Tearing of the acetabular labrum is a common cause of chronic hip pain. MR arthrography (MRA) of the hip is the imaging procedure of choice for the evaluation of acetabular labrum. Familiarity with the various imaging findings of MRA of the hip allows recognition of normal variants and differentiation from true pathologic conditions. This article comprehensively reviews the technical aspects and interpretation of MRA of the hip. The appearances of normal and abnormal labra in MRA are discussed. Potential pitfalls in labral tear interpretation, such as sulci around the hip, normal variants of the labrum and plicae, and osseocartilaginous and soft tissue lesions identified on MRA of the hip are also described.
Purpose: There have been some efforts to diagnose intracranial aneurysm through a non-invasive method using MRA, although the process may be difficult when the lesion is less than 3mm. The present study prospectively compares the results of high resolution, fast speed slice interpolation MRA and DSA thereby examing the potentiality of primary non-invasive screening test. Materials and Methods: A total of 26 cerebral aneurysm lesions from 14 patients with subarachnoid hemorrhage from ruptured aneurysm (RA) and 5 patients with unruptured aneurysm(UA). In all subjects, MRA was taken to confirm the vessel of origin, definition of aneurysm neck and the relationship of the aneurysm to nearby small vessels, and the results were compared with the results of DSA. The images were obtained with 1.5T superconductive machine (Vision, Siemens, Erlangen, Germany) on 4 slabs of MRA using slice interpolation. The settings include TR/TE/FA=30/6.4/25, matrix $160{\times}512$, FOV $150{\times}200$, 7minutes 42 seconds of scan time, effective thickness of 0.7 mm and an entire thickness of 102. 2mm. The images included structures from foramen magnum to A3 portion of anterior cerebral artery. MIP was used for the image analysis, and multiplanar reconstruction (MPR) technique was used in cases of intracranial aneurysm. Results: A total of 26 intracranial aneurysm lesions from 19 patients with 2 patients having 3 lesion, 3 patients having 2 lesions and the rest of 14 patients having 1 lesion each were examined. Among those, 14 were RA and 12 were UA. Eight lesions were less than 2mm in size, 9 lesions were 3-5mm, 7 were 6-9mm and 2 were larger than IOmm. On initial exams, 25 out of 26 aneurysm lesions were detected in either MRA or DSA showing 96% sensitivity. Specificity cannot be estimated since there was no true negative of false positive findings. When MRA and MPR were used concurrently for the confirmation of size and shape, the results were equivalent to those of DSA, while in the confirmation of aneurysm neck and parent vessels, the concurrent use of MRA and MPR was far superior to the sole use of either MRA or DSA. Conclusion: High resolution MRA using slice interpolation technique showed equal results as those of DSA for the detection of intracranial aneurysm, and may be used as a primary non-invasive screening test in the future.
Purpose : In order to investigate the functional brain anatomy associated with mental calculation, functional magnetic resonance imaging was performed. Materials and Methods : In six normal right handed subjects, functional MR images were obtained using a 1.57 MR scanner and the EPI BOLD technique. The study included experiment I and experiment II. Each experiment consisted of five resting and four activation periods with each period of 30 seconds. During the activation period of both experiment I and II, calculation equations[an example: $(4+5)\times8=72$] were presented and the subjects were instructed to decide true or false of them. During the resting period of experiment I, the subjects were instructed to visually fixate on a crosshair. During the resting period of experiment II, two diagrams (an example: $(\bullet,\;\blacksquare)$)were presented and the subjects were instructed to decide they are same or not. For the post-processing of images, the SPM program was used, with the threshold of significance set at p<0.00001. The activated areas during the tasks were assessed. Results : In experiment 1, the inferior frontal gyrus, prefrontal cortex, promoter area, supplementary motor area, and intraparietal sulcus including superior parietal cortex were activated bilaterally. Although these areas were also activated in experiment II, the activated signals in the right frontal and parietal lobes were lessened. Conclusion : The left inferior frontal gyrus and prefrontal cortex and bilateral intraparietal sulci were activated during mental calculation. The right frontal and parietal lobes might be related to attention and decision making.
Purpose : Early detection of small brain metastases is important. The purpose of this study was to compare the detectability of brain metastases according to the size between 1.5 T and 3.0 T MRI. Materials and Methods : We reviewed 162 patients with primary lung cancer who were examined for TNM staging. After administration of double dose of Gd-DTPA, MR imaging was performed with SPGR by 3.0 T MRI and then with T1 SE sequence by 1.5 T MRI. In each patient, three readers performed qualitative assessment. Sensitivity, positive predictive value, and diagnostic accuracy were calculated in 3.0 T and 1.5 T MRI according to size. Using the signal intensity (SI) measurements between the metastatic nodules and adjacent tissue, nodule-to-adjacent tissue SI ratio was calculated. Results : Thirty-one of 162 patients had apparent metastatic nodules in the brain at either 1.5 T or 3.0 T MR imaging. 143 nodules were detected in 3.0 T MRI, whereas 137 nodules were detected at 1.5 T MRI. Six nodules, only detected in 3.0 T MRI, were smaller than 3.0 mm in dimension. Sensitivity, positive predictive value, and diagnostic accuracy in 3.0 T MRI were 100 %, 100 %, and 100 % respectively, and in 1.5 T MRI were 95.8%, 88.3%, and 85.1% respectively. SI ratio was significantly higher in the 3.0 T MRI than 1.5 T MRI (p=0.025). Conclusion : True positive rate of 3.0 T MRI with Gd-DTPA was superior to 1.5 T MRI with Gd-DTPA in detection of metastatic nodules smaller than 3.0 mm.

