Objective: This study examined the effects of working chair that was developed for farmers who work in grapevine's cluster thinning. Background: Agricultural work involves some of the nation's highest occupational risk exposures. Fruit cultivation has been recognized as one of the most hazardous crops in which to work. Grapevine cluster thinning task involve activities related to the main risk factors associated with upper limb work-related musculoskeletal disorders. Method: An experiment was designed to test the working chair. Six healthy male($25.8{\pm}4.9years$) were selected as the subjects for this study. Electromyography(EMG) was used to monitor the muscles activity, and Electro-goniometer was used to measure the ranges of motions of the elbow, trunk and knee. Subjective test was also carried out to examine discomfort body parts and their pain intensity. A grapevine's working space was built for the experiment and working chair was installed on it. In order to examine the muscle activity and range of motion, subjects used to the working chair for 30 minutes for each experimental condition. Another test without working chair was also performed for comparison. %MVC was used to quantify the level of muscle activity. Results: Decreases of muscle activity was found in all leg muscles and significant decrease of muscle activity was found in left Gastrocnemius. The range of motion of the trunk and knee also decreased when working chair used. Discomfort in lower back, thigh and shank region were reduced significantly. However, in upper limbs muscle activity tended to increase in working chair compared with conventional task. Conclusion: Improvement for cushion in seat back and pan required to reduce discomfort in buttocks. Application: Overall findings verified that the working chair might help to prevent upper limb and lower back MSDs based on the current study. These results can be practically used for work improvement for the grapevine growers to prevent MSDs.
WMSD occurs by accumulating the visible position and movements. Therefore, it is easier to prevent WMSD from occurring than other diseases when each employee is aware of the factors which cause WMSD. Hence, We need to develop exercise remedy, which can be done without using any exercise equipments and regardless places. The exercise prescription for those for LBP was to do the upper body exercise remedy twice in the morning and another twice in the afternoon and the spinal exercise remedy and the lower body exercise remedy once in the morning and once in the afternoon. We measured their maximum muscular strength every second week using a fitness machine so that we could compare the differences of the muscular strength of the test group and the comparison group. Therefore this research presented the fact that the exercise remedy is effective to prevent and cure LBP through a scientific test. And it confirmed that the exercise remedy by the Ergonomic exercise prescriptions is effective on LBP.
The purpose of this study was to investigate the characteristics and the current condition of musculoskeletal pain sufferers among fitness center users. The investigation used self-reporting questionnaire with the 797 subjects. On average, the subjects were 30.6 years old, 168.8 cm in height and 65.2 kg in weight. Among them, 423 were male (53.1%) and 374 were female (46.9%). Out of them, 276 suffered from pain, (35.2%). More men felt muscle aches than women (p<.05). In terms of pain, lower back area topped the list with 44.9%, followed by the lower extremities (30.1%), the neck (17.8%) and the upper part of the body (17.8%). Dieters suffered the most from pain (86.2%) while those exercising to build strength suffered the least with 26.8%. There was a significant relationship between the purpose for exercising and the amount of pain suffered (p<.05). By kind of exercise, pain stroke aerobic exercisers disproportionately with 42.4% while it almost steered clear of exercisers for strengthening with 26.9%. There was significant relationship between the kind of exercise and pain suffered (p<.05). The type of suffering differed by body shape. A significantly higher number of overweight people experienced pain than underweight people (p<.05). 32.8% of pain sufferers commented that the pain affects their workout, and on this issue there was no gender difference (p>.05). The pain caused more difficulties in doing activities of daily living for overweight or obese peoples than underweight peoples (p<.05). More than a third of health club and fitness center users are experiencing musculoskeletal pains. Measures such as professional training or information provision is required to prevent injury or disorder caused by improper exercise.
