Determination of Lead in Urine by Atomic Absorption Spectrophotometry (원자흡광법을 이용한 요중 연 배설량의 정량)

  • Determination of lead in urine is important in industrial hygiene and toxicology. Dithizone method has been principally used for the determination of lead in urine, which gives accurate results in skilful hands but is usually complex and time-consuming. Atomic absorption spectrophotometry is a new simple method and several procedures have been described. However, the influences of pH and the presence of chelating agents during treatment of lead poisoning are not clear. The purpose of this study was to find out the effect of pH and chelating agents on the determination of lead using Shimadzu atomic absorption/flame spectrophotometer, model AA-610. The results obtained were as follows: 1. The atomic absorption spectrophotometry(AAS) could be applied without prior acid digestion to specimens in the absence of chelating agents. The absorbance at $2,170\;{\AA}$, though more sensitive, was more noisy electronically. Therefore, we selected the wavelength of $2,833\;{\AA}$ plus scale expansion. 2. The optimal pH was in the range from 2 to 3. 3. The sensitivity was $0.075{\mu}g/ml/%$ and detection limit was about $0.2{\mu}g/ml$. 4. In the presence of EDTA, lead could not be completely determined without prior acid digestion. 5. On specimens from patients receiving penicillamine therapy, a comparison was made between the values obtained with dithizone method and AAS method with prior acid digestion. The results of comparison showed a very good agreement.

  • Urinothorax occurs when urine leakes into thoracic cavity and it was first reported by Correie in 1968. The mechanism of its occurrence is an accumulation of urine in thoracic cavity by retroperitoneal urinoma formed by urinary tract obstruction or trauma. Retroperitoneal inflammation, malignant neoplasm, kidney transplantation, and kidney biopsy can also induce urinothorax. The diagnosis of urinothorax may be delayed due to its rare incidence rate, but if diagnosis is confirmed the treatment is possible by urinary diversion. We report our experience of a case of urinothorax associated with retroperitoneal urinoma followed by blunt renal trauma with review of literatures.

  • Three crossbred gilts weighing $61{\pm}2kg$ ($mean{\pm}SD$) and three gilts weighing $52{\pm}3kg $ on the day before the first treatment began (d -1) were used for each of two experiments (Exp. 1 and Exp. 2), respectively. In Exp. 1, all pigs were fed the experimental diet (CP 19%) from d -7 to the end of study (d 21) to verify that nitrogen retention is constant during the 21 -d period. In Exp. 2, pigs were fed the control diet (CP 15.5 %) from d -7 to d 8 and then the low-lysine diet from d 9 to d 16 in order to determine how rapidly dietary changes in amino acid composition results in a new equilibrium for nitrogen metabolism. The amount of urine nitrogen loss was not different over 21 days (p > 0.10). Rates of nitrogen retention were not different among pigs (p > 0.10) nor over time (p > 0.10). Average nitrogen retention during the period was 1.00 g/kg $BW^{0.75}$ per day. The apparent biological value was 41%, which did not change over the 3-week period (p > 0.10). The overall efficiency of nitrogen use for nitrogen retention was 35% (Exp. 1). The amount of nitrogen loss in urine and the efficiency of nitrogen utilization for nitrogen gain reached a new equilibrium within 2 to 3 d after the diet was changed. The low-lysine diet resulted in a 20% increase of nitrogen loss in urine (p < 0.001) and a 9% decline in efficiency of nitrogen use for nitrogen retention (p < 0.001). Nitrogen retention while the pigs were fed the control diet was also higher than the retention when pigs were fed the low lysine diet (p < 0.001). The efficiency of nitrogen use for nitrogen retention in pigs fed the control diet was 57% (Exp. 2), which was higher (p < 0.001) than that from pigs fed the low-lysine diets (52%).

Quantitative Analysis of Lovastatin in Human Plasma and urine by Reversed-Phase High-Performance Liquid Chromatography (역상 고속액체크로마토그라프법을 이용한 혈장 및 뇨 중 로바스타틴의 정량)

  • Lovastatin (LOVA), a fungal metabolite isolated from cultures of Aspergillus terreus, is a competitive HMG-CoA reductase inhibitor used for the treatment of primary hyper cholesterolemia, and has also been shown to suppress growth in a variety of non-glioma tumor cell lines. A sensitive reversed-phase high-perfonnance liquid chromatographic method with ultraviolet (UV) absorbance detection has been developed to quantitate LOVA in human plasma and urine samples using liquid-liquid extraction procedure. Baseline separation of LOVA and internal standard, simvastatin was achieved on a Novapak $C_{18}$ analytical column with a mobile phase containing 0.025M $NaH_2PO_4$: CAN (35:65, v/v%), adjusted pH to 4.5. The flow rate was set at 1.5ml/min, and the column effluent was monitored by a UV detection at 238nm. The limit of quantification was determined to be 0.5${\mu}$g/ml while extraction efficiency of LOVA ranged from 73.4-82.9% at LOVA concentrations of 0.5 to 10${\mu}$g/ml. Good linearity with correlation coefficients greater than 0.999 was obtained in the range of LOVA concentrations from 0.5 to 10${\mu}$g/ml. The accuracy and the precision were proven excellent with relative standard deviation (RSD, %) and relative error (RE, %) of less than 4.2 and 4.0, respectively. Intraday precision, evaluated at five LOVA concentrations (0.5, 1, 2, 5, 10${\mu}$g/ml) and expressed as RSD ranged from 0-1.82% while the interday precision at the same concentrations ranged from 0.7-10.5%. The analytical method described was then successfully employed for the determination of LOVA concentrations in plasma samples obtained during a phase II clinical trial using high doses of LOVA (30-40mg/kg/day). This method could be further utilized for the ongoing pharmacolkinetic studies and therapeutic drug monitoring of the high-dose LOVA therapy in adenocarcinoma patients.

