균형 향상을 위한 율동적 안정 또는 안정적 반전의 효과적인 적용방법은? (What is the Optimal Application Method of Rhythmic Stabilization or Stabilizing Reversals to Improve Balance?)

  • Purpose: This study reviewed articles to identify the optimal rhythmic stabilization or stabilizing reversals application method for improving balance. Methods: The Cochrane, EBSCO, eArticle, Embase, DBpia, KISS, Medline, ProQuest, PubMed, SAGE, ScienceDirect, Scopus, Springer, and Wiley databases were used to search articles from 1990 to January 2017. The search terms included: "rhythmic stabilization" and "stabilizing reversals." Only experimental human studies (randomized controlled trials) that compared the effects of varying the optimal application of rhythmic stabilization or stabilizing reversals to improve balance were included in the review. Non-English language (except Korean) and unpublished studies were excluded. Results: During the research, 1,098 articles were initially identified. Of these articles, nine were randomized controlled trials. Of these nine articles, five were in English, and four were in Korean. In addition, three of the trials did not measure the patients' balance, two did not report the intensity and location of the resistance, and three performed the rhythmic stabilization incorrectly. Only one article met the inclusion and exclusion criteria. The remaining study evaluated the participants' performance of the alternating trunk flexor and extensor isometric contraction of the scapulae using the optimal resistance for 10 seconds in a sitting position. The participants completed three sets of eight repetitions with rest intervals of 30 seconds between the repetitions and 60 seconds between the sets. Conclusion: Due to the lack of the scientific research on the topic, this review may not provide the evidence needed to support the optimal application of rhythmic stabilization or stabilizing reversals to improve balance. Future research should consider the methodological quality to identify the proper rhythmic standardization and stabilizing reversals application method.

경쟁적 게임을 적용한 집단 운동 프로그램이 만성 뇌졸중 환자의 균형에 미치는 영향 (The Effects of a Group Exercise Program with Competitive Games on Balance in Chronic Stroke Patients)

  • PURPOSE: The purpose of this study was to investigate the effects of a group exercise program with competitive games on balance in chronic stroke patients. METHODS: The subjects were chronic stroke patients living at home after their discharge from the hospital. A total of 32 subjects were divided into two groups. The experimental group performed a group exercise program with competitive games, and the control group performed individual exercises. The exercise program duration was one hour, and it comprised 10 minutes of stretching, 40 minutes of the main exercise, and 10 minutes of stretching at the end. The experimental group was divided into two teams of 8 players each and played competitive games (transfer a gym ball to another person, relay 10 m walking and return, transfer an object to the next person using both arms while sitting). The control group performed an arm/trunk exercise for 15 minutes and gait training for 25 minutes. These exercises were conducted twice a week (4 weeks). The Berg Balance Scale (BBS) and timed up-and-go (TUG) tests were performed to confirm the change in balance. RESULTS: In the experimental group, BBS showed a significant increase (p < .05) and TUG showed a significant decrease (p < .05). In the control group, there was no statistically significant change in the BBS. However, the TUG showed a significant decrease (p < .05). The BBS and TUG both showed statistically significant differences (p < .05) in the between-group comparison of the change in pre-post intervention values. CONCLUSION: A group exercise program with competitive games to improve the balance ability for fall prevention in chronic stroke patients could be used as a good intervention method.

교사와의 상호작용을 통한 주기적인 척추운동이 청소년의 요부근력과 균형 감각에 미치는 영향 (Effect of Regularly Spine Strength Exercise on Lumbar Muscle Strength and Balance Ability in Teenager with Tutor)

  • Purpose: The purpose of this study was to investigate the effect of regularly strength exercise of spine in teenagers with tutor. Method: Study subjects were 24 middle school students. They were arranged to 2 groups(n=12, respectively). Then experimental group did regularly strength exercise of spine as the handout maneuver during 50min/day and 1day/week in 8 weeks with tutor, but control group did regularly exercise of spine as same maneuver without tutor. For muscle strength of both extensor and flexor of lumbar, the balancing ability and weight distribution index were measured before and after the intervention. Results: For muscle strength of both extensor and flexor of lumbar, there was not significant differences after the intervention, but both flexor and extensor increase mean value of muscle strength than control group. For balancing ability and weight distribution index, there were not significant difference after intervention(p>.05), but experimental group show a tendency to decrease in a few position than control group. Conclusion: When teenagers regularly exercise for spine strengthening for a long time with tutor, it may be increased muscle strength of lumbar and trunk balance.