Purpose: Diastolic dysfunction is a common problem in patients with hypertrophic cardiomyopathy (HCM). The purpose of this study was to assess the role of MRI in the assessment of diastolic function using mitral valve and pulmonary vein flow velocities in HCM patients. Methods and Results: Phase-contrast MRI (mitral valve and pulmonary vein) and transthoracic echocardiography was successfully performed for 59 HCM patients (44 men and 15 women; mean age, 51 years). Forty-nine patients had a diastolic dysfunction; grade 1 (n = 20), grade 2 (n = 27), and grade 3 (n = 2) using echocardiography, and ten patients had normal diastolic function. The transmitral inflow parameters (E, A, and E/A ratios) obtained by MRI showed positive correlation with the same parameters measured by echocardiography (Pearson's r values were 0.47, 0.60, and 0.75 for E, A, E/A, respectively, all P < 0.001). With the flow information of the pulmonary vein from cardiac MRI, pseudo-normalized pattern (n = 8) could be distinguished from true normal filling pattern (n = 17), and the diastolic function grades by cardiac MRI showed moderate agreement with those of echocardiography (kappa value = 0.45, P < 0.001). Conclusions: Assessment of left ventricle diastolic function is feasible using phase-contrast MRI in HCM patients. Analysis of pulmonary vein flow velocity on MRI is useful for differentiating pseudo-normal from normal diastolic function in HCM patients.
An intensive analysis of 185 timings of WZ Cep, including our new three timings, was made to understand the dynamical picture of this active W UMa-type binary. It was found that the orbital period of the system has complexly varied in two cyclical components superposed on a secularly downward parabola over about 80y. The downward parabola, corresponding to a secular period decrease of $-9.{^d}97{\times}10^{-8}y^{-1}$, is most probably produced by the action of both angular momentum loss (AML) due to magnetic braking and mass-transfer from the massive primary component to the secondary. The period decrease rate of $-6.^{d}72{\times}10^{-8}y^{-1}$ due to AML contributes about 67% to the observed period decrease. The mass flow of about $5.16{\times}10^{-8}M_{\odot}y^{-1}$ from the primary to the secondary results the remaining 33% period decrease. Two cyclical components have an $11.^{y}8$ period with amplitude of $0.^{d}0054$ and a $41.^{y}3$ period with amplitude of $0.^{d}0178$. It is very interesting that there seems to be exactly in a commensurable 7:2 relation between their mean motions. As the possible causes, two rival interpretations (i.e., light-time effects (LTE) by additional bodies and the Applegate model) were considered. In the LTE interpretation, the minimum masses of $0.30M_{\odot}$ for the shorter period and $0.49M_{\odot}$ for the longer one were calculated. Their contributions to the total light were at most within 2%, if they were assumed to be main-sequence stars. If the LTE explanation is true for the WZ Cep system, the 7:2 relation found between their mean motions would be interpreted as a stable 7:2 orbit resonance produced by a long-term gravitational interaction between two tertiary bodies. In the Applegate model interpretation, the deduced model parameters indicate that the mechanism could work only in the primary star for both of the two period modulations, but could not in the secondary. However, we couldn't find any meaningful relation between the light variation and the period variability from the historical light curve data. At present, we prefer the interpretation of the mechanical perturbation from the third and fourth stars as the possible cause of two cycling period changes.

Background: Superior migration of humeral head has been conventionally determined by measuring the acromiohumeral distance (AHD), We sought to devise a novel measurement system more reliably and accurately than AHD. We described a structural landmark called 'C-line'. In this study, we investigated the clinical usefulness of 'step-off of the C-line (SOC)' compared to that of AHD. Methods: The C-line formed from the medial margin of the proximal humeral head continuing up to the inferior margin of the articular glenoid and then to the lateral border of the scapula. The superior migration of the humeral head triggered by a rotator cuff tear introduces a discontinuity in this C-line. We measured the distance of this discontinuity. We enrolled 144 patients who underwent a rotator cuff repair. We selected 58 controls who didn't have any cuff lesions apparent on magnetic resonance imaging. Using radiographs derived from standardized true anteroposterior views of the shoulder, we measured the SOC and the AHD. We used t-tests for statistical analyses. Results: A rotator cuff tear was associated with an increase in SOC and a decrease in AHD. In control group, the mean SOC was $1.29{\pm}1.71mm$ and AHD was $9.71{\pm}2.65mm$. In cuff tear group, the mean SOC was $3.15{\pm}3.41mm$ and AHD was $8.28{\pm}1.76mm$. The mean SOCs of the patient group in relation to the mean SOC of the control group according to tear size, the SOCs of medium tear and lager groups showed statistically significant increase (p<0.05). Conclusions: The SOC may be a similarly effective to diagnose cuff tears of medium size and larger compared with AHD.

