Relationship between Income and Healthcare Utilization in Cancer Patients (암환자의 소득수준과 의료이용의 관련성)

  • Purpose: The purpose of this study is to examine the relationship between the income level and the healthcare utilization by health insurance type in all cancer patients in year 2005. Methods: The target population was cancer patients with health insurance who used healthcare as a diagnosis code (C00-C97) from January 1 to December 31 of 2005. The Korea Central Cancer Registry Center's Cancer Patient Registry Data, the list of cancer patients of the National Health Insurance Corporation, and the claim data of the Health Insurance Review & Assessment Service were used. The I was the wealthiest, followed by II, III, IV. The V was the poorest in this study. For the analysis, the $x^2$-test, ANOVA (and Kruskal-Wallis test), and regression were used. Results: Outpatient and hospitalization medical expenses, and outpatient visit days of cancer patients with self-employed health insurance were highest in I (p<.001, respectively), and the hospitalization days were the highest in II (p<.001, respectively). Outpatient and hospitalization medical expenses, and outpatient visit and hospitalization days of cancer patients with occupational health insurance were the highest in I (p<.001, respectively). Outpatient and hospitalization medical expenses, and outpatient visit and hospitalization days in cancer patients were higher in I compared to V, and higher in II and III, IV compared to V (p<.001, respectively). Conclusion: Supporting plan for cancer patients' outpatient healthcare utilization are necessary. Moreover, we should make specialized strategy for low income cancer patients with self-employed health insurance when we develop quality improvement policy for inpatient service.

Association between Inhaled Corticosteroid Use and SARS-CoV-2 Infection: A Nationwide Population-Based Study in South Korea

  • Background: Although it is known that inhaled corticosteroid (ICS) use may increase the risk of respiratory infection, its influence on the risk of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection remains unknown. Thus, the aim of this study was to investigate the association between ICS use and the positivity of SARS-CoV-2 infection among patients with chronic respiratory diseases. Methods: Nationwide data of 44,968 individuals with chronic respiratory diseases tested for SARS-CoV-2 until May 15, 2021 were obtained from the Ministry of Health and Welfare and Health Insurance Review and Assessment Service in Korea. The positivity of SARS-CoV-2 infection was retrospectively analysed according to the prescription, type, and dose of ICS taken one year before SARS-CoV-2 test. Results: Among 44,968 individuals tested, 931 (2.1%) were positive for SARS-CoV-2. A total of 7,019 patients (15.6%) were prescribed ICS one year prior to being tested for SARS-CoV-2. Low, medium, and high doses of ICS were prescribed in 7.5%, 1.6%, and 6.5% of total cases, respectively. Among types of ICS, budesonide, fluticasone, beclomethasone, and ciclesonide were prescribed in 3.7%, 8.9%, 2.3%, and 0.6% of total cases, respectively. A multivariate analysis showed no significant increase in infection with ICS use (odds ratio, 0.84; 95% confidence interval, 0.66-1.03). Moreover, there were no associations between the positivity of infection and the dose or type of ICS prescribed. Conclusion: Prior ICS use did not increase the positivity for SARS-CoV-2 infection. Moreover, different doses or types of ICS did not affect this positivity.

A study on the socio-economic study on the characteristics and oral health of national basic livelihood security (우리나라 국민기초생활수급자의 사회경제적인 특성과 구강건강에 관한 연구)

  • Objectives : The purpose of the study is to provide basic data for oral health promotion in national basic livelihood security. Methods : The data were extracted from the 4th National Health and Nutrition Survey (2007-2009) in Korea. The data were analyzed using SPSS 18.0. to $x^2$-test and CSGLM. Results : 1. The subjects were not able to receive dental treatment because of the financial difficulty. 2. The infuencing factors of DMFT include gender (p<0.05), type of health insurance (p<0.05), membership for private health insurance (p<0.05), frequency of toothbrushing (p<0.05), use of oral hygiene products (p<0.01), smoking (p<0.05), drinking (p<0.05). The influencing factors of CPI include gender (p<0.01) and smoking (p<0.05). The influencing factors of fixed bridge include age (p<0.05), income (p<0.05) and use of accessory oral hygiene products (p<0.05). The influencing factors of fixed bridge include age (p<0.001), income (p<0.01), education (p<0.001), type of health insurance (p<0.001), membership for private health insurance (p<0.001), frequency of toothbrushing (p<0.01), use of oral hygiene products (p<0.01), and drinking (p<0.05). Conclusions : It is necessary to provide oral health care and health promotion to the recipients of National basic livelihood security. The government must give them better quality of dental health care in the near future.

