Chemotherapy for Malignant Gliomas Based on Histoculture Drug Response Assay : A Pilot Study

  • Objective : The Histoculture Drug Response Assay (HDRA), which measures chemosensitivity using minced tumor tissue on drug-soaked gelfoam, has been expected to overcome the limitations of in vitro chemosensitivity test in part. We analyzed interim results of HDRA in malignant gliomas to see if the test can deserve further clinical trials. Methods : Thirty-three patients with malignant gliomas were operated and their tumor samples were examined for the chemosensitivity to 10 chosen drugs by HDRA. The most sensitive chemotherapy regimen among those pre-established was chosen based on the number of sensitive drugs or total inhibition rate (IR) of the regimen. The response was evaluated by 3 month magnetic resonance image. Results : Among 13 patients who underwent total resection of the tumor, 12 showed no evidence of disease and one patient revealed progression. The response rate in 20 patients with residual tumors was 55% (3 complete and 8 partial responses). HDRA sensitivity at the cut-off value of more than one sensitive drug in the applied regimen showed a sensitivity of 100%, specificity of 60% and predictability of 70%. Another cut-off value of >80% of total IR revealed a sensitivity of 100%, specificity of 69%, and predictability of 80%. For 12 newly diagnosed glioblastoma patients, median progression-free survival of the HDRA sensitive group was 21 months, while that of the non-sensitive group was 6 months ($p$=0.07). Conclusion : HDRA for malignant glioma was inferred as a feasible method to predict the chemotherapy response. We are encouraged to launch phase 2 clinical trial with chemosensitivity on HDRA.

Serum Levels of Interleukin-8 and Tumor Necrosis Factor-alpha in Coal Workers' Pneumoconiosis: One-year Follow-up Study

  • Objectives: Various cytokines induced by inhalation of coal dust may mediate inflammation and lead to tissue damage or fibrosis, such as coal workers' pneumoconiosis (CWP). Methods: To investigate the relevance of serum cytokines in CWP, the levels of serum interleukin-8 (IL-8) and tumor necrosis factor-alpha (TNF-${\alpha}$) as CWP biomarkers in 110 retired coal miners (22 controls and 88 CWP subjects) were related to cross sectional findings and 1-year progressive changes of the pneumoconiosis. Progressive changes of CWP were evaluated by paired comparison of chest radiographs. Analysis by a receiver operating characteristic curve assessed the biomarker potential of each cytokine. Results: The mean serum IL-8 level was significantly higher in CWP compared to controls and IL-8 levels correlated with the degree of CWP. The median serum TNF-${\alpha}$ level was significantly higher in subjects with progressive CWP compared to subjects without CWP progression. The area under the ROC curve for IL-8 (0.70) and TNF-${\alpha}$ (0.72) for CWP identification and progression, respectively, indicated the biomarker potential of the two cytokines. Serum cutoff values of IL-8 and TNF-${\alpha}$ were 11.63 pg/mL(sensitivity, 69%; specificity, 64%) and 4.52 pg/mL (sensitivity, 67%; specificity, 79%), respectively. Conclusion: The results suggest that high levels of serum IL-8 are associated with the presence of CWP and those of serum TNF-${\alpha}$ are associated with the progression of CWP.

종격동 종양의 전산화 단층촬영(CT)소견, 수술소견 및 병리조직학적 침윤도 사이의 상관성 분석 (Analysis of Relativity Between Invasiveness on Chest of Tomographic Finding and Histopathologic Invasiveness)

