Studies of Nutrent Intake and Serum Lipids Level in Adult Women in Taegu (대구지역 성인여성의 영양섭취 상태와 혈청지질에 관한 연구)

  • The purpose of this study to investigate the relationship between nutrient intake and serum lipid levels in 165 healthy women in Taegu. A convenient method was to assess nutritional intake. Anthropometric measurement of body weight and height were measured and average energy expenditure calculated . The mean body mass index (BMI) was 23.4$\pm$3.1 and it was higher than the mean BMI of Korean women. Obesity rates for the study subjects were 15.7% by RBW (relative body weight : >120%) and 28.5% by BMI(body mass index) ; >25). Daily energy intake was sufficient at 106% of recommended dietary allowances and the energy percentage ration of carbohydrate , fat and protein was 68 : 18 : 14. Mean intake of vitamin A, B1 , niacin , and Ca were higher than RDA. The incidence of hypertension(>140mmHg) and hypercholoesterolemia(>240mg/dl) as 18.2% and 23.6% of the subjects, respectively . Postmenopasusal women showed significantly higher blood pressure, RBW, and ALP (alkaline phosphatase) than premenopausal women. The concentration of cholesterol and average blood pressure in the irregular meal eater group were significantly higher than in the regular meal eater group. Skipping meals and unenvendurnal distribution(no breakfast, or no lunch , and large evening meals) are associated with high total cholesterol level in this population . Especially, the atherogenic index was significantly lower in the regular meal eater group than that in irregular meal eaters. The values for serum cholesterol, triglyceride, blood pressure, BMI, and atherogenic index increased with age in middle-aged women. There was a highly significant correlation between body weight and plasma lipids. HDL -cholesterol was inversely correlated with BMI. The above dta provides valuable information for community program planning and health providers who work with individual female and adults to meet their nutrietional needs to control blood lipids.

The effect of food habits on blood component profile and health condition (식생활 습관이 인체의 혈액성상 및 건강상태에 미치는 영향)

  • The purpose of this study was to investigate the effect of food habits on blood components level(blood glucose, uric acid, hemoglobin, some lipids levels) and health condition in Taejon. The study was conducted with 250 subjects (male 130, female120) who visited in a general hospital from May to June in 1997. Questionaires were used for food habits. The data were analyzed by using $x^2$-test, t-test, ANOVA and multiple range test. Results were summerized s follow : mean of Broca Index were 110.0 $\pm$13.7% in male, 110.6$\pm$16.6% in female. Mean values of systolic blood pressure and diastolic blood pressure were 130.9$\pm$17.6mmHg, 76.2$\pm$12.6mmHg, respectively. The systolic blood pressure was significantly higher in man than woman, Mean serum total cholesterol, LDL-cholesterol, and HDL-cholesterol and triglyceride levels were 202.8$\pm$40.2, 121.1$\pm$35.5, 50.3$\pm$12.9, 157.1$\pm$112.6mg/dl, respectively. Triglyceride level was significantly higher in man than woman. But HDL-cholesterol was significantly lower in man than woman. And also, the value of LPH and AI were higher in man. So Man has more risk of atherogenesis than woman. Mean values of blood sugar, uric acid and hemoglobin was 102.7$\pm$1.36mg/dl and 14.1$\pm$1.56g/dl were higher in man than woman except blood sugar. Food habit of woman were better than it of man. The levels of education and income did not affect to food habit score. The most problem of habits were intake of salty food and lack of exercise. The intake of nutrients were higher as food habit score was higher. Intakes of energy, Ca, vitamin A and vitamin $B^1$ were lower than RDA's of nutrients in bad food habit group. This result did not show that blood components level and health condition were significantly different by food habits. But Broca Index and triglyceride levels were higher in bad food habit group than other groups, but not statistically significant. By this result, it is more needed food habit education to man and young people. The education of food habits will include the real methods of weight control, low intake of salt and exercise to bad food habit group.

Sitagliptin and Vildagliptin Use Evaluation among Dipeptidyl Peptidase 4 Inhibitors in Adult Koreans with Type 2 Diabetes Mellitus (한국인 제2형 당뇨병 환자에 대한 디펩티딜 펩티다제 4 억제제 중 Sitagliptin과 Vildagliptin의 약물 사용 평가)

