Etoposide is a semi-synthetic derivative of podophyllotoxin that is effective against many cancers including small cell lung cancer. We report a case of etoposide-induced anaphylaxis in a 51-year-old woman who tolerated etoposide during her first cycle chemotherapy regimen. During the second cycle, she complained of generalized urticaria and dyspnea 5 minutes after being infused with etoposide. She recovered completely with antihistamine, corticosteroid and fluid replacement. The intradermal skin test with etoposide showed a clear positive immediate reaction. This case suggests that etoposide can induce IgE-mediated anaphylaxis.
Ulmi radicis cortex is a herb medicine which has been used for the treatment of such allergic disease as urticaria, allergic rhinitis and athma. To assess the contribution of an aqueous extract of Ulmi radicis cortex(URC) in systemic anaphylaxis, we used compound 48/80 as a fatal anaphylaxis inducer in rats. URC inhibited anaphylactic shock 100% with a dose of 1.0 mg/g body weight (BW) 1 hr before injection of compound 48/80. URC significantly inhibited serum histamine levels induced by compound 48/80. URC (1.0 mg/g BW) also inhibited to 79.1% passive cutaneous anaphylaxis activated by anti-dinitrophenyl (DNP) IgE. URC dose-dependently inhibited the histamine release from the rat peritoneal mast cells (RPMC) by compound 48/80. Moreover, URC had a significant inhibitory effect on anti-DNP IgE-induced histamine release or tumor necrosis $factor-{\alpha}$ production from RPMC. The level of cAMP in RPMC, when URC was added, significantly increased compared with that of normal control. These results indicate that URC may possess strong antianaphylactic effect.

Objectives : Atopic dermatitis is a recurrent or chronic eczematous skin disease with severe pruritus. Although the pathogenic mechanisms of atopic dermatitis are yet unknown, recently hyperresponsive Th2 cells in the acute phase are reported as the important mechanisms. Cheonggisan(CGS) is used in oriental clinics for curing acute skin lesions of eczema, atopic dermatitis or urticaria. There have been no studies on the therapeutic mechanism of CGS for curing atopic dermatitis. We aimed to find out the therapeutic mechanism of CGS on atopic dermatitis, so we observed Th2 cell differentiation in EL 4 cells and $NF-kB$ p65 activation in RAW 264.7 cells. Materials and Methods : EL 4 cells were induced the increase of IL-4 mRNA expression by phorbol-12-myristate-13-acetate(PMA) and 4-tert-Octylphenol(OP) and treated with CGS extract. RAW 264.7 cells were induced the increase of cyclooxygenase(COX)-2 mRNA expression by lipopolysaccharide(LPS) and treated with CGS extract. Results : The PMA and OP induced IL-4 mRNA expression was dose-dependantly decreased in CGS treated EL 4 cells. The LPS-induced COX-2 mRNA expression was dose-dependantly decreased in CGS treated RAW 264.7 cells. Conclusion : The results may suggest that the CGS inhibits Th2 cell differentiation in EL 4 cells and inhibits $NF-kB$ p65 activation in RAW 264.7 cells.
Fexofenadine, one of selective histamine $H_1$ receptor antagonists, has been used for the treatment of seasonal allergic rhinitis and chronic idiopathic urticaria. The bioequivalence of two fexofenadine hydrochloride preparations, containing 120 mg fexofenadine hydrochloride, was evaluated according to the guidelines of Korea Food & Drug Administration(KFDA). The test product was Hanmi Fexofenadine Hydrochloride Tablet $120mg^{(R)}$ made by Hanmi Pharm. Co. and the reference product was Allegra Tablet $120mg^{(R)}$ made by Handok Parmaceuticals Co.. Twenty healthy male subjects were randomly divided into two groups and a $2{\times}2$ cross-over study was employed. After one tablet was orally administered, blood was taken at predetermined time intervals and the concentration of fexofenadine in plasma was determined using a validated HPLC method with fluorescence detector. Two pharmacokinetic parameters, $AUC_t\;and\;C_{max}$, were calculated and analyzed statistically for the evaluation of bioequivalence of the two products. Analysis of variance was carried out using logarithmically transformed parameter values. The 90% confidence intervals of $AUC_t\;and\;C_{max}$ were log $0.844{\sim}log$ 1.149 and log $0.833{\sim}log$ 1.109, respectively. These values were within the acceptable bioequivalence intervals of log 0.8 to log 1.25. Thus, the criteria of the KFDA guidelines for the bioequivalence was satisfied, indicating that Hanmi Fexofenadine Hydrochloride Tablet 120 mg is bioequivalent to Allegra Tablet 120 mg.

