The voluntary contracted EMG signal of spinal cord injured patients is very small because the information from central nervous system is not sufficiently transmitted to $\alpha$ motor neuron or muscle fiber. Therefore the acquisited EMG signal from needle or surface electrodes can not be identified obvious voluntary contraction pattern by muscle movement. In this paper we propose the extraction technique of voluntary muscle contraction and relaxation pattern from EMG signal of spinal cord injured patient whose EMG signal is composed of the linear sum of mo색 unit action potentials with two noise sources, additive noise assumed to be white Gaussian noise and high frequency discharge assumed to be not motor unit action potential but impulsive noise. In order to eliminate impulsive noise and additive noise from voluntary contracted EMG signal, we use the FatBear filter which is a nonarithmetic piecewise constant filter, and multiscale nonlinear wavelet denoising processing, respectively. The proposed technique is applied to the EMG signal acquisited from transverse myelitis patients to extract voluntary muscle contraction pattern.
Objectives: It is a statistical study to examine the data of CaPSPI (Diagnostic System for Climacteric and Postmenopausal Syndrome Pattern Identification), developed for diagnosis of menopause disorders and to record the status of treatment of it. Methods: From November 1, 2020 to June 19, 2021, 36 cases of data of 33 respondents of the CaPSPI were analyzed. For the use of the basic data of the clinical menopausal disorder, we investigated frequency of menopausal symptoms and the difference between them depending on the period of menopause, and the presentation of usage prescriptions. And the diagnostic results for three kinds' diagnosis [for examination (D1), for treatment (D2), by doctors (D3)] were compared. The diagnostic consistency of D1 and D3 and the statistical significance between DT and disease elements (證素) was investigated. Results: 1. Hot flush was the highest in the symptom survey of the menopause that the subjects complained of, followed by insomnia. There was no significant difference in symptom expression according to menopausal period. 2. The diagnostic consistency of D1 and D2 showed significant diagnostic consistency only in liver depression, and the diagnostic consistency of D1 and D3 showed significant consistency in liver depression and Dual Deficiency of Heart and Spleen. 3. D3' diagnosis and disease elements had statistical significance for cases of P<0.1 was found to be related to the theory of oriental medicine. Conclusions: It is needed to continue to accumulate diagnosis and treatment results through CaPSPI in the future to strengthen the basis for patten identification and treatment of menopause disorders.

The results of treatment of eighteen lesions of fibrous dysplasia which of them seventeen lesions were treated with surgery were reviewed at the Department of Orthopedic Surgery, College of Medicine, Hanyang University Hospital. We studied to evaluate the functional clinical results and the recurrence according to the type of disease, grafted bone, methods of treatment and location of lesion. We treated sixteen patients(five males and eleven females) and their mean age was 22.6 years. There was no association with skin pigmentation or dysfunction of endocrine system. Twelve patients had a monostotic pattern and four patients had a polyostotic pattern. Twelve lesions were treated with curettage and bone grafting and four lesions in the proximal femur were treated by internal fixation with curettage and bone grafting. One lesion was treated by en-bloc resection. There were eleven satisfactory results in twelve monostotic lesions and there were four satisfactory results in five polyostotic lesions, but the recurrence were four cases, respectively. The two unsatisfactory results were seen in two patients treated by autografting, but there were three recurrence of four lesions in autografting only, one of five in autografting and allografting, four of eight in allgrafting or xenografting. Four of six lesions in upper extremity were recurred after curettage and bone grafting and five of them showed satisfactory results. In pelvic and lower extremity lesions, the recurrence were occurred in two lesions after curettage and bone grafting and in two lesions after internal fixation and bone grafting. The recurrence does not always provide an unsatisfactory functional results and the recurrence showed higher rate in radiologic pattern of ground glass appearance with-out marginal sclerotic rim, but the recurrence according to grafted bone showed similar rates. Curettage and bone grafting is adequate for a symptomatic lesion and firm internal fixation is needed for a lesion in proximal femur. In use of grafted bone, xenograft or allograft may be preferable to autograft because of the disadvantage of autografting like a increased blood loss, prolonged operation times, etc.
