토트백의 무게가 젊은 성인여성의 상부 체간근육의 근두께와 근긴장도에 미치는 영향에 관한 융합연구 (A Convergence Study on the Effect of Tote Bag Weight on Muscle Thickness and Muscle Tone of Upper Trunk Muscles in Young Adult Women)

  • 본 연구는 토트백이 젊은 여성의 작은가슴근 두께와 위등세모근의 긴장도에 어떤 영향을 미치는지를 확인하였다. 습관적으로 토트백을 들고 다니는 27명의 여성이 본 연구에 동원되었다. 대상자들은 대상자들의 체중의 0 %, 5 %, 7.5%, 10 % 무게에 해당하는 토트백을 한쪽 손목으로 들고 있는 동안 작은가슴근의 두께와 위등세모근의 긴장도를 측정하였다. 가방의 무게가 증가할수록 위등세모근의 긴장도는 유의하게 증가되었으며(p<.05), 작은가슴근의 두께 역시 가방 무게에 따라 유의하게 증가되는 것을 확인하였다(p<.05). 이러한 결과를 바탕으로, 손목으로 토트백을 드는 습관이 젊은 여성의 위등세모근과 작은가슴근에 해로운 영향을 끼칠 수 있다는 것을 확인하였다. 그러므로, 본 연구는 토트백 즐겨 사용하는 여성들에게 가능하다면 토트백 무게를 감소시킬 것을 제안한다.

The Impact of Cervical Stretching Exercise and Cervical Traction on Cervical Pain and Muscle Activity among Patients with Cervical Hypolordosis

  • The purpose of this study is to provide fundamental clinical data for the treatment plan and rehabilitation of patients with cervical hypolordosis by comparing the cervical headache and muscle activity after cervical stretching exercise and cervical traction, which are generally applied to patients with cervical hypolordosis. The research subjects included 20 patients without gender division who were diagnosed with cervical hypolordosis. After applying cervical stretching exercise and cervical traction for six weeks, cervical headache and the activity of the muscles around the cervical vertebra(upper trapezius muscle, sternocleidomastoid muscle, splenius capitis muscle, and anterior temporal muscle) were investigated and the following results were obtained. In a comparison of the within group intervention effects of the two groups, cervical pain statistically significantly decreased in the cervical stretching exercise group. According to the results of analyzing the change of muscle tension in the upper trapezius muscle, both the cervical traction group and showed a statistically significant within group difference in the left and right side. According to the results of analyzing the change in the muscle tension of the splenius capitis muscle, both groups showed a statistically significant within group difference in the left and right side. In a between-group comparison, a statistically significant difference in the right side was observed. These results confirm that cervical vertebra traction and cervical stretching exercise decrease the cervical headache and muscle activity of the upper trapezius muscle and the splenius capitis muscle among patients with cervical hypolordosis.

다이나믹 테이핑을 적용한 복합 운동프로그램이 긴장성 두통과 전방머리자세를 동반한 만성 목 통증 환자에게 미치는 효과 (Effects of a Complex Exercise Program using Dynamic Taping on Patients with Tension-Type Headache and Chronic Neck Pain with Forward Head Posture)

  • Purpose : This study examined the effects of pain, neck dysfunction, psychosocial level, headache impact test (HIT), postural alignment, and trapezius muscle tone of the complex exercise program using dynamic taping in patients with tension headache and chronic neck pain with forward head posture. Methods : The design of this is a randomized controlled trial (RCT). Thirty-four patients with chronic neck pain were screened using a randomized assignment program and assigned to experimental group (n=17) and control group (n=17). Both groups underwent a complex exercise program. In addition, the experimental group dynamic taping was applied to the upper trapezius muscle. All interventions were applied three times per week for four weeks. The visual analogue scale (VAS), the neck disability index (NDI), short form-12 health survey questionnaire (SF-12), Headache impact test-6 (HIT-6), Craniovertebral angle (CVA), Cranial rotation angle (CRA), upper trapezius muscle tone were compared to evaluate the effect on intervention. Results : Both groups showed significant differences before and after the intervention in VAS, NDI, SF-12, HIT-6, and CVA, CRA (p<.05). In addition, significant differences in NDI and upper trapezius muscle tone were observed between the experimental group and control group (p<.05). Conclusion : A complex exercise program using dynamic taping for patients with tension headache and chronic neck pain with forward head posture are effective method with clinical significance in improving the function and reducing upper trapezius muscle tone.

