Percutaneous Pig-tail Catheter Drainage in the Management of Lung Abscesses (폐농양의 치료에 Pig-tail 도관을 통한 경피적 배농)

  • Background: Patients with pyogenic lung abscess who do not respond to medical therapy are usually candidates for thoracotomy with pulmonary resection. Percutaneous tube drainage, used routinely and with good results before the antibiotic era, has nearly been forgotten. Methods: We treated 8 patients who had poor cadidates for a lobectomy and didn't respond to medical management. A pig-tail catheter (6 to 10 Fr.) was inserted into the abscess under the fluoroscopic guidance. Results: All patients brought about dramatic clinical responses with prompt closure of cavity and radiographic improvement. Conclusion: Percutaneous pig-tail catheter drainage provides excellent clnical results with ease and safety, and avoids unnecessary loss of functioning lung parenchyma. But lobectomy should be considered in patients who major life-threatening bleeding or massive pulmonary necrosis.

  • Recently, dewatering process method of high water content materials that utilize geotextile has many applications in variety fields. It is method of dewatering to solid step through self-weight consolidation process after pour sludge using filtering efficiency and dewatering efficiency. Analyzed application of domestic manufactured geotextile tube that can examine physical characteristics of geotextile tube and filling soil and achieve filtering efficiency and dewatering efficiency. Based on the various laboratory and field test results mixing proportions of water and soil is about 6:4 at least. Polypropylene geotextile is more effective for drainage and dewatering function of geotextile tube application.

Clinical Study after Video-Assisted Thoracic Surgery and Mid-Axillary Thoracotomy for Recurrent Spontaneous Pneumothorax (재발성 자연 기흉에 대한 정중액과 개흉술과 비디오 흉강경수술의 비교연구)

  • Background: Although treatment of spontaneous pneumothorax by video-assisted thoracic surgery(VATS) has generally shown better clinical results than conventional thoracotomy, treatment of spontaneous pneumothorax by midaxillary thoracotomy(MAXT) has also shown good clinical results. The author studied to compare the clinical results of MAXT group I and VATS group II. Material and Method : Group I included 30 midaxillary thoracotomy among 83 operative cases of spontaneous pneumothorax from Jan. 1992 to Dec. 1993. Group II included 30 VATS among 101 operative cases of pneumothorax from Jan. 1994 to Aug. 1995. The author selected the 30 patients, age 18 to 25 years, with recurrent spontaneous pneumothorax in each group. The author analyzed the operative indication, gender, operating time, amounts of the used staplers, tube drainage, total amounts of analgesics used during postoperative 24hr, tube stay time, postoperative complications and mortality. Result: The follow-up periods of both procedures were from 6 to 43 months. The operating time from start of skin incision to end of skin closure was 84.79${\pm}$21.70(from 40 to 150) minutes in MAXT group I and 108.8${\pm}$42.02(from 58 to 120) minutes in VATS group II(P<0.001). The numbers of the used staples useo was 1.31${\pm}$0.6(from 0 to 3) in group I and 3.41${\pm}$2.37(from 0 to 11) in group II. The amounts of postoperative 24hour tube drainage were 220.76${\pm}$106.73(from 65 to 400) ml in group I and 260.63${\pm}$233.18(from 70 to 320) in group II(P>0.05). The amounts of postoperative 24 hourly used analgesics(Tarasyn ) was 1.38${\pm}$1.32(from 0 to 5) amples in group I and 0.72${\pm}$1.02(from 0 to 4) amples in group II (P<0.05). The postoperative tube stay is 5.45${\pm}$30.9 (from 3 to 7) days in group I and 4.75${\pm}$3.1(9 from 2 to 14) days in group II(P>0.05). The number of complications after operations was 2 cases of prolonged air leakage in group I, and in group II. (P is not significant). The number of recurrence after the operation was one in group I and also one in group II(P is not significant). In conclusion, there were no statistical differences in the postoperative 24 hour chest tube drainage, days of postoperative tube stay, postoperative complications and recurrence in the analysis between group I and group II. Conclusion: The author found that group II of VATS between 18 years and 25 years of age in recurrent spontaneous pneumothorax, statistically, requires longer operative time, more number of autosuture staples and less postoperative analgesic dosage than the midaxillary thoracotomy group I.

