Pre-Coronavirus Disease 2019 Pediatric Acute Appendicitis: Risk Factors Model and Diagnosis Modality in a Developing Low-Income Country

  • Purpose: Pediatric acute appendicitis has a stable incidence rate in Western countries with an annual change of -0.36%. However, a sharp increase was observed in the Asian region. The Indonesian Health Department reveals appendicitis as the fourth most infectious disease, with more than 64,000 patients annually. Hence, there is an urgent need to identify and evaluate the risk factors and diagnostic modalities for accurate diagnosis and early treatment. This study also clarifies the usage of pediatric appendicitis score (PAS) for children <5 years of age. Methods: The current study employed a cross-sectional design with purposive sampling through demographic and PAS questionnaires with ultrasound sonography (USG) results. The analysis was performed using the chi-square and Mann-Whitney tests and logistic regression. Results: This study included 21 qualified patients with an average age of 6.76±4.679 years, weighing 21.72±10.437 kg, and who had been hospitalized for 4.24±1.513 days in Siloam Teaching Hospital. Compared to the surgical gold standard, PAS and USG have moderate sensitivity and specificity. Bodyweight and stay duration were significant for appendicitis (p<0.05); however, all were confounders in the multivariate regression analysis. Incidentally, a risk prediction model was generated with an area under the curve of 72.73%, sensitivity of 100.0%, specificity of 54.5%, and a cut-off value of 151. Conclusion: PAS outperforms USG in the sensitivity of diagnosing appendicitis, whereas USG outperforms PAS in terms of specificity. This study demonstrates the use of PAS in children under 5 years old. Meanwhile, no risk factors were significant in multivariate pediatric acute appendicitis risk factors.

Hypoadrenocorticism in a 1-Year-Old Korean Shorthair Cat

  • A one-year-old spayed female Korean Shorthair cat presented to Kangwon National University Veterinary Hospital with vomiting, weight loss, lethargy, loss of appetite, and polyuria that lasted for more than two weeks. The body condition score, blood pressure, heart rate, and body temperature were abnormally low, and the physical examination findings were consistent with moderate dehydration. Hematological and biochemical tests demonstrated mild azotemia and a low Na:K ratio. Additional abdominal ultrasound imaging revealed reduced size of both adrenal glands. The adrenocorticotropic hormone (ACTH) stimulation test showed decreased post-ACTH cortisol and aldosterone levels and increased endogenous ACTH levels, confirming a diagnosis of primary hypoadrenocorticism. The cat was treated with subcutaneous injections of desoxycorticosterone pivalate (DOCP) and oral prednisolone supplementation, and subsequent electrolyte analysis showed a normal Na:K ratio. Clinical symptoms were also improved in response to treatment. Hypoadrenocorticism in cats is a very rare disease, but it should not be excluded as a potential diagnosis in favor of kidney diseases or other conditions, especially when the Na:K ratio is low. In addition, the prognosis for the disease and the response to DOCP treatment should be further evaluated in cats.

  • Newborn gastrointestinal tract emergency diseases include various disorders that occur anywhere along the digestive tract. Most of them are congenital malformations, not acquired diseases. Many of them cannot be confirmed on prenatal examination, and consequently, postnatal evaluation is required. Unlike adults, in newborn babies, detailed evaluations with computed tomography (CT) and magnetic resonance imaging (MRI) are often limited, so making the diagnosis based on early abdominal radiography is most important. Therefore, it is necessary to be familiar with the radiologic findings that may be seen on imaging studies. In addition, it is important to understand abdominal ultrasound and fluoroscopy findings, which can be used in addition to simple radiography and indications that can be used for further diagnosis and appropriate treatment. Therefore, this study describes and organizes the various imaging findings that may occur in neonatal gastrointestinal emergencies.

  • In patients with thyroid nodules, ultrasonography (US) has been established as a primary diagnostic imaging method and is essential for treatment decision. The Korean Thyroid Imaging Reporting and Data System (K-TIRADS) is a pattern-based, US malignancy risk stratification system that can easily diagnose nodules during real-time ultrasound examinations. The 2021 K-TIRADS clarified the US criteria for nodule classification and revised the size thresholds for nodule biopsy, thereby reducing unnecessary biopsies for benign nodules while maintaining the appropriate sensitivity to detect malignant tumors in patients without feature of high risk thyroid cancer. Thyroid radiology practice has an important clinical role in the diagnosis and non-surgical treatment of patients with thyroid nodules, and should be performed according to standard practice guidelines for proper and effective clinical care.

