High Dose Rate Cobalt-60 After Loading Intracavitary Therapy of the Uterine Cervical Carcinoma in Srinagarind Hospital, Analysis of Residual Disease

  • Objectives: To evaluate residual disease in uterine cervical cancer patients treated with teletherapy using combined high dose rate Cobalt-60 brachytherapy. Materials and Methods: A retrospective study of uterine cervical cancer patients, FIGO stages IB-IVB (International Federation of Gynecologists and Obstetricians recommendations), treated by radiotherapy alone between April 1986 and December 1988 was conducted and the outcomes analysed. The patients were treated using teletherapy 50 Gy/25 fractions, five fractions per week to the whole pelvis together with HDR Cobalt -60 afterloading brachytherapy of 850 cGy/fraction, weekly to point A for 2 fractions. Results: The study covered 141 patients with uterine cervical cancer. The mean age was 50.0 years with a range of 30-78 years. The mean tumor size was 4.1 cm in diameter (range 1-8 cm). Mean follow - up time was 2.94 years (range 1 month-6.92 years). The overall incidence of residual locoregional disease was 3.5%. Residual disease, according to stage IIB, IIIB and IVA was present in 2.78%, 3.37% and 50.0%. It was noted that there was no evidence of residual disease in stage IB and IIA cases. Conclusion: Combined teletherapy along with high dose rate Cobalt -60 brachytherapy of 850 cGy/fraction, weekly to point A for 2 fractions resulted in overall 3.5% residual disease and a 96.5% complete response. The proposed recommendation for improving outcome is initiation of measurements for early detection of disease.

Impact of Treatment Strategies on Local Control and Survival in Uterine Carcinosarcomas in Turkey

  • Background: The purpose of this study was to determine the clinical characteristics, patterns of recurrence and survival outcomes in patients with uterine carcinosarcomas treated in our institution. Materials and Methods: Records of 26 patients diagnosed between 2007 and 2011 with uterine carcinosarcoma were retrospectively evaluated for demographic features, tumor characteristics, treatment regimens and patient outcomes in terms of DFS and OS Results: Median age was 61 (range 43-78). 10 patients (38%) had stage I disease at diagnosis, 3 (12%) had stage II, 4 (15%) had stage III and 9 (35%) had stage IV. Sixteen patients (62%) received chemotherapy with paclitaxel and carboplatin for 6 cycles. One patient underwent radiotherapy. Median follow up was 17 months. Sixteen patients relapsed and 13 died during follow up. Considering recurrence, 5 out of 16 patients had lung metastases, one had brain metastases and 9 had only intraabdominal recurrence. The 3 year DFS was 37% and the 3 year OS was 30%. Conclusions: Our data show that uterine carcinosarcomas tend to be at advanced stage at diagnosis and despite the use of chemotherapy, overall prognosis is poor. Surgery remains the mainstay of treatment. More effective adjuvant strategies are needed to reduce relapse and death rates.

푸마르산 수산화효소 유전자 결함에 따른 다발성 피부 평활근종의 치험례 (Multiple Cutaneous Leiomyoma Derived from the Mutation in Fumarate Hydratase Gene: A Case Report)

  • Purpose: Multiple skin leiomyoma and uterine myoma bearing autosomal dominant traits are benign smooth muscle tumors which originate in skin or female uterus. Skin leiomyoma occurs after gene mutation originating from arrector pili muscle of hair follicle where its clinical manifestations vary significantly from person to person. Our department hereby reports the histological findings and genetic evaluations of this very rare disease. Methods: A 57-year-old woman presented in our institute with multiple tumors in the left and central parts of her back that started to appear since 19 years ago. The patient was diagnosed as having uterine myoma 15 years ago and underwent hysterectomy. Biopsy has been done on the specimen, and genomic DNA was separated from Fumarate hydratase gene for it to go through PCR amplification. The results of PCR amplification were aligned by sequencer. Results: According to the results of biopsy, tumor cells were spindle-shaped and were aligned in a bundle where there was no dysplasia or mitosis. Moreover, these cells had abundant eosinophilic cytoplasm with elongated nucleus, and benign leiomyoma that showed positive reactions to SMA stain were found. In genetic examination, mutations such as heterozygous single nucleotide substitutions were found in alignments of amplified DNA. Conclusion: Multiple skin leiomyoma and uterine myoma are relatively uncommon diseases that are transmitted through autosomally dominant traits from genetic mutations. When a patient's chief complaint lies upon skin-colored or brown masses that occur in multiples appearing in the trunk or extremities with characteristic clinical symptoms and histological findings, and when the patient's family history is acknowledged such as skin or uterine leiomyoma or renal tumor, necessary genetic examination on multiple skin leiomyoma and uterine myoma could be done, and thereby precise diagnosis could also be made.