Effects of hand grip strength on shoulder muscle activity in breast cancer patients

  • Objective: The purpose of this study was to investigate effects of hand grip strength on the muscle activation of shoulder joint in breast cancer patients. Design: Cross-sectional study. Methods: Ten breast cancer patients who agreed to active participation were included. These patients were operated with either conservative surgery or segmental mastectomy, and then were treated with radiation therapy and chemotherapy. The activity of the upper trapezius, lower trapezius, supraspinatus and serratus anterior muscle were measured using surface electrodes during 4 hand gripping tasks (lowered their arms in standing position, 0%, 30%, and 50% of maximum voluntary contraction) in the scapular abduction plane. Results: The results were analyzed using a one-way repeated measures ANOVA. There was a significant difference in the lower trapezius and supraspinatus muscles according to grip strength, lower trapezius and supraspinatus muscles showed significantly difference according to grip strength (p<0.05). The result of the muscle activation according to hand strength (0%, 30%, and 50%) it showed a significant difference between the upper trapezius and supraspinatus in 0% grip strength (p<0.05). In addition, it did not show a significant difference between muscles in 30%, 50% hand strength. Conclusions: This study showed an increase in shoulder muscle activation with increasing hand grip strength with the upper trapezius muscle being more activated than other muscles in 0% grip strength. The finding of this study suggests usefulness for development of preventative measures and rehabilitation strategies for increasing shoulder motor function in patients with breast cancer.

목빗근에 대한 허혈성 압박법이 위등세모근의 만성 통증에 미치는 영향 (Effect of Ischemic Compression on Sternomastoidcleido Muscle on Chronic pain in Upper Trapezius Muscle)

  • 최근 스마트폰 사용 등과 같은 이유로 잘못된 자세가 발생하고 있다. 이로 인하여 근육들의 불균형 단축이 발생하고 있는데 특히 목빗근이 단축되면 더부신경이 압박되어 위등세모근의 허혈성 통증을 일으킬 수 있다. 그리하여 본 연구는 목빗근 1/3지점의 더부신경 포착점에 허혈성 압박법을 중재하여 위등세모근의 통증에 미치는 영향을 알아보았다. 본 연구는 여성 8명을 대상으로 4주간 주 2회 중재하였고, 중재 전, 후 그리고 중재 종료 3주 후 평가하였다. 대상자들의 통증을 알아보기 위해 시각적 사상 척도와 맥길 통증 설문지 그리고 압력 통각계를 사용하였다. 본 연구 결과 치료 후 통증이 유의하게 감소한 것을 확인하였으며, 중재 종료 3주 후 역시 통증 감소에 효과가 있는 것을 확인하여 목빗근의 더부신경 압박점의 중재가 위등세모근의 통증 감소와 지속성이 있음을 확인하였다. 차후 연구에서 대상자를 늘려 위등세모근의 통증 감소뿐만 아니라 목의 기능개선도 확인할 필요성이 있을 것이다.

수동적 어깨뼈 정렬이 작은가슴근 단축된 사람의 등세모근 근활성도에 미치는 영향 (Effects of Passive Scapular Alignment on Electromyographic Activity of Trapezius in People With Shortness of Pectoralis Minor Muscle)

  • The purpose of this study was to assess the influence of scapular alignment on the electromyographic (EMG) activity of the trapezius in people with a short pectoralis minor muscle. For the study, we recruited 15 volunteers who had positive results for short on a pectoralis minor muscle length test. We measured the EMG activity of the upper, middle and lower trapezius muscles. The participants lifted their dominant arm to ear level with the thumb up toward the ceiling in the prone position on a table with the shoulder at a flexion angle of 180 degrees and a horizontal abduction angle of 120 degrees. Scapula was manually aligned by an experienced physical therapist prior to arm lift for the scapular alignment condition. A paired t-test was used to compare the effects of scapular alignment on the EMG activity of the trapezius muscles. The EMG activity of the lower trapezius muscle was significantly increased during the test with the scapular alignment compared to that without scapular alignment (p<.05), while the upper trapezius and middle trapezius exhibited no significant difference between the two conditions (p>.05). The findings of this study suggest that a scapular alignment may alter the recruitment of the lower trapezius muscle during arm lifting in the prone position in people with a short pectoralis minor muscle.

필라테스 슈퍼맨 동작 시 탄성밴드 사용유무와 지면의 종류에 따른 근활성도 분석 (The Analysis of EMG According to Surface Type and Elastic Band Usage During the Pilates Superman Movement)

  • Objective : The purpose of this study was to analyze EMG according to surface type and elastic band usage during the pilates Superman movement. Method : The subjects were 10 female university students with a mean age of 27. The independent variables were surface type (yoga mat, air filled equipment, elastic mat) and elastic band usage. In order to measure muscle activity, the Noraxon(USA) was used. Eight muscles (upper trapezius, lower trapezius, thoracic spine, lumbar spine, gluteus maximus, gluteus medius, biceps femoris, semitendinosus) activation were analyzed. For the statistical analysis, MANOVA, independent t-test and Scheffe test for the post-hoc via SPSS 20.0 was used. Results : The left and right upper trapezius muscle activities were significantly reduced when using the elastic bands. In addition gluteus medius muscle activities significantly increased with the elastic band as well. Conclusion : According to the usage of the elastic band and the different types of surfaces, different muscle groups were recruited. Elastic bands were found to have more activation on the gluteus medius muscle meanwhile there was less activation on the upper trapezius during the pilates Superman movement.