  • 종격동 종양은 그 발생 위치가 심장과 대혈관, 식도, 그리고 신경조직 등과 같이 인체에 중요한 조직과 인접해 있는 점과 수술적 치료가 비교적 용이하고 그 결과 또한 우수하여 흉부외과 영역에서 매우 흥미있는 질환으로 관심을 끌어 왔다. 저자는 울산의대 서울중앙병원 흉부외과에서 1990년 1월부터 1995년 6월까지 5년 6개월간 수술적 치료를 받은 원발성 종격동 종양 87명을 대상으로 연령과 성별, 진단방법, 임상적 징후 및 증상, 크기, 해부학적 위 치, 수술소견, 병리조직학적 진단에 대하여 관찰하고 전산화 단층촬영(U)소견과 병리조직학적으로 침윤성 을 보였던 종격동 종양에서의 임상적 의의를 분석하여 종격동 종양에 대한 적절한 외과적 치료지침을 얻고자 하였다. 종양의 발생 부위는 전상부 종격동이 50명(57%)으로 가장 많았으며 후부 종격동 30명(35%), 중부 종격동 7 명(8%)순이었다. 병리조직학적 분류로 보면 흉부종 27명(31%), 원발성 낭종 19명(22%), 신경성 종양 19명 (22%), 기형종 10명(10%)순이었다. 병리조직학적으로 침윤성을 보인 경우는 17명(20%)이었으며 전상부 종격 동에 14명(16%), 후부 종격동에 3명(4%)이었다. 종격동 종양 \ulcorner대한 전산화 단층촬영은 87명 모두 시행하였으며, 주위 조직에 침윤성을 보인 경우는 15명(17%)이었다. 종양의 외과적 치료로 완전 절제 80명(92%), 부분 절제 6명(7%), 개흉후 조직검사 1명(2%)을 시행하였다 수술 소견상 14명(16%)에서 주위 조직의 침윤이 의심 되었다. 종양의 크기는 전체 평균 6.0$\pm$3.2 cm 전상부 종격동 종양 6.2$\pm$3.1 cm 중부 종격동 종양 3.9$\pm$ 1.1 cm 후부 종격동 종양 5.8그2.6 cm등이었다. 양성 종양의 크기는 평균 5.5$\pm$2.6 cm 악성종양의 크기는 7.3$\pm$ 4.6 cm이었다. 수술소견상 침윤이 의심된 경우들의 평균 크기는 7.2$\pm$3.8 cm이었고, 소견상 침윤이 의심 된 경우들의 평균 크기는 8.0$\pm$3.8 cm이었다. 병리조직학적으로 침윤성을 보였덜 17명과 CT소견상 침윤성을 보였던 15명과의 상관성은 민감도 (sensitivity) 35%, 특이도(specificity) 87%, 예측도(predictability) 35%이며, 수술소견상 침윤성을 보였던 14명과 의 상관성은 민감도 53%,특이도 93%, 예측도 64%이었다. 또한 종양의 크기와 악성도와의 상관관계에서 크기가 클수록 악성일 가능성이 증가하는 통계적으로 의의 있는 유의성이 있었다. 결론적으로 종격동 종양에 대한 수 珦\ulcorner진단으로 또는 수술시 예측할 수 있는 침윤성은 낭성 종양을 제외 한 경우 병리조직학적 진단과의 일치성이 낮아 종격동 종양의 수술적 치료는 종양을 포함하여 가능한 범위까지 광범위하게 절제하는 것이 수술 결과를 좋게 하는 방법이라고 사료된다.

구강 악안면 영역의 암종 진단에 있어서 $[^{18}F]$-Fluorodeoxyglucose를 이용한 양전자방출 단층촬영의 임상적 연구 (CLINICAL STUDY OF POSITRON EMISSION TOMOGRAPHY WITH $[^{18}F]$-FLUORODEOXYGLUCOSE IN MAXILLOFACIAL TUMOR DIAGNOSIS)

  • Positron Emission Tomography(PET) is a new diagnostic method that can create functional images of the distribution of positron emitting radionuclides, which when administered intravenously in the body, makes possible anatomical and functional analysis by quantity of biochemical and physiological process. After genetic and biochemical changes in initial stage, malignant tumor undergoes functional changes before undergoing anatomical changes. So, early diagnosis of malignant tumors by functional analysis with PET can be achieved, replacing traditional anatomical analysis, such as computed tomography(CT) and magnetic resonance image(MRI), etc. Similarly, PET can identify malignant tumor without confusion with scar and fibrosis in follow up check. In the Korea Cancer Center Hospital(KCCH) from October 1997 to September 1999, clinical study was performed in 79 cases that underwent 89 times PET evaluation with [18F]-Fluorodeoxyglucose for diagnosis of oral and maxillofacial tumors, and the data was analysed by Bayesian $2{\times}2$ Classification Table. The results were as follows : Evaluation for initial diagnosis with FDG-PET (P<0.005) 1. Agreement rate or accuracy rate is 88.9%. 2. Sensitivity is 95.2%, and specificity 66.7%. 3. Positive predictive rate is 90.9%, and negative predictive rate 80.0%. 4. In consideration of tumor stage, diagnostic rate in less than stage II was 90% and in greater than stage III 100%. 5. In consideration of tumor size, diagnostic rate in less than T2 was 92.3% and in greater than T3 100%. After primary treatment, evaluation for follow up check with FDG-PET (P < 0.001) 1. Agreement rate or accuracy rate is 85.4%. 2. Sensitivity is 87.5%, and specificity 82.4%. 3. Positive predictive rate is 87.5%, and negative predictive rate 82.4%. 4. In 24 recurred cases, 6 had distant metastasis, and 5 of them were diagnosed with FDG-PET, resulting in diagnostic rate of FDG-PET of 83.3%. From the above results, Positron Emission Tomography with [18F]- Fluorodeoxyglucose appears to be more sensitive and accurate for detecting the presence of oral and maxillofacial tumors, and has various clinical applications such as early diagnosis of tumor in initial and follow up check and detection of distant metastasis.

