Oral ulcerative and vesicular diseases (구강궤양 및 수포성 질환)

  • Oral ulcerative and/or vesicular diseases have similar characteristics of clinical phenotypes. Detailed and careful history taking is the cornerstone of the diagnosis of oral mucosal disease. Moreover, complete screening of review of system for patient is important. Through this article, the simple ulcerative, recurrent ulcerative, acute multiple ulcer, chronic multiple ulcerative diseases will be discussed. Clinicians have to keep in mind its differential diagnosis and management.

Study of The Correlation of Lung-Large intestine-Skin by Ulcerative Colitis-Induced 3 Weeks Old Mice (3주된 생쥐에서 궤양성 대장염 유발을 통한 폐-대장-피부의 상관관계 연구)

  • Objectives The purpose of this study is to understand the correlations between lung, large intestine, and skin of 3-week-old mice in which ulcerative colitis was induced, up on administration of Coptidis rhizome and Glycyrrhiza uralensis mixed extract. Methods Mice were divided into 4 groups as follows; no treatment group (Ctrl group), ulcerative colitis-induced mice group (UE group), ulcerative colitis-induced mice group after administering Pentasa (PT group), ulcerative colitis-induced mice group after administering Coptidis rhizoma and Glycyrrhiza uralensis mixed extract (CGT group). Mice were induced ulcerative colitis by Dextran sulfate sodium (DSS). After 5 days of administration, We obvserved anti-inflammatory effect, alveolar formation, and skin barrier control in the colon mucosa. Results The CGT group was observed arrangement of normal intestinal cells, Infiltration of less inflammatory cells. The CGT significantly decreased positive rseponse of $TNF-{\alpha}$, p-IkB, Caspase 3 in large intestine, and significantly increased positive rseponse of EGF, IGF, catalase, Filaggrin, involucrin, loricrin. Conclusions The results of this study show the correlation of Lung-Large intestine-Skin by administering Coptidis rhizoma and Glycyrrhiza uralensis mixed extract to ulcerative colitis-induced mice.

Associations between an MDM2 gene polymorphism and ulcerative colitis by ARMS-PCR

  • Ulcerative colitis is a form of inflammatory bowel disease characterized by chronic inflammation of the colon and rectum. The abnormal lesions in the digestive system caused by ulcerative colitis and intermittent colitis are of major clinical importance. MDM2 is a phospho-protein that functions as a ubiquitin ligase for p53. Recently, a T>G substitution in the promoter of the MDM2 gene (rs309) has been identified. In this case-control study, 174 ulcerative colitis biopsy samples and 82 control samples were collected from colonoscopy centers, hospitals, and clinics in Mazandaran and Gilan Provinces in Iran from October 2014 to May 2015. This MDM2 polymorphism was investigated in DNA samples (extracted from biopsy samples) by amplification-refractory mutation system polymerase chain reaction. The mean age of patients with ulcerative colitis was 46.5 years (range, 28 to 69 years) and that of control individuals was 45.3 years (range, 26 to 71 years). Seventy-eight patients (44.82%) were men and 96 (55.18%) were women. The distribution of the TT, TG, and GG genotypes was 17.93%, 27.59%, and 34.48%, respectively, in the ulcerative colitis patients and 31.70%, 24.40%, and 43.90%, respectively, in the control individuals (odds ratio of GG for ulcerative colitis, 7.142; 95% confidence interval, 2.400 to 9.542; p = 0.001). It was found that a single-nucleotide polymorphism at rs309 in the MDM2 gene was associated with ulcerative colitis. A direct relationship was found between age and ulcerative colitis, while no relationship was found with sex. This finding is of note because the occurrence of intestinal inflammation and subsequent ulcers can precede the development of cancer.

  • Extracolonic manifestations which occur in approximately 10~20% of patients with ulcerative colitis most commonly affect joints, skin, liver and eyes. In contrast, pulmonary involvement in ulcerative colitis is very rare. However, a variety of respiratory disorders has been associated with ulcerative colitis, including pulmonary vasculitis, bronchiectasis, chronic bronchitis, interstitial fibrosis, pleural effusion. Since the first observation of pulmonary involvement in ulcerative colitis by Kraft in 1976, a few cases have been reported, and probably no such case have been reported in Korea yet. Here we report an experience concerning 56 year-old man interstitial lung disease in ulcerative colitis, who was diagnosed by clinical, radiographic, endoscopic, histologic findings.

