순구개열이 영구치 근원심 폭경에 미치는 영향 (THE EFFECTS OF CLEFT ON MESIODISTAL DIMENSIONS OF PERMANENT TEETH IN UNILATERAL CLEFT LIP AND PALATE PATIENTS)

  • 본 연구는 편측성 순구개열 환자에서 영구치의 근원심 폭경에 대한 순구개열의 영향을 조사하기 위하여 시행하였다. 연구 대상으로는 편측성 순구개열자 50명과 그의 형제나 자매 10명을 실험군으로 선정하고, 대조군으로는 별다른 골격적 부조화를 보이지 않는 정상교합자 50명을 선정하여, 인상채득 후 석고모형을 채득하여 영구치 근원심 폭경을 계측하여 다음과 같은 결과를 얻었다. 1. 순구개열군에서 상하악 견치와 하악 제1소구치를 제외한 모든 치아에서 파열측 치아가 비파열측 치아보다 작았다. 2. 대조군에서는 상악 측절치를 제외한 모든 치아에서 좌우측 치아크기의 차이가 나타나지 않았다. 3. 형제군에서는 상악 제1대구치를 제외한 모든 치아에서 치아크기의 차이가 나타나지 않았다.

Comparative evaluation of nasal and alveolar changes in complete unilateral cleft lip and palate patients using intraoral and extraoral nasoalveolar molding techniques: randomized controlled trial

  • Objectives: Cleft lip and palate is a common congenital anomaly that impairs the aesthetics, speech, hearing, and psychological and social life of an individual. To achieve good aesthetic outcomes, presurgical nasoalveolar molding (NAM) has become important. Currently, the intraoral NAM technique is widely practiced. Numerous modifications have been made to intraoral NAM techniques, but the original problem of compliance leading to discontinuation of treatment remains unsolved. Therefore, the present study compared an extraoral NAM technique with the intraoral NAM technique. Materials and Methods: Twenty infants with complete unilateral cleft lip and palate were included and divided into two equal groups. Group A received the intraoral NAM technique, and Group B received the extraoral NAM technique. Pre- and postoperative extraoral and intraoral measurements were recorded. Results: Groups A and B did not differ significantly in any extraoral or intraoral parameter. Conclusion: The extraoral NAM technique is as effective as the intraoral NAM technique in achieving significant nasal and alveolar changes in complete unilateral cleft lip and palate patients. Additionally, it reduces the need for frequent hospital visits for activation and the stress associated with the insertion and removal of the intraoral NAM plate, thereby improving compliance.

Evaluation of facial appearance in patients with repaired cleft lip and palate: comparing the assessment of laypeople and healthcare professionals

  • Background: The present study aimed to determine whether laypeople and professionals rate the facial appearance of individuals with repaired complete unilateral or bilateral cleft lip and palate (UCLP, BCLP) similarly based on viewing full facial images. Methods: The study followed a cross-sectional analytical design where five young patients aged 10 to 14 years, who had completed all stages of their unilateral or bilateral cleft lip and palate treatment (bilateral: three, unilateral: two), were evaluated by two groups. The assessment was done by laypeople and 97 qualified professionals (33 orthodontists, 32 plastic surgeons, and 32 oral and maxillofacial surgeons). Professionals were not involved in any stage of the patients' treatment. Results: The facial appearance assessment of the professional groups on different facial aesthetics was significantly lower than that of laypeople, and they had higher perceived need for further treatment. On the other hand, laypeople had higher aesthetic ratings and lower perceived need for further treatment. Differences were also observed between the assessments of the professional groups. Participants who had lower aesthetic assessments of the repair tended to report a higher influence of cleft lip and palate on social activities and professional life. Conclusion: Differences in perception exist between healthcare professionals and laypeople. The discrepancies between the professional groups could be attributed to different treatment modalities and protocols.

순구개열자의 선천결손치와 과잉치의 발생빈도에 관한 연구 (A STUDY OF THE FREQUENCY OF CONGENITAL MISSING AND SUPERNUMERARY TEETH IN CLEFT LIP AND PALATE PATIENTS)

  • The purpose of this study was to evaluate the frequency of congenital missing teeth and supernumerary teeth in cleft patients. The subjects were divided into bilateral cleft lip and palate(BCLP), unilateral cleft lip and palate(UCLP) and cleft palate alone(CP alone) groups. 97 cleft patients(BCLP 15, UCLP 70, CP alone 12) between 6-20 years old were evaluated. Panorama film, Orthodontic chart and initial intraoral photogram were employed for this research. The obtained results were as follows. 1. The incidence of congenital missing teeth in total cleft samples was $57.7\%$, and the incidence of supernumerary teeth was $26.8\%$. Each incidence was higher than non-cleft. 2. The incidence of congenital missing teeth was the highest in BCLP and the lowest in CP alone. 3. The number of congenital missing teeth per perso was usually one, and the frequency was higher in the maxillary lateral incisors$(67.8\%)$, and maxillary second premolar$(14.9\%)$ than other teeth. 4. Most of tooth number anomalies in cleft patients were found in maxilla, especially adjacent region to the cleft site.

