Effect of Cyclosporine A on Plasma and Urine Levels of Endothelin-1 in Steroid Dependent Minimal Change Nephrotic Syndrome (미세변화형 신증에서 cyclosporine A 투여 후 혈액 및 요중 endothelin-1치의 변화)

  • Purpose : It has been reported that endothelin-1 can be associated with the development of cyclosporine A nephrotoxicity. Levels of endothelin-1 was assayed to determine the effect of cyclosporine A on the plasma and urine levels of this peptide. Method : Nine patients who were less than 15 years of age and were also diagnosed as steroid dependent minimal change nephrotic syndrome were included in this study. Assay of endothelin-1 was done before and 3 months after cyclosporine A treatment. The mean age of the patients was $8.3{\pm}4.6$ years. Result : There was no significant differences in the plasma levels of endothelin-1 before and after 3 months of cyclosporine A treatment ($3.22{\pm}0.39$ pg/mL vs. $3.84{\pm}1.52$ pg/mL, P=0.64). Also there was no changes in the urine levels of endothelin-1 ($21.8{\pm}5.8$ pg/mL vs. $20.3{\pm}3.1$ pg/mL, P=0.30). Conclusion : No significant changes of endothelin-1 levels were found with 3 months of cyclosporine A treatment. Further studies with longer durations will help clarifying the effect.

Renal Effects of Chronic Treatment Of Adenosine Analogues (Adenosine 수용체 작동제 장기 투여의 신장효과)

  • Evidence for the existance of at least two subclasses of renal adenosine receptors has been presented. N-6-cyclohexyladenosine (CHA) is a relatively selective $A_1$ adenosine agonists, whereas 5'-N-ethylcarboxamidoadenosine (NECA) acts as a preferential agonist of $A_2$ adenoisne receptor. N6-(L-2-phenylisoproryl)-adenosine (PIA) almost unselectively activates both $A_1\;and\;A_2$ adenosine receptors at micromolar concentrations. During the characterization of adenosine receptor in the kidney, we have discovered a novel phenomenon, that is, an intramuscular administration of CHA for 3 days caused a diuresis and a suppression of urinary concentrating ability. To further characterize this novel phenomenon, an intramuscular administration of adenosine and other adenosine angonists, PIA and NECA, and prior treatment of adenosine antagonists, caffeine, theophylline and 1,3-diethyl-8-phenyl-xanthine (DPX) were performed. Systemic administration of CHA, PIA, and NECA for 3 days caused a suppression in heart rate, blood pressure and general motor activity without change in rectal temperature. Systemic administration of CHA, 0.5, 1 and 2 mg/kg/day, for 3 days caused a dose-dependent increase in urine volume and decrease in urinary osmolarity and free water reabsorption. This phenomenon was reversible and repeatable. Administration of adenosine (40 mg/kg/day) produced no apparent effect on the renal function, whereas PIA (2 mg/kg/day) produced an similar effect to CHA on the renal function. Systemic adminstration of NECA, 0.025, 0.05 and 0.25 mg/kg/day, for 3 days caused a dose-dependent increase in urine volume and dose-dependent increases in excreted amount of creatinine, urinary osmolarity and free water reabsorption. These renal effects of adenosine agonist were maximum at second day during the drug administration. In terms of increase in urine volume and the suppression of urinary concentrating ability, NECA was potent than CHA. Prior treatment of caffeine (50 mg/kg/day) or theophylline (50 mg/kg/day) abolished the diuretic effect of CHA, whereas DPX (50 mg/kg/day) did not affect the CHA effect. CHA, 0.5 mg/kg/day, produced no change in plasma renin activity and plasma levels of aldosterone, epinephrine, and norepinephrine. These results suggest that this novel phenomenon produced by an activation of renal adenosine receptors plays an important role in urinary concentrating mechanism.

  • Objectives : This study aims to report the effect of Korean medicine treatment on Sequela of Bladder Cancer. Methods : The patient was treated with korean medicine of Paljeongsan-gamibang, acupuncture at Qihai (CV6), Guanyuan (CV4), Qugu (CV2), Hyeolhae (SP10), Sameumgyo (SP6) and we evaluated treatment effects by visual analogue scale (VAS) and urine analysis (UA) finding. Results : After treatments, the symptoms such as perineal pain, painful urination, dysuria, anorexia were improved and taking number of narcotic analgesic was reduced. Conclusions : This clinical study suggests that korean medicine treatment shows possibility to care for sequela of bladder cancer patient.