The Effects of Private Health Insurance on Healthcare Utilization of People with Physical and Mental Illness: According to 2018 Korea Health Panel Survey Data (신체·정신복합질환자의 민간의료보험 가입 여부가 의료이용에 미치는 영향: 한국의료패널 데이터 2018년 자료 활용)

  • Background: Private health insurance supplements the coverage of national health insurance in Korea. In this situation, the subject of the study is to identify the healthcare utilization of people with physical and mental illnesses according to private health insurance. Methods: This study used data from the Korea Health Panel Survey 2018. The study population consisted of 813 individuals with physical and mental illnesses (PMI). Multiple logistic regression analysis and binominal logistic regression analysis were conducted about the utilization of emergency, inpatient, and outpatient medical services of people with PMI depending on enrollment in private health insurance (PHI). Results: The results of this study indicated that individuals with PHI utilized emergency and outpatient medical services less frequently compared to those without PHI. Conversely, having PHI was associated with a higher utilization of inpatient medical services compared to not having PHI. Binomial logistic regression analysis revealed that individuals with PMI who had PHI exhibited a higher frequency of visits to emergency and outpatient medical services compared to those without PHI. However, the significance of this trend was not observed in the case of emergency medical services. On the other hand, individuals with PMI who had PHI showed a lower frequency of visits to inpatient medical services compared to those without PHI. Conclusion: In conclusion, there was a significant relationship between having PHI and the utilization of medical services in people with PMI. There is a need for a follow-up study considering the type of mental illnesses, length of stay, and health outcome of people with PMI depending on having PHI.

The Utilization of Health Service by Psychiatric Outpatients (정신과 외래환자들의 의료이용에 관한 연구)

  • Objective of the study represents experiences of medical utilization of Psychiatric Outpatients. This research draws on information obtained from the 2008 National-wide sampling study of the Korean Healthcare Panel(KHP) conducted. The results of our study suggest the significance of variables such as the gender(p<.001) in the social demographical characteristics, the medical security type(p<.016), medical institution utilization(p<.012) treatment type(p<.004) in the utilization factors. In the case of medical utilization cost, the financial support factor(p<.018) showed a significant relationship. Depending on the particularities of gender, medical security type, financial support, medical institution utilization, treatment type were determined through a multiple logistics regression analysis. Gender showed that, among Psychiatric outpatient age of 30-59 level was 5.358 times and 60 years and older, their the second medical examination level was 4.490 times higher than Psychiatric outpatients under the age of 29. Health insurance type showed for the others medical allowance was 6.712 times higher than job health insurance and the other treatment was 0.395 times lower than drug treatment. Psychiatric outpatients utilization are rooted in the thoughts and factor levels of the Psychiatric patients and must be understood from the Psychiatric outpatients's perspective. The point is not only to gauge the patients research during Psychiatric medical utilization. The important of social community mental healthcare services levels goes beyond the goal of enhancing healthcare.

Factors Affecting Early Cancer Screening for Lung Cancer: Focusing on Lung Cancer Screening Subjects (폐암의 조기 암검진 여부에 미치는 요인: 폐암 검진 사업대상자를 중심으로)

