  • The rapid aging is increasing as the shortage of medical facilities and the resulting of decline in the quality of public health. In order to ease the burden of rising medical expenses, advanced medical institutions are expanding their remote medical care to lower the cost of services. U-healthcare detects the changes in physical and chemical phenomena occurring in the human body and converts them into electrical signals that can be processed and feeds back to the results through analytical and visualization processes to select only the desired information from the measured signals. The service is provided through a process of providing an alarm to a user. However, traditional biometric methods of attaching sensors directly to the body can be annoying and rejected in daily life. Therefore, there is a need for a method of continuously measuring biometric information without causing inconvenience to daily life. In this paper, we propose an IR-UWB-based non-contact and non-responsive respiratory measurement system that can continuously monitor biological information without any inconveniences to daily life.

  • This study aims to derive implications on current problems in the nursing manpower management in Korea through the comparison with the nursing workforce and employmentrent in the United States and Japan. There are various issues in nursing policy, such as nursing shortage, quality of the nursing service, and the increased cost of management due to the high turnover rate of nursing staffs and it is urgent to seek various policy measures to resolve this. Although nursing shortage is a commom problem in the world, the U.S. and Japan were higher rate of employment than Korea in nursing staffs, which implicates the importance of the legislation of mandatory minimum staffing ratios, the establishment of policies such as the fees and charge policy and the nursing work environments. For quality nursing care and patient safety through the stable workforce of qualified nursing staffs, administrative mechanisms that support adequate nurse staffing and promote positive work conditions are needed, for which the improvement of legal system is required.

Viruses, Bacteria and Helminths of Invasive Carp: Insights from an In Vitro Assay and a Survey with Native Fishes in a Large Midwestern River

  • Pathogen introductions associated with aquatic invasive species threaten ecosystems and biodiversity worldwide. Bigheaded carps (BHC), including Silver Carp Hypophthalmichthys molitrix, Bighead Carp H. nobilis, and their hybrids, are prolific, invasive pests in central US rivers. However, little is known about pathogen effects on invading BHC or how BHC affect the disease risk profile for native fishes in receiving ecosystems. We therefore conducted, from May 2013-December 2014, a systematic pathogen survey for BHC and native fishes in the Wabash River watershed, Indiana, USA. We found Pseudomonas fluorescens, P. putida, and Salmonella enterica DNA in BHC as well as native fishes, although none of these bacteria were exclusively present in BHC. DNA from other bacterial taxa was detected only in native fishes and Common Carp Cyprinus carpio. No gastrointestinal helminths were detected in BHC, although they were common in most native fishes examined. We also conducted in vitro studies on BHC tissues (skin, gill, fin, and fry) and found high sensitivity to Largemouth Bass virus, viral hemorrhagic septicemia virus, and infectious pancreatic necrosis virus. We conclude that BHC are not heavily burdened by bacteria, viruses and parasites in the invaded study ecosystems, although they do harbor native bacteria and show potential for high sensitivity to endemic viruses.

  • Wireless Sensor Networks is applied to improvement of life convenience or service like U-City as well as environment pollution, tunnel and structural health monitoring, storm, and earthquake diagnostic system. To increase the usability of sensor data and applicability, mobile devices and their facilities allow the applications of sensor networks to give mobile users and actuators the results of event detection at anytime and anywhere. In this paper, we present MUSNEMO(Multi-sensor centric Ubiquitous Smart sensor NEtwork using Mobile devices) developed system for providing more efficient and valuable information services with a variety of mobile devices and network camera integrated to WSN. Our system is performed based on IEEE 802.15.4 protocol stack. To validate system usability, we built sensor network environments where were equipped with five application sensors such magnetic, photodiode, microphone, motion and vibration. We also built and tested proposed MUSNEMO to provide a novel model for event detection systems with mobile framework.

A Comparative Study on Hospital Choice Factors between First-visit and Revisit patients in General Hospitals (초진과 재진환자의 종합병원 선택요인 비교분석)

  • The purposes of this study is to analysis the differences of the hospital choice factors between first-visit and revisit patients in general hospitals. The samples used for data analysis are 499 outpatients of G, H, S, U hospitals using a standardized questionnaires in 4 provinces. In case of first-visit patients, human, effectiveness and accessibility factors were found to have a significant positive(+) effect on hospital choice factors. In case of revisit patients, human, physical, accessibility, image of hospital and salary factors were found to have a significant positive(+) effect on hospital choice factors. Based on these results, this study suggests implications to improve customer satisfaction, profitability and provide a high-quality medical service by making effectively use of planning strategies of hospital management with a variety of factors.