편평발과 정상발의 무게중심한계 비교 (Comparison of the Limitation of Stability between Flatfeet and Neutral Feet)

  • Purpose: Flatfeet are one of the risk factors of foot dysfunction and postural imbalance. The purpose of this study was to compare the limitation of stability (LOS) for the center of gravity (COG) between flatfeet and neutral feet on stable and unstable support surfaces. Methods: The study included 26 healthy, adult male participants: 14 with normal feet and 12 with flatfeet. The subjects were asked to incline the trunk maximumly to the left, right, anterior, and posterior directions and were asked to keep their feet on the floor with the knee extension. The subjects had 30 seconds of rest time between the tasks. The LOS (anterior, posterior, left, right) of COG was measured by Balance Trainer (BT4, Hur-labs, Tampere, Finland). An independent t- test was used to compare the LOS of COG between flatfeet and normal feet on stable and unstable support surfaces, respectively. Results: The LOS of the flatfeet group was generally decreased on stable support surfaces as compared to that of neutral feet, but it was not significantly different (p>0.05), while the LOS of the flatfeet group was significantly decreased compared to that of neutral feet on unstable support surfaces (p<0.05). Conclusion: This study suggested that the LOS of individuals with flatfeet may be decreased on unstable support surfaces and the postural balance of the flatfeet group may easily be disturbed on an unstable support surface.

  • Dynamic postural control varies with the environmental context, specific task and intentions of the subject. In this paper, dynamic postural control against forward-backward perturbations of a platform was estimated using tri-axial accelerometers and a force plate. Ten young healthy volunteers stood upright in comfortable condition on the perturbation system which was controlled by an AC servo motor. With anterior-posterior perturbations, movements of ankle, knee and hip Joints were obtained by tri-axial accelerometers. and ground reaction forces with corresponding displacements of the center of pressure(CoP) by the force plate. The result showed that the ankle moved first and the trunk forward, which implies that the mechanism of the dynamic postural control in forward-backward perturbations, occurred in the procedure of the ankle, the knee and the hip. Knee flexion and hip extension in the period of acceleration, constant velocity and deceleration phase is very important fur the balance recovery. These responses depends on the magnitude and timing of the perturbation. From the present study the accelerometry-system appears to be a promising tool for understanding kinematic accelerative In response to a transient platform perturbation. A more through understanding of balance recovery mechanism may aid in designing methods for reducing falls and the resulting injuries.

국내·외 문헌적 비교를 통한 뇌졸중 환자에 대한 근력강화 운동의 고찰연구 (A Study on Strengthening Exercise for Stroke Patients through Comparison of Literature between Domestic and Foreign)

  • Purpose : The purpose of this study is to investigate the effect of strengthening program for improving the muscle strength and body function in rehabilitation of patients with stroke Methods : We found the 15 precedent studies at online portal site of "Korea education and Research information service" and found 5 precedent studies at "Pubmed". We analyzed and describe the total 19 studies involving national and international research. Results : The strengthening exercise was adapted to lower extremities and trunk muscle of participants and the isokinetic (concentric and eccentric) exercise was most of exercise type and then manual isometric exercise, functional activities, progressive task oriented resistance exercise, PNF pattern exercise was following. The studies reported that the strengthening program increased target muscle strength and improved balance capacity and walking function for the participants positively. Conclusion : Conclusionally the several strengthening program can be the intervention to increase the muscle strength without increasing spasticity for patients with stroke. We think that the strengthening of lower extremities an improve balance capacity and walking ability and it can use the intervention to change the quality of life in patients with stroke. More than 3weeks strength program might be effectiveness, in case of acute patients with stroke, the improving of muscle strength is available but need to study for improving balance and walking capacity more in the future.

운동과 이침요범을 병행한 체중조절 프로그램이 비만아동의 체성분, 혈액구성 및 체력에 미치는 영향 (Effects of Body Weight Control Program Contains Exercise and Auricular Acupuncture on Body Composition, Blood Composition and Physical Ability in Obese Children)

  • Objectives: The purpose of this study was to investigate the changes of obesity, blood composition and physical ability in obese children on body weight control program. Methods : The body weight control program included exercise, nutrition education and auricular acupuncture was performed 4 times a week during 12 weeks. 32 obese children whose body fat ratio were over 30% participated in this study. Measuring materials were body composition(weight, muscle mass, fat mass, fat ratio, BMI), physical measurement(height, waist circumference, chest circumference, hip circumference), physical ability(grip strength, sit-ups, sitting trunk flexion, sit-down stand-up test, standing long jump, opened eyes foot balance, side step), blood composition(WBC, Hb, ESR, Total Cholesterol, TG, GOT, GPT, BUN, Creatinine, Glucose) and survey(changes of eating habits, changes of living habits, knowledge of obesity, self-confidence, self-esteem). Results : From the results, positive changes were made in BMI, height, waist circumferences, chest circumferences, hip circumferences, sit-ups, sitting trunk flexion, sit-down stand-up test, standing long jump, opened eyes foot balance, side step, Hb, ESR, GOT, GPT and changes of eating habits through the body weight control program. Conclusions : This results suggest that body weight control program including exercise, nutrition education and auricular acupuncture may be effective for helping obese children.

