갑상선암 수술과 수술 전후 음성관리 (Perioperative Management of the Voice in Thyroid Cancer)

  • Evaluating the patient's voice before thyroidectomy is useful for the purpose of identifying patients with vocal cord paralysis without symptoms, identifying other patient's voice abnormalities, and whether it is related to voice disorders that may occur after surgery. Also voice evaluation after thyroid surgery is helpful in diagnosis, treatment, and rehabilitation and follow-up of voice disorders that occur without clear nerve damage after thyroidectomy. And it is helpful for rapid recovery through active early rehabilitation treatment for patients who complain of speech impairment without paralysis. In particular, neck exercise can improve the adhesion of the surgical site and increase the range of motion of the neck as well as improve subjective neck discomfort. In addition, hearing, voice and breathing functions should be improved, and voice hygiene education and counseling should be provided. Vocal cord injection is the first treatment option for unilateral vocal cord palsy. By establishing a protocol for voice disorders before and after thyroid surgery and providing appropriate treatment, the quality of life of patients can be improved.

일측성 성대 마비에서 $Radiesse^{(R)}$를 이용한 성대 주입술 : 예비보고 (Injection Laryngoplasty with $Radiesse^{(R)}$ in Unilateral Vocal Fold Palsy : Preliminary Report)

  • Background and Objectives: $Radiesse^{(R)}$ is a gell-formed material of calcium hydroxylapatite (CaHA) and carboxymethylcellulose (CMC) used for vocal fold injections. The authors aimed to study injection laryngoplasty with $Radiesse^{(R)}$, and determine the efficacy of $Radiesse^{(R)}$ for unilateral vocal cord palsy using objective and subjective measures. Materials and Method: Nine patients with unilateral vocal cord palsy received injection laryngoplasty with $Radiesse^{(R)}$ under general anesthesia from Jul. 2007 to Jan. 2008. $Radiesse^{(R)}$ was injected with 25gauze long needle perorally or percutaneously. The Acoustic, aerodynamic, stroboscopic analysis and pre-injection/post-injection perceptual assessment were evaluated in all patients. Results: Postoperative jitter and shimmer scores tended to diminish compared to preoperative scores, and maximum phonation time increased with statistical significantly. Stroboscopic findings demonstrated improvement postoperatively. The degree of hoarseness, which is a part of perceptual assessment, decreased after the procedure. Conclusion: $Radiesse^{(R)}$ may be an altemative material for injection laryngoplasty. We suggest long-term follow up with more cases.

발성장애 평가 시 /a/ 모음연장발성 및 문장검사의 켑스트럼 분석 비교 (Comparison of Vowel and Text-Based Cepstral Analysis in Dysphonia Evaluation)

  • Background : Cepstral analysis which is obtained from Fourier transformation of spectrum has been known to be effective indicator to analyze the voice disorder. To evaluate the voice disorder, phonation of sustained vowel /a/ sound or continuous speech have been used but the former was limited to capture hoarseness properly. This study is aimed to compare the effectiveness in analysis of cepstrum between the sustained vowel /a/ sound and continuous speech. Methods : From March 2012 to December 2014, total 72 patients was enrolled in this study, including 24 unilateral vocal cord palsy, vocal nodule and vocal polyp patients, respectively. The entire patient evaluated their voice quality by VHI (Voice Handicap Index) before and after treatment. Phonation of sustained vowel /a/ sample and continuous speech using the first sentence of autumn paragraph was subjected by cepstral analysis and compare the pre-treatment group and post-treatment group. Results : The measured values of pre and post treatment in CPP-a (cepstral peak prominence in /a/ vowel sound) was 13.80, 13.91 in vocal cord palsy, 16.62, 17.99 in vocal cord nodule, 14.19, 18.50 in vocal cord polyp respectively. Values of CPP-s (cepstral peak prominence in text-based speech) in pre and post treatment was 11.11, 12.09 in vocal cord palsy, 12.11, 14.09 in vocal cord nodule, 12.63, 14.17 in vocal cord polyp. All 72 patients showed subjective improvement in VHI after treatment. CPP-a showed statistical improvement only in vocal polyp group, but CPP-s showed statistical improvement in all three groups (p<0.05). Conclusion : In analysis of cepstrum, text-based analysis is more representative in voice disorder than vowel sound speech. So when the acoustic analysis of voice by cepstrum, both phonation of sustained vowel /a/ sound and text based speech should be performed to obtain more accurate result.

Autologous Cartilage Intracordal Injection in Unilateral Vocal Fold Paralysis

  • Objectives : The methods to treat glottic incompetence include thyroplasty type I, arytenoid abduction, and intracordal injection using various materials. The intracordal injection is easy and simple and does not require skin incision. In general, the grafted cartilage shows a high survival rate, a low absorption rate and small voluminous change. The authors performed injection of minced autologous auricular cartilage and fat using a Bruning injector in unilateral vocal cord palsy We evaluate the effect and safety of autologous auricular cartilage intracordal injection. Study Design : Retrospective study. Methods : Auricular cartilage was obtained by incising tragus vertically and it was minced with a scalpel and #15 blade. About 2g of abdominal fat was obtained by small periumbrical incision and cut into small pieces. The minced cartilage was put into a 1$m\ell$ injector and then the injector was filled with fat. The operation was conducted under laryngeal microscope. Minced cartilage was injected into the vocalis muscle at the junction of the middle and posterior third of the vocal fold. In three cases, we performed autologous cartilage intracordal injection. Results : We observed no postoperative complications, such as dyspnea, granulation, inflammation, in any of the cases. The voice was improved compared with the voice prior to operation in all cases. Conclusion : Although the cases are still limited and the observation period is short, we suggest that the autologous cartilage using the auricular cartilage is the ideal and new effective augmentative material in vocal cord palsy.

