  • Purpose: Studies on cytomegalovirus (CMV) infections in immunocompetent children are lacking, and minimal information is available in the medical literature on hepatic manifestations and complications of CMV. The aims of this study were to evaluate the clinical characteristics, laboratory data, and prognosis of children with CMV hepatitis, and to investigate its prevalence at a single medical center in Korea over a 10-year period. Methods: One hundred thirty-two children diagnosed with CMV infection based on specific markers (anti-CMV IgM, CMV polymerase chain reaction in blood and urine, or CMV culture of urine) were included in the study. Clinical and biochemical characteristics, immunological markers, and outcomes of hepatic CMV infection were determined. Results: The median age of patients (n=132) was 8.5 months (range, 14 days-11.3 years). Peak total bilirubin and alanine aminotransferase levels in serum ranged from 0.11-21.97 mg/dL, and 5-1,517 IU/L, respectively. Alanine aminotransferase remained elevated from 2-48 weeks. Jaundice was the most common clinical feature of hepatic CMV infection during infancy. The hematologic findings revealed anemia, leukocytosis, and monocytosis in CMV-infected patients. All participants recovered without administration of ganciclovir. Conclusion: In children with CMV hepatitis, fever was the most common symptom at presentation, and jaundice was the most common clinical feature of hepatic CMV infection in infants younger than 3 months of age. Hepatic CMV infection in immunocompetent children is often a self-limited illness that does not require antiviral therapy, as most patients in this study had favorable outcomes.

  • The total number of the dogs bred in Korea as of 2007 was 1,917,709, and among them, 77%, 1,476,776 dogs were edible dogs. Dogmeat has been legalized edible as food from Choseon dynasty, Daehan imperial state, Japan-occupied era till the present Korea. Dogs had been included in the article 2, Enforcement Ordinance of Processing and Disposal Rule of Livestock and Its Products until the end of Jan. 1979, but it was crossed out by the Notification No 3,005(Feb. 1 1979 effective) of the Minister of Agriculture and Marine Products, and as a result, the obligation that dogs should be slaughtered at the slaughtering ground was defunct. Thus, the arbitrarily dog slaughtering was empowered. As a matter of fact, the new law was not legalized in order to ban dogs from being slaughtered. The waste amount of slaughtered edible dogs amounts to 7,282 tons annually, and most of its waste from the arbitrarily-slaughtered dog is being illegally dumped without proper management and supervision. Edible dogs defecate 292,509 tons(calculates urine as dung) annually, but it is sanitarily disposed according to the Law of Management and Use of Livestock's Dung and Urine which took effective from Sep. 2009. Annual sales amount of edible dogs comes to 590 billion won on the basis of the shipment at breeding ground, but after passing through various level of marketing, and being processed as Gaesoju, and Boshintang, it forms 4 trillion won market when it reaches customers. The amount of food waste in Korea in 2007 came to 5,274,944 tons, and 633 billion won was spent for its disposal cost. Korean edible dogs of 1,476,776 heads consumed 1,266,705 tons, the 24% of total food waste. Edible dogs are the most effective means to convert food waste into food for man, not entailing the cost of disposal. On the other hand, pet dog culture brought about disposal cost, and the 51,188 dogs were abandoned at 2007, while 7 billion won was spent for the protection, euthanasia of them and the disposal of their dead bodies.

