Poor marginal fidelity resulting in a large marginal gap increases plaque accumulation, gingival inflammation and dental caries. The purpose of this study was to evaluate the marginal fit of three different cervical finishing methods of prepared teeth. A stereomicroscope was used to measure the space between the margin of restoration and the finishing line of prepared tooth. The results were statistically analyzed using the ANOVA and Multiple Range Test(Tukey's HSD). The results were as follows : 1. There were no significant differences concerning the types of tooth and positon (P>0.05), whereas the differences were statistically significant in case of cervical finishing methods (P<0.05). 2. There were statistically significant differences between before and after cementation (P<0.05). 3. In comparison according to variable margins after cementation, the gap discrepancies were increased in $130^{\circ}$ shoulder margin, chamfer margin and $90^{\circ}$ shoulder margin in ascending order, and there were significant differences between $90^{\circ}$ shoulder margin and chamfer, $130^{\circ}$ shoulder margin 4. In comparison according to variable margins, the gap discrepancies were increased in chamfer margin, $130^{\circ}$ shoulder margin and $90^{\circ}$ shoulder margin in ascending order, and there were significant differences between $90^{\circ}$ shoulder margin and chamfer, $130^{\circ}$ shoulder margin. 5. This study demonstrated a better marginal fit with all-ceramic crowns fabricated on chamfer and $130^{\circ}$ shoulder margin compared with $90^{\circ}$ shoulder margin.
Background : Patients with frozen shoulder have pain and limited range of motion (ROM) at the affected shoulder. Shoulder pain of these patients usually decreases with recovery of range of motion. The aim of this study is to identify effects of static stretching and hold-relax on recovering ROM and reducing pain of patients with frozen shoulder. Methods : In this study, 15 patients with frozen shoulder who were only treated with physical therapy were recruited. Patients who received any surgical procedure or hydrotherapy were excluded. The subjects were divided randomly into 3 groups; control(n=5), static stretching(n=5) and hold-relax(n=5) groups. Patients were treated with stretching and the total number of treatments was 12 times. The ROM was measured at baseline, after fourth, eighth and final treatments. Shoulder pain and disability index(SPADI) scores were assessed before and after doing all treatments. Repeated measures ANOVA was used to analyze ROM change within each group. Changes of ROM between groups was analyzed by one way ANOVA followed by post hot analysis(Tukey's HSD). SPADI score within each group analyzed using Wilcoxon signed ranks test. A p-value<0.05 was considered statistically significant. Study Design : Repeated measures design. Result : There were significant differences of ROM in both treatment groups except internal rotation of static stretching group. ROM was significantly increased in treatment groups compared with control group, but the difference of ROM change between two treatment groups did not differ significantly. SPADI scores for treatment groups significantly decreased compared with pre-SPADI score. Conclusion : The result suggests that hold-relax can be more effective than static stretching to recover range of motion and alleviate shoulder pain on patients with frozen shoulder.
This study was done to elucidate the electron microscopic characteristics of certain pathogenic fungi. Four Candida species, (C. albicans, C. tropicalis, C. parapsilosis and C. glabrate) and Cryptococcus neoformans were cultured for 3 days at $30^{\circ}C$ in the Sabouraud dextrose medium. After incubation, they were stored at $4^{\circ}C$ for 24hours. Fine structures were analyzed by morphometry, and Tukey's HSD test was used for statistics. On scanning electron microscopy C. albicans and C. neoformans were similar in size but different in shape, showing sphero-shape or ovalo-shape in C. neoformans. Surface of C. neoformans was coarse and spiny, but Candida species examined were uniformly smooth. In size, C. glabrata was the smallest among them. Budding scar as seen on the surface of Candida species by the number ranging from 1 to 7. Cryptococcus neoformans showed one or two budding scar. On transmission electron microscopy the cytoplasm of most yeast cells showed plentiful glycogen particles, mitochondria, peroxisomes and vacuoles. However, cell walls were different among four Candida species and Cryptococcus neoformans. The cell wall of Candida species consisted of fibrous layer, that was electron dense layer and transparent layer, in contrast to Cryptococcus neoformans consisted of electron dense layer with lamellar structure. This layer was two times thicker than that of Candida species. The outer layer of cell wall was of radiating pattern.
Objectives: This study investigated the effect of red and white wine on color changes of nanofilled and nanohybrid resin composite. Materials and Methods: Sixty specimens of each resin composite were prepared. Baseline data color values were recorded using a spectrophotometer. Three groups of discs (n = 20) were then alternately immersed in red, white wine, and deionized water (as a control) for twenty five minutes and artificial saliva for five minutes for four cycles. Specimens were then stored in artificial saliva for twenty two hours. This process was repeated for five days following immersion in artificial saliva for two days. Subsequently, the process was repeated again. Data were analyzed by two-way repeated ANOVA, one-way ANOVA, and Tukey's HSD. Results: Red wine caused significantly higher color change (${\Delta}E^*$ > 3.3) than did white wine and deionized water (p < 0.05). Nanohybrid resin composites had significantly more color changes than nanofilled resin composite (p < 0.05). Conclusions: The effect of red and white wine on the color changes of resin composite restorative materials depended upon the physical and chemical composition of the restorative materials and the types of wine.