Prognostic Factors and Treatment Outcomes in 93 Patients with Uterine Sarcoma from 4 Centers in Turkey

  • Introduction: Uterine sarcomas are a group of heterogenous and rare malignancies of the female genital tract and there is a lack of consensus on prognostic factors and optimal treatment. Objective and Methodology: To perform a retrospective evaluation of clinicopathological characteristics, prognostic factors and treatment outcomes of 93 patients with uterine sarcomas who were diagnosed and treated at 4 different centers from November 2000 to October 2010. Results: Of the 93 patients, 58.0% had leiomyosarcomas, 26.9% malignant mixed Mullerian tumors, 9.7% endometrial stromal sarcomas, and 5.4% other histological types. According to the last International Federation of Gynecology and Obstetrics (FIGO) staging, 43.0% were stage I, 20.4% were stage II, 22.6% were stage III and 14.0 % were stage IV. Median relapse free survival (RFS) was 20 months (95% confidence interval (CI), 12.4-27.6 months), RFS after 1, 2, 5 years were 66.6%, 44.1%, 16.5% respectively. Median overall survival (OS) was 56 months (95% CI, 22.5-89.5 months), and OS after 1, 2, 5 years was 84.7%, 78%, 49.4% respectively. Multivariate analysis showed that age ${\geq}60$ years and high grade tumor were significantly associated with poor OS and RFS; patients administered adjuvant treatment with sequential chemotherapy and radiotherapy had longer RFS time. Among patients with leiomyosarcoma, in addition to age and grade, adjuvant treatment with sequential chemotherapy and radiotherapy after surgery had significant effects on OS. Conclusion: Uterine sarcomas have poor progrosis even at early stages. Prognostic factors affecting OS were found to be age and grade.

가미홍등탕(加味紅藤湯) 직장 내 투여가 자궁내막증을 유발한 흰쥐에 미치는 영향 (Effects of Retention Enema Therapy Gamihongdeung-tang on Surgically Induced Endometriosis in Rats)

  • Purpose : To observe the effect of retention enema therapy Gamihongdeung-tang on experimentally induced endometriosis. Methods : Rats with surgically induced endometriosis were administered in Gamihongdeung-tang rectally. After treatment for 40 days, the serum concentrations of progesterone, estradiol, tumor necrosis factor-${\alpha}$(TNF-${\alpha}$), interleukin(IL)-2, IL-4, IL-6 and IL-10 were measured. Results : The size of the ectopic uterine implants in the treated group was much smaller than that in the control group. The concentration of estradiol was significantly lower in the experimental group than in the control group. The IL-4, IL-6 levels were significantly lower in the experimental groups than the control groups. TNF-${\alpha}$, IL-2 levels were lower and IL-10 level was higher in the experimental groups than in the control groups but the difference was not significent. The progesterone level was similar in the experimental and control group. Conclusion : These results indicate that Retention enema therapy with Gamihongdeung-tang reduces the size of ectopic uterine implants at the serosal wall and inhibits the growth of ectopic uterine implants. This suggests that retention enema therapy with Gamihongdeung-tang is an effective treatment for endometriosis.

트로카 삽입 부위에 생긴 림프관 확장을 동반한 기생 평활근종: 증례 보고 (Parasitic Leiomyoma with Lymphatic Dilatation in Trocar Port-Site of Abdominal Wall: A Case Report)

  • 자궁 근종은 여성에서 가장 흔한 양성 골반 강 내 종양이며 증상이 있을 경우 수술적 제거를 하게 된다. 내시경 수술이 발달하게 되면서 포트 삽입 부위의 기생 평활 근종들이 보고되기 시작하였다. 40세의 여자 환자가 왼쪽 아랫배에서 만져지는 종괴를 주소로 외래로 내원하였고 2년 전 내시경 근종 제거를 받은 과거력이 있었다. 조영증강 CT와 MRI를 시행하여 해당 종괴에 대한 평가를 시행하였으며 수술적 제거를 시행하였다. 병리 검사에서 해당 종괴는 기생 평활 근종으로 최종 진단되었는데 이는 이전 내시경 근종 제거술을 시행한 환자에서 수술부위에 만져지는 종괴를 주소로 내원하였을 때 반드시 기생 평활 근종을 감별 진단으로 고려하여야 함을 시사한다.