A pancreatic pseudocyst is the most common cystic lesion in the pancreas. There are several treatment methods with variable results. Recently, conservative treatment has been preferred because it has fewer complications. In this case, a 37-year-old male patient had pancreatic pseudocysts with upper abdominal pain, left upper back pain, diarrhea, weight loss, and general weakness. We prescribed him Banchongsangamibang (蟠蔥散加味方). For about 1 year, he was treated with oriental medicine. All of his pancreatic pseudocysts disappeared on abdominal CT follow-up and the symptoms mostly subsided. There had been no recurrence of pancreatic pseudocysts on abdominal CT follow-up after 1 year.
Twenty five cases of the patient admission-treatment from chiefly complained of neck pain observed clinically from first January 1997 to thirty first December 1997 in the Dept. of Acupuncture and Moxibustion, Oriental Hospital, Taejon University, Taejon, Korea. And the results were obtained as follows; 1. The distribution of sex was male 13(52%) cases, female 12(48%) cases and the male to female ratio was 1.1 : 1. The distribution of duration was the most predominant of 9(36%) cases in a week. 4. The distribution of before admission-treatments were the most predominant as west-medication 13(32.5%) cases. 5. The distribution of patient's condition of first treatment was the most predominant as Gr.III 13(52%) cases. 6. The distribution of duration for admission was the most predominant as 13(52%) cases in a week. 7. The distribution of radiological studies were the nust predominant as the HIVD 26(63.4%) cases. 8. The distribution of clinical symptoms were the most predominant as neck pain 25(21.0%) cases, radiating pain to the upper extremities 23(19.3%) cases. 9. The distribution of located on clinical syrnptoms were the most predominant as neck-shoulder and back-upper extremities 11(44%) cases. 10. The distribution of clinical diagnosis was the most predominant as HIVD 16(45.7%) cases. 11. The distribution of methods of treatment were the most predominant as Acupuncture - Herb Medication - Electro Acupuncture treatment 8(32%) cases. 12. The effect of treatment by discharge was the most predominant of 11(44%) cases as Good result.
Follicular thyroid cancer(FTC) accounts for about 10-15% of thyroid cancer. Distant metastasis is common, usually to lung, bone and brain. 71-years-old man visited neurosurgery outpatient department. He complained of recent 6kg weight loss, left upper extremity pain with weakness and back pain. The radiologic findings showed multiple bone metastasis including thoracic spine and left scapular resulting from FTC. There was a probable brain metastatic lesion on right temporal fossa. The core biopsy of thyroid and thoracic spine(T11) confirmed metastatic follicular carcinoma. Radioactive iodine therapy and radiotherapy was done following total thyroidectomy. We report a unique case of multiple bone metastasis from follicular carcinoma of thyroid with literature review.
We present a case of chondroblastoma in the thoracic vertebra. A 40-year-old patient with upper back pain and lower extremity weakness was admitted to our clinic. On neurological examination, the patient exhibited lower extremity spastic paraparesis. Magnetic resonance imaging revealed a mass infiltrating the 7th thoracic vertebra and its adjacent structures with concomitant compression of the epidural space. After right upper lung tuberculoma was resected through the transthoracic approach, T7 total corpectomy was done with anterior stabilization using a MESH cage and T7 rib bone graft. Two weeks after the first operation, remained part of vertebra was removed and posterior stabilization was performed using a pedicle screw fixation and cross linkage bar with the assistance of the navigation system. The final pathologic diagnosis of the vertebral lesion was benign chondroblastoma.
Background: We aimed to explore the American College of Rheumatology (ACR) 1990 and 2011 fibromyalgia (FM) classification criteria's items and the components of Fibromyalgia Impact Questionnaire (FIQ) to identify features best discriminating FM features. Finally, we developed a combined FM diagnostic (C-FM) model using the FM's key features. Methods: The means and frequency on tender points (TPs), ACR 2011 components and FIQ items were calculated in the FM and non-FM (osteoarthritis [OA] and non-OA) patients. Then, two-step multiple logistic regression analysis was performed to order these variables according to their maximal statistical contribution in predicting group membership. Partial correlations assessed their unique contribution, and two-group discriminant analysis provided a classification table. Using receiver operator characteristic analyses, we determined the sensitivity and specificity of the final model. Results: A total of 172 patients with FM, 75 with OA and 21 with periarthritis or regional pain syndromes were enrolled. Two steps multiple logistic regression analysis identified 8 key features of FM which accounted for 64.8% of variance associated with FM group membership: lateral epicondyle TP with variance percentages (36.9%), neck pain (14.5%), fatigue (4.7%), insomnia (3%), upper back pain (2.2%), shoulder pain (1.5%), gluteal TP (1.2%), and FIQ fatigue (0.9%). The C-FM model demonstrated a 91.4% correct classification rate, 91.9% for sensitivity and 91.7% for specificity. Conclusions: The C-FM model can accurately detect FM patients among other pain disorders. Re-inclusion of TPs along with saving of FM main symptoms in the C-FM model is a unique feature of this model.