Effects of Body-Adhesive Backpack Condition on Craniovertebral Angle, Sagittal Shoulder Angle and Muscle Fatigue

  • Background: Backpacks are one of the most widely used accessories in modern society. However, previous studies have shown that carrying a backpack can cause various musculoskeletal problems. Objectives: To investigate the effects of a body-adhesive backpack on craniovertebral angle, sagittal shoulder angle and the muscle fatigue in the upper extremity. Design: Randomized cross-over design. Methods: For this study, 36 healthy university students participated in this study. The experiment was conducted three times using common backpack, and body-adhesive backpack condition. The angles of the cervical spine and shoulder joints of the subjects were calculated without the backpack. Electrodes were placed at upper trapezius, lower trapezius, rectus abdominis and erector spinae to check for muscle fatigue. Subjects carried a backpack and walked on a treadmill for 15 minutes at 4 km/h. The muscle fatigue signal was also measured while walking. After walking, the craniovertebral and sagittal shoulder angles were measured again while subjects carried backpack. Results: As a result of this study, body-adhesive backpack condition showed significant decrease and significant increase in craniovertebral angle and sagittal shoulder angle than common backpack (P<.05). Body-adhesive backpack condition showed significant decrease in upper trapezius, lower trapezius, erector spinae, and rectus abdominis when compared to a common backpack (P<.05). Conclusion: This study suggests that a body-adhesive backpack is more beneficial in correcting body posture than a common backpack.

The Study of Muscle Fatigue and Risks of Musculoskeletal System Disorders from Text Inputting on a Smartphone

  • Objective: This study aims to analyze and evaluate fatigue in upper trapezius muscles of adults when inputting text on a smart phone, thereby examining the risk of cumulative trauma disorders in the musculoskeletal system of the upper extremity. Method: Forty-five healthy adults were divided equally into three groups: a smartphone group, a notebook group, and a desktop group. A surface electromyogram was placed on the upper trapezius muscle of each participant, after which they inputted text for ten minutes within an ergonomic setting; their side posture was recorded on video and analyzed. Results: The spectral edge frequency values for the smartphone group, as recorded from the upper right trapezius muscles on the surface electromyogram, showed a significant decrease(p<.05). Regarding the RULA, all three groups had results indicating "action level 2". Conclusion: All three groups had results indicating "action level 2" according to the RULA, which means that additional research on the posture of the participants while performing tasks is necessary, and their posture needs to be corrected. In particular, the smartphone group has the highest levels of fatigue in the upper trapezius muscles, suggesting that the task of inputting text on a smartphone is a high risk for causing cumulative trauma disorders.

고주파심부투열 치료가 앞쪽머리자세를 가진 성인의 자세정렬, 통증 그리고 기능에 미치는 즉각적인 효과 (Immediate Effects of High-Frequency Diathermy on Neck, Shoulder Alignment, Pain and Function in Adults with Forward Head Posture)

  • Purpose : Forward head posture (FHP), characterized by the anterior positioning of the head relative to the spine, is a common postural deviation that can lead to neck pain, reduced mobility, and muscle imbalances. Recently, high-frequency deep heat therapy (HFDT) has been gaining attention for the intervention of FHP. This research aims to investigate the efficacy of HFDT in comparison to instrument assisted soft-tissue mobilization (IASTM) for treating FHP among 30 young adults. Methods : Participants were randomly assigned to either the HFDT or IASTM group. The study focused on examining changes in neck joint mobility, pain thresholds, rounded shoulder distance, lower trapezius muscle strength, and neck dysfunction. Measurements were taken before and after the interventions. Paired t-tests were used for within-group analyses, and independent t-tests were employed for between-group comparisons. The statistical significance level α was set to .05. Results : Statistically significant improvements were observed across all measured parameters in both groups (p<.05). The HFDT group showed significantly greater enhancements in neck joint mobility, pain thresholds, rounded shoulder distance, lower trapezius muscle strength, and neck dysfunction parameters. Specifically, HFDT was more effective than IASTM in improving neck joint mobility, right upper trapezius pain threshold, left rounded shoulder distance, and right lower trapezius strength. The only exceptions were neck flexion range of motion, left upper trapezius pain threshold, right rounded shoulder distance, and left lower trapezius strength, where no significant differences were found between the groups. Conclusion : The findings suggest that HFDT, by combining the benefits of high-frequency therapy and manual therapy, effectively alleviates upper trapezius muscle pain and tension, enhances neck mobility, and strengthens lower trapezius muscles. Thus, HFDT could be considered a valuable intervention for clinicians aiming to address FHP and associated musculoskeletal problems.