It is well known that reflected waves and resonance affect phase distortion. In addition, phase delay can be distorted by antenna impedance. In this study, we analyze the phase delay variation caused by the antenna impedance, considering mutual coupling effects. In addition, we confirm the beam steering characteristics. When was -10 dB and -7 dB, the maximum phase delay error was $18.5^{\circ}$ and $26.5^{\circ}$, respectively. The Monte Carlo simulation with an eight-element linear array antenna demonstrated that the RMS error of the beam steering angle ranged from $0.19^{\circ}$ to $0.4^{\circ}$, and the standard deviation ranged from $0.14^{\circ}$ to $0.33^{\circ}$ when the beam steering angle was in the range of $0^{\circ}$ to $30^{\circ}$, with the uniformly distributed phase error of $18.5^{\circ}$ and $26.5^{\circ}$. The side lobe level increased from 0.74 dB to 1.21 dB by the phase error from the theoretical value of -12.8 dB, with a standard deviation of 0.31 dB to 0.51 dB. This is verified by designing an eight-element spiral array antenna.

Purpose : The purpose of this study is to evaluate clinical applicability of Co-60 ViewRay treatment plan to increase the skin dose in case of high skin dose is required such as Malignant Fungating Wound By measuring the presence / absence of Bolus application and skin dose by the treatment device and comparing it Materials and Methods : Nine inner measuring points of 2.5 cm lattice arrangement and all 13 measuring points including upper and lower left and right measuring points touching the chest and skin were marked. After CT was taken, each treatment plan was formulated through Eclipse and ViewRay-TPS, and a Fixed beam-IMRT treatment plan was formulated so that the left chest V2Gy=95 % is delivered. Before measurement QED detector was calibrated and the QED detector was positioned at the 13 measurement points displayed on Phantom and surface dose of each treatment planner was measured using 5 mm Bolus application using True-beam and View-ray before and after, measure three times and compare each before applying 5 mm Bolus. Results : The surface dose of the Co-60 ViewRay and the linear accelerator appeared at $76.8%{\pm}5.2%$ vs. $67.3{\pm}%7.5%$ and the surface dose after application of 5 mm Bolus was $87.6%{\pm}8.9%$ vs. $80.3%{\pm}10.2%$ It was measured at 10.2 % (p<0.001). Conclusion : As a result of the surface dose measurement of each treatment instrument, Co-60 ViewRay confirmed that the surface dose reached 95.6 % of 6 MV Linac with conventional 5 mm bolus, despite not using Bolus (p<0.001). Also, by utilizing magnetic resonance images for each treatment, it is possible to observe the change in the treatment site without the problem of exposure, it is easy to formulate an adaptive treatment plan and it is easy to secure the skin dose, so the size In the case of Malignant Fungating Wound patients who need fast skin changes and need high skin doses, Co-60 ViewRay is considered to be more useful than linear accelerators.

Purpose : To compare dynamic susceptibility contrast imaging, diffusion-weighted imaging, and susceptibility-weighted imaging (SWI) for the differentiation of tumor recurrence and delayed radiation therapy (RT)-related changes in patients treated with RT for primary brain tumors. Materials and Methods: We enrolled 24 patients treated with RT for various primary brain tumors, who showed newly appearing enhancing lesions more than one year after completion of RT on follow-up MRI. The enhancing-lesions were confirmed as recurrences (n=14) or RT-changes (n=10). We calculated the mean values of normalized cerebral blood volume (nCBV), apparent diffusion coefficient (ADC), and proportion of dark signal intensity on SWI (proSWI) for the enhancing-lesions. All the values between the two groups were compared using t-test. A multivariable logistic regression model was used to determine the best predictor of differential diagnosis. The cutoff value of the best predictor obtained from receiver-operating characteristic curve analysis was applied to calculate the sensitivity, specificity, and accuracy for the diagnosis. Results: The mean nCBV value was significantly higher in the recurrence group than in the RT-change group (P=.004), and the mean proSWI was significantly lower in the recurrence group (P<.001). However, no significant difference was observed in the mean ADC values between the two groups. A multivariable logistic regression analysis showed that proSWI was the only independent variable for the differentiation; the sensitivity, specificity, and accuracy were 78.6% (11 of 14), 100% (10 of 10), and 87.5% (21 of 24), respectively. Conclusion: The proSWI was the most promising parameter for the differentiation of newly developed enhancing-lesions more than one year after RT completion in brain tumor patients.