In Vivo Target RNA Specificity of Trans-Splicing Phenomena by the Group I Intron

  • The Tetrahymena group I intron has been shown to employ a trans-splicing reaction and has been modified to specifically target and replace human telomerase reverse transcriptase (hTERT) RNA with a suicide gene transcript, resulting in the induction of selective cytotoxicity in cancer cells that express the target RNA, in animal models as well as in cell cultures. In this study, we evaluated the target RNA specificity of trans-splicing phenomena by the group I intron in mice that were intraperitoneally inoculated with hTERT-expressing human cancer cells to validate the anti-cancer therapeutic applicability of the group I intron. To this end, an adenoviral vector that encoded for the hTERT-targeting group I intron was constructed and systemically injected into the animal. 5'-end RACE-PCR and sequencing analyses of the trans-spliced cDNA clones revealed that all of the analyzed products in the tumor tissue of the virus-infected mice resulted from reactions that were generated only with the targeted hTERT RNA. This study implies the in vivo target specificity of the trans-splicing group I intron and hence suggests that RNA replacement via a trans-splicing reaction by the group I intron is a potent anti-cancer genetic approach.

Current advances in adenovirus nanocomplexes: more specificity and less immunogenicity

  • An often overlooked issue in the field of adenovirus (Ad)-mediated cancer gene therapy is its limited capacity for effective systemic delivery. Although primary tumors can be treated effectively with intralesional injection of conventional Ad vectors, systemic metastasis is difficult to cure. Systemic administration of conventional naked Ads leads to acute accumulation of Ad particles in the liver, induction of neutralizing antibody, short blood circulation half-life, non-specific biodistribution in undesired organs, and low selective accumulation in the target disease site. Versatile strategies involving the modification of viral surfaces with polymers and nanomaterials have been designed for the purpose of maximizing Ad anti-tumor activity and specificity by systemic administration. Integration of viral and non-viral nanomaterials will substantially advance both fields, creating new concepts in gene therapeutics. This review focuses on current advances in the development of smart Ad hybrid nanocomplexes based on various design-based strategies for optimal Ad systemic administration.

타액선 질환의 세침흡인생검에 관한 세포학적 연구 (A Cytologic Study of Fine Needle Aspiration Biopsy of Salivary Gland Diseases)

  • Fine needle aspiration biopsy cytology is a widely recognized and useful technique which can provide diagnosis in lesions of the head and neck, enabling appropriate management plans for individual patient to be made. Fifty one fine needle aspirates from salivary gland masses were examined. Four aspirates (8%) were inadequate for examination. Of the remaning 47 samples, 42 cases (82%) were benign lesions which consist of 30 pleomorphic adenoma(58%), 7 inflammatory lesion (14%), 4 Warthin's tumor(8%) and 1 benign lesion(2%). Two cases(4%) were atypical lesions. Three cases(6%) were malignant lesions consisting of 2 adenoid cystic carcinomas(4%) and 1 mucoepidermoid carcinoma (2%). The cytologic diagnoses were compared with the subsequent histologic diagnosis of surgical resected specimen in 24 cases. 19 cases of 21 aspirates from benign tumors were correctly diagnosed by fine needle aspiration cytology, with a specificity of 90%. All 3 aspirates from the 3 patients with malignant tumor were correctly diagnosed by fine needle aspiration cytology, with a sensitivity at 100%. Overall acurracy was 88%. Diagnostic error was encountered in adenoid cystic carcinoma, mucoepidermoid carcinoma and Warthin's tumor Correct histologic diagnosis was made in 86% of benign tumors(84% for pleomorphic adenoma and 100% for Warthin's tumor) and in 100% of malignant tumors.

