Urethroperineal Fistula Associated with a Severe Ulcerative Dermatosis in a Puppy (자견에서 발생한 중증의 궤양성 피부증과 관련된 요도회음누관증)

  • 40-day-old mixed male puppy was presented with a 10-day history of tenesmus and extensive ulcerative lesion of perineum. At physical examination, the dog was voiding mainly from an orifice in the perineal fistula. On fistulogram, a urethroperineal fistula was diagnosed. The dog was treated by wet dressing for wound management and antibiotics for 4weeks. Subsequently, the patient improved and no complication or recurrence was presented.

  • Painful, ulcerative lesions of various systemic disease can affect the oral mucosa membrane at first. If you don't consider the skin lesion, followed or accompanied by oral mucosa, you are likely to fail in differential diagnosis. In this cases, we introduced erythema multiforme and Stevens-Johnson syndrome(SJS) patients with painful, ulcerative lesions on oral mucosa and skin. Also we review oral mucosal diseases come with the skin lesions.

  • Ulcerative colitis (UC) is an inflammatory bowel disease, which is a chronic gastrointestinal disorder. Adenophorae Radix (AR) has been used as a traditional medicine for various diseases including strengthening cardiac function, allaying a fever, and easing pain and cough. However, the regulatory effects of AR in intestinal inflammation are not yet understood. This study attempted to determine the effect of AR in dextran sulfate sodium (DSS) - induced colitis in mice. The colitis mice were induced by drinking water containing 5% DSS for 7 days. The results showed that mice treated with DSS showed remarkable clinical signs, including weight loss, and reduced colon length. Administration of AR attenuated weight loss, colon shortening and inhibited the levels of interleukin (IL) - 6 in DSS - treated colon tissues. These results provide experimental evidence that AR might be a useful therapeutic medicine for patients with UC.

  • Schizandra chinensis fruits (SC) have been used as a traditional Oriental medicine for treatments of many stress-induced diseases. In the present study, we investigated the protective effect of SC aqueous extract(SC-Ex) in the inflammatory diseases of intestine using a mouse model of ulcerative colitis. An experimental colitis was induced by daily treatment with 5% dextran sulfate sodium (DSS). SC-Ex was orally administered from day 2 of DSS treatment in a dose-dependent manner. Administration of SC-Ex reduced significantly clinic signs of DSS-induced colitis, including body weight loss, shorten colon length, increased disease activity index, and histological colon injury. Moreover, SC-Ex suppressed significantly not only the activities of myeloperoxidase (MPO) and chymase, but also the expressions of $TNF-{\acute{a}}$ and COX-2 in DSS-treated colon tissues. Inhibitory effect of SC-Ex was effective at a dose over 20 mg/kg. Our results indicate that SC-Ex may possess therapeutic effect on the development of DSS-induced colitis.

Studies on the effect of Kamikuibitang on the Gastric Ulcer in Rats (가미귀비탕(加味歸脾湯)이 흰쥐의 위궤양(胃潰瘍)에 미치는 영향(影響))

  • This study was aimed to evaluate the anti-pain effect of Kamikuibitang in acetic acid method and the anti-ulceration effect of Kamikuibitang in indomethacin, aspirin and immobilization stress method in rats. The results were follows; 1. The anti-pain effects of Kuibitang and Kamikuibitang were decreased compared with those of control group. 2. In indomethacin and aspirin method, the anti-ulcerative effects of experimental groups were shown compared with those of control group. 3. In immobilization stress method, the anti-ulcerative effect of experimental groups was significantly shown compared with that of control group. 4. The serum gastrin levels of Kuibitang groups showed very significant decrease in indomethacin-induced and immobilization stress-induced ulcers. The serum gastrin levels of Kamikuibitang groups showed very significant decrease in indomethacin-induced, aspirin- induced and immobilization stress-induced ulcers. 5. The serum $V_{B12}$ levels of Kuibitang groups showed very significant increase in both indomethacin-induced and immobilization stress-induced ulcers. The serum $V_{B12}$ levels of Kamikuibitang groups showed significant increase in aspirin-induced and immobilization stress-induced ulcers whereas very significant increase in indomethacin-induced ulcer. According to the above results, it was concluded that Kamikuibitang had very significant anti-ulceration effect as well as anti-pain effect on gastric ulcer in rats.