Objectives: It is difficult to achieve adhesion between resin cement and zirconia ceramics using routine surface preparation methods. The aim of this study was to evaluate the effects of $CO_2$ and Er:YAG laser treatment on the bond strength of resin cement to zirconia ceramics. Materials and Methods: In this in-vitro study 45 zirconia disks (6 mm in diameter and 2 mm in thickness) were assigned to 3 groups (n = 15). In control group (CNT) no laser treatment was used. In groups COL and EYL, $CO_2$ and Er:YAG lasers were used for pretreatment of zirconia surface, respectively. Composite resin disks were cemented on zirconia disk using dual-curing resin cement. Shear bond strength tests were performed at a crosshead speed of 0.5 mm/min after 24 hr distilled water storage. Data were analyzed by one-way ANOVA and post hoc Tukey's HSD tests. Results: The means and standard deviations of shear bond strength values in the EYL, COL and CNT groups were $8.65{\pm}1.75$, $12.12{\pm}3.02$, and $5.97{\pm}1.14MPa$, respectively. Data showed that application of $CO_2$ and Er:YAG lasers resulted in a significant higher shear bond strength of resin cement to zirconia ceramics (p < 0.0001). The highest bond strength was recorded in the COL group (p < 0.0001). In the CNT group all the failures were adhesive. However, in the laser groups, 80% of the failures were of the adhesive type. Conclusions: Pretreatment of zirconia ceramic via $CO_2$ and Er:YAG laser improves the bond strength of resin cement to zirconia ceramic, with higher bond strength values in the $CO_2$ laser treated samples.

The aim of this study was to investigate the effects of chitosan powder addition on the strengthening of conventional glass ionomer cement. Two types of chitosan powders with different molecular weight were mixed with conventional glass ionomer cement (GIC): low-molecular weight chitosan (CL; 50~190 kDa), high-molecular weight chitosan (CH; 310~375 kDa). The chitosan powders (CL and CH) were separately added into the GIC liquid (0.25-0.5 wt%) under magnetic stirring, or mixed with the GIC powder by ball-milling for 24 h using zirconia balls. The mixing ratio of prepared cement was 2:1 for powder to liquid. Net setting time of cements was measured by ISO 9917-1. The specimens for the compressive strength (CS; $4{\times}6mm$), diametral tensile strength (DTS; $6{\times}4mm$), three-point flexure (FS; $2{\times}2{\times}25mm$) with flexure modulus (FM) were obtained from cements at 1, 7, and 14 days after storing in distilled water at $(37{\pm}1)^{\circ}C$. All mechanical strength tests were conducted with a cross-head speed of 1 mm/min. Data were statistically analyzed by one-way ANOVA and Tukey HSD post-hoc test. The mechanical properties of conventional glass ionomer cement was significantly enhanced by addition of 0.5 wt% CL to cement liquid (CS, DTS), or by addition of 10 wt% CH (FS) to cement powder. The CL particles incorporated into the set cement were firmly bonded to the GIC matrix (SEM). Within the limitation of this study, the results indicated that chitosan powders can be successfully added to enhance the mechanical properties of conventional GIC.

PURPOSE: This study was conducted to determine the effects of treadmill gait training with obstacle-crossing on the static and dynamic balance ability of patients with post stroke hemiplegia. METHODS: Twenty-one patients with post stroke hemiplegia were divided into three groups as: treadmill gait training with obstacle-crossing (TOG, n=7), treadmill gait training without obstacle-crossing (TGG, n=7) and a control (CON, n=7). TOG and TGG performed exercise for 20 minutes, three times a week for 8 weeks. Static balance ability (stability typical, ST; weight distribution index, WDI; fourier harmony index, FHI; and fall index, FI) and dynamic balance ability (berg balance scale, BBS and timed up and go test, TUG) were measured before and after 8 -weeks in each exercise group. Statistical analyses were conducted using two-way ANOVA with repeated measures, a paired t-test, and multiple comparisons according to Tukey's HSD. RESULTS: FHI and BBS were significantly increased at TOG (p<.01) and TGG (p<.05) after 8-weeks compared to before treadmill gait training with obstacle-crossing. FHI and BBS were significantly increased at TOG compared with CON and TGG (p<.05). CONCLUSION: Treadmill gait training with obstacle-crossing was more effective than that without obstacle-crossing to improve posture control and independent daily life performance of hemiplegia patients.