Brain Tumor Detection Based on Amended Convolution Neural Network Using MRI Images

  • Brain tumors are one of the most threatening malignancies for humans. Misdiagnosis of brain tumors can result in false medical intervention, which ultimately reduces a patient's chance of survival. Manual identification and segmentation of brain tumors from Magnetic Resonance Imaging (MRI) scans can be difficult and error-prone because of the great range of tumor tissues that exist in various individuals and the similarity of normal tissues. To overcome this limitation, the Amended Convolutional Neural Network (ACNN) model has been introduced, a unique combination of three techniques that have not been previously explored for brain tumor detection. The three techniques integrated into the ACNN model are image tissue preprocessing using the Kalman Bucy Smoothing Filter to remove noisy pixels from the input, image tissue segmentation using the Isotonic Regressive Image Tissue Segmentation Process, and feature extraction using the Marr Wavelet Transformation. The extracted features are compared with the testing features using a sigmoid activation function in the output layer. The experimental findings show that the suggested model outperforms existing techniques concerning accuracy, precision, sensitivity, dice score, Jaccard index, specificity, Positive Predictive Value, Hausdorff distance, recall, and F1 score. The proposed ACNN model achieved a maximum accuracy of 98.8%, which is higher than other existing models, according to the experimental results.

Tumor Markers in Serum and Ascites in the Diagnosis of Benign and Malignant Ascites

  • Objective: To evaluate the values of 4 tumor markers in serum and ascites and their ascites/serum ratios in the identification and diagnosis of benign and malignant ascites. Materials and Methods: A total of 76 patients were selected as subjects and divided into malignant ascites group (45 cases) and benign ascites group (31 cases). Samples of ascites and serum of all hospitalized patients were collected before treatment. The levels of carcinoembryonic antigen (CEA), alpha fetoprotein (AFP), cancer antigen 125 (CA125) and carbohydrate antigen 19-9 (CA19-9) were detected by chemiluminescence (CLIA). Results: CEA, AFP and CA19-9 in both serum and ascites as well as CA125 in ascites were evidently higher in the malignant ascites group than in the benign ascites group (P<0.01). Malignant ascites was associated with elevated ascites/serum ratios for AFP and CA125 (P<0.01). The areas under receiver operating characteristic (AUROCs) of CEA and CA125 in ascites and the ratios of ascites/serum of AFP, CEA, CA125 and CA19-9 were all >0.7, suggesting certain values, while those of ascites CA19-9 and serum CEA were 0.697 and 0.629 respectively, indicating low accuracy in the identification and diagnosis of benign and malignant ascites. However, the AUROCs of the remaining indexes were <0.5, with no value for identification and diagnosis. Compared with single index, the sensitivity of combined detection increased significantly (P<0.05), in which the combined detection of CEA, CA19-9 and CA125 in ascites as well as the ratio of ascites/serum of CEA, CA19-9, CA125 and AFP had the highest sensitivity (98.4%) but with relevantly low specificity. Both sensitivity and specificity of combined detection should be comprehensively considered so as to choose the most appropriate index. Conclusions: Compared with single index, combined detection of tumor markers in serum and ascites can significantly improve the diagnostic sensitivity and specificity.

Texture Analysis of Three-Dimensional MRI Images May Differentiate Borderline and Malignant Epithelial Ovarian Tumors

  • Objective: To explore the value of magnetic resonance imaging (MRI)-based whole tumor texture analysis in differentiating borderline epithelial ovarian tumors (BEOTs) from FIGO stage I/II malignant epithelial ovarian tumors (MEOTs). Materials and Methods: A total of 88 patients with histopathologically confirmed ovarian epithelial tumors after surgical resection, including 30 BEOT and 58 MEOT patients, were divided into a training group (n = 62) and a test group (n = 26). The clinical and conventional MRI features were retrospectively reviewed. The texture features of tumors, based on T2-weighted imaging, diffusion-weighted imaging, and contrast-enhanced T1-weighted imaging, were extracted using MaZda software and the three top weighted texture features were selected by using the Random Forest algorithm. A non-texture logistic regression model in the training group was built to include those clinical and conventional MRI variables with p value < 0.10. Subsequently, a combined model integrating non-texture information and texture features was built for the training group. The model, evaluated using patients in the training group, was then applied to patients in the test group. Finally, receiver operating characteristic (ROC) curves were used to assess the diagnostic performance of the models. Results: The combined model showed superior performance in categorizing BEOTs and MEOTs (sensitivity, 92.5%; specificity, 86.4%; accuracy, 90.3%; area under the ROC curve [AUC], 0.962) than the non-texture model (sensitivity, 78.3%; specificity, 84.6%; accuracy, 82.3%; AUC, 0.818). The AUCs were statistically different (p value = 0.038). In the test group, the AUCs, sensitivity, specificity, and accuracy were 0.840, 73.3%, 90.1%, and 80.8% when the non-texture model was used and 0.896, 75.0%, 94.0%, and 88.5% when the combined model was used. Conclusion: MRI-based texture features combined with clinical and conventional MRI features may assist in differentitating between BEOT and FIGO stage I/II MEOT patients.