  • Objectives: The purpose of this study is to examine the current status of cancer screening among subjects in the lung cancer screening cycle and to analyze the factors affecting the cancer screening of subjects in the lung cancer screening cycle. Methods: This study used the 'National Health and Nutrition Survey 7th Year (2017)' surveyed nationwide as the main data. The subjects are lung cancer screening projects, the dependent variable is early cancer screening, the independent variables are gender, age, marital status, household income level, education level, national health insurance type, private health insurance, The number of chronic diseases, general health examination, smoking status, drinking status, moderate intensity physical activity, stress perception rate, and weight control efforts were determined. Results: The results of this study showed that factors affecting early cancer screening of lung cancer screening subjects were gender, age, marital status, education level, national health insurance, smoking status, drinking status, moderate physical activity, and weight. Irrespective of the control effort, it was found that the private medical insurance, the number of chronic diseases, the medical examination, and the stress perception rate were affected. Conclusion: If the lung cancer screening subjects recognize the importance of early cancer screening themselves and create a social environment to increase their participation rate, lung cancer screening patients and their families will help them to live a healthy life.

A study on the characteristics of severe trauma patients by health insurance system (건강보험제도에 따른 중증외상 환자 특성 비교 연구)

  • This study was attempted to analyze the status of emergency room use of severe trauma patients using the health insurance system and to understand their characteristics. This study used data from the 'Community-based Severe Trauma Surveillance' investigated from January 1, 2018 to December 31, 2019. As a result, there were differences in the degree of disability after injury and whether treatment(surgery, trauma embolism, transfusion) was performed according to the type of medical insurance (p< .001), it was found that there was a statistically significant difference between the degree of disability before and after damage depending on the type of medical insurance (p< .001). Reviews of the health insurance system located for the well-being of the people should be continued from various angles, and specific improvement plans should be proposed.

Factors influencing the frequency of denture adjustments after delivery of complete denture: a retrospective study (총의치 장착 후 의치 조정 횟수에 영향을 미치는 요인에 관한 후향적 연구)

  • Purpose. The purpose of this study was to analyze the frequency and duration of adjustments after delivery of complete denture according to age, sex, arch with complete denture, insurance coverage of a denture, type of antagonist, the experience of wearing denture, the period of edentulism, and the type of denture base. Materials and methods. For 5 years, medical records of patients aged 65 or older who had treated full dentures were assessed for the frequency and duration of follow-up visits after complete denture delivery. Statistical analysis was performed at the 5% level of significance to analyze the correlation between the frequency and duration of follow-up according to sex, insurance coverage, arch with the complete denture, type of denture base, type of antagonist, experience of wearing a denture, age, and healing period. Results. 247 complete dentures were included in this study. The median frequency of follow-up visits was 3, and the median duration of follow-up was 36 days. Lower dentures had significantly higher frequency of follow-up visits than upper dentures (P = .036). According to the type of antagonist, dentures opposing a complete denture had a significantly higher frequency of follow-up visits than dentures opposing a removable partial denture (P = .016). There was no statistically significant difference in the frequency and duration of adjustments after delivery of complete denture by age, sex, insurance coverage, healing period, the experience of wearing a denture, and type of denture base. Conclusion. Within the limitations of the present study, lower complete dentures or dentures opposing a complete denture had an increased frequency of follow-up visits.

Estimation for the half triangle distribution based on Type-I hybrid censored samples

  • A hybrid censoring is a mixture of Type-I and Type-II censoring schemes. This paper deals with estimation based on Type-I hybrid censored samples from the half triangle distribution. We derive some estimators of the scale parameter of the half triangle distribution based on Type-I hybrid censored samples. We compare the proposed estimators in the sense of the mean squared error for various censored samples.

  • Health insurance is the main instrument to protect the people against sickness. To examine the task of in the future it would be necessary to extract and understand the components formed in its formation and development, benefit related and normative characteristics of the health insurance itself. The health insurance oriented itself to the universal coverage at a law level. Paradoxically this worked positively for the universal development of the health insurance. The task of the health insurance is on the one hand universal, positive and open. On the other hand it has to shape type of allowed method, art and content of the medical treatments into the regulation to ensure the equal benefit as well as the financial stability. That is, the health insurance should check the aberrant medical treatment, and at the same time should be compensated for the their necessity and effectiveness. However there are always some structural differences between both requirements. This article aims to restate and analyse the development of the health insurance, based on the characteristics formed hitherto show the way to reform the health insurance. The problem to enhance the coverage of health insurance, its institutional as well as financial crisis, its peculiar governance would be handled.