노인의 운동능력이 IADL에 미치는 영향 (The Influence of Motor Ability of Elderly on IADL)

  • Motor ability were hypothesized to be major factors that may have an influence on IADL(instrumental activities of daily living). The purpose of this study was to investi gate the effect of motor ability of elderly on IADL. This study consisted of forty eight elder male and ninety elder female. The subjects were selected from L, S, Y-institution in kyoung ki do. The average age of elder male and female was 73.81, 71.38 years. The data were analyzed with t-test, repeated measurement, correlation test, regression test, using SPSS PC+ and MINITAB program. The measurement item of motor ability was muscle strength(left, right- grip strength), flexibility(sitting trunk flexion, trunk extension, leg raising while in a standing position), agility(whole body reaction test, standing up), power(sargent jump), balance (stork stand). IADL was measured with Philadelphia Geriatric Center IADL. The result of this study were the following : 1. The factor of significant difference of between elder male and female was muscle strength, flexibility, power but no significant difference was agility, balance. 2. Between IADL of elder male and female was no significant difference. 3. Correlation of IADL and Motor ability of elder male had effect on muscle strength and power, agility. Correlation of Body composition and Motor ability of elder female had effect on muscle strength and power, agility. As a result of this study, optimal motor ability of elder male and female had improved IADL. In addition to, this result of this study, it can suggested the consideration of the Health promotion program for elderly.

Effects of the Abdominal Drawing-in Maneuver and the Abdominal Expansion Maneuver on Grip Strength, Balance and Pulmonary Function in Stroke Patients

  • Purpose: The purpose was to determine whether the application of the abdominal drawing-in maneuver (ADIM) and abdominal expansion maneuver (AEM) to stroke patients would affects their trunk stability, balance, pulmonary function, and grip strength. Methods: The subjects were 36 stroke patients who were randomly and equally assigned to an ADIM group (n=12), an AEM group (n=12), and a control group (n=12). The intervention was applied to each group three times per week, 30 minutes each time, for four weeks. Outcome measures were grip strength, modified functional reach test (mFRT) and pulmonary function. Pulmonary function were measured force expiratory volume at one second (FEV1), forced vital capacity (FVC), FEV1/FVC and peak expiratory flow (PEF) values. Results: The results of the three groups showed statistically significant improvements in grip strength. The AEM group showed significantly greater differences in grip strength than either the ADIM group or the control group. In the anterior mFRT, the ADIM group showed significantly improvements than the control group. The ADIM and AEM groups were showed statistically significant greater improvements in PEF between the baseline and post-intervention and the post-analysis revealed that the AEM group showed significantly greater improvements than the control group. Conclusion: The results of this study indicate that the ADIM and AEM were effective in improving the PEF of pulmonary function. The ADIM was more effective than AEM in trunk stabilization.

Effects of Deep Abdominal Muscle Strengthening Exercises on Pulmonary Function and the Ability to Balance in Stroke Patients

  • Purpose: This study investigated effects of deep abdominal muscle strengthening exercises on pulmonary function and the ability to balance in stroke patients and was conducted to propose an exercise program for improving cardiovascular function. Methods: Study subjects were 20 patients with hemiplegia due to stroke, who were divided into the deep abdominal muscle strengthening exercise group (experimental group), 10, and the control group, 10. Pulmonary function tests measured FVC and FEV1, dynamic balance ability was measured using TUG. Static and dynamic balance ability was measured using BBS. The experimental group performed exercises during a period of 6 weeks, 5 times a week for 40 minutes, whereas the control group did not participate in regular exercise. The difference before and after the exercise was compared using paired t-test, difference in exercise before and after between groups was ANCOVA and level of significance was set at ${\alpha}=0.05$. Results: The changes in FVC and FEV1 within the group showed a significant difference only in the experimental group (p<0.001) (p<0.01), between-group difference was statistically significant only in FVC and FEV1 changes in the experimental group (p<0.001). The TUG changes within the group showed a significant difference in the experimental group and control group (p<0.001) (p<0.05), while BBS changes showed a significant difference only in the experimental group. Between-group difference was statistically significant only in TUG and BBS changes in the experimental group. The experimental group showed a more effective significant difference than the control group (p<0.001). Conclusion: Can exercise involving a deep abdominal muscle strengthening program be applied in patients with stroke with difficulty in control of trunk and decreased breathing ability?
