Hyeongok's Bobee-tang was Combined according to the Theory for Properties and Tastes of Herbal Medicines (현곡(玄谷) 보비탕(補脾湯)의 구성한약과 그 기미배오(氣味配伍) 분석)

  • The objectives of this study lie in theoretical establishment of Bobee-tang for curing the asthenic syndrome of spleen through analyzing the component medicines and combination principles of Hyeongok's Bobee-tang, and furthermore, maximizing the clinical use of Bobee-tang. This study analyzed the component medicines and combination principles of Hyeongok's Bobee-tang based on the theory for properties and tastes of herbal medicines from the ${\ulcorner}$Yellow Emperor's Canon of Internal Medicine${\lrcorner}$, the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine, and the five elements doctrine. Hyeongok's Bobee-tang is an odd prescription, composed of 7 kinds of ingredients : No.1 Radix Ginseng (2don;7.5 g), No.2 Rhizoma Atractylodis Macrocephalae (1don;3.75 g), No.3 Carapax Et Plastrumtestudinis (1don), No.4 Concha Ostreae (1don), No.5 Cortex Cinnamomi (1don), No.6 Rhizoma Zingiberis Praeparatae (1don), and No.7 Radix Glycyrrhizae (1don). First, to cure the asthenic syndrome of the spleen, the methods of invigorating the spleen and the heart, and purging the liver should be used according to the five elements doctrine. Secondly, herbal medicines appropriate for those treatment methods should be chosen according to the theory for properties and tastes of herbal medicine and thirdly, the combination of those herbal medicines should be carried out according to the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine. As a good example, Hyeongok's Bobee-tang is combined according to the above theories. In conclusion, this formula was created by applying to the theory for properties and tastes of herbal medicines.

Anti-inflammatory effects of herbal medicines(Rubus coreanus, Rehmanniae Radix, Houttuynia cordata, Betulae cortex) EtOH extract on acute atopic dermatitis mice (급성 아토피 피부염 마우스 모델에서 한약조합약물의 염증반응 진정효과)

  • Object : The object of this study was effected of composed of 4 herbal medicines(Rubus coreanus, Rehmanniae Radix, Houttuynia cordata, Betulae cortex) on acute atopic dermatitis mice. Methods : BALB/c mice were divided 4 groups ; normal group, negative control group of acute atopic dermatitis mice induced by DNCB, treated apply to the back skin with the herbal medicine group, and treated orally with herbal medicine group. Treated with herbal medicine and DNCB during 2 weeks. After treated, measured WBCs, RBCs, and platelet in the blood, and analysed mRNA expression of spleen. Result : Composed herbal medicines could reduce WBCs, lymphocytes, monocytes, neutrophils, and eosinophils in the mouse blood. And, could down TNF-${\alpha}$ levels in spleen. In the skin tissue histology results, herbal medicines reduce the cells and T cells. Conclusion : Composed herbal medicines help to another inflammatory disease because they reduce the cells and T cells in the skin tissues. And they have inhibitory effects of TNF-${\alpha}$ mRNA expression, and Th 2 immune responses. Therefore herbal medicines use for an acute atopic dermatitis treatment.

Analysis of the Properties and Tastes In Hyeongok's Formulas for Invigorating Five Viscera and their Ingredients (현곡(玄谷) 오장(五臟) 보방(補方)의 조성 약물과 그 기미배오(氣味配伍) 분석)