Minimally Invasive Cardiac Surgery versus Conventional Median Sternotomy for Atrial Septal Defect Closure

  • Background: Median sternotomy is the standard approach for atrial septal defect (ASD) closure. However, minimally invasive cardiac surgery (MICS) has been introduced at many centers in adult/grown-up congenital heart patients. We retrospectively reviewed the results of right anterolateral thoracotomy compared with conventional median sternotomy (CMS) for ASD closure at Seoul National University Hospital. Methods: We retrospectively analyzed 60 adult patients who underwent isolated ASD closure from January 2004 to December 2013 (42 in the CMS group, 18 in the MICS group). Preoperative, operative, and postoperative data were collected and compared between the 2 groups. Results: The MICS group was younger (44.6 years vs. 32.4 years, p=0.002) and included more females (66.7% vs. 94.4%, p=0.025) than the CMS group. Operation time (188.4 minutes vs. 286.7 minutes, p<0.001), cardiopulmonary bypass time (72.7 minutes vs. 125.8 minutes, p<0.001), and aortic cross-clamp time (25.5 minutes vs. 45.6 minutes, p<0.001) were significantly longer in the MICS group. However, there were no significant differences in morbidity and mortality between groups. Only chest tube drainage in the first 24 hours (627.1 mL vs. 306.1 mL, p<0.001) exhibited a significant difference. Conclusion: MICS via right anterolateral thoracotomy is an alternative choice for ASD closure. The results demonstrated similar morbidity and mortality between groups, and favored MICS in chest tube drainage in the first 24 hours.

Chylothorax after Surgery for Congenital Cardiac Disease: A Prevention and Management Protocol

  • Background: Chylothorax after congenital heart surgery is not an uncommon complication, and it is associated with significant morbidity. However, consensus treatment guidelines are lacking. To improve the treatment outcomes of patients with postoperative chylothorax, we implemented a standardized management protocol at Severance Hospital in September 2014. Methods: A retrospective review of patients treated at a single center was done. All corrective and palliative operations for congenital heart disease performed at our institution between January 2008 and April 2018 were reviewed. The incidence and treatment outcomes of postoperative chylothorax were analyzed. Results: The incidence of chylothorax was 1.9%. Sixty-one percent of the patients could be managed with a low-fat diet, while 28% of the patients required complete restriction of enteral feeding. Thoracic duct embolization was performed in 2 patients and chest tube drainage decreased immediately after the procedure. No patient required thoracic duct ligation or pleurodesis. After implementation of the institutional management protocol, the number of chest tube drainage days decreased (median, 24 vs. 14 days; p=0.45). Conclusion: Implementing a strategy to reduce postoperative chylothorax resulted in an acceptable incidence of postoperative chylothorax. Instituting a clinical practice protocol helped to curtail the treatment duration and to decrease the requirement for surgical treatment. Image-guided embolization of the thoracic duct is an effective treatment for postoperative chylothorax.

Variation of Soil Physical Characteristics by Drainage Improvement in Poorly Drained Sloping Paddy Field (배수불량 경사지 논 토양의 배수방법에 따른 토양 물리성 변화)

  • The lower portion of sloping paddy fields normally contains excessive moisture and the higher water table caused by the inflow of ground water from the upper part of the field resulting in non-uniform water content distribution. Four drainage methods namely Open Ditch, Vinyl Barrier, Pipe Drainage and Tube Bundle for multiple land use were installed within 1-m position from the lower edge of the upper embankment of sloping alluvial paddy fields. This study was conducted to evaluate soil physical characteristics by drainage improvement in poorly drained sloping paddy field. The results showed that subsurface drainage by Pipe Drainage improves the productivity of poorly drained soils by lowering the water table and improving root zone soil layer condition. In an Pipe drainage plot, soil moisture drained faster as compared to the other drainage methods. Infiltration rate showed high tendency to Piper Drainage method about $20.87mm\;hr^{-1}$ than in Open Ditch method $0.15mm\;hr^{-1}$. And Similarly soil water and degree of hardness and shear strength phase of soil profile showed a tendency to decrease. From the above results, we found that when an subsurface drainage was established with at 1m position from the lower edge paddy levee of the upper field in sloping poorly drained paddy fields Pipe Drainage was the most effective drainage system for multiple land use.

Early Pleurodesis for Postoperative Air Leak with Autologous Blood and 50% Glucose Solution

  • Background: Postoperative air leaks after pulmonary resection prolong the duration of chest drainage and the length of hospital stay. One of the many treatment options is bedside pleurodesis using various agents. This study evaluated the feasibility of an early intervention to stop postoperative air leaks with either autologous blood or a 50% glucose solution. Methods: We retrospectively reviewed 323 patients who underwent bedside pleurodesis between January 2017 and March 2022. Sixty-four patients received autologous blood patch pleurodesis, and 36 were treated with a 50% glucose solution after pulmonary resection. The primary endpoints were the total postoperative tube indwelling time, post-pleurodesis tube indwelling time, and hospital stay. A propensity score-matched analysis was performed. Results: In the autologous blood patch pleurodesis and 50% glucose solution groups, the mean initiation timing of postoperative pleurodesis were 2.06±1.62 and 3.28±1.56 days, the mean duration of the tube indwelling time after surgery was 6.58±3.02 and 6.42±4.92 days, and the mean duration of the tube indwelling time after pleurodesis, it was 4.53±3.10 and 3.11±4.80 days, respectively. In addition, the total length of hospital stay was 9.11±5.42 and 7.83±4.75 days in the autologous blood patch pleurodesis and 50% glucose solution groups, respectively. Conclusion: Early postoperative air leak cessation with autologous blood patch pleurodesis or 50% glucose solution pleurodesis is a feasible procedure with acceptable outcomes that effectively shortens the hospital stay.

