고혈압을 동반한 제2형 당뇨병 환자에서의 creatinine과 미세 단백뇨 증가 (Creatinine and microalbuminuria levels are increased in type 2 diabetic patients with hypertension)

  • The purpose of this study was to identify clinical characteristics of type 2 diabetic patients with hypertension. The subjects were 209 type 2 diabetic patients who visited at the endocrine center at Kangnam St. Mary's Hospital of Catholic University in Seoul from beginning of March through the end of April in 2001. The patient's clinical laboratory data were assessed at medical record review. The data were analyzed using for t-test, $x^2$ test. The results were as follows: 1) There were no significant differences in age, body mass index, sex, family history of diabetes and oral hypoglycemic agents between hypertensive group and normotensive group, However, percentage of patients receiving insulin treatment was higher significantly in the hypertensive group. 2) Creatinine and microalbuminuria levels were higher significantly in the hypertensive group. However, fasting blood glucose levels were lower significantly in the hypertensive group. There were no significant differences in $HbA_1c$, 2-hour postprandial blood glucose, total cholesterol, triglyceride, high density lipoprotein cholesterol, lipoprotein(a) and blood urea nitrogen between two groups. Our present study supports that Creatinine and microalbuminuria levels were higher significantly in the hypertensive group.

The subjects were 390 out type 2 diabetic patients from beginning of March through the end of April in 2001, who visited at the endocrine center at Kangnam St. Mary's Hospital of Catholic University in Seoul. The patients' clinical laboratory data were assessed at medical record review. The data were analyzed using for t-test and $\chi^2$ test. The results were as follows : 1. There were no significant differences in age, body mass index, family history of diabetes and hypoglycemic agents between normoalbuminuria group and microalbuminuria group. 2. The level of systolic blood pressure and creatinine of microalbuminuria group were higher than those of normoalbuminuria group. There were no significant differences in HbAlc, fasting blood glucose, 2-hour postprandial blood glucose, diastolic blood pressure, total cholesterol, triglyceride, high density lipoprotein cholesterol, lipoprotein(a) and blood urea nitrogen between normoalbuminuria group and microalbuminuria group.

Fermented Ginseng Attenuates Hepatic Lipid Accumulation and Hyperglycemia through AMPK Activation

  • Fermented ginseng (FG) is an ethanol extract of ginseng radix processed with $\beta$-galactosidase. It was hypothesized that FG may exert anti-hyperlipidemic and anti-diabetic activities through modulating AMP-activated protein kinase (AMPK) in HepG2 human hepatoma cells. In this study, we showed that AMPK phosphorylation was stimulated by FG. These effects were abolished by pretreatment with an AMPK inhibitor, compound C. In addition, FG regulated the expression of genes associated with lipogenesis and lipolysis, thus causing suppression of hepatic triglyceride accumulation. In vivo study using db/db mice, FG reduced fasting plasma glucose, HbAlc, and insulin resistance index, when compared to diabetic control. FG also increased the phospho-AMPK and glucose transporter 4 (GLUT4) expressions in liver and skeletal muscle, respectively. In liver, expressions of lipogenic gene were decreased whereas expressions of lipolytic genes were induced, when compared to diabetic control. Taken together, we may suggest that FG ameliorates hyperglycemia and hyperlipidemia through activation of AMPK and could be developed as a health functional food or therapeutic agent for type 2 diabetic patients.

  • The purpose of this study to examine lipid profiles and lipoprotein (a) concentrations and identify the risk factors of type 2 diabetes mellitus patients with macrovascular complications. The subjects were consisted of 618 out patients with type 2 diabetes mellitus from beginning of March through the end of April in 2001, who visited at the endocrinology department at Kangnam St. Mary's Hospital of Catholic University in Seoul. The patient's clinical laboratory data and the occurrence of chronic complications of diabetes were assessed at medical record review. The data were analyzed using for t-test. chi-square test and logistic regression. The results were as follows : 1. There were no significant differences in age. duration of diabetes. body mass index, sex, fasting blood glucose, 2-hour postprandial blood glucose and $HbA_{1}c$ level between macrovascular complication group and non macrovascular complication group. 2. There were significant differences in the level of total cholesterol. triglyceride. HDL(High density lipoprotein) cholesterol, LDL(Low density lipoprotein) cholesterol and lipoprotein(a) between macrovascular complication group and non macrovascular complication group. 3. Significant factor associated with macrovascular complication in the logistic regression best gut model was HDL cholesterol.