Differentiating Uterine Sarcoma From Atypical Leiomyoma on Preoperative Magnetic Resonance Imaging Using Logistic Regression Classifier: Added Value of Diffusion-Weighted Imaging-Based Quantitative Parameters

  • Objective: To evaluate the added value of diffusion-weighted imaging (DWI)-based quantitative parameters to distinguish uterine sarcomas from atypical leiomyomas on preoperative magnetic resonance imaging (MRI). Materials and Methods: A total of 138 patients (age, 43.7 ± 10.3 years) with uterine sarcoma (n = 44) and atypical leiomyoma (n = 94) were retrospectively collected from four institutions. The cohort was randomly divided into training (84/138, 60.0%) and validation (54/138, 40.0%) sets. Two independent readers evaluated six qualitative MRI features and two DWI-based quantitative parameters for each index tumor. Multivariable logistic regression was used to identify the relevant qualitative MRI features. Diagnostic classifiers based on qualitative MRI features alone and in combination with DWI-based quantitative parameters were developed using a logistic regression algorithm. The diagnostic performance of the classifiers was evaluated using a cross-table analysis and calculation of the area under the receiver operating characteristic curve (AUC). Results: Mean apparent diffusion coefficient value of uterine sarcoma was lower than that of atypical leiomyoma (mean ± standard deviation, 0.94 ± 0.30 10-3 mm2/s vs. 1.23 ± 0.25 10-3 mm2/s; P < 0.001), and the relative contrast ratio was higher in the uterine sarcoma (8.16 ± 2.94 vs. 4.19 ± 2.66; P < 0.001). Selected qualitative MRI features included ill-defined margin (adjusted odds ratio [aOR], 17.9; 95% confidence interval [CI], 1.41-503, P = 0.040), intratumoral hemorrhage (aOR, 27.3; 95% CI, 3.74-596, P = 0.006), and absence of T2 dark area (aOR, 83.5; 95% CI, 12.4-1916, P < 0.001). The classifier that combined qualitative MRI features and DWI-based quantitative parameters showed significantly better performance than without DWI-based parameters in the validation set (AUC, 0.92 vs. 0.78; P < 0.001). Conclusion: The addition of DWI-based quantitative parameters to qualitative MRI features improved the diagnostic performance of the logistic regression classifier in differentiating uterine sarcomas from atypical leiomyomas on preoperative MRI.

Mitochondrial tumor suppressor 1 is a target of AT-rich interactive domain 1A and progesterone receptor in the murine uterus

  • Objective: Progesterone receptor (PGR) and AT-rich interactive domain 1A (ARID1A) have important roles in the establishment and maintenance of pregnancy in the uterus. In present studies, we examined the expression of mitochondrial tumor suppressor 1 (MTUS1) in the murine uterus during early pregnancy as well as in response to ovarian steroid hormone treatment. Methods: We performed quantitative reverse transcription polymerase chain reaction and immunohistochemistry analysis to investigate the regulation of MTUS1 by ARID1A and determined expression patterns of MTUS1 in the uterus during early pregnancy. Results: The expression of MTUS1 was detected on day 0.5 of gestation (GD 0.5) and then gradually increased until GD 3.5 in the luminal and glandular epithelium. However, the expression of MTUS1 was significantly reduced in the uterine epithelial cells of $Pgr^{cre/+}Arid1a^{f/f}$ and Pgr knockout (PRKO) mice at GD 3.5. Furthermore, MTUS1 expression was remarkably induced after P4 treatment in the luminal and glandular epithelium of the wild-type mice. However, the induction of MTUS1 expression was not detected in uteri of $Pgr^{cre/+}Arid1a^{f/f}$ or PRKO mice treated with P4. Conclusion: These results suggest that MTUS1 is a novel target gene by ARID1A and PGR in the uterine epithelial cells.

자궁경부암 방사선치료시 Squamous Cell Carcinoma Antigen(SCC) 고k Carcinoembryonic Antigen(CEA) 의 종양표지자로서의 의의 (Pre-radiotherapy and Post-radiotherapy Serial Serum Squamous Cell Carcinoma Antigen (SCC) and Carcinoembryonic Antigen (CEA) in the Monitoring of Squamous Cell Carcinoma of Uterine Cervix)