  • Acrodermatitis enteropethica (AE) is a rare autosomal recessive disorder of zinc absorption leading to chronic diarrhea and characteristic skin lesion. The term is also applied to any acquired zinc deficiency state resulting in the same clinical pictures. We experienced one case of AE in 1 month old male infant who had bacterial enterocolitis. The skin around mouth, anus, eyes, ears, hands and legs became reddish, vesicular and eczematoid. Serum zinc level was decreased to $51.4\;{\mu}g/dL$ (N=70~150). Endoscopic finding revealed pale gastric mucosa and villous atrophy of small intestine. Biopsy finding of small intestine showed no villi due to mucosal atrophy. On 13 day of admission jaundice with DIC were noted and AST & ALT were elevated to 110 & 36.8 IU/L, respectively. Diarrhea was improved but jaundice and liver function were not recovered until discharge from hospital. After discharge when the patient was 4 months of age serum bilirubin and AST/ALT had not been normalized. CMV shell vial culture of urine and CMV Ig G antibody were positive. So intravenous ganciclovir injection of 7.5 mg/kg, two times a day for 2 weeks and then 10 mg/kg/day for 3 months was done from 4 to 6 months of age. No virus was found in the urine and AST & ALT were normalized at 2 months after stopping ganciclovir treatment.

  • The genus Lactococcus is a gram-positive, catalase-negative, non-motile bacterium. It is a facultative anaerobe and can be cultured at 10~40℃. The genus Lactococcus consists of 16 species, of which Lactococcus garvieae and Lactococcus lactis are known to cause disease in humans. This study reports the first case in which L. garvieae was identified in the urine culture of a 74-year-old woman. The patient confirmed the findings of acute urinary tract infection through blood tests, microbial identification tests, antibiotic susceptibility tests, and computed tomography performed at a hospital. The patient was admitted to the nephrology ward and was treated with IV fluids and erythromycin antibiotics and discharged 5 days later. This is the first case in Korea in which L. garvieae was isolated from the urine of a patient with a urinary tract infection and is expected to be useful in treating patients with L. garvieae infection in the future.

  • Growth rates, photosystem II photosynthesis, and the levels of chlorophyll $\alpha$ and secondary metabolites of Chlorella ovalis were estimated to determine if they were enhanced by the addition of swine urine (BM) or cow compost water (EP) that had been fermented by soil bacteria to deep seawater (DSW) in an attempt to develop media that enabled batch mass culture at lower costs. Growth of C. ovalis in f/2, f/2-EDTA+BM60%, DSW+BM30%, and DSW+EP60% was enhanced and maintained in the log phase of growth for 16 days. The cell densities of C. ovalis in DSW+EP60% ($4.1{\times}10^6$ Cells/ml) were higher than those of f/2 ($2.9{\times}10^6$ Cells/ml), f/2-E+BM60% ($3.7{\times}10^6$ Cells/ml), and DSW+BM30% ($2.7{\times}10^6$ Cells/ml). The growth rate was also more favorable for C. ovalis cultured in DSW+EP60% ($0.15\;day^{-1}$) than that of C. ovalis cultured in the control medium (f/2) ($0.12\;day^{-1}$). Furthermore, the chlorophyll a concentration of C. ovalis cultured in DSW+EP60% (4.56 mg/l) was more than 2-fold greater than that of C. ovalis cultured in f/2 (2.35 mg/l). Moreover, the maximal quantum yields of photo system II at 470 nm (Fv/Fm) were significantly higher in organisms cultured at f/2-E+BM60% (0.53) and DSW+EP60% (0.52) than in the other treatment groups. Finally, Fourier transformation infrared (FT-IR) spectroscopy revealed that C. ovalis grown in DSW+EP60% had more typical peaks and various biochemical pool shifts than those grown in other types of media. Taken together, the results of this study indicate that the use of DSW+EP60% to culture C. ovalis can reduce maintenance expenses and promote higher yields.

  • Isolation and identification of causative microorganisms and susceptibility testing are important in selecting appropriate antimicrobial agent. The purpose of this study was to determine the antimicrobial susceptibility patterns and identification of bacteria for the selection of a therapeutic antibiotic agent for treatment. Specimens were cultured aerobically from dog patients brought to the veterinary medical teaching hospital of Seoul National University between July 1999 and September 2000. A total of 157 isolates were from skin(63), urine(45), ear canal(31) and conjunctiva(18). The result is that the most common organisms isolated from dog patients were S. intermedius was the most common isolates from the skin, ear canal, and conjunctiva. E. coli was the most common isolated from urine. Most of gram-positive isolates were resistant to ampicillin(80.6%), erythromycin(68.8%), penicillin(86.2%), tetracycline(89.2%). Otherwise most of gam-negative isolates were resistant to ampicillin(73.4%), trimethoprim-sulfa(53.3%). E. coli was resistant to ciprofloxacin(61.5%), piperacillin (69.2%). Antimicrobial susceptibility pattern by the sampling site was not remarkably different except 5. aureus isolated from urine.