  • Type 2 Diabetes Mellitus (T2DM) is characterized by high blood glucose in the context of insulin resistance and relative insulin deficiency. Diabetes is often initially managed by increasing exercise and dietary modification. As the condition progresses, medications may be needed such as oral sulfonylurea or others. Recently, dipeptidyl peptidase 4 (DPP- 4) Inhibitor is new drug which can control blood glucose by increasing the active levels of incretin hormone in the body. However, researches have been carried out for mostly Caucasian and Japanese, not for Koreans at all. Therefore, this study was to evaluate the efficacy and safety of DPP-4 inhibitor (Sitagliptin, Vildagliptin) in patients with T2DM in Koreans. This study was carried out retrospectively with reviewing of medical records from the 141 patients who received sitagliptin or vildagliptin over 24 week periods from January 2009, to December 2009. Information including demographics, concomitant medication, disease duration, and exercise was evaluated. $HbA_{1c}$, random blood glucose, post prandial 2 hour glucose, blood pressure, AST, ALT, serum creatinine, total cholesterol, triglyceride levels were also collected at baseline and endpoint (at 24 weeks). In each post-treatment group, $HbA_{1c}$, random blood glucose and post prandial 2 hour glucose levels were decreased significantly from baseline in the sitagliptin group (-0.82%, -28.76 mg/dl, -46.65 mg/dl) and vildagliptin group(-1.22%, -27.96 mg/dl, -67.2 mg/dl). Greater $HbA_{1c}$ mean reductions from baseline to 24 weeks were seen in patients with higher baseline values (>7.0%), with shorter disease durations (${\leq}1$ year) compared with those with lower baseline values (<7.0%), with longer disease durations (>1 year) in both sitagliptin and vildagliptin groups. The incidences of hypoglycemia, headache and upper respiratory infection were 0%, 8.7%, 5.8% in sitagliptin group and 2.8%, 8.3%, 6.9% in vildagliptin group. In conclusion, our results showed DPP-4 inhibitor provided similar efficacy compared with sulfonylurea after 24 weeks of treatment and were safer than sulfonylurea in hypoglycemia for Korean T2DM. Also vildagliptin was associated with significant improvement in $HbA_{1c}$ reduction in Korean patient with subgroup (body mass index<25 $kg/m^2$, metformin dose${\geq}$1000 mg, p<0.05) compared to sitagliptin. Therefore, even though DPP-4 inhibitor use for Korean needs to be studied more consistently in the future, DPP-4 inhibitor is a safe and effective drug for Korean T2DM based on our result.

Anthropometry, Blood Pressure, Serum Lipid Levels and Nutrient Intakes in People with Impaired Fasting Glucose and with Diabetes (공복혈당장애군 및 당뇨군의 비만도, 혈압, 혈청 지질 농도 및 영양소 섭취 상태)

  • The purpose of this study was to compare the anthropometry, blood pressure, blood lipid levels, and nutrient intakes in three groups: a group with an impaired fasting blood glucose (IFG) condition (62 males and 30 females); a group with normal blood glucose (73 males and 50 females); and a group with diabetes mellitus (DM) (79 males and 49 females). In male subjects, body mass index (BMI) and obesity index values were significantly higher in the IFG group than in normal and DM groups, but waist/hip ratio (WHR), systolic blood pressure, and diastolic blood pressure were not significantly different from those of the DM group. In female subjects, there was no difference in body weight, BMI, obesity index and WHR values between the IFG and the DM groups. The prevalence of obesity in the IFG group (males 43.6%, females 76.7%) was the highest among the three groups. The percentage of high SBP (40.0%) and high DBP (52.0%) in male subjects of the IFG group was significantly greater than in the DM group or the normal group. In the male subjects, the serum triglyceride concentration was significantly higher in the DM group than in the normal and IFG groups. There was no difference in various nutrient intakes(energy, carbohydrate, protein, fat, calcium, iron, vitamin A and cholesterol) among the three groups. In male subjects, the percentage of energy intake from alcohol was higher in the IFG group (7.0%) than that of the normal (4.5%) and the DM (5.6%) groups. The fasting blood glucose level had a positive correlation with WHR, TG, the athrogenic index and LDL-cholesterol/HDL-cholesterol. This study suggested that the IFG group had a higher obesity index, BMI, blood pressure and serum lipid levels (TG, cholesterol) than the normal or the DM group; furthermore, the IFG group had higher levels of alcohol intake and habits of taking unbalanced diets. Therefore, guidelines for IFG group should emphasize weight control, diet therapy, physical activities and regular balanced diets, in order to prevent diabetes in this group.

