체간 안정화 운동이 무릎관절 전치환술 환자의 무릎기능, 균형, 보행에 미치는 영향 (The Effects of Trunk Stability Exercise on Knee function, Balance, Gait in patients after Total Knee Arthroplasty)

  • 본 연구는 퇴행성 관절염으로 인해 무릎관절 전치환술을 실시한 환자를 대상으로 체간 안정화 운동이 무릎기능, 균형, 보행능력 향상에 어떠한 영향을 미치는지 알아보고자 한다. 본 연구 대상자는 퇴행성 관절염으로 진단 받아 무릎관절 전치환술을 실시한 환자 24명을 선정하여, 무작위 배분으로 일반 물리치료를 실시한 대조군 12명, 체간 안정화 운동을 실시한 실험군 12명으로 구성하였다. 체간 안정화 운동은 선행 연구를 참고하여 구성한 3가지 운동을 총 4주간 실시하였다. 측정도구로는 무릎기능 측정은 무릎관절 주관적 기능 점수 척도를, 균형능력은 일어나 걷기 검사, 한발 서기 검사를 보행능력은 10미터 걷기 검사를 이용하였다. 통계학적 분석은 각 그룹 내 비교는 대응표본 t-검정, 각 그룹 간 비교는 독립표본 t-검정으로 분석하였다. 본 연구 결과 중재 후 실험군이 대조군에 비해 모든 측정 결과가 유의하게 향상되어 나타났다(p<.05). 이와 같은 결과를 바탕으로 무릎관절 전치환술 환자를 대상으로 무릎관절 치료와 함께 체간 안정화 운동을 실시한다면, 환자의 무릎기능, 균형, 보행에 좀 더 효과적이라고 판단된다.

Comparison of Trunk Control on Gross Motor Function and Topography in Children with Spastic Cerebral Palsy

  • PURPOSE: This study examined the differences in the trunk impairment scores according to the levels of the gross motor classification system by evaluating trunk control in children with spastic cerebral palsy using the index of trunk impairment. In addition, the characteristics of trunk control disabilities were investigated according to the cerebral palsy type. METHODS: The subjects were 49 children (mean age 8.57±1.83 years, 11 with hemiplegia, 26 with diplegia, and 12 with quadriplegia) with spastic cerebral palsy levels I to IV under the gross motor function classification system (GMFCS). The coordination and balance of the children with cerebral palsy were evaluated using the index for trunk impairment. Statistical analyses were performed using a Kruskal-Wallis test, and Bonferroni analyses were used as a post-hoc comparison for any significant results. RESULTS: The median of the total scores of trunk impairment was 13 (range, 9-17), which was 56% of the maximum score. The total score of trunk impairment and subscales differed significantly according to the disease severity and type of motor disability. The scores for children with quadriplegia were the lowest compared to children with hemiplegia and diplegia. CONCLUSION: Trunk control function in children with spastic cerebral palsy was reduced, and varied according to the disease severity and types of motor disabilities. The degree of trunk impairment differed from the trunk control ability according to the degree of motor disability of children with cerebral palsy.

1년생 '대능' 대추 회초리 묘목 재식 시 주간 절단 정도 설정 (Establishment of Heading-back Pruning Severities of Trunk at Planting in the One-year-old 'Daeneung' Jujube Whip Nursery Tree)

  • BACKGROUND: The successful management of jujube trees depends on maintaining a balance between reproductive and vegetative growth. In general, heading cuts of trunk stimulate rather vegetative growth, but could decrease flower initiation. This study was conducted to establish a heading-back pruning severities at planting in the one year old 'Daeneung' jujube whip nursery tree by investigating vegetative growth and fruit yield. METHODS AND RESULTS: The heading back pruning severity was assigned as 5 different ranges as follow: one bud (TR-10, heading back pruning was 10 cm above the ground), three buds (TR-30), five buds (TR-50), seven buds (TR-70), and nine buds (TR-90) were left of scion. The number of buds on whip trunk was correlated to the central leader length and increment of trunk cross-sectional area, negatively, and to the tree height and canopy volume, positively. The yields in the TR-50 treatment were higher about 2 times than other treatments. The fruit diameter was not significantly different among the treatments. CONCLUSION: These results indicated that the optimum location of heading-back pruning in one year old 'Daeneung' jujube tree was 50 cm above the ground as evaluated by the balance of reproductive and vegetative growth.