  • 목적 : 자궁경부암의 치료후의 재발이나 병소의 지속을 발견하기 위한 종양표지자로서의 squamous cell carcinoma antigen(SCC) 와 carcinoembryonic antigen(CEA) 의 의의를 확인하고 향후의 치료시에 참고하고자 한다. 대상 및 방법 : 1995년 10월부터 1998년 8월까지 단국대학병원 치료방사선과에서 원발성 자궁경부암으로 근치적 방사선치료를 시행 받은 환자 중 22례를 대상으로 하였고 방사선치료전에 MRI 나 초음파를 시행하여 종양의 체적을 구하였으며 방사선치료 전에 SCC 와 CEA를 측정하였고 치료 후에도 주기적으로 SCC 와 CEA를 측정 하여 분석 하였다 결과 : 방사선 치료전에 측정한 SCC 치는 전체 22례 중 68.2$\%$ 에서 상승되어 있었으며, CEA 치는 측정이 이루어진 21례 중 19.0$\%$에서 상승되어 있었다. 종괴의 부피와 치료전의 SCC 치의 관련성은 극단적으로 벗어난 1례를 제외하면 상관계수가 0.59382 로 상당히 컸다. 종괴의 부피증가에 따른 CEA 치의 증가경향은 없었다. 치료후의 SCC 치는 9.1$\%$ 에서 CEA 치는 측정이 이루어진 21례 중 4.8$\%$ 에서 상승되어 있었다. 방사선치료 전에SCC 치가 상승되어 있었던 15례 중 14례에서 완전관해의 소견을 보였으며 그 14례 중 SCC 치가 정상 범위로 낮아진 경우가 13례(92.9$\%$) 였다. SCC 치가 치료 전에 상승되어 있다가 치료 후에 정상화 된 13례 중 5례에서 SCC 치의 정상 범위 이상의 재상승이 나타났는데 그 중 4례는 국소실패(3례) 혹은 국소 및 주변 림프절 실패(1례) 였고 나머지 1례는 임상적으로 무병상태(NED) 이다. 따라서 재발과 관련된 SCC 치 상승의 민감도는 100$\%$, 특이도는 93.8$\%$, 양성예측도는 85.7$\%$ 였다. SCC 의 재상승이 있은 후 임상적 재발을 발견할 때까지의 시간차이(lead time) 는 평균 1.2개월이었다. CEA 는 치료전 높았던 4례 모두에서 치료 후 완전 관해시에 정상화 되었고 이들 중 재발한 경우는 없었다. 결론 : 본 연구에서 SCC 와 CEA 는 치료전의 값이 상승되어 있었던 경우는 치료에 대한 반응과 재발을monitor 하는데 좋은 종양표지자로 나타났다. 그러나 치료전 SCC 치가 민감도가 높은데 비해 CEA 치는 민감도가 낮으며 CEA 와 SCC 를 모두 측정한 경우가 SCC 만 측정한 경우에 비해 예후의 관찰에 유리한 점이 없었다.

Relaxant and anti-inflammatory effect of two thalidomide analogs as PDE-4 inhibitors in pregnant rat uterus

  • The aim of this study was to evaluate the relaxant and anti-inflammatory effects of two thalidomide analogs as phosphodiesterase-4 (PDE-4) inhibitors in pregnant rat uterus. Uteri from Wistar female rats were isolated at 19 day of pregnancy. Uterine samples were used in functional studies to evaluate the inhibitory effects of the thalidomide analogs, methyl 3-(4-nitrophthalimido)-3-(3,4- dimethoxyphenyl)-propanoate (4NO2PDPMe) and methyl 3-(4-aminophthalimido)- 3-(3,4-dimethoxyphenyl)-propanoate (4APDPMe), on prostaglandin-$F2{\alpha}$ ($PGF2{\alpha}$)-induced phasic, $K^+$-induced tonic, and $Ca^{2+}$-induced contractions. Accumulation of cAMP was quantified in uterine homogenates by ELISA. Anti-inflammatory effect was assessed by using ELISA for determination of the pro-inflammatory cytokines tumor necrosis factor-${\alpha}$ ($TNF{\alpha}$) and interleukin (IL)-$1{\beta}$, and anti-inflammatory IL-10, from uterine explants stimulated with lipopolysaccharide (LPS). Nifedipine, forskolin and rolipram were used as positive controls where required. Both thalidomide analogs induced a significant inhibition of the uterine contractions induced by the pharmaco- and electro-mechanic stimuli. Nifedipine and forskolin were more potent than the analogs to inhibit the uterine contractility, but these were more potent than rolipram, and 4APDPMe was equieffective to nifedipine. Thalidomide analogs increased uterine cAMP-levels in a concentration-dependent manner. The LPS-induced $TNF{\alpha}$ and $IL-1{\beta}$ uterine secretion was diminished in a concentration-dependent fashion by both analogs, whereas IL-10 secretion was increased significantly. The thalidomide analogs induced utero-relaxant and anti-inflammatory effects, which were associated with the increased cAMP levels as PDE-4 inhibitors in the pregnant rat uterus. Such properties place these thalidomide analogs as potentially safe and effective tocolytic agents in a field that urgently needs improved pharmacological treatments, as in cases of preterm labor.