Effect of Tea Fungus/Kombucha Beverage on Lipid Metabolism in Streptozotocin-induced Diabetic Male Rats (발효홍차가 당뇨숫쥐의 지질 대사에 미치는 영향)

  • This study investigated the effect of tea fungus/kombucha(TF) beverage on body weights, pancreas weights, serum glucose and lipid concentrations of both normal and diabetic male rats. Sprague Dawley male rats weighing $140{\pm}10g$ were randomly assigned to one control and five streptozotocin(STZ) diabetic groups. Diabetic groups were divided into D control(TF free water), 20% TFD(20% TF in water), 40% TFD(40% TF in water), 20% TFSD(20% TF disinfection in water) and 40% TFSD(40% TF disinfection in water) according to the level of TF supplementation. Diabetes was experimentally induced by intraperitoneally administration of STZ in citrate buffer(pH4.3) after 3 week feeding of four experimental water. Animals were sacrificed at the 4 week of diabetic state. The diabetic groups showed significant decrease of body weight(-29.4~48.6g) compared with the control group(72.4g). Pancreatic weights relative to body weights in all diabetic groups were heavier than those of the control group. The levels of fasting serum glucose were higher in all diabetic groups than that of the control group. The concentrations of triglyceride in serum was lower in 20% TFD and 20% TFSD groups than in those D control group. The levels of total cholesterol, LDL cholesterol in serum and atherogenic index were significantly decreased in all(20 or 40%) TFD and TFSD groups than in those D control group, but those were similar to control group. The levels of HDL cholesterol in serum was similar to all groups, but HDL cholesterol/total cholesterol ratio were significantly increased in all(20 or 40%) TFD and TFSD groups than in those D control group. Liver triglyceride concentration of the all diabetic groups significantly decreased compared with the control group. Liver total cholesterol and phospholipid were not affected by TF and TFS feeding. The results indicated that tea fungus/kombucha beverage feeding improved the cholesterol, triglyceride and atherogenic index levels in diabetic rats.

Functional Characteristics of Egg Oil Extracted from Duck's (청둥오리 압란유의 기능적 특성)

  • The aims of this study are conducted to investigate the effect of Duck's egg oil no new functional characteristics in the diabetics induced by streptozotocin(STZ) and inhibition of antitumor activity toward Sarcoma-180 bearing mice. Serum glucose content by administration of Duck's egg oil in diabetic group induced by STZ were maintained with normal state than that of control. Phospholipid and triglyceride contents were significant decrease in group of administration with Duck's egg oil than that of control group. Total cholesterol, LDL + VLDL cholesterol and atherosclerotic index were decreased in the group of treatment of Duck's egg oil when does level compared with that of control group. On the other hand, the growth inhibition of the Sarcoma-180 showed the highest level of 63.89% when 150㎎/㎏ Duck's egg oil administrated. The life prolongation effect showed 19.7% at dose of 150㎎/㎏.

Renal Cell Carcinoma is More Aggressive in Turkish Patients with the Metabolic Syndrome

  • Background: Metabolic syndrome (MetS) is a multifactorial disease characterized by impaired glucose tolerance/diabetes, obesity, high triglyceride levels, low HDL levels, and hypertension. In this study we evaluate the relationship between tumor size and grade, and presence of the metabolic syndrome in patients with renal cell carcinoma. Materials and Methods: Between 2007-2013, radical nephrectomy was performed for 310 patients with renal tumors in our clinic and those with pathology reported renal cell carcinoma were enrolled and divided into two groups, with and without metabolic syndrome diagnosed on the basis of the National Cholesterol Education Program (NCEP) Adult Treatment Panel III (ATP III) criteria. The relationship between tumor size and grade of the two groups (Fuhrman nuclear degree) was evaluated statistically. Results: The metabolic syndrome was found in 70 patients, with a mean age of 65.5 (40-87), as compared to 58.8 (31-84) years in the non-metabolic syndrome group. Tumor size over 7 cm was found in 54% and 33%, respectively, and tumor grade over Fuhrman 3 in 56% and 32% of patients. Patients with metabolic syndrome had significantly higher tumor size and grade (p<0.05). In the presence of hypertension, diabetes and high triglyceride levels, significant assocations were again observed (p<0.05). Tumor size and degree also increased with increasing body mass index but this was not statistically significant (p>0.05). Conclusions: Renal cancer is more aggressive in patients with metabolic syndrome. Lifestyle and risk factors were revealed to be significant influences in renal cancer patients.

Clinical characteristics of obese boys and girls in a high school: focused on abdominal fat indices, fatty liver and carotid intima-media thickness

  • Purpose: Our study aimed to evaluated sex differences in clinical features of obese high school students. Methods: One hundred three obese high school students (body mass index [BMI]${\geq}$85th percentile) and 51 control students (BMI<85th percentile) were enrolled in this study. Anthropometric measurements were performed. Fasting serum glucose, insulin, aspartate aminotransferase, alanine aminotransferase, total cholesterol, triglyceride, low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and high-sensitive C-reactive protein were measured. Abdominal fat thickness, degree of fatty liver, and carotid intima-media thickness were measured by ultrasound. Results: In control and obese groups, waist circumference was significantly longer in boys but body fat mass was Significantly higher in girls. In the control group, total cholesterol and LDL-C were higher in girls. In the obese group, however, aspartate aminotransferase, alanine aminotransferase and triglyceride were higher and HDL-C was lower in boys. Preperitoneal fat thickness was significantly higher in obese girls. In obese group, the degree of fatty liver was significantly higher in boys. Carotid intima-media thickness was not significantly different between boys and girls. Conclusion: Obese adolescents had distinguishable sex differences in body measurements, metabolic abnormalities, abdominal fat thickness and fatty liver. We can infer that these characteristics may extend into adult obesity.