편측성 완전 구순구개열 환자의 포괄적 치료 (COMPREHENSIVE TREATMENT OF UNILATERAL COMPLETE CLEFT LIP AND PALATE)

  • Cleft lip and palate is one of the congenital anomalies which need comprehensive and multidisciplinary treatment plan because 1) oral cavity is an important organ with masticatory function as a start of digestive tract, 2) anatomic symmetry and balance is esthetically important in midfacial area, and 3) it is also important to prevent psycho-social problems by adequate restoration of normal facial appearance. There are many different protocols in the treatment of cleft lip and palate, but our department has adopted and modified the $Z{\"{u}}rich$ protocol, as published in the Journal of Korean Cleft Lip and Palate Association in 1998. The first challenge is feeding. Type of feeding aid ranges from simple obturators to active orthopedic appliances. In our department we use passive-type plate made up of soft and hard acrylic resin which permits normal maxillary growth. We use Millard's method to restore normal appearance and function of unilateral complete cleft lip. In consideration of both maxillary growth and phonetic problems, we first close soft palate at 18 months of age and delay the hard palate palatoplasty until 4 to 5 years of age. When soft palate is closed, posterior third of the hard palate is intentionally not denuded to allow normal maxillary growth. In hard palate palatoplasty the mucoperiosteum of affected site is not mobilized to permit residual growth of the maxilla. We have treated a patient with unilateral complete cleft lip and palate by Ajou protocol, which is a kind of modified $Z{\"{u}}rich$ protocol. It is as follows: Infantile orthopedics with passive-type plate such as Hotz plate, cheiloplasty with Millard's rotation-advancement flap, and two stage palatoplasty. It is followed by orthodontic treatment and secondary osteoplasty to augment cleft alveolus, orthognathic surgery, and finally rehabilitation with conventional prosthodontic treatment or implant installation. The result was good up to now, but we are later to investigate the final result with longitudinal follow-up study according to master plan by Ajou protocol.

교과서 분석을 통해 본 북한의 구순$\cdot$구개열 현황 (The status of Cleft Lip and Palate in North Korea; Analysis of North Korean textbooks)

  • The dissimilarities between South and North Korea have persisted in spite of the reconciliation campaign by both countries. The situation of the cleft lip & palate of North Korea was very unclear until now. The purpose of this study is to understand all the current facts of cleft lip & palate in North Korea so that we can find ways of helping North Korea in this field of medicine. The present data and analysis are extracted from North Korean textbooks. The results are as follow. 1. In North Korea, patients with CLP are treated by oral surgeons or maxillofacial surgeons. The detailed contents about the CLP are well described in the North Korean textbooks for the dental students. 2. The terminology of CLP in North Korea has changed from time to time, but the present terminology not being so different from South Korean counterpart. So there will be no particular problems in mutual communication. 3. The main classification for CLP in North Korea originated from Kernahan & Stark's classification as is with South Korea. 4. The incidence of CLP is 1 : 1,000-1,200 in North Korea, which is lower than that of South Korea. There is, however, some difference between the North and South Korean CLP in detailed statistics. 5. We found the North Korean physicians have shown much interest in pursuing the etiology and the prevention of CLP. 6. The timing of CLP operations varied a lot in North Korea. There was recommendation by few for the operation in much late age than in South Korea. 7. The classical operation techniques of cleft lip have changed. For unilateral cleft lip Tennison-Randall method was replaced by Millard I method: and for bilateral cleft lip LeMesurier method was replaced by Veau III and Tennison methods. But for cleft palate Pushback palatoplasty has been utilized consistently.

  • The author presents Bardach' s technique for the residual unilateral cleft lip nasal deformity, The key to a successful and stable correction of the nasal deformity is to lengthen the columella on the cleft side and to mobilize alar cartilage from its surrounding tissue, creating a symmetric shape and length, The major advantages of the technique are lengthening of the cleft columella and creation of a symmetric and well-projected nasal tip.

  • The alar base on the cleft side in unilateral complete cleft lip, alveolus and palate is markedly displaced laterally, caudally and dorsally, By incising the pyriform margin from the cleft margin of the alveolar process, including mucosa of the anterior part of the inferior turbinate, to the upper end of the postnasal vestibular fold, the alar base is released from the maxilla, A physiological correction of nasal deformity can be accomplished by careful reconstruction of nasolabial muscle integrity, functional repair of the orbicular muscle, raising and rotating the displaced alar cartilage, and finally by lining the lateral nasal vestibule, The inferior maxillary head of the nasal muscle complex is identified as the deeper muscle just below the web of the nostril, The muscle is repositioned inframedially, so that it is sutured to the periosteum that overlies the facial aspect of the premaxilla in the region of the developing lateral incisor tooth, And then, the deep superior part of the orbicular muscle is sutured to the periosteum and the fibrous tissue at the base of the septum, just in front of the anterior nasal spine, The nasal floor is surgically created by insertions of the nasal muscle complex in deep plane and of the orbicular muscle in superficial one, The upper part of the lateral nasal vestibular defect is sutured by shifting the alar flap cephalically, The middle and lower parts of this defect are closed by use of cleft margin flaps of the philtral and lateral segments, respectively, Authors stress the importance of nasal floor reconstruction at primary surgery and report the technique and postoperative results.

  • The authors had treated 27 cleft lip and cleft palate patients including 16 male patients and 11 female patients. There were 17 cases of unilateral cleft lip patients, 4 cases of bilateral cleft lip patients, and 6 cases with cleft lip and cleft palate patients. We had accomplished that the surgical plastic closures of cleft lips and cleft palates had been performed by using of Millard's, Hagedorn's, Meyer's, LeMesurier's, Wunderer's, Lindemann's methods and Veau's Method with Z-plasty.