Development of the Critical Pathway for Cesarean Section Patient (제왕절개술 환자를 위한 Critical Pathway 개발)

  • With the opening of healthcare market, the health care environment in Korea is anticipating a drastic change. In this Internationally open market environment, it is necessary to introduce a systematic health care plan and DRG system which offer qualitative medical services as well as reduced cost. Purpose of this study is to develop and test the critical pathway for Cesarean section patient in the way to be possible the integrated inpatient management. It was adopted the process of six phases to develop the critical pathway as the theoretical framework implemented by Johns Hopkins Hospital, Maryland, U.S.A. In the first phase, make a selection of diagnosis/procedures to develop. In the second phase, organize a development team consisted of eight expertises working in maternity nursing area. In the third phase, analyze the overall medical service offered to patient through review medical records and decided the service content and the implementation period for the Cesarean section patient. In the forth phase, make out a preliminary critical pathway after verification of expert group on content validity. In the fifth phase, validity operate to ten Cesarean section patients to test implementation in practice by using the preliminary critical pathway, In the sixth phase, defined the final critical pathway. The result of this study was as follows. 1. There were classified 8 categories as monitoring/assesment, treatment, medication, activity, diet, test, consult, education/discharge plan for vertical axis and showed hospital stayed from admission to discharge for horizontal axis of critical pathway through analysis 68 Cesarean section patients medical records. 2. After critical review 68 medical records to make out a preliminary critical pathway, hospital stays for horizontal axis were showed 6 days, mean hospital stays were 7.5 days, 2.1 days were to be taken operation after admission and 4.2 days were stayed until discharge after operation. 3. After making out a questionare in 90 items of a medical service content of eight categories and verifying the content validity of expertises, the 85 items of the preliminary critical pathway were selected by expertises agreement over 88% and modified or deleted 5 items showing agreement below 75%. 4. After verifying a validity to 10 patients for 4 weeks, hospital stays were 5.9 days. There were deleted 1 item and modified or supplemented the 9 items of the 10 items.

Discussion on Expansion and Establishment of Home Nursing Care (가정ㆍ방문간호사업의 확대 및 정착을 위한 시론(時論))

  • The purpose of study is to develop a home nursing care project model suitable for Korea's heath care system and policy directions for expansion and establishment of home nursing care. Method: The first, status of home nursing care program in medical institution and public health center was evaluated respectively in view of structure, process and performance. And then, issues of the program were analyzed in view of accessibility, safety, and sufficient supply. The second, demand projection of home nursing care according to income level and technical level of service that is needed to the subject was tried. In addition, the level of supply for home nursing care at present was estimated. The third, home nursing care program in advanced countries (Japan and U.S.A.) was examined. Result and Conclusion: Community-based home nursing care program is developed into public-operation model and private-operation model from above the sub-subject result. Functional network for referral system among related institutions is built up to meet various needs, regardless of accessibility to distance and economy. And prior settlements and policy directions for expansion and establishment of home nursing care are suggested.

  • Please Interests in the medical service are increasing in internet environment as life quality of the people improves because of development in information and medical technology. The medical information in today's modern internet environment can violate privacy of the patients. Many medical institutions in Korea are very passive in the privacy protection of patients in the internet environment. The law, standard scheme and systematic guidance to prevent drain of medical information are not developed. This study examines cases of infringement pattern on information of each patient in the internet environment. This study will also try to find a solution to protect the personal information of patients in the internet environment in the measures of law system, technique and management.

  • 사람들의 건강에 대한 관심이 증가하면서 이러한 사람들의 요구사항을 만족시킬 수 있는 다양한 서비스가 개발되어지고 있다. 헬스-관광 서비스는 관광 서비스와 헬스 서비스를 접목하여 관광객의 건강관리를 도와주는 서비스이다. 헬스-관광 서비스는 PDA의 지리정보 및 관광정보를 활용하여 방문할 관광지역과 이동방법을 선정한다. 시스템은 사용자의 기본 정보와 이동반경을 계산하여 사용자의 예상 운동량을 산출하여 보여주고, 사용자는 해당 정보를 바탕으로 전체 관광코스를 선정한다. 실 운동량 측정은 관광객의 이동거리, 이동속도, 고도, 맥박, 체온 등의 정보를 이용하여 측정하고, 측정된 운동량을 사용자에게 알려준다. 또한 실 운동량이 예상 운동량과 비교하여 과도하거나 부족한 경우 관광코스의 수정을 유도하여 적정량의 운동을 할 수 있도록 코치할 수 있다. 본 논문에서는 헬스-관광 서비스 시스템의 서비스 방법을 제안하고, 헬스-관광 서비스 시스템을 설계하였다.

Implementation of Dynamic Situation Authentication System for Accessing Medical Information (의료정보 접근을 위한 동적상황인증시스템의 구현)

  • With the development of IT technology recently, medical information systems are being constructed in an integrated u-health environment through cloud services, IoT technologies, and mobile applications. These kinds of medical information systems should provide the medical staff with authorities to access patients' medical information for emergency status treatments or therapeutic purposes. Therefore, in the medical information systems, the reliable and prompt authentication processes are necessary to access the biometric information and the medical information of the patients in charge of the medical staff. However, medical information systems are accessing with simple and static user authentication mechanism using only medical ID / PWD in the present system environment. For this reason, in this paper, we suggest a dynamic situation authentication mechanism that provides transparency of medical information access including various authentication factors considering patient's emergency status condition and dynamic situation authentication system supporting it. Our dynamic Situation Authentication is a combination of user authentication and mobile device authentication, which includes various authentication factor attributes such as emergency status, role of medical staff, their working hours, and their working positions and so forth. We designed and implemented a dynamic situation authentication system including emergency status decision, dynamic situation authentication, and authentication support DB construction. Finally, in order to verify the serviceability of the suggested dynamic situation authentication system, the medical staffs download the mobile application from the medical information server to the medical staff's own mobile device together with the dynamic situation authentication process and the permission to access medical information to the patient and showed access to medical information.