  • Objectives The purpose of this case study is to report the effect of Yukmijihwangtang-gamibang on recurrent hematuria in childhood. Methods The subject was a male child with macroscopic hematuria. This patient was only treated with oriental herbal medicine. The improvement was observed by subjective symptoms and routine urinalysis. Results After the treatment with Yukmijihwangtang-gamibang, the symptoms of macroscopic hematuria were relieved. The occult blood test was negative as well. The number of red blood cells in urine was within the normal range. He was less tired but hematuria did not recur even after intense exercise. Conclusions This study showed that Korean medical treatment with using herbal medicine such as Yukmijihwangtanggamibang can be an effective treatment option for asymptomatic macroscopic hematuria.

NMR-based metabolomic profiling of the liver, serum, and urine of piglets treated with deoxynivalenol

  • Deoxynivalenol (DON), a Fusarium mycotoxin, causes health hazards for both humans and livestock. Therefore, the aim of this study was to investigate the metabolic profiles of the liver, serum, and urine of piglets fed DON using proton nuclear magnetic resonance ($^1H-NMR$) spectroscopy. The $^1H-NMR$ spectra of the liver, serum, and urine samples of the piglets provided with feed containing 8 mg DON/kg for 4 weeks were aligned and identified using the icoshift algorithm of MATLAB $R^2013b$. The data were analyzed by multivariate analysis and by MetaboAnalyst 4.0. The DON-treated groups exhibited discriminating metabolites in the three different sample types. Metabolic profiling by $^1H-NMR$ spectroscopy revealed potential metabolites including lactate, glucose, taurine, alanine, glycine, glutamate, creatine, and glutamine upon mycotoxin exposure (variable importance in the projection, VIP > 1). Forty-six metabolites selected from the principal component analysis (PCA) helped to predict sixty-five pathways in the DON-treated piglets using metabolite sets containing at least two compounds. The DON treatment catalyzed the citrate synthase reactions which led to an increase in the acetate and a decrease in the glucose concentrations. Therefore, our findings suggest that glyceraldehyde-3-phosphate dehydrogenase, citrate synthase, ATP synthase, and pyruvate carboxylase should be considered important in piglets fed DON contaminated feed. Metabolomics analysis could be a powerful method for the discovery of novel indicators underlying mycotoxin treatments.

Effects of the Administration of 5-aryl-2,3-dihydroimidazol [2,1-a] isoquinolines (SDZ-62434) on Kidney

  • The effects of the anti-tumor agent, SDZ-y2434, on rat kidney were investigated to predict the toxicities of its derivatives and to develope less toxic derivatives. After adjusted in metabolic cages for 5 days, rats were treated SDZ-62434(acute : 25mg/kg, i.p, once and 50mg/kg, i.p., once; subacute ; 10mg/kg, i.p., daily for 7 days). Kidney weights and urine volume during the treatment were observed. Creatinine concentration, protein concentration and the activities of N-acetyl-${\beta}$-D-glucosaminidase (NAG), alanine aminopeptidase (AAP), ${\gamma}$-glutamyl transpeptidase (GGT) and lactate dehydrogenase(LDH) in 24 hr urine were also determined. The kidney weights after the acute and subacute administration didn't show any difference. Urine volume increased 5 days after the acute administration (50mg/kg) and 3 days after the subacute administration. The excretion of creatinine was increased 5 days after the acute (50mg/kg) and subacute administration. However, the protein excretion didn't show any change. NAG acivity declined 7 days after the subacute administration. AAP and GGT activites increased 3 days after the acute administration (50mg/kg) but, returned to the control value. LDH activity showed continuousely high value after the subacute administration. These results indicates that the acute administration of SDZ-62434 might damage on glomerulus and that the subacute administration might be cytotoxic to kidney cells.