PURPOSE. The purpose of this study was to evaluate the shear bond strength (SBS) of an indirect resin composite (IRC) to the various resin matrix ceramic (RMC) blocks using different surface treatments. MATERIALS AND METHODS. Ninety-nine cubic RMC specimens consisting of a resin nanoceramic (RNC), a polymer-infiltrated hybrid ceramic (PIHC), and a flexible hybrid ceramic (FHC) were divided randomly into three surface treatment subgroups (n = 11). In the experimental groups, untreated (Cnt), tribochemical silica coating (Tbc), and Neodymium-Doped Yttrium Aluminum Garnet (Nd:YAG) laser irradiation (Lsr) with 3 W (150 mJ/pulse, 20 Hz for 20 sec.) were used as surface treatments. An indirect composite resin (IRC) was layered with a disc-shape mold ($2{\times}3mm$) onto the treated-ceramic surfaces and the specimens submitted to thermal cycling (6000 cycles, $5-55^{\circ}C$). The SBS test of specimens was performed using a universal testing machine and the specimens were examined with a scanning electron microscope to determine the failure mode. Data were statistically analyzed with two-way analysis of variance (ANOVA) and Tukey HSD test (${\alpha}=.05$). RESULTS. According to the two-way ANOVA, only the surface treatment parameter was statistically significant (P<.05) on the SBS of IRC to RMC. The SBS values of Lsr-applied RMC groups were significantly higher than Cnt groups for each RMC material, (P<.05). Significant differences were also determined between Tbc surface treatment applied and untreated (Cnt) PIHC materials (P=.039). CONCLUSION. For promoting a reliable bond strength during characterization of RMC with IRC, Nd:YAG laser or Tbc surface treatment technique should be used, putting in consideration the microstructure and composition of RMC materials and appropriate parameters for each material.

PURPOSE. The aim of this study was to evaluate the effect of occlusal adjustments on the surface roughness of yttria-tetragonal zirconia polycrystal (Y-TZP) and wear of opposing artificial enamel. MATERIALS AND METHODS. Twenty-five Y-TZP slabs from each brand (Lava, 3M and Bruxzir, Glidewell Laboratories) with different surface conditions (Control polished - CPZ; Polished/ground - GRZ; Polished/ground/repolished - RPZ; Glazed - GZ; Porcelain-veneered - PVZ; n=5) were abraded (500,000 cycles, 80 N) against artificial enamel (6 mm diameter steatite). Y-TZP roughness (in ${\mu}m$) before and after chewing simulation (CS) and antagonist steatite volume loss (in $mm^3$) were evaluated using a contact surface profilometer. Y-TZP roughness was analyzed by three-way analysis of variance (ANOVA) and steatite wear by two-way ANOVA and Tukey Honest Difference (HSD) (P=.05). RESULTS. There was no effect of Y-TZP brand on surface roughness (P=.216) and steatite loss (P=.064). A significant interaction effect (P<.001) between surface condition and CS on Y-TZP roughness was observed. GZ specimens showed higher roughness after CS (before CS - $3.7{\pm}1.8{\mu}m$; after CS - $13.54{\pm}3.11{\mu}m$), with partial removal of the glaze layer. Indenters abraded against CPZ ($0.09{\pm}0.03mm^3$) were worn more than those abraded against PVZ ($0.02{\pm}0.01mm^3$) and GZ ($0.02{\pm}0.01mm^3$). Higher wear caused by direct abrasion against zirconia was confirmed by SEM. CONCLUSION. Polishing with an intraoral polishing system did not reduce the roughness of zirconia. Wear of the opposing artificial enamel was affected by the material on the surface rather than the finishing technique applied, indicating that polished zirconia is more deleterious to artificial enamel than are glazed and porcelain-veneered restorations.
Purpose. This study aimed to assess the influence of various micromechanical surface conditioning (MSC) strategies with or without coupling agent (silane) application on the micro-shear bond strength (µSBS) of resin- matrix ceramics (RMCs). Materials and Methods. GC Cerasmart (GC), Lava Ultimate (LU), Vita Enamic (VE), Voco Grandio (VG), and Brilliant Crios (BC) were cut into 1.0-mm-thick slices (n = 32 per RMC) and separated into four groups according to the MSC strategy applied: control-no conditioning (C), air-borne particle abrasion with aluminum oxide particles (APA), 2W- and 3W-Er,Cr:YSGG group coding is missing. The specimens in each group were further separated into silane-applied and silane-free subgroups. Each specimen received two resin cement microtubules (n = 8 per subgroup). A shear force was applied to the adhesive interface through a universal test machine and µSBS values were measured. Data were statistically analyzed by using 3-way ANOVA and Tukey HSD test. Failure patterns were scrutinized under stereomicroscope. Results. RMC material type, MSC strategy, and silanization influenced the µSBS values (P<.05). In comparison to the control group, µSBS values increased after all other MSC strategies (P<.05) while the differences among these strategies were insignificant (P>.05). For control and APA, there were insignificant differences between RMCs (P>.05). The silanization decreased µSBS values of RMCs except for VE. Considerable declines were observed in GC and BC (P<.05). Conclusion. MSC strategies can enhance bond strength values at the RMC - cement interface. However, the choice of MSC strategy is dependent on RMC material type and each RMC can require a dedicated way of conditioning.