Generalized anaphylaxis is a most dramatic and acutely life-threatening allergic reaction and may cause death within a few minutes. Differential diagnosis of anaphylaxis is made by clinical signs, such as, mental change, respiratory distress, hypotension, hypoglycemia, urticaria and angioedema. Especially, insulin reaction, myocardial infarction and vasovagal syncope are considered as differential diagnosis. In cases of fatal anaphylaxis, respiratory and cardiovascular disturbances predominate and are evident early in the reaction. This is a case report of the intensive care of anaphylactic shock after intravenous injection of the penicillin in a old medically compromised patient with the maxillary osteonecrosis. The anaphylactic shock symptoms, such as, unconsciousness, respiratory disorder, no pulsation on carotid artery and cardiopulmonary arrest are occurred in intravenous injection of augmentin 1.2 g after the skin test. In spite of immediate emergency cares, such as intravenous injection of epinephrine, endotracheal intubation, cardiopulmonary resuscitation, and continuous intensive care, the patient is expired in 58 hours after anaphylactic shock attack.

A 6-year-old boy with underlying hemolytic anemia of unknown etiology, atopic dermatitis, and recurrent urticaria visited our hospital because of acute respiratory failure induced by influenza A. Despite mechanical ventilation after endotracheal intubation along with inhalation of nitric oxide, respiratory acidosis and hypoxemia persisted. Veno-venous extracorporeal membrane oxygenation (VV ECMO) insertion was performed to provide respiratory support. After performing flexible bronchoscopy, we found that thick mucus plugs were obstructing the right bronchus intermedius and the upper lobe orifice. After bronchial washing and removal of the plugs, we were able to wean the patient off VV ECMO and transfer him to the general ward. He was discharged without any neurologic or pulmonary sequelae.
A servey on bovine fascioliasis in 52,876 cattle of Euiseong, Andong, Yecheon, Wolseong areas in Gyeongbug Province was carried out by the intradermal test, and the clinical side action of the positive cattle administered with DS-6 intramuscularly was observed. In addition pathological examination of 2 cattle died with side action of DS-6 was also conducted. The results of the studies are summarized as follows; 1. The average infestation rate of bovine fascioliasis was 31.1% and regional infestation rates were 35.1% in Eiseong, 34.5% in Yecheon, 32.2% in Andong, 26.9% in Wolseong, respectively. 2. The side actions were occurred in 1.5%(115/7,667) cf cattle administered with DS-6 and the clinical observations were salivation, dyspnea, hyperamia of conjunctiva, lacrimation, perspiration, urticaria, tremor, diarrhea, staggering, convulsion, edema and abortion. 3. Within 1 hour after the injection, side actions were occurred in 61.7% of the 115 cattle while they were observed in 37.4% of the 115 catte from 1 hour to 2 hours post injection. 4. The duration of the side action was less than 2 hours in 29.5%, 2 to 10 hours in 47.0% and more than 10 hours in 23.5% of the 115 cattle. 5. Autopsy findings and histopathological observations indicate that the death of cattle is caused by anaphylactic shock.
Continuous epidural pain block with a local anesthetic agents is a commonly employed technique for pain relief after thoracotomy. In this study, we evaluated the effectiveness of the continuous epidural pain block in 19 patients undergoing elective lateral or posterolatrral thoracotomy with control group(n=19) from November 1994 to July 1995, Epidural lidocaine and morphine mixtures were injected via an epidural catheter as a bolus after operation, and then bupivacaine and morphine mixtures were injected continuously following 5 or 6 days. The pain score, upper arm elevation(ROM score), and respiratory rate were significantly changed(P<0.05) from 30min after injection. The CO2 tension of arterial blood was decreased significantly(P<0.05) from 2hr after injection. The postoperative hospital days were decreased significantly(P<0.05). Side effects of the epidural pain block were urinary retention(n= 10), urticaria(n=2) and a case of headache. There was no postoperative lung atelectasis. We conclude that the continuous epidural pain block is good for prevention of the postoperative lung complication and early recovery after thoracotomy.
The dried unripe fruit of Poncirus trifoliata L. is widely used to treat urticaria, itch and indigestion in folk medicine. And recently it was reported the component of the fruit was found to exhibit an inhibitory effect on histamine release from mast cells. So to evaluate the effect of aqueous extract of Poncirus trifoliata L.(PTFE) on compound 48/80-induced histamine release, the study was carried out in rat peritoneal mast cell. PTFE $(10^{-3}{\sim}1mg/ml)$ inhibits the histamine release induced by compound 48/80 $(5{\mu}g/ml)$ in rat peritoneal mast cells. To clarify the mechanism of these inhibition, we investigated the effect of PTFE on cAMP and intracellular calcium content. The increase in cAMP content, when PTFE was added, was transient. At concentration of 1mg/ml, the cAMP content of mast cells was significantly increased at a rate of 53 times of basal cells at 10sec. PTFE inhibits histamine release by augmenting the cAMP content in mast cells. Moreover, PTFE inhibits intracellular calcium release induced by compound 48/80. This result suggests that PTFE may be useful for the prevention and treatment of allergy-related disease.