The Clinical Study for Empyema: 176 Cases (농흉의 임상적 고찰[176예])

  • For the past 5 year 6 months from January 1975 to June 1980, 176 patients with empyema have been treated in Chonnam University Hospital. They were 134 males and 42 females ranging from ] 8 days to 69 years of age. [mean age: 26.1 years] The duration of illness prior to treatment was relatively shorter in pediatric group than in adult group, that is, the duration of less than 1 month was 89.5% in pediatric group and 38.0% in adult group. In bacteria study there were Staphylococcus 26.1%, Streptococcus 17.6%, E. coil 10.8%, Pseudomonas 10.8%, Diplococcus pneumoniae 5.7% and Candidia. And 4 children and 3 adults had infections of two species of bacteria. The underlying pathologic lesions were pyogenic pneumonia 34.7%, tuberculosis 29.5%, paragonimiasis 15.3%, trauma 9.7% and postoperative state. The over-all mortality rate was 1.7% [3 patients]. The causes of death were sepsis In 1 child and sepsis secondary to esophageal fistula in 2 adults. Adequate drainage and obliteration of the pleural space seems to be the most important aspect of treatment and can frequently be achieved by initial tube drainage in acute empyema, especially in the pediatric group. The chronic thick walled or loculated cavities required open window therapy, decortication, resection therapy and sterilization. Modified Eloesser`s operation and 0.3-0.5% potadine irrigation brought good result in the patients who had general weakness, marked pulmonary parenchymal destruction due to pyothorax, and pyothorax with severe bronchopleural fistula.

Closed Thoracostomy of Spontaneous Pneumothorax : Clinical Comparison of Suction with No Suction (자연기흉의 흡인법 치료 효과)

  • We have performed a prospective study to assess the efficacy of suction drainage in 45 patients with primary spontaneous pneumothorax and 15 patients with secondary spontaneous pneumothorax, treated by closed chest tube drainage with underwater seal during the period Jan. 1990 to Dec. 1990 at the Department of Thoracic and Cardiovascular Surgery, Yeungnam University Hospital. The patients were divided randomly into two groups, 28 cases receiving suction and the other 32 cases no suction. The success rate was 82.1% for the former and 87.5% for the latter with the overall success rate of 85.0% and there was no significant difference in success rate between the two groups According to the causes and the extent of pneumothorax, the difference between the success rates of the two groups was also not significant statistically. But according to the duration of air leakage, suction group under 2 days showed a high success rate[46.4%] and the same group with 3-4 days, a relatively low success rate[21.4%] compared with that of the former. We conclude that the suction treatment is somewhat valuable in shortening the tubing time in patients with small amounts of air leakage, but it doesn`t seem to increase the success rate in all patients.

Clinical Study of Empyema Thoracis: a review of 64 cases (농흉의 임상적 고찰64례 보고)

  • A Clinical analysis of 64 patients of thoracic empyema was done who received surgical intervention at Dept. of Thoracic Surgery of the Chosun University Hospital in the period of 3 years from September 1976 to October 1979. Following was the results: 1. Seven cases [10.9%] were under the age of 15 years, 16 cases [25%] was between 15-30 years and 41 cases [64.1%] was above the age of 30 years. A proportion of children and adult was 1:8. 2. Male and female ratio was 3:1. Right and left side pleural cavity ratio was 2.4:1. 3. Predisposing factors were pneumonia [35.9%] and pulmonary tuberculosis [28.1%]. 4. Most frequently encountered symptoms were dyspnea, cough, chest pain and fever in order. 5. Etiologic organisms were confirmed in 39 cases [86.7%] which requested in 45 cases. Staphylococcal infections were 11 cases and streptococcal, pneumococcal pseudomonas infection was infected in order. 6. Pneumothorax was associated with empyema on 21 cases [32.8%]; among those 13 cases [61.9%] were tuberculous in nature. 7. Sensitivity test was revealed that Minocin was most very sensitive drug, and next Erythromycin, Gentamycin and Penbrex in order. But most resistant drugs were Penicillin, Kanamycin, Streptomycin and Tetracycline in order. 8. Treatments were combined with antibiotics therapy and several surgical procedures for empyema. 26 cases [40.6%] were treated with closed thoracotomy drainage, 17 cases [26.6%] with open thoracotomy tube drainage and 9 cases decortication and 9 cases thoracoplasty. 9. 2 death cases occurred in 64 cases of thoracic empyema, and 79.7% cases were discharged with recovery and improvement.