인슐린 비의존형 당뇨병의 가족력을 가진 정상 자녀의 혈청 지질 및 식습관에 관한 연구 (The Study of Serum lipid Profile and Food Behaviors in Healthy Offsprings of Korean NIDDM Patients)

  • This study was to investigate the risk factors for the development of diabetes mellitus on healthy offsprings of Korea non-insulin dependent diabetes mellitus(NIDDM) patients. Forty four offsprings who have at least 1 parent with NIDDM and age, sex and body mass index(BMI) matched forty four healthy control subjects were collected for this study. BMI, skinfold thickness, waist/hip circumference ratio(WHR), fasting blood glucose, serum insulin, serum lipid profile, nutrient intakes and food behaviors were measured. There were no significant difference in BMI, WHR, fasting blood glucose, insulin, total cholesterol, triglyceride and daily nutrients intake between offsprings and controls. But skinfold thickness, LDL-cholesterol level and sweety food eating frequency were significantly higher in male offsprings than in male controls. And HDL-cholesterol was significantly lower in male offsprings than in male controls. Although there were no significant difference, offsprings had a overeating habit and ate more confectionery and greasy food than controls.

돌복숭아(Prunus persica Batsch var. davidiana Max.) 추출액이 Streptozotocin 유발 당뇨성 흰쥐의 혈당 및 지질 성분에 미치는 영향 (Effects of the Prunus persica Batsch var. davidiana Max. Extract on the Blood Glucose and Serum Lipid Components in Streptozotocin-Induced Diabetic Rats)

  • Streptozotocin(STZ 55mg/kg., I.P. injection)으로 유발된 당뇨성 Sprague Dawley계 수컷 흰쥐에 있어서, 돌복숭아(Prunu persica Batsch var. davidiana Max.) 생리활성 추출액(5.0 g% extract)의 급여에 의한 혈당, 혈청지질 개선 효과 및 간 기능 효소의 활성 변화를 생리생화학적 측면에서 검토하기 위하여 본 실험을 수행하였다. 기본 식이 만을 급여한 대조군을 비롯한 STZ 당뇨 유발군과 당뇨 유발군에 돌복숭아 추출액을 급여한 군을 5주간 실험 사육한 결과, 혈당 농도는 당뇨 유발군에 돌복숭아 추출액을 급여하므로서 유의적으로 저하됨을 관찰할 수가 있었다. 또한 혈청 총 콜레스테롤, 동맥경화지수, LDL, LDL-콜레스테롤, 유리 콜레스테롤, 콜레스테롤 에스테르비, 중성지방, 인지질 농도 등도 돌복숭아 추출액을 급여한 군에서 농도가 감소되는 것으로 나타났다. 반면, 총 콜레스테롤에 대한 HDL -콜레스테롤 비 등은 추출액 급여로 인해 높게 나타났다. 한편, 혈청 AST, 혈청 ALT, LDH 및 ALP 활성은 당뇨 유발군에 돌복숭아 추출액 급여에 의해 저하되는 것 등으로 미루어 보아, 돌복숭아 중의 생리 활성 물질이 혈당 조절 기능 이상, 혈청 지질 대사 이상 및 간 기능 장애 등에서 오는 성인병 예방 차원 및 개선 작용에 효과가 있을 것으로 생각된다.

누에 유래 실크 세리신이 당뇨성 흰쥐의 혈당 및 지질 농도 개선에 미치는 영향 (Improvements Caused by Silk Sericin Extract Derived from Silkworm in Blood Glucose and Lipid Concentration in Diabetic Rats)

  • 누에로부터 추출한 실크 세리신 추출물이 streptozotocin(STZ 55 mg/kg B.W., I.P. injection)으로 유도된 당뇨성 Sprague Dawley계 수컷 흰쥐에 있어서, 혈당 및 혈청 지질 농도 개선 효과와 당질대사 이상 등에 관여하는 효소의 활성 변동을 생리생화학적 측면에서 검토하기 위하여 본 실험을 수행하였다. 대조군인 CG군을 비롯한 STZ 당뇨 유발군(BSW군), 당뇨 유발에 실크 세리신을 급여시킨 군(ESS군)을 5주간 실험 사육한 결과, 혈당 농도는 당뇨 유발군에 실크 세리신을 섭취함으로써 유의적인 감소 효과를 보였다. 또한, 혈청 총 콜레스테롤, 동맥경화지수, LDL, LDL-콜레스테롤, 중성지방 및 인지질 농도, 유리 콜레스테롤, 콜레스테롤 에스테르 비 등은 실크 세리신을 섭취시킨 군(ESS군)에서 농도가 저하되는 것으로 나타났다. 한편, HDL-콜레스테롤 및 총 콜레스테롤에 대한 HDL-콜레스테롤 비 등은 실크 세리신 섭취에 의해서 증가되는 것으로 나타났다. 혈청 중 alkaline phosphatase(ALP) 및 aminotransferase(AST, ALT)의 활성은 당뇨를 유도시킨 후 실크 세리신을 섭취시킴으로써 저하되는 것으로 나타났다. 따라서 본 실험 결과, 실크 세리신 추출물이 STZ으로 유발된 당뇨성 흰쥐의 혈당 조절 기능 및 지질대사 이상 등에서 오는 당뇨성 질환의 예방 및 개선에 효과가 있을 것으로 추정된다.