Purpose : Instrument-assisted soft tissue mobilization (IASTM) has been widely used to improve pain and range of motion. However, research on IASTM using vibration stimulation is lacking. Therefore, this study investigates how IASTM using vibration stimulation affects chronic pain on muscle activity and flexibility of the shoulders. Methods : A total of 20 patients with chronic shoulder pain were divided into the experimental group and the control group. The experimental group (n=10) used vibration stimulus for IASTM, while the control group (n=10) used general IASTM. For six weeks, this training lasted for 30 min a day, twice a week. Muscle activity was evaluated using the surface electromyograph. The back and reach test was used to assess flexibility, the visual analogue scale to measure pain, and the paired t-test to compare the groups before and after the experiment. An independent ANCOVA was conducted to assess differences in the degree of transition between the two groups before and after the experiment. Results : Significant differences in muscle activity, flexibility, and pain in both groups before and after the experiment (p<0.05) were observed, as well as in the muscle activity of the pectoralis major and associated pain (p<0.05). however, muscle activity and flexibility of the upper trapezius and infraspinatus were not significantly different between groups (p>0.05). Conclusion : This study confirms the importance of IASTM using vibration. Design and manufacture of IASTM using vibration based on our results would be useful in the management of shoulder pain, Further clinical evidence are needed.
Many out patients of the rehabilitation center complaint a pain caused by suffering of MPS, and it brings many different kind of social and ecinomical problems such as medical expenses and the reduction in work efficiency. Therefore, we conducted a research to present a fast and effective treatment to the MPS patients. this research was conducted from November, 1996 to January 1998 with eighteen outpatients who agreed to be the subjects to our reserach. We applied the electric acupuncture on 28 different trigger point on the patients with mostly muscular-skeletal pain and some tender and radiating pain. After the treatment, VRS, VAS, PRI were used to measure the degree of the pain on the patients third and seventh visits, and following results were found. 1. To apply EAP treatment, we acupunctured 6em-Iong needles on Tps region, then the electrodes of pulse generater pg-306 E.S.T were connected on the top of the needles. the electric acupuncture therapy was conducted for 20 minutes with the intensity of 4hz - 60hz(auto wave). The treated electric intensity was the level at which the patients did not feel discomfort. 2. Thirteen out of the 18 participated patients were in their 30s and 40s(72.2%), showing highest frequency. There were more female than male with the ratio of 1 to 1:2. 3. six out of the participated patients (33.3%) had the pain for less then a week, and the average duration of the pain of the participated patients wear 0.8 years. 4. The pain occured mostly in the upper trapezius by 6 part (21.4%), then in the gluteus medius region by 4 part (14.3%). Many of the patients with the pain in the upper back area accompanied varios kind of the referred pains such as radiculopathy, HIVD and Frozen shoulder. 5. MPS occured more frequently on the right side than left side then left side and it seemed to be due to the frequent use of the right hand. 6. There was almost no difference in the measurement of the intensity of the pain right before and after the EAP treatment. Howerver, there was significant decrease in the numerical values of the VAS, and a little bit of decrease in the numerical values of the PRI after the EAP treatment. 7. Based on the results of this present research, it can be concluded that EAP can be used for the treating the myofascial pain syndrome with promptness and safety in most cases.