Change of Flavonoid Composition in Scutellaria baicalensis by Processing (수치에 의한 황금의 성분변화)

  • Scutellaria baicalensis Georgi (Labiatae) has been extensively used in treatment of diarrhea, conjunctivitis, gastritis, enteritis and skin disease. As instructed in old description, the Scutellariae Radix should sometimes be processed before use. To 노디 light on the changes of chemical compositions by processing as well as processing method-activity relationships, Scutellariae Radix was heated at 16$0^{\circ}C$-20$0^{\circ}C$ for 30 min. in furnace or soaked with 20% EtOH (SPE) and boy's urine(SPU), respectively, which are one of processing methods and then heated at 17$0^{\circ}C$ for 30 min. To isolate the chemical components, Scutellariae Radix with/without processing were extracted with EtOH and EtOH extract was fractionated with ether, ethyl acetate and butanol to give respective fractions. Ether and EtOAc fractions obtained from the processed drug with urine (SPU) were subjected to chromatography to obtainsix components, oroxylin A, Wogonin, chrysin, baicalein, baicalein 7-O-$\beta$-D-glucopyranoside and $\beta$-sitosterol 3-O-$\beta$-D-glucopyranoside. All the isolated compounds were identified by means of physicochemical and spectroscopic methods (IR, $^1H-NMR,{\;}^13C-NMR$, Mass). By HPLC determination, the changes of the contents for each isolated components from SPE and SPU samples were observed. It was found that the content of nonglycosidic flavones such as oroxylin A, wogonin, chrysin and baicalein was increased markedly, whereas the content of baicalin and baicalein 7-O-$\beta$-D-glucopyranoside was significantly decreased in both samples as compared with those of Scutellariae Radix. When the sample was soaked with boy's urine, the total amount of nonglycosidic flavones was higher than that of processing with 20% EtOH.

The Effect of Phaseoli Semen Herbal-acupuncture at $KI_{10}$ in Lipopolysaccharide Induced Acute Nephritis in Rats (음곡에 시행한 적소두약침이 LPS로 유도된 흰쥐의 급성 신장염에 미치는 영향)

  • Objectives : This study was designed to evaluate the effects of Phaseoli Semen Herbal-acupuncture(PS-HA) at $KI_{10}$ in acute nephritis induced by lipopolysaccharide(LPS) in rat. Methods : The rats were divided into 5 groups, which were control, LPS, PS-HA, NP and saline group. LPS, PS-HA, NP and saline groups were given LPS to induce acute nephritis and control group did not receive LPS. LPS group did not receive any treatment after the onset of acute nephritis. PS-HA, NP and saline group received PS-HA, normal acupuncture, and saline injection at $KI_{10}$ three times per week, respectively. To evaluate the effect of PS-HA at $KI_{10}$, the complete blood count, BUN, creatinine, TNF-${\alpha}$, and CINC-1 in serum were measured. To show its effect on renal function, creatinine, and total protein in urine was measured as well as urine output. The level of myeloperoxidase in renal tissue was quantified and complete histology was done in kidney samples obtained from the rats. Results : PS-HA group showed a significant reduction in the proportion of WBC and neutrophil, serum BUN, TNF-${\alpha}$, and CINC-1 compared to LPS group. Furthermore, a significant increase in urine output and a decrease in urinary creatinine level, MPO in renal tissue, and number of neutrophils at glomerulus was observed in PS-HA group compared to LPS group Conclusions : PS-HA at $KI_{10}$ was shown to have a significantly effect on treating LPS induced acute nephrits. Therefore, future study is needed to further evaluate the clinical usefulness of PS-HA at $KI_{10}$ in treating acute nephritis.

Trace Analysis of Racemic Isomers of Flurbiprofen in Human Urine using Column Switching-HPLC (컬럼스위칭 액체크로마토그래피를 이용한 뇨중 플루비프로펜의 광학이성질체의 미량분석)

  • The separation and determination method of racemic isomers of flurbiprofen in urine samples have been investigated using column switching- HPLC, which include the sample treatment and concentration column. The optical rotation of two isomers separated were measured to identify d-form and l-form. The calibration curves are liear in the ranges of $0.11-5.4{\mu}g/mL$ for both d-form and l-form. The detection limits were $0.031{\mu}g/mL$ for l-isomes and $0.027{\mu}g/mL$ for d-isomer. The coefficients of variation of intra-day and inter-day precision of this method were about 1.8%. The present method was apllied to determine the concentration of racemic isomers in urine sample from human eating the drug.