  • Objectives : The objective of this study lies in theoretical establishment of formulas for invigorating five viscera for curing the asthenic syndrome of five viscera through analyzing the component medicines and combination principles of Hyeongok's formulas for invigorating five viscera, and furthermore, maximizing the clinical use of them. Methods : This study analyzed the component medicines and combination principles of Hyeongok's formulas for invigorating five viscera based on the theory for properties and tastes of herbal medicines from the "Yellow Emperor's Canon of Internal Medicine", the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine, and the five elements doctrine. Hyeongok's formulas for invigorating five viscera are Bogantang, Boseemtang, Bobeetang, Bopyeotang, and Boshintang. Results : There are three methods for curing the asthenic syndrome of five viscera according to the five elements doctrine: in case of Bogantang invigorating the liver, invigorating the kidney and purging the lung; Boseemtang, invigorating the heart, invigorating the liver and purging the kidney; Bobeetang, invigorating the spleen, invigorating the heart and purging the liver; Bopyeotang, invigorating the lung, invigorating the spleen and purging the heart; Boshintang, invigorating the kidney, invigorating the lung and purging the spleen. Conclusions : First, to cure the asthenic syndrome of five viscera, the methods of invigorating five viscera, and purging five viscera should be used according to the five elements doctrine. Secondly, herbal medicines appropriate for those treatment methods should be chosen according to the theory for properties and tastes of herbal medicine and thirdly, the combination of those herbal medicines should be carried out according to the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine.   • There are four kinds of formulas for purging the liver to cure its sthenic syndrome based on the types of preparation formulas : Sagan-tang, Saganhwan, Saseem-san and Saganeum. Another formula called Sacheonghwan, Sacheong-tang and Sacheong-san is to purge the green colour of liver. There are 38 kinds of Sagan-tang, 2 kinds of Saganhwan, 29 kinds of Sagan-san, 5 kinds of Saganeum, 4 kinds of Sacheonghwan, 3 kinds of Sacheong-tang and 1 kind of Sacheong-san. Combination of herbal medicines, carried out in formulas for purging the liver, consists of various kinds depending on medical scientists' personal experience in medical treatment without any general principles, which makes it difficult to apply it to clinical use. The objectives of this study lie in theoretical establishment of Sagan-tang for curing the sthenic syndrome of liver through analyzing the component medicines and combination principles of Hyeongok's Sagan-tang, and furthermore, maximizing the clinical use of Sagan-tang. In the case of taste purgation, two herbal medicines with sour taste, Radix Paeoniae and Fructus Chaenomelis, are combined into the principal and assistant herbal medicine, respectively. For property purgation, two herbal medicines with the cool property, Radix Bupleuri and Radix Scutellariae, are combined into adjuvant herbal medicines. Both sweet and cold herbal medicines, Radix Rehmanniae and Folium Phyllostachydis Henonis, are combined into adjuvant herbal medicines. Sour herbal medicines, Radix Paeoniae and Fructus Chaenomelis, were combined to invigorate the lung. Cool herbal medicines, Radix Bupleuri and Radix Scutellariae, were combined to invigorate the lung and to purge the liver. In addition, Radix Glycyrrhizae are combined as dispatcher herbal medicine, harmonizing all the herbal medicines composing the formula. First, to cure the sthenic syndrome of the liver, the methods of purging the liver and the heart, and invigorating the lung should be used according to the five elements doctrine. Secondly, herbal medicines appropriate for those treatment methods should be chosen according to the theory for properties and tastes of herbal medicine and thirdly, the combination of those herbal medicines should be carried out according to the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine. As a good example, Hyeongok's Sagan-tang is combined according to the above theories.

Commonly used herbal medicines in the treatment for female reproductive dysfunction

  • Herbal medicines are widely used for centuries to treat illness and improve health in Asia and the use of these remedies has become a worldwide form of alternative therapy. However, there is very limited evidence from randomized controlled trials to support the efficacy of the vast majority of herbal products. Of the commonly used herbal medicines, systemic clinical experiences and promising experiments in animals have found potential evidence of efficacy for six herbal medicines (Unkei-to, Hachimijio-gan, Keishi-bukuryo-gan, Toki-shakuyaku-san, Shakuyaku-kanzo-to, Sairei-to) in the treatment for female reproductive dysfunction. Because herbs may contain potent bioactive substances, the physician should have an opportunity to outline more stringent regulation, similar to over-the counter drug.

A Study on the Trend and Meaning of Searching for Herbal Medicines in Online Portal Using Naver DataLab Search Trend Service (네이버 데이터랩 검색어 트렌드 서비스를 이용한 온라인 포털에서의 한약재 검색 트렌드와 의미에 대한 고찰)

  • Objectives : From January 2020, when the first confirmed case of COVID-19 in Korea, the use of health information using the Internet is expected to increase. It is expected that there will be a significant change in the general public's interest in Korean herbal medicines for health care. Therefore, in this study, we tried to confirm the change in the search trend of Korean herbal medicines after the COVID-19 epidemic. Methods : Using the "Naver DataLab (http://datalab.naver.com)" service of a Korean portal site Naver, search volume was investigated with 606 Korean herbal medicines as keywords. The search period was from January 2020, right after the onset of COVID-19, to June 2021. The search results were sorted by the peak search volume and the total search volume. Results : 'Cheonsangap (천산갑, 穿山甲, Manitis Squama)' was the most searched Korean herbal medicine in the peak search volume and total search volume with least bias. Conclusions : The problem of supply and demand of Korean herbal medicines of high public interest was identified. Broadcasting and media exposure were the factors that had a big impact on the search volume for Korean herbal medicines. As it was confirmed that the search volume for Korean herbal medicines increased rapidly due to media exposure, it is necessary to provide correct information about Korean herbal medicines, improve public awareness, and manage stable supply and demand based on continuous search trend monitoring.