  • Herpes zoster oticus is one of complication of varicella zoster virus (VZV) reactivation in the geniculate ganglion of the facial nerve, which is the most common presentation of herpes zoster in the head and neck region. However, VZV infection of the larynx has rarely been described in the literature compared with Herpes zoster oticus. Moreover, zoster laryngopharyngitis simultaneously occurred with recurred Herpes zoster oticus which has no newly developing motor dysfunction has not been reported yet. Therefore, these diseases are difficult to diagnose due to its rareness. However, distinctive appearances such as unilateral herpetic mucosal eruptions and vesicles are useful and essential in making a quick and accurate diagnosis. Thus, we report a characteristic case of zoster laryngopharyngitis simultaneously occurred with recurred Herpes zoster oticus not accompanied by any newly developing motor palsy.

  • Background and Objectives: $VoCoM^{(R)}$ is a set composed of prefabricated hydroxylapatite implants and shims of various sizes that are designed for the type I thyroplasty, Authors aimed to evaluate the efficacy of $VoCoM^{(R)}$ System in type I thyroplasty. Materials and Method: Twenty three patients with unilateral vocal cord palsy were included in the study, who received type I thyroplasty with $VoCoM^{(R)}$ between May 2000 and May 2007 in St. Mary's Hospital, The Catholic University of Korea, Seoul, Korea, Acoustic, aerodynamic and stroboscopic analyses were performed pre-and post-operatively, Subjective voice improvement was analysed by Voice handicap index, Results : Preoperative jitter was $4.68{\pm}2.46%$ and improved to $3.19{\pm}1.94%$(P<0,05), Preoperative NHR was $0.26{\pm}0.1$ and improved to $0.18{\pm}0,07$(P<0.05), Preoperative MPT was $6.16{\pm}4.9$secs improved to $9.55{\pm}4.67$secs(p<0.05), The postoperative stroboscopy revealed an effective medialization of vocal fold of all patients, Conclusion: Type I thyroplasty using $VoCoM^{(R)}$ is a efficient, safe and convenient way of vocal fold medialization at the expense of its high cost and difficulty in removal.

  • The causes of vocal cord paralysis include iatrogenic injury during thyroid or cervical surgery, heart and chest surgery, and tumorous lesion such as laryngeal cancer and lung cancer. In addition to these common causes, rarely, cardiovascular disease can also cause vocal fold paralysis. A disease known as Cardiovocal syndrome, or Ortner's syndrome, causes left vocal fold paralysis when the left recurrent laryngeal nerve is compressed by the pulmonary artery and aorta, which is occurred by pulmonary hypertension from heart disease. We report for the first case in Korea the diagnosis of vocal fold paralysis caused by Ortner's syndrome.

실시간 광유도 성대주입술의 개발과 임상적 적용 (Development and Clinical Application of Real-Time Light-Guided Vocal Fold Injection)

  • Vocal fold injection (VFI) is widely accepted as a first line treatment in treating unilateral vocal fold paralysis and other vocal fold diseases. Although VFI is advantageous for its minimal invasiveness and efficiency, the invisibility of the needle tip remains an essential handicap in precise localization. Real-time light-guided vocal fold injection (RL-VFI) is a novel technique that was developed under the concept of performing simultaneous injection with precise placement of the needle tip under light guidance. RL-VFI has confirmed its possibility of technical implementation and the feasibility in injecting the needle from various directions through ex vivo animal studies. Further in vivo animal study has approved the safety and feasibility of the procedure when various transcutaneous approaches were applied. Currently, RL-VFI device is authorized for clinical use by the Ministry of Food and Drug Safety in South Korea and is clinically applied to patients with safe and favorable outcome. Several clinical studies are currently under process to approve the safety and the efficiency of RL-VFI. RL-VFI is expected to improve the complication rate and the functional outcome of voice. Furthermore, it will support laryngologists in overcoming the steep learning curve by its intuitive guidance.

단단문합술에 의한 기관재건의 임상적 고찰 (Clinical Analysis of Cases of Laryngotracheal Reconstruction Treated with End to End Anastomasis)

  • From 1988 to 1995, 59 patients with laryngotracheal stenosis were operated with resection of stenotic segment and end to end anastomosis in Seoul national university. Among these patients, 36 were tracheal stenosis, 17 were combined lesion of subglottis and trachea and 5 were subglottic stenosis. The success of procedure was determined by stoma closure or decannulation and an overall 89.8% success rate was achieved following 165 procedures. The success rate showed correlation with severity of stenosis and the number of procedures done was related to the site of stenosis. There was no mortality and granulation formation, unilateral vocal cord palsy were the common complication of end to end procedures.