Prevention effect of Sunkiwhajung-tang, a prescription, on the gastric ulcer induced by indomethacin in rats (순기화중탕이 Indomethacin으로 유발된 위궤양에 미치는 영향)

  • In order to evaluate the prevention effect of Sunkiwhajung-tang (SWT) which has been used as a traditional prescription for the treatment of digestive disease in Korea on the gastric ulcer induced by indomethacin in rats, the changes of number and size of ulcerative lesions, parietal, chief, Grimelius and Serotonin-positive cells in the peri-ulcerative tissues were detected with histological examinations of ulcerative and peri-ulcerative lesions after oral injections of SWT extracts (125, 250 and 500 mg/kg, respectively). SWT prevented to a great extent the expected indomethacin-induced elevation in hemorrhagic ulcerative lesions, the number and size of ulcerative lesions, and the number of parietal cell, chief cell, Grimelius-positive cells and Serotonin-positive cells in the peri-ulcerative lesions in a dose dependent manner. These results provide a story evidence that SWT produced an protective effect on gastric ulcer induced by indomethacin. Determination of the specific mechanisms involved in the protective effect of SWT on the gastric ulcer will require additional study.

The anti-oxidative and anti-inflammatory effect of Psoralea corylifolia on Ulcerative Colitis Induced by Dextran Sulfate Sodium in Mice

  • Objectives: This study was to investigate the anti-oxidative and anti-inflammatory effect of Psoralea corylifolia water extract (PE) on ulcerative colitis which was induced by dextran sulfate sodium (DSS) in mice. Methods: Ulcerative colitis was induced by DSS in male BALB/c mice. The mice were divided into 3 groups. The control group (Ctrl) was not induced ulcerative colitis. The pathological group (CE) was induced the colitis. The experimental group (PT) was administered PE after inducing the colitis. The effects of the PE on ulcerative colitis were evaluated by morphological change in the colon tissue and cells, substance P production, activity of tumor necrosis factor $(TNF)-{\alpha}$ and nuclear factor $(NF)-{\kappa}B$, cyclooxygenase (COX)-2 production, and anti-oxidative activity. Results: In the PT group, PE alleviated hemorrhagic erosion in colon mucosa and infiltration of inflammatory cells in lamina propria mucosae. In the colon of the PT group, COX-2 production was inhibited via regulating the activity of $TNF-{\alpha}$ and $NF-{\kappa}B$ p65. PE also had an anti-oxidative effect via activating nuclear factor (erythroid-derived 2)-like 2 (Nrf2). Conclusions: In this study, we found the utility of treatment with PE and the potential of developing a medicine for ulcerative colitis by applying our results. Further investigations for the anti-inflammatory mechanism of PE may be needed.

Mitigating Effect of Jageum-jung on Dextran Sulfate Sodium-induced Ulcerative Colitis through Anti-inflammatory Regulation (항염증조절을 통한 자금정의 DSS 유도 궤양성 대장염 완화 효과)

  • Objectives: This study investigated the anti-inflammatory effects of Jageum-jung extract on Dextran sulfate sodium (DSS-induced) ulcerative colitis in mice. Methods: Ulcerative colitis was induced by DSS in Balb/C male mice. Ten mice were assigned to each of four groups: Ctrl (control), UE (ulcerative colitis-induced), PT (treated with pentasaccharide after induction of ulcerative colitis), and JT (treated with Jageum-jung extract after induction of ulcerative colitis). The effects of Jageum-jung extract were measured by restoration of the length of the intestine, degree of mucosal damage as seen with histochemistry, and changes of p-IkB, iNOS, COX-2, and caspase-3 determined by immunohistochemistry. Results: The recovered intestinal length of the JT group was longer than that of the UE group. In the colon mucosa of JT group, hemorrhagic lesions were reduced, and the mucus barrier was recovered. This group also showed inhibited production of inflammatory enzymes (iNOS, COX-2) through regulation of proinflammatory enzyme (NF-kB, p65) activity in the colon. In addition, caspase 3 activation induced apoptosis. By GC/MS analysis, azetidine was identified. Conclusions: This study confirmed the anti-inflammatory effects of jageum-jung extract, and suggests the possibility of using Jageum-jung extract to treat ulcerative colitis. Further experiments and research on the mechanism of Jageum-jung effects are needed.