The Influence of Restricted Arm Swing on Symmetry, Movement of Trunk and Pelvis Rotation according to Using a Mobile Phone

  • Purpose: This study was conducted to investigate the effects of variations in arm swing during gait on movement of the trunk and pelvis. During the gait task, the angle of the trunk and pelvic rotation were analyzed according to arm swing conditions. Methods: Seventeen healthy males participated in this study. All subjects were analyzed for gait on a treadmill three times each under three different types of arm swing conditions - natural arm swing, restricted arm swing using a phone, restricted swing in both arms. 3-D motion analysis systems were used to collect and analyze the kinematic data of trunk and pelvic movements, and repeated one-way ANOVA was used to compare the trunk and pelvic kinematic data and symmetry index. The level of significance was ${\alpha}=0.05$. Results: The results showed kinematic differences in trunk and pelvic during gait based on the arm swing conditions. Specifically, there were significant differences in trunk rotation, left and right trunk rotation and symmetry index of trunk rotation during gait among the three arm swing conditions. ROM was used to calculate a symmetry index (SI) based on the average left and right trunk rotation in which a value closer to zero indicated better balance. The SI obtained for arm swing restricted with the phone was closer to -1 than the other conditions. Conclusion: Restricted arm swing due to use of a phone had the possibility to induce instability of postural control while walking, which could be seen to suggest a risk of falling during gait.

20대 여성 요통환자의 6주간 슬링운동이 유연성, 균형, 근력 및 골반경사각에 미치는 영향 (The Effect of a Six-Week Sling Exercise on Flexibility, Balance, Muscular Strength, Pelvic Tilt Angle in Age of 20 Woman Low Back Pain)

  • Purpose : The purpose of this study was to evaluate the effect of 6 weeks of sling exercises on female patients 20 years of age with low back pain according to flexibility, balance, muscle strength and pelvic tilt. Method : The 20 female patients who complained of back pain were divided into two groups. The experimental group exercised for 40 minutes, 3 times a week for 6 weeks. The control group did not. Both groups were measure for flexibility, balance, muscular strength, and pelvic tilt angle. Result : Curvature of the experimental flexibility, stretch, balance, muscle strength of the trunk forward, backward, left rotation, right rotation for the sling exercise group had significant differences. However, pelvic tilt did not show a significant difference. Control of the stretch of the flexibility, balance, and muscle strength of the trunk posterior showed significant differences for the sling exercise group. Conclusion : Although significant differences were found in flexibility, balance, muscular strength, there was no difference in pelvic tilt. When selecting subjects, we believe patients with regular patterns of pain will be more effective for statistical analysis of the changes in pelvic tilt.

백수오(白首烏)의 명칭과 수확 시기 연구 (Determination of Harvest Time and Nominal Origin from Cynanchi wilfordii Radix)

  • Objectives: Cynanchi wilfordii Radix has commonly been used with or instead of Polygoni multiflori Radix in Korea. The purpose of this study was to investigate their history in the traditional Oriental medical literature and quantitative determination of conduritol F from Cynanchi wilfordii Radix according to various harvest times. Methods: We conducted studies of the traditional Oriental medical literature and analysis of the component, conduritol F, simultaneously. Efficacy and origin of Cynanchi wilfordii Radix in literature study, time of harvest in component research. Chromatographic analysis performed using Silica(2) 100A column (Phenomenex 5 um, 250 mm 4.6 mm) with a mixture of acetonitrile and water (98 : 2)(v/v). The analyses detected at UV (210 nm). Results: Conduritol F was detected in Cynanchi wilfordii Radix, but in Cynanchi auriculati Radix was not. We indicate that conduritol F is as a distinguishable indicator with other materia medica. We think the proper harvesting time is October and November. Conclusions: Measures of trunk control were significantly related with values of dynamic balance and gait. Based on these results, trunk control is an essential core component of balance and gait. Trunk control training programs after stroke should be developed and emphasized.

Immediate Effects of Lumbar Rotational Mobilization on the One-Legged Standing Ability in Healthy Individuals: A Randomized Controlled Trial

  • The original focus of this study was to investigate the immediate effects of lumbar rotational mobilization on the one-legged standing ability. Fifteen subjects (6 men and 9 women, mean age = 22.77 (SD = 1.21), mean height = 165.46cm (SD = 11.65), mean weight = 61.46kg (SD = 8.29) volunteers from healthy individuals were recruited and randomized to a lumbar rotational mobilization (LRM) group and a trunk rotational exercise (TRE) group. Mobilization (grade 3 or 4) was applied to the LRM group on the lumbar spine (L1 to L5) in a side-lying, and trunk twist exercise (left and right side) was applied the to the TRE group with lunge position. Center of pressure (COP) and the velocity of the center of pressure (VCOP) of each participant were measured as a balance ability through one leg standing position. Results are as follows. In within-group difference, the COP of the LRM group reduced during standing with the right foot, but the VCOP change of the LRM was not statistically significant. In between-groups difference, COP of TRE group was decreased compared with LRM group only during left leg standing in the eyes (p <.05). The results of this study suggest that LRM is more effective than TRE in improving balance ability.