Prevalence and pathologic study of porcine salmonellosis in Jeju (제주도 내 돼지 살모넬라증의 발생 양상 분석 및 병리학적 연구)

  • Salmonella (S.) Typhimurium is highly contagious, and its infection may rapidly spread within pig populations of herd. According to the survey (1,191 pigs) from 2003 to 2012, 155 pigs (13.0%) were diagnosed as salmonellosis in Jeju. Major porcine salmonellosis cases (88.4%) were concentrated in 4- to 12-week-old weaned pigs, but 6 pigs (3.9%) under 4 weeks old were also diagnosed. Based on the histopathologic examinations, ulcerative enteritis (63.9%) in the large intestine and/or paratyphoid nodules formation (57.4%) in the liver were most prevalent lesions in porcine salmonellosis. Single infection of S. Typhimurium and mixed infection with more than 2 pathogens were detected in 38 (24.5%) and 117 (75.5%) in pigs, respectively. Co-infections of Porcine reproductive and respiratory syndrome virus and Porcine circovirus type 2 were very common in porcine salmonellosis in Jeju and detected in 84 (54.2%) and 59 (38.1%) pigs, respectively. Based on the serotyping tests using 41 bacterial isolates, S. Typhimurium and S. Rissen were confirmed in 39 (95.1%) and 2 (4.9%) cases, respectively.

Perforator Based Tibialis Anterior Segmental Muscle Island Flap in Lower Extremity Reconstruction

  • Reconstruction of the lower extremities is difficult due to a lack of skin laxity and muscular tissues. Here, we present a case of lower extremity reconstruction via the anterior tibial artery perforator based segmental muscle island flap. Our patient was a 75-year-old male with a chronic ulcerative wound on the right lower leg from an old car accident. A $5.0{\times}0.5cm$ size ulcerative wound with tibial bone exposure was noted. We planned to reconstruct the lower extremity defect with a free flap, but the vessel status was severely compromised intraoperatively. Thus, we found the anterior tibial artery perforator using Doppler ultrasound, elevated the tibialis anterior muscle segment flap, and transposed it to cover the defect successfully. The flap presented with a nice contour and the skin graft covering the flap survived completely. There were no complications of the surgical site at three months follow-up and no gait morbidity. This is a meaningful case applying the concept of segmental muscle flap based on a perforator that had advantages including proper bulkiness, vascularization, and preservation of function, which were well applied, leading to great success.

  • Erythema multiforme(EM), a blistering and ulcerative inflammatory disorder, affect skins or mucosa, and is thought to be triggered mainly by preceding infection, such as Herpes simplex virus, or exposure to drugs and medication, particular antibiotics or analgesics. Symptoms include typical cutaneous target lesions on skins and in case of oral manifestation, erosive and ulcerative lesions on lips, buccal mucosa, and tongue are known to occur, which needs differential diagnosis with other intraoral lesions. In this case, EM assumed that it is occurred by giving Trichomonas infection or Metronidazole in oral region is introduced with a review of diagnosis and treatment of EM.

The Improving Effect of Gastrodia elata Blume on DSS-induced Colitis in Mice

  • Ulcerative colitis (UC) is a chronic inflammatory bowel disease characterized by abdominal pain, rectal bleeding and diarrhea. Gastrodia elata Blume (GE) has been used for the treatment of various diseases including neurodegenerative diseases and inflammatory disease. However, there has been no information on whether GE regulates intestinal inflammation. The aim of this study is to elucidate whether GE can protect against dextran sulfate sodium (DSS)-induced colitis in a mouse model. The colitis mice were induced by drinking water containing 5% DSS for 7 days. Body weight, colon length and clinical score were assessed to determine the effects on colitis. The levels of inflammatory cytokines, tumor necrosis factor $(TNF)-{\alpha}$ and interleukin (IL)-6 in colitis tissue were also measured. The results showed that mice administrated with DSS showed clinical signs including weight loss and reduced colon length. GE inhibited the DSS-induced loss of body weight and shortening of colon and increased Disease activity index score. Additionally, we observed that GE suppressed the levels of $TNF-{\alpha}$ and IL-6 in DSS-treated colon tissues. Collectively, these findings provide experimental evidence that GE might be a useful therapeutic agent for patients with UC.

  • Hyangsayangwee-Tang, a combined preparation of crude drugs, which has been used for stomach and intestinal disorder, was examined for anti-spasmodic, anti-ulcerative and anti-emetive effects. Spontaneous motility of isolated ileum was strongly suppressed and inhibitory effects against contraction of isolated ileum induced by acetylcholine and barium chloride were shown in mice. And, contraction of isolated guinea-pig ileum by histamine was inhibited. In rats fundus preparations, Hyangsayangwee-Tang elicited strong relaxation and had antagonist effects against the spasm induced by acetylcholine and barium chloride. A significant inhibitory effect on the intestinal propulsion of barium sulfate in mice was shown. In pylorus-ligated rats, Hyangsangwee-Tang inhibited gastric secretion and showed a strong anti-peptic activity. Protective effects against gastric ulceration induced by pyloric ligation, water-immersion stress, histamine and aspirin were significantly recognized in mice and rats. Hyangsayangwee-Tang decreased cupric sulfate-induced vomitting in frogs.
