 2.54, all below 20% of the maximum possible score, indicating reliable measurement. The optimal mTIS cut-off score for distinguishing between mild (75-95 points) and severe (50-74 points) dependence on the BI was ≥ 9.5, with an accuracy of 79%. Patients with an mTIS score ≥ 9.5 (out of 15) showed an 18-fold higher likelihood of achieving a mild level of functional independence than those with a score < 9.5. CONCLUSION: The mTIS, TCT, and PASS-TC showed high test-retest reliability and no systematic errors in chronic stroke patients. The MDC values were reliable, indicating meaningful change. Among these, the mTIS is a sensitive and a useful tool for predicting functional independence in clinical practice and is straightforward to apply.

Shortening of Korean Patient Classification System-1 and Classification of Nursing Care Needs (한국형 환자분류체계의 단축형 개발과 간호요구 유형 분류)

  • Purpose: The purpose of the study was to shorten the KPCS-1 (Korean Patient Classification System-1) for predicting nursing care need level and to explore whether the patients can be clustered by their acuity and dependency. Methods: The participants were inpatients in two surgical wards and two internal medicine wards at a teritory hospital during 14 days investigations. The KPCS-1 was evaluated once a day for all inpatients and 2,082 cases of data from a total of 411 patients were analyzed. Results: The items were reducted from 50 items to 26 items by partial least squares analysis and expert review. Through factor analysis, it was confirmed that hygiene, diet, elimination, and exercise were categorized as dependence factors. Patients were clustered with low acuity/low dependency (average score: 7.68±2.81/1.05±1.33), high acuity/low dependency (average score: 17.20±4.15/1.94±2.40), medium acuity/high dependency (average score: 13.56±5.30/9.66±2.64) through cluster analysis. The total score of the three groups for their nursing care needs was 8.73±3.36, 19.14±5.74, and 23.24±6.31 in order, and the results showed a statistically significant difference (F=1712.12, p<.001). Conclusion: The shortening of the KPCS-1 and the new criteria for categorizing patients according to acuity and dependence will increase clinical utility and be useful for manpower assignment criteria in detail.

  • As the utility value of GPS in surveying field is on the increase after the conversion into the world geodetic system, most of the baseline processing programs seeking the relative baseline vector for the roving point based on the base point are dependent on the foreign software, and such dependence remains a stumbling block to its wide application. In this study an algorithm was established settling ambiguity through LAMBDA techniques and the baseline processing program was developed for Ll carrier phase using visual c++ 6.0, which is an object-oriented language. And the developed program proved that it maintained a difference of less than 4.9 cm over the short baseline of 4.9 km or shorter when compared with other commercialized programs.

Development of a Time-temperature Indicator for Managing the Distribution Temperature of Frozen Mackerel Scomber japonicus (냉동 고등어(Scomber japonicus)의 유통 온도 관리를 위한 Time-temperature Indicator 개발)

  • We developed a time-temperature indicator (TTI) that rapidly determines whether frozen mackerel Scomber japonicus has thawed during its distribution. Our TTI was made from filter paper capable of absorbing mackerel exudate that was soaked in a 20% citric acid solution at pH 4.0 and then dried. The dried absorbent paper was cut and attached to blue litmus paper with a 2 mm overhang. The fixed litmus paper was covered with a polypropylene film and sealed. The indicator was placed inside a polyvinyl vacuum package containing the mackerel sample. The vacuum-sealed packaged was frozen at $-20^{\circ}C$ for 24 h. After freezing, the color change and time dependence of the indicator were observed at room temperature ($25^{\circ}C$) and demonstrated the utility of this TTI for rapidly determining whether frozen mackerel underwent thawing during distribution.

  • In this paper, solute transport in heterogeneous aquifers using a modified Fokker-Planck equation (MFPE) is investigated. This newly developed mathematical model is characterised with a time-, scale-dependent dispersivity. A two-dimensional finite volume quadrilateral mesh method (FVQMM) based on a quadrilateral background interpolation mesh is developed for analysing the model. The FVQMM transforms the coupled non-linear partial differential equations into a system of differential equations, which is solved using backward differentiation formulae of order one through five in order to advance the solution in time. Three examples are presented to demonstrate the model verification and utility. Henry's classic benchmark problem is used to show that the MFPE captures significant features of transport phenomena in heterogeneous porous media including enhanced transport of salt in the upper layer due to its parameters that represent the dependence of transport processes on scale and time. The time and scale effects are investigated. Numerical results are compared with published results on the some problems.

  • Although many theoretical studies have tried to explain the volatility in financial markets using models of herd behavior, there have been few empirical studies on dynamic herding due to the technical difficulty of detecting herd behavior with time-series data. Thus, this paper theoretically extends a continuous beliefs system belonging to an agent based economic model by introducing a term representing agents'mutual dependence into each agent's utility function and derives a SV(stochastic volatility)-type econometric model. From this model the time-varying herding parameters are efficiently estimated by a Markov chain Monte Carlo method. Using monthly data of KOSPI and DOW, this paper provides some empirical evidences for stronger herding in the Korean stock market than in the U.S. stock market, and further stronger herding after the global financial crisis than before it. More interesting finding is that time-varying herd behavior has weak autocorrelation and the global financial crisis may increase its volatility significantly.

Range of physicochemical parameters for active ingredients of herbicides (제초제의 활성 성분에 대한 물리-화학 파라미터의 범위)

  • Various physicochemical parameters for the active ingredients of 245 herbicides were calculated to develope a diagnosis and estimation system for utility as herbicide. The range of physico-chemical parameters for each inhibitors of photo system II (H1), acetolactate synthase (ALS) (H2) and herbicides were confirmed. The distribution ranges of 85% dependence for each physicochemical parameters were Obs.logP :$-0.90\sim4.50$, dipol moment: $1.80\sim12.22$ (debye), molecular refractivity: $53.0\sim104.0(cm^3/mol)$, polarizability: $19.0\sim37.0(\AA^3)$, HOMO energy: $-9.98\sim-7.34$ (eV), LUMO energy:$-2.76\sim0.40$ (eV), Van der Waals molecular volumes: $558.0\sim995.0(cm^3)$, molecular weight: $202.0\sim430.0$ (amu) and surface areas (Grids): $194.0\sim356.0(\AA^2)$, hydration energy: $-10.16\sim114.7$ Kcal/mol, respectively. It is suggested that MR and polarizability constants will be able to distinguish between herbicides and medicinal drugs. Results revealed that various compounds based on the range of physicochemical parameters of herbicides could be diagnosed and estimated.