Gross motor dysfunction and balance impairments in children and adolescents with Down syndrome: a systematic review

  • Background: Individuals with Down syndrome present with several impairments such as hypotonia, ligament laxity, decreased muscle strength, insufficient muscular cocontraction, inadequate postural control, and disturbed proprioception. These factors are responsible for the developmental challenges faced by children with Down syndrome. These individuals also present with balance dysfunctions. Purpose: This systematic review aims to describe the motor dysfunction and balance impairments in children and adolescents with Down syndrome. Methods: We searched the Scopus, ScienceDirect, MEDLINE, Wiley, and EBSCO databases for observational studies evaluating the motor abilities and balance performance in individuals with Down syndrome. The review was registered on PROSPERO. Results: A total of 1,096 articles were retrieved; after careful screening and scrutinizing against the inclusion and exclusion criteria, 10 articles were included in the review. Overall, the children and adolescents with Down syndrome showed delays and dysfunction in performing various activities such as sitting, pulling to stand, standing, and walking. They also presented with compensatory mechanisms to maintain their equilibrium in static and dynamic activities. Conclusion: The motor development of children with Down syndrome is significantly delayed due to structural differences in the brain. These individuals have inefficient compensatory strategies like increasing step width, increasing frequency of mediolateral center of pressure displacement, decreasing anteroposterior displacement, increasing trunk stiffness, and increasing posterior trunk displacement to maintain equilibrium. Down syndrome presents with interindividual variations; therefore, a thorough evaluation is required before a structured intervention is developed to improve motor and balance dysfunction.

Effect of Elastic Aids (Bands) on Functional Reach Test in Patients with Spinal Cord Injury: Pilot Study

  • Purpose: This study aimed: to confirm the balance ability of patients with spinal cord injury in the sitting state through a functional reach test using an elastic aid; and to propose a balance improvement plan. Methods: The study evaluated seven patients with spinal cord injury who could maintain a sitting posture through minimum assistance. A functional reach test was performed before and after wearing an elastic aid while sitting on a chair, and the effects before and after use of the elastic aid were compared and analyzed through a motion analyzer. Results: In the functional reach test, the forward movement distance of the hand was 97.45 mm before wearing the elastic aid, but significantly increased to 131 mm after wearing the aid (p<0.05). Corresponding forward movement distances for the shoulder were 81.26 mm and 113 mm (p<0.05 for the increase). There was no statistically significant change in lateral functional arm extension. Conclusion: It was confirmed, through a functional reach test, that trunk stability in patients with spinal cord injury increased with use of an elastic aid. In future, more efficient rehabilitation treatment programs will be possible if trunk stability in patients with spinal cord injury is improved by using elastic aids, and if various exercise treatments are also included in the rehabilitation programs.

보행 방향 전환 시 입각기 하지 및 체간의 운동형상학적 분석 (A Study on Kinematic Analysis of Trunk and Lower Extremities in Stance Phase of Walking according to Turning Direction)

  • Purpose: The purpose of this study was to conduct an analysis of kinematics of lower extremities and trunk in stance phase of walking according to turning direction. Methods: Ten university students (five male, five female) who were in their 20s (mean age was 20.6 years old) participated in this study. Participants did not have participants did not have any problem with skeletal muscular system. We used the "Qualisys motion capture system" for analysis of trunk and lower extremity movement in stance phase of walking according to turning direction. We collected data while subjects walked a distance of 10 m, and at the 6 m line, subjects were required to turn to the left side and the right leg was positioned in stance phase and the left leg was positioned in swing. For data analysis, the SPSS for Windows ver. 20.0 statistics program was used in performance of one way analysis of variance according to turning direction. Results: Significant difference of trunk and lower extremities was observed for turning direction according to walking cycle (p<0.05). Upper trunk movement showed a greater increase at three dimensions than lower trunk, and in heel off phase, pelvic movement showed a greater increase than lower trunk (p<0.05). In 45 degree and 90 degrees of turning direction, all movements of trunk and lower extremities were significantly different among three events of stance phase (p<0.05). Conclusion: We suggest that three-dimensional movement analysis of trunk and lower extremities during turning movement was very important in order to indicate increasing balance or walking ability for people with impaired movement or walking.