Prevalence of Schistosomes and Soil-Transmitted Helminths and Morbidity Associated with Schistosomiasis among Adult Population in Lake Victoria Basin, Tanzania

  • The objective of this study was to carry out a community survey on schistosomiais and soil-transmitted helminth (STH) infections in order to suggest feasible and effective intervention strategies in Lake Victoria basin, Tanzania. A total of 37 communities selected from 23 districts of the 4 regions in the Lake Victoria basin of Tanzania were involved in the study. From each of the selected locality, 50 adult community members, 25 males and 25 females, were recruited for the study. Each study participant was requested to submit stool and urine specimens. From each stool specimen, duplicate Kato-Katz thick smears were prepared and microscopically examined for Schistosoma mansoni and STH eggs. Urine specimens were processed by the filtration technique and microscopically examined for Schistosoma haematobium eggs. Ultrasound examination for morbidity due to schistosomiasis was performed. Mass treatment was done using praziquantel and albendazole for schistosome and STHs infections, respectively. Out of 1,606 adults who provided stool specimens, 199 (12.4%) were positive for S. mansoni, 349 (21.7%) for hookworms, 133 (8.3%) for Ascaris lumbricoides, and 33 (2.0%) for Trichuris trichiura. Out of 1,400 participants who provided urine specimens, 25 (1.8%) were positive for S. haematobium eggs. Because of the co-endemicity of these afflictions and their impact on vulnerable population groups, the helminthiasis could be simultaneously treated with 2 drugs, praziquantel for schistosomiasis and albendazole for STHs.

Effect of Combined Exposure to EDTA and Zinc Pyrithione on Pyrithione Absorption in Rats

  • Zinc pyrithione (ZnPT) is a coordination complex of zinc and has been used widely as an anti-dandruff agent in shampoos. Many shampoos contain both ZnPT and EDTA, a chelating agent speculated to increase ZnPT absorption, thereby raising concerns about neurotoxicity. Here, we investigated the effect of EDTA on ZnPT absorption by direct comparison of ZnPT and pyrithione (PT) concentrations in shampoo formulations, and by pharmacokinetic analysis of ZnPT, PT, and 2-methanesulfonylpyridine (MSP), the main ZnPT metabolite, in rat plasma or urine following exposure to shampoo containing ZnPT alone or a combination of ZnPT and EDTA. Approximately 17.3% of ZnPT was converted to PT by the addition of EDTA in the shampoo formulation. Plasma ZnPT and PT concentrations were not measured up to 24 hr after treatment with shampoo containing 1% ZnPT or 1% ZnPT + 2% EDTA in all rats. However, PT amount in 24-hr urine sample, MSP concentration in plasma, and MSP amount in 24-hr urine sample were approximately 4-, 2.6-, and 2.7-fold higher, respectively, in the 1% ZnPT + 2% EDTA shampoo group than in the 1% ZnPT shampoo group. As confirmed by the formulation analysis and in vivo pharmacokinetic analysis, the exposure of ZnPT could be increased by the absorption of PT due to partial dissociation of ZnPT into PT.

Maple Syrup Urine Disease (MSUD) Diagnosis & Treatment Guidelines: Past and Present in Korea (한국에서의 단풍당뇨병 진단 치료 지침: 과거와 현재)

  • Maple syrup urine disease (MSUD) is an autosomal recessive metabolic disorder caused by a deficiency in branched chain α-keto acid dehydrogenase (BCKAD). Between 1997, when Korea's MSUD case was first reported, and 2023, 14 cases were reported in the literature. 29% of the cases experienced developmental delay, and 29% expired. The prevalence of MSUD in Korea was estimated to be 1 in 230,000. Of 21 MSUD patients currently being treated at the Korea Genetics Research Center, 19 were detected through newborn screening program, and 2 were diagnosed by the symptoms. 14 MSUD patients had confirmed genetic mutations; 6 (43%) were BCKDHA and 8 (57%) were BCKDHB. In one case, a large deletion was observed. 4 patients had leucine levels above 2,000 (umo/L), and post-dialysis diet therapy was initiated in the newborn period. No patient required further dialysis as diet therapy and regular monitoring proved highly effective. Most MSUD patients were growing normally; weight and height growth were above the 50th percentile in 76% of the cases while BMI values were higher than normal in 71% of cases. Developmental delays were observed only in 2 cases (10%) and anticonvulsant use in 3 cases (14%). With newborn screening available to all Korean infants, early diagnosis and intervention should allow most patients to remain asymptomatic. However, ongoing surveillance, dietary management and continued patient compliance as well as rapid correction of acute metabolic decompensations remain critical to a favorable long-term prognosis.