Constitution Therapy of Adolescent Ulcerative Colitis: A Case Report (청소년궤양성 대장염 환자의 사상체질요법에 대한 증례보고)

  • Objectives: This study discusses the effects of constitution therapy on adolescent ulcerative colitis. Methods: A 12-year-old male patient was treated with western medication for six months and herbal medicine for one year and six years; however, his abdominal pain, mucousy stool, bloody stool, and diarrhea persisted. He was diagnosed as having Taeumin according to Sasang constitution classification and treated with Sasang constitutional medicine (i.e., Yeuldahanso-tang and food restrictions based on constitutional medicine theory). Before the first treatment, his Pediatric Ulcerative Colitis Activity Index (PUCAI) score was 35, but this score decreased to 15 within six months and was maintained at 10 by 18 months of treatment. He took the herbal medication for 38 months, and his PUCAI score was 0 at the end of treatment. His liver functioned normally despite long-term drug use, and a follow-up colonoscopy showed no ulcerative colitis except melanosis, which was presumed to be caused by the herbal medicine. Ulcerative colitis in pediatric adolescents requires treatment with pharmacotherapy and dietary control based on constitutional medicine to maintain disease remission. Conclusions: Constitutional therapy is effective for treating adolescent ulcerative colitis. More clinical data are needed for patients with ulcerative colitis.

A Pathologic Study on the Gastric Ulcers of the Pigs (돼지의 위궤양에 관한 병리학적 관찰)

  • A total of 499 stomachs consisted of 393 slaughter and 106 breeding pigs from industrial farm was examined for ulcerative lesions grossly and microscopically. The relationship between the ulcerative lesions and stomach contents was also examined. 1. The majority of stomachs has one or more lesions of ulcerative process, including parakeratosis(64.1%), erosions(10.2%), acute ulcers(2.8%), chronic ulcers(1.4%), and scars(0.8%). 2. The incidence and the degree of the esophagogastric lesions in the breeding pigs were similar to those of the slaughter pigs. But ulcerative lesions of the fundic lesions in the breeding pigs were found to be more remarkable than those of the slaughter pigs. 3. The stomach contents of the pigs having gastric ulcers were mostly fluid in nature, which were dry in the normal stomach. 4. Histologically, muscular hypertrophy, connective tissue hyperplasia and vascularity were shown in the ulcerative lesions of the esophagogastric portion. Thrombosis, necrosis and granulation tissue formation were found frequently.

A Case Report of a Patient with Active Ulcerative Colitis Treated with Do-Che Decoction-based Korean Medicine (도체탕(導滯湯) 위주의 한방치료로 호전된 활동성 궤양성 대장염 환자 치험 1례)

  • Background : Sulfasalazine for treating ulcerative colitis has a clinical limitation due to its adverse effects. This case is about a patient with ulcerative colitis who could not tolerate sulfasalazine and was improved by Do-che decoction-based Korean medicine. Method and Results : Purulent and bloody diarrhea more than 20 times a day, mild fever, chilling and tenesmus were main symptoms of this patient who was diagnosed as ulcerative colitis 10 years ago. Do-che decoction-based Korean medicine which was recommended for purulent and bloody diarrhea in Dong-Ui-Bo-Gam, the classic literature of Traditional Korean Medicine was given to the patient for more than 1 month. After treatment, the frequency of purulent and bloody diarrhea, tenesmus and fever was decreased remarkably, and C-reactive protein, erythrocyte sedimentation rate, white blood cell count and maximum temperature also dropped to the normal range. Conclusion : Do-che decoction-based Korean medicine has a potential effect and may be a alternative therapeutic option in patients with ulcerative colitis who cannot tolerate sulfasalazine due to its adverse effects.