Objectives : The aim of this study was to survey the present utilization of pattern identification(PI) by chronic disease and using PI system in the clinical field among Korean medicine doctors. Methods : This survey was conducted from Oct. 1 to Oct. 31, 2014 by face-to-face interview using structured questionnaire. The subjects were 400 Korean medicine doctors who worked in hospitals or clinics. The questionnaire consisted of whether use clinical experience by chronic disease and the utilization of PI, the utilization of PI systems, the PI utilization rate, and the correlation between utilization PI and so on. General elements consisted of sex, age, clinical experience, place of work, district and so on. Results and Conclusions : This study revealed that clinical experience by chronic disease used 24.0-90.8%. The most chronic diseases were chronic fatigue and chronic pain, and least disease was cancer. Experience of the utilization of PI among Korean medicine doctors who have clinical experience was 87.8-97.1%. Also, the most utilized disease was chronic gastritis & gastric ulcer, and least disease was dyslipidemia. In this case, the most common cases were using the one PI between the utilization of PI systems, and the utilization ratio of Visceral PI was the highest. Phi coefficient between the PI systems in chronic diseases divided into positive correlation and negative correlation. The correlation of dementia between Triple energizer PI and Six excesses PI was the highest at positive correlation, and the frequency of occurrence between Defense, Qi, Nutrient and Blood PI was the highest at positive correlation. The correlation of osteoarthropathy between Meridian and Collateral PI and Sasang Constitutional Medicine was the highest at negative correlation. Also the frequency of occurrence between Qi, Blood, Fluid and Humor PI and Sasang Constitutional Medicine was the highest at negative correlation. We hope that additional studies on systematic PI research of chronic disease which needed to be in korean oriental medicine and meet the needs of clinical consumer continue based on this study.

PURPOSE: This study was conducted to compare different standing postures with the use of standing aids for lumbar spine posture and muscle activity, and to identify the most desirable standing posture. METHODS: The lumbopelvic angle was assessed based on static radiographic measurement on the sagittal plane. Lumbar lordosis, lumbosacral lordosis, and the intervertebral joint angle at L1/L5 and L5/S1 were measured using radiography in three standing postures (standing on level ground, standing with one foot on a platform, and standing on a sloped surface). In addition, muscle activity was measured using surface electromyography to examine the co-contraction of the lumbar and hip muscles. RESULTS: Lumbar lordosis, lumbosacral lordosis, and L5/S1 intervertebral joint flexion occurred with one foot on the platform. No significant differences were found between standing on a sloped surface and standing on level ground. However, muscle co-contraction was reduced with the use of standing aids. CONCLUSION: This study demonstrated that standing with a foot on a platform induced lumbar lordosis, but that there was no significant difference between standing on a sloped surface and standing on level ground. However, muscle co-contraction was reduced with the use of standing aids. Based on the motor control pattern as a predictor of LBP, the use of standing aids would help workers during prolonged standing.
The proliferation of new technologies has significantly enhanced the diagnostic capabilities of flexible bronchoscopy compared with traditional methods. Narrow band imaging (NBI), an optical technique in which filtered light enhances superficial neoplasms based on their neoangiogenic patterns, was developed to screen for central intraepithelial moderate or severe dysplasia, carcinoma in situ (CIS), and microinvasive neoplasia in patients at risk for lung cancer. Because angiogenesis occurs preferentially in dysplastic and neoplastic lesions, NBI may identify early dysplastic lesions better than white light bronchoscopy (WLB) currently in use. NBI bronchoscopy can be used not only to detect precancerous lesions, but also to screen for cancerous lesions. We prospectively evaluated 101 patients with suspected lung cancer between July 2009 and June 2010. All were previously scheduled for flexible bronchoscopy CT scans. Abnormal NBI was defined by Shibuya's descriptors (tortuous, dotted, or spiral and screw patterns). Biopsies of 132 lesions in 92 patients showed that 78 lesions (59.1%) were malignant and 54 (40.9%) were benign. The diagnostic sensitivity of bronchoscopy in detecting malignancy was 96.2% (75/78). When assorted by lesion pattern, the sensitivity and specificity of NBI bronchoscopy in detecting malignancies were 69.2% (54/78) and 96.3% (52/54), respectively, for the spiral and screw pattern and 14.1% (11/78) and 96.3% (52/54), respectively, for the dotted pattern. Unexpectedly, additional cancerous lesions were detected in five patients (2 dotted and 3 spiral and screw). As a screening tool for malignant lesions, NBI bronchoscopy should assess combinations of all three lesion. The dotted and spiral and screw patterns may be helpful in determining which lesions should be biopsied. NBI bronchoscopy may be useful not only for the diagnosis of early-stage lung cancer but also for more accurate local staging of lung cancer.