  • Purpose : Urinary tract infections (UTIs) of children require prompt and correct diagnosis and treatment to reduce the risk of renal damage. As a first step to improve the outcome of UTI in Korea, we investigated the practical variations in the methods of diagnosis, treatment, and evaluation of children with UTI and UTI prevention. Method :A questionnaire related to the individual policy on UTI diagnosis. treatment, imaging test, and prevention was submitted to 26 experts. Result Majority of the experts used bag-collected urine specimen for infants and mid-stream urine specimen for children for urinary culture. With a negative result of culture study, they diagnosed UTI when there was pyuria, positive results of the nitrite test, or bacteriuria. 80 $\%$ of experts prescribed prophylactic antibiotics after upper tract UTI. Operative treatment of vesicoureteral reflux (VUR) was indicated for children older than one or two years old with high-grade VUR, refractory breakthrough infections, or recurrent UTIs. Most of them performed kidney ultrasonography on the diagnosis of UTI and more than half of them evaluated children treated of UTI with vesicocystourethrogram and/or DMSA scintigraphy. Majority did not recommend circumcision. Half of the experts were screening siblings of VUR patients. Conclusion : Considering the variations exposed through this study systematic guidelines for management of children with UTI in Korean would be necessary. (J. • Cytomegalovirus (CMV) is one of the most commonly encountered viral pathogens in newborn infants and is found in 0.3-2.4% of all live births. It has been demonstrated that 40-96% of seropositive mothers shed the virus via their breast milk. Breast milk containing CMV can cause almost one-third of CMV infections occurring in infants. A case of postnatal CMV infection in an extremely premature infant (gestational age $24^{+5}$ weeks, birth weight 750 g) transmitted via breast milk is presented. For neonatal intensive care unit (NICU) management of severe thrombocytopenia, anemia, and sepsis syndrome, the infant received repeated transfusions of platelets; intravenous (IV) immunoglobulins; and gamma- irradiated, filtrated packed red cells and was fed her mother's breast milk since the second week of life. CMV infection was diagnosed with positive CMV immunoglobulin M (IgM) and positive urine CMV culture at the second month of life. Considering the negative CMV IgM and urine CMV culture at birth, postnatally-acquired CMV infection was suspected and confirmed with completely identical nucleotide sequence alignments of the infantile blood isolate and the maternal breast milk isolate. To our knowledge, this is the first case of proven postnatal CMV infection transmitted via breast milk in an extremely premature infant in Korea.

  • To evaluate the features of pyuria related to the bacteriuria, 140 subjects were studied from Jan. 1987 to Dec. 1987. The pyuria was frequently developed from the age 30 to 60 years old, and male to female ratio was 1:1.41. The most common disease was urethrocystitis that was shown 42.8%. Common precipitating factors were urethral catheterization(25%) and urinary tract obstruction(11.4 %). Through the observation, symptomatic pyuric patients were 66 subjects(47.1%), and the subjects with significant urine culture were 121 subjects(86.4%). In the urine culture, the most common bacteria was E. coli(41.4%), and the next was Pseudomonas(19.3%). A large percentage of E. coli and Pseudomonas was susceptible of amikin. The pyuria due to S. epidermidis and Accinatobacter was well treated. High therapeutic rate was observed in the acute pyelonephritis(71%) and urethrocystitis(67%). In the persistent urinary tract infection, there were relapsing(22 cases) and recurrent urinary tract infection(16 cases).