Assessment for Nutrient Intakes in Korean Women according to Obesity and Metabolic Syndrome (비만과 대사증후군 동반 여부에 따른 한국 성인 여성의 영양소 섭취 상태 평가)

  • Recent studies have reported that a subset of obese individuals who were metabolically healthy but obese had more favorable clinical outcomes than obese subjects with metabolic disturbances. The purpose of this study was to evaluate the distribution and agreement of obesity subtypes according to body mass index (BMI) and metabolic syndrome (MS). Furthermore, we examined the differences of nutrient intake among the groups. Data was analyzed for 1,095 female subjects older than 40 years using Korean National Health and Nutrition Survey in 2008. The degree of obesity was classified by two methods, using BMI (obese ${\geq}\;25\;kg/m^2$, not obese < $25\;kg/m^2$) and MS (meet ${\geq}\;3$ criteria among 5 index: waist circumference, triglyceride, glucose, HDL-cholesterol and blood pressure). Subjects were divided into 4 groups according to $2{\times}2$ cross table: non-obese without MS, non-obese with MS, obese without MS and obese with MS. Nutrient intakes were compared among 4 groups. The results showed that the proportions of non-obese without MS, non-obese with MS, obese without MS and obese with MS were 47.6%, 13.6%, 16.6%, and 22.2% of total subjects, respectively. The agreement (kappa value) of two methods was 0.354 (fair) in total subjects, 0.365 (fair) in 40-60 year old subjects and 0.304 (fair) in ${\geq}\;61$ year old subjects. In ${\geq}\;61$ years old subjects, intakes of percentage energy from carbohydrate, percentage of energy from fat, calcium, phosphorous, sodium, vitamin A, carotene, thiamine, riboflavin and niacin were significantly different among the groups. In contrast, the subjects of 40-60 years old, no differences in nutrient intakes were observed. In conclusion, there were differences in nutrient intakes among the groups subdivided by obesity and MS, especially in elderly female subjects. Individualized dietary guideline for subtype of obesity will be needed to treat metabolic disturbance of obesity.

The Effectiveness of a Hospital-Based Obesity Management Program using Korean Medicine in Uzbekistan (우즈베키스탄 한방병원에서 진행한 한의약 비만관리 프로그램의 효과)

  • Objective : As part of the Korean Medicine Official Development Assistance(ODA) and Public Health Project, we implemented a obesity management program(OMP) using Korean medicine in Uzbekistan and its follow-up study was conducted to evaluate the effectiveness of the program. Method : We recruited the participants of whose Body Mass Index(BMI) were over $25kg/m^2$ and who agreed to participate in the program at the South Korea-Uzbekistan Friendship Korean Medical Hospital in Uzbekistan. The program consisted of auricular acupuncture, functional food, and education program on diet and exercise. It was provided once a week during 7-week period. Anthropometric measurements and blood test measuring total cholesterol, triglyceride, and glucose were performed at the baseline, the end of the program, and 10-month follow-up. At the end of the program and the follow-up, survey on satisfaction and self-evaluation of the effectiveness of the program was additionally conducted. Results : 43 out of 78 participants completed the program and were included in the analysis. The analysis of the main outcomes showed that there were significant decreases in anthropometric measurements such as body weight, waist circumference, body mass index (BMI) and body fat content as well as blood test including triglycerides, total cholesterol and blood glucose levels. In the follow-up study, 24 subjects participated and 19 subjects (79.1%) reported that they succeeded in weight control. Furthermore, many participants reported that they maintained healthy lifestyles like healthy diet and regular exercise. The satisfaction with the program was also relatively high, and the education program was selected as the most motivating intervention for weight control. The outcomes such as body weight, BMI, body fat content, and blood glucose level were maintained to be at the decreased level; however, waist circumference, total cholesterol and triglycerides levels either returned back to the baseline level or were elevated to the level higher than the baseline. Conclusion : This study suggests that the OMP using Korean medicine may contribute to weight control of obese population in Uzbekistan. It is meaningful in that the study shows the possibility of implementing health promotion programs using Korean medicine in other countries with different cultures. In the future, more efforts to evaluate the effectiveness of the programs using rigorous methodologies and utilize the effective programs in ODA project will be needed.