  • Most Hepatic hemangiomas are asymptomatic and small in size, making them difficult to find by pathological examination. Therefore, radiological diagnosis is essential for the early finding and diagnosis of Hepatic hemangioma. Three-phase method using contrast medium in computed tomography, T1, T2-weighted imaging in magnetic resonance imaging, dynamic magnetic resonance imaging using contrast medium, echo planar imaging method, diffusion-weighted imaging method, blood pool scan using 99mTc-labeled red blood cells in nuclear medicine, we looked at the color doppler method In ultrasound, and it is important to accurately understand the imaging findings of hepatic hemangioma and perform the examination in order to make an accurate diagnosis. most hepatic hemangioma are benign tumors, care should be taken not to confuse them with malignant tumors such as hepatocellular carcinoma to prevent unnecessary procedures. Therefore, in order to make an accurate diagnosis, it is important to accurately understand the imaging findings of hemangioma and perform the examination.

  • A method of reflection-transmission type ultrasonic inverse scattering image was presented using linearly arrayed transducers in inner surface of half-cylinder. In this method, to reduce the number of data, the mirror effect using a reflector behind object and pulse wave with finite frequency band, To verify the proposed method, a computer simulation was performed for organic phantom specimen, As the results. it was verified that the reconstructed image was satisfactory even when the limitation view angle was limited to around 30 deg.

Effect of Plank Exercise on the Abdominal Muscle Thickness of Subjects with Asymmetric Pelvic Anterior Tilt (플랭크 운동이 비대칭 골반 앞기울임을 가진 대상자의 복부근육 두께 변화에 미치는 영향)

  • Purpose : This study investigated the effects of different types of plank exercise on abdominal muscle thickness in subjects with asymmetric pelvic anterior tilt. Methods : Participants with a diagnosis of pelvic anterior tilt were divided into an experimental group (n=12) and a control group (n=13). The thicknesses of the transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) muscles were measured using an ultrasound diagnostic apparatus. The data were analyzed using an SPSS (Ver. 21) program. Results : In the experimental group, there was a significant difference in the thicknesses of the left and right muscles in the standing and elbow plank exercises (p<.05). All the muscle thicknesses increased significantly during the plank exercises(elbow plank, right-side plank, and left-side plank) as compared with those in the standing position (p<.05). In addition, the side-plank exercise resulted in significantly higher muscle thickness of the supported side (p<.01, p<.001). Conclusion : These findings suggest that asymmetric exercise should be applied to the change of the left and right muscles of the abdominal when applying plank exercise according to pelvic anterior tilt.

  • Purpose: The purpose of this study was to investigate about missed abortion pathophysiology, diagnosis, medical treatment and to research the trend of the study related to missed abortion. Methods: We referred a PubMed site by using search word of "missed abortion"(Limits: 3 Year, only items with abstracts, Human). Results: 37 journals with 49 papers were searched. Conclusion: 1. The study of missed abortion pathophysiology was the following. The first was that important pathologies such as molar pregnancy and placental trophoblastic disease can be diagnosed by routine histopathologic analysis of product of conception following first-trimester spontaneous miscarriages. The second was that coelomic fluid leptin concentration in missed abortion is higher than in normal. The third was that adenosine deaminae activity in serum and placenta of patients with anembryonic pregnancies and missed abortions was low. The forth was that Leptotrichia amnionii sp. nov. was the etiopathogenetic factor in missed abortion. 2. Transvaginal ultrasound assessment of irregular vaginal bleeding is effective in diagonosis of missed abortion. 3. There were medical therapy with misoprostol, mifepristone or anti progesterone for missed abortion. Misoprostol was administrated oral(sublingual) and vaginal.

A Case of Pulmonary Sequestration Diagnosed by Doppler Sonography (도플러초음파로 진단한 폐분리증 1례)

  • Pulmonary Sequestration is a rare congenital malformation during embryouic development which results in a cystic mass of nonfunctioning lung tissue. A diagnosis of a pulmonary sequestration has traditionally relied upon identifying the pathological arterial vessels by arteriography, computed tomography, and magnetic resonance imaging. We reported a case of pulmonary sequestration diagnosed by Doppler sonography and subsequent CT angiography. A 21-year-old-woman admitted to hospital for an investigation of recurrent pneumonia with left lower chest pain. A doppler ultrasound sonography showed an aberrant blood supply from the descending thoracic aorta to the left lower chest lesion. The pulmonary sequestration was confirmed by the subsequent CT angiograph.

Neck metastasis of invasive ductal carcinoma of breast causing voice change: a case report (음성변화를 주증상으로 내원한 유방암의 경부연조직 전이환자 1례)

  • We present a metastatic carcinoma from the breast to the neck soft tissue around common carotid artery, with a rare finding of voice change. A 60 year-old female patient presented with voice change for 7 months. Neck ultrasound revealed a soft tissue mass between internal jugular vein and common carotid artery. Result of fine needle aspiration biopsy was a metastatic carcinoma. Computed tomography and magnetic resonance image revealed $2.5{\times}3.0cm$ sized irregular marginated soft tissue mass in right lower neck encasing common carotid artery and internal jugular vein. Surgical resection was performed and pathologic result with immunohistochemical analysis confirmed the diagnosis of a metastatic invasive ductal carcinoma originated from breast.