Clinical characteristics of Kawasaki disease with sterile pyuria

  • Purpose: Kawasaki disease (KD) is a systemic vasculitis and affects many organ systems. It often presents sterile pyuria, microscopic hematuria, and proteinuria due to renal involvement. The aims of this study were to define clinical characteristics of acute KD patients with pyuria and to analyze meaning of pyuria in KD. Methods: The medical records and laboratory findings including serum and urine test of 133 patients with KD admitted to Yeungnam University Hospital from March 2006 to December 2010 were reviewed retrospectively. Results: Forty patients had sterile pyuria and their clinical characteristics including age, gender and body weight were not significantly different with those who did not have pyuria. Fever duration after treatment was significantly longer in KD patients with pyuria. Erythrocyte sedimentation rate, C-reactive protein and serum concentration of alanine aminotransferase were significantly higher in patients with pyuria. Hyponatremia and coronary artery lesion were seen more often in patients with pyuria but there was no significant difference. Also serum blood urea nitrogen was significantly higher in KD patients with pyuria. Urine ${\beta}_2$-microglobulin was elevated in both patients groups and showed no difference between two groups. Conclusion: We found more severe inflammatory reaction in KD patients with pyuria. We also found elevation of some useful parameters like ${\beta}_2$-microglobulin that indicate renal involvement of KD through the urine test. Careful management and follow up will need for KD patients with pyuria and it is necessary in the future to study the specific parameters for renal involvement of KD.

CMV Infection in AIDS Patients Depends on the Decrease of CD4+ T Cell Count (AIDS 환자에서 CD4+ T 세포수 감소에 따른 CMV 감염)

  • Cytomegalovirus is the most common cause of life-threatening viral infection in HIV-infected patients. This study was done prospectively to investigate the incidence of CMV infection according to the decrease of CD4+ T cell count (CD4+) in Korean AIDS patients. Thirty-nine HIV-infected patients diagnosed before 1994 were followed for regular immunological monitoring. We have used urine shell vial method for the CMV detection from 1994 and have also checked clinical findings. Positive urine culture rate definitely depended on the CD4+ as follows; 45%, 22%, 17%, 11% and 0%, CD4+ <50, 50-100, 100-200, 200-500 and >500, respectively. Except culture positive 2 patients with CD4+ of $200{\sim}300/{\mu}l$, all eight culture positive patients with CD4+ less than $200/{\mu}l$ showed CMV related diseases on or before urine culture. But, we could not get a positive culture for a late AIDS patient with vision loss. With ganciclovir therapy, all culture results were at least negative just after or on late of first 14 days-ganciclovir infusion-course. These data suggest that the incidence of CMV disease in Korean AIDS patients is very high, and early diagnosis and treatment for CMV diseases is required for the prevention of life threatening results.

A Study on the Diuretic Action of Buxuletin (Buxuletin의 가토에 대한 이뇨작용)

  • Buxus leaves(Buxus microphylla var. koreana Nakai) have been used as a folk medicine in treating woman's disease. The chemical constituents, which have teen identified from Buxus sempervirens L. in Europe, are so far known in isolation of buxus alkaloids, cyclobuxine, buxamine, norbuxamine and buxaminol. But But any study on the pharmacological action of Buxus microphylla var. koreana Nakai has not yet teen rallied out, however, active substances have not been identified. As an attempt to isolate the biologically active substances from this plant, the examination of chemical constituents was undertaken. In the experiment, the crystalline component$(C_{10}H_8O_4)$ obtained from Buxus microphylla var. koreana Nakai was identified as a 6-methoxy, 7-hydroxy coumarin by chemical and physical methods. We named this crystal buxuletin. Therefore, we intended to screen the pharmacological action of buxuletin, especially, its diuretic effects in rabbits. Buxuletin was intraveneously injected to the rabbit in the dose of 20 mg/kg, 10 mg/kg and 5 mg/kg. The variations of urine volume and minerals $(Na^+,\;K^+\;and\;Cl^-)$ in urine were measured at an interval of 15 minutes for 4 hours after the treatment of buxuletin. The observed results are as follows: 1) With the administration of buxuletin (20 mg/kg, 10 mg/kg), the remarkable increment of the excreted urine volume was observed during the relatively long period. Excretion rates of urinary minerals $(Na^+,\;K^+\;and\;Cl^-)$ were also increased. 2) In a dose of 5 mg/kg, the slight increment on the diuretic actions of rabbits was observed. From the above results, authors name this crystal buxuletin and buxuletin shows the diuretic action.