The purpose of this study was to make an analysis of the impact of the DRG payment system on medical care pattern and cost of cataract surgery in a general hospital. The subjects were 173 patients whose DRG severity grade was zero, selected from among the hospitalized who underwent cataract surgery before and after the joining to the demonstrational operation of the third year DRG payment system. Their medical records and the details of their medical bills were examined to find out the length of hospital stay, medical care pattern provided to them, the cost of medical care, and the quality of medical care. The length of stay and the amount of medical care supplied during being in hospital dropped significantly for both single-eye and double-eyes cataract surgery groups. The amount of antibiotic use went down during the hospitalization and upon discharge from the hospital, but decreased after discharge. The total medical bills and the rate of basic examination implementation increased in the OPD before hospitalization but after discharge dropped. For double-eyes cataract patients, the rate of double-eyes cataract surgery went down. The total medical bills of DRG payment system converted into the fee-for-service system was greater by 113.3% for the single-eye cataract surgery group and by 102.9% for the doble-eyes cataract surgery group, compared to that by the fee-for-service. The contribution shared by the insurance corporation increased for both single-eye and double-eyes cataract surgery groups, but the copayment by the insured went down. Regarding the treatment outcome, no difference was found in complication rate, resurgery rate and mortality rate before and after the joining to the DRG payment system was implemented. The use of special lens lessened significantly. The amount of medical care supplied during hospitalization decreased but the complication rate didn't increase. But the increased use of low-price artificial cataract and the avoidance of double-eyes cataract surgery was observed. The phenomenon decreased number of OPD visit and the decreased total medical bills of OPD care after discharge in this hospital required further evaluation.
Objectives: The purpose of this study was to evaluate the achievements of Ryodoraku and to suggest new diagnostic ideas. Methods: A search of six Korean databases and one Japanese was performed using the terms such as Ryodoraku, diagnosis, foreigner, etc. and the search results classified and summarized. Results: From the initial search results, 21 Korean papers and 26 Japanese papers were selected and classified into 4 categories, that is, classification by pattern, classification by physiological limit, classification by setting various sections using the average Ryodoraku score, and classification by formula. Conclusions: Each of the 4 methods has its own benefits; however, it is hard to find disease-specific common characteristics from the Ryodoraku diagnostic values with the methods. Further studies which use the number of Pyesaek and Gyeokcha are necessary.

The purpose of this study was to evaluate the program for the control of acute respiratory infections(ARI) in children in a Korean rural area(Yonchon county). Evaluating the program, we focused on the pattern of prescription and appropriateness of antibiotics prescribed by the health personnel who had participated in the ARI Control Program. It was implemented at the primary health care setting in rural area, such as district hospital, health subcenters, and health posts. During six-months programme monitoring period, medical records were reviewed and collected data were analysed by the pediatrician, research coordinator of this study. The baseline data were collected from medical records of the same period(six months) of one year before the implementation of the ARI programme. The study results were as follow : 1. Common cold was the most prevalent disease(78.7%. 594 cases) among the all ARI cases (755 cases). The less frequent cases were bronchitis(11.9%), acute pharyngitis(5.2%), and pneumonia(1.8%). 2. Significant reduction in the use of antibiotics was observed after the programme implementation. Ninety three(15.7%) of 594 common cold cases were received antibiotics compared with 282(35.2%) of 802 in the baseline period. In the cases of bronchitis and acute pharyngitis, the reduction rates were 15.1% and 23.2% respectively compared to the baseline period. 3. Mean duration of antibiotics prescription was 1.81-1.75 days, similar to the baseline data. 4. The appropriateness rate of antibiotics prescriptions were 84.3%(common cold), 35.6% (bronchitis) and 28.2%(acute pharyngitis). In the case of pneumonia, the antibiotics prescription was compatible to the criteria developed. 5. Pediatrician prescribed antibiotics more appropriately for all cases than general practitioners in health sub-center, and nurse practitioners in health posts. 6. Antibiotics therapy was shown to be of no effect in the treatment of the all ARI cases. At the 5 and 10 days check-up of common cold cases after visits, proportion of improved patients were 58.3% in the antibiotics-used group and 51.4% in the control group. In the other cases of ARI, the patterns of response were similar to common cold. None of the differences in outcome between the antibiotics-used and control group was statistically significant. This ARI programme may have substantial a substantial impact on antibiotics use at the public health institutions(district hospital, health subcenters, health posts) which are of major domain for primary health care in Korean rural areas.
Objective: In this case study, we describe the use of Shihogayonggolmoryo-tang-gami to treat an in-patient with delirium. Methods: An in-patient with symptoms of delirium was treated with Korean medicine therapies, including the herbal medication Shihogagyonggolmoryo-tang-gami, for 7 days. To evaluate its therapeutic effect, we used the Nursing Delirium Screening Scale (Nu-DESC) and Memorial Delirium Assessment Scale (MDAS). Results: After the treatment, Nu-DESC and MDAS scores improved. Of note, instances of the patient calling out improved, in addition to the patient's sleeping pattern. Although all the symptoms of delirium did not disappear completely, Shihogayonggolmoryo-tang-gami had therapeutic effects. Conclusion: These results suggest that Shihogayonggolmoryo-tang-gami may have a beneficial effect on delirium, without using other antipsychotic medications.