  • Hepatitis, liver cirrhosis and liver cancer caused by viral infection are among the most prevalent causes of death in Korea. Several medicines have been in use despite their nonsatisfactory effects on these disease. Some herbal medicines put to use recently have not shown beneficial effects, either. This paper evaluates the effects of extracts from 10 traditional Korean herbal medicines on rats with hepatic damage induced by galactosamine. Rubus coreanus showed an anti-inflammatory effect as shown on the data of activities of serum transaminases.

Characteristics of Korean Medicine Clinic Affecting the Form of Extramural Herbal Dispensaries - Focusing on the 2021 Korean Herbal Medicine Consumption Survey (원외탕전 형태에 영향을 미치는 한의원의 특성 - 「2021 한약소비실태조사」를 중심으로)

  • Objectives : This study was conducted to prepare basic data for improving the system related to Extramural herbal dispensaries by analyzing the characteristic factors of Korean medicine clinics that affect the type of Extramural herbal dispensaries. Methods : This study utilized '2021 Korean Herbal Medicine Consumption Survey', with 1,817 final participants. We performed ANOVA to compare the characteristics of Korean medicine clinics according to the type of extramural herbal dispensary. Multinomial logistic regression analysis were conducted to analyze characteristics of Korean medicine clinics affecting the form of extramural dispensaries. Results : Among Korean medicine clinics, 71.22% of the institutions used only the intraherbal dispensaries, 11.12% of the institutions used the Extramural herbal dispensaries in parallel, and 17.67% of the institutions used only the Extramural herbal dispensaries. The characteristics of Korean medicine clinics that increased the probability of concurrently using extramural herbal dispensaries were institution area, bed presence or absence, number of regular workers, region, number of prescriptions for insurance herbal medicines, and number of prescriptions for uninsured herbal medicines. The characteristics of Korean medicine clinics, which increase the probability of using only extramural herbal dispensaries, were age, durations of clinical experience, specialist license, franchise, area, bed presence or absence, number of regular workers, number of prescriptions for uninsured herbal medicines, and price of treatment for diseases. Conclusion : The reason why Korean medicine clinics use extramural herbal dispensaries is the result of a combination of efficient use of clinic size and the preparation of uninsured herbal medicines.

Evidence-based herbal medicine in efficacy and safety assessments

  • Herbal medicine is the use of medicinal plants for prevention and treatment of diseases: it ranges from traditional and popular medicines of every country to the use of standardized and tritated herbal extracts. Generally cultural rootedness enduring and widespread use in a traditional medical system may indicate safety, but also efficacy of treatments, especially in herbal medicine where tradition is almost completely based on remedies containing active principles at very low and ultra low concentrations or relying on magical-energetic principles. The efficacy and safety assessments of medicines, whether modern or herbal, invariably encounter challenges or problems during the course of pre-clinical and clinical research. Some of the challenges in evidence-based herbal medicinal research are unique, and the researcher must be cognizant of them in order to safeguard the quality of the data obtained. Key challenges are: the quality of raw materials; appropriateness of biological/pharmacological activity assessment methodology, and data interpretation; standardization methodology; pharmacokinetics and bioavailability of active constituents and metabolites; clinical dosage formulation/production; and clinical study designs and outcome measures.

The Clinical Effectiveness of Frequently Used Herbal Medicines for Cerebrovascular Disease: A Retrospective Study (뇌혈관질환에 대한 다빈도 처방 한약의 임상적 유효성 : 후향적 연구)

  • Objectives: With the aging of the population, the incidence of a variety of diseases is increasing. Cerebrovascular disease is a major cause of mortality worldwide and therefore requires timely treatment and management. We aimed to evaluate the clinical effectiveness of herbal medicines in treating cerebrovascular disease by screening the most frequently prescribed herbal medicines and analyzing the clinical data of patients with cerebrovascular disease taking these medicines. Methods: In this retrospective study, we analyzed the electronic medical records of patients with cerebrovascular disease who received herbal medication at the Pusan National University Korean Medical Hospital from 10 March 2010 to 31 March 2017. The inclusion criteria were as follows: diagnosis of cerebrovascular disease, use of frequently prescribed herbal medicines, available records of laboratory tests and functional evaluation that was conducted within two weeks before and after admission and discharge, and herbal medicine usage exceeding half the period of hospitalization. Results: Of the total patients with cerebrovascular disease, the number of patients analyzed in this study was 49. There were some changes in the various variables examined after frequently used herbal medicines had been taken, such as complete blood count, liver function test, lipid test, and functional evaluation. No serious adverse events related to herbal medicine were found in this study. Conclusions: We concluded that herbal medicines frequently used for cerebrovascular disease may have some clinical effectiveness. However, because of the retrospective nature of this study and only one hospital involved, the generalizability of our findings is limited.