대추씨(Zizyphus jujuba Seed) 추출액이 Streptozotocin 유발 당뇨성 흰쥐의 혈당 및 지질성분에 미치는 영향 (Effects of the Zizyphus jujuba Seed Extract on the Blood Glucose and Serum Lipid Components in Streptozotocin-Induced Diabetic Rats)

  • 약용 및 가공 식품 등의 재료로 활용되고 있는 대추의 가공 부산물 중 대추씨(Zizyphus jujuba seed)의 각종 생리 활성물질을 구명하여 생체 대사학적 측면에서 검증하고 당뇨병 등 성인병 예방 및 치료효과에 대한 개선 방안을 검토하여 기능성 건강음료 등의 자원으로서 이용 가능성을 검토하기 위하여, Sprague Dawley계 숫 횐쥐에 기본식이를 급여한 대조군(1군)과 streptozotocin(STZ 55mg/kg B.W., I.P injection)으로 유발된 당뇨성 실험동물군(2군), 당뇨유발 실험군에 대추씨 3.5g% 추출액(3군)을 각각 급여하여 4주간 실험 사육한 결과는 다음과 같다. 혈당 농도, 혈청 총 콜레스테롤 농도, 동맥경화지수, LDL 및 LDL-콜레스테롤, 유리 콜레스테롤, 콜레스테롤 에스테르, 중성지방 및 인지질 농도 등은 STZ으로 유발된 당뇨성 실험동물군(2군)에 비해 대추씨 생리활성 추출액 급여군(3군)에서 저하 효과를 관찰할 수가 있었다. HDL-콜레스테롤 농도 및 총콜레스테롤에 대한 HDL-콜레스테롤의 농도비는 2군 보다 대추씨 추출액을 급여한 3군의 농도 및 비율이 높은 것으로 나타났다. AST, ALT, LDH 및 ALP 활성변동은 2군에 비해 대추씨 추출액을 급여 (3군) 함으로써 감소되는 것으로 나타났다. 이상의 결과들을 미루어 볼 때, 대추씨 중의 생리활성 물질이 혈당 및 혈청의 지질 개선 효과가 있는 것으로 사료되며, 당질 및 지질대사 장애 등에서 오는 당뇨병 등 성인병 예방 및 개선에 효과가 있을 것으로 추정된다.

양파껍질 열수 추출물이 혈중지질 성분과 혈당에 미치는 영향 (Effects of Onion Peel Water Extract on the Blood Lipid Profiles and Glucose)

  • Objectives : Onion (AlliumcepaL.) is one of the richest sources of flavonoids in human diet. In this study, we studied the effects of onion peel water extract (OPE) on the blood lipid profiles and glucose in rats. Methods : The experimental groups were divided with 5 groups (n = 6) of SD rats: normal diet + distilled water (NC), high-fat diet + Quercetin (PC), high-fat diet + onion peel water extract 4 mg/kg (OPE-4), high-fat diet + onion peel water extract 20 mg/kg (OPE-20), high-fat diet + onion peel water extract 100 mg/kg (OPE-100). Results : The liver fat showed significantly lower weights and size in the OPE-100 group as compared with NC group (p<0.05), The epididymal fat and retroperitoneal fat showed significantly lower weights and sizes in the OPE-4 and OPE-20 group as compared with NC group (p<0.05). The serum levels of total cholesterol, LDL cholesterol and triglyceride were significantly lower in the OPE-4 and OPE-20 group as compared with NC group (p<0.05). The OPE-4 and OPE-20 group showed higher HDL cholesterol concentration than NC group (p<0.05). Atherogenic index of OPE-4 and OPE-20 group was significantly lower in as compared with NC group (p<0.05). The serum levels of glucose significantly lower in the OPE-20 group as compared with NC group (p<0.05). Conclusions : In these results, we suggests that onion peel water extracts supplementation can reduces the serum lipid components and improves the lipid metabolism in hyperlipidemic SD rat induced with a high-fat diet.

표고버섯 보충이 제 2 형 당뇨병 환자의 혈당, 지질 대사 및 항산화 효소 활성에 미치는 영향 (Effects of Mushroom Supplementation on Blood Glucose Concentration, Lipid Profile, and Antioxidant Enzyme Activities in Patients with Type 2 Diabetes Mellitus)

  • The present study was carried out to evaluate the physiological effects of mushroom supplementation on blood glucose levels, lipid profile, and antioxidant enzyme activities in subjects with type 2 diabetes mellitus. Subjects were randomized into either a control group or mushroom supplementation group. Mushroom supplementation was provided 3 times a day for 4 weeks. We found that total dietary fiber intake was about 2.5 times higher (30.3 g vs. 12.3 g) in subjects receiving mushroom supplementation than in the control group. Two groups maintained the same food intake and amount of activity, exercise during the supplementation. We observed no difference in age, height, weight, BMI (body mass index), blood pressure between the groups. Nutrient intake did not differ appreciably between the two groups, except for fiber intake, during the supplementation. Fasting blood glucose levels and 2-hour postprandial blood glucose levels were significantly lower in those ingesting mushroom than in controls. Furthermore, the concentrations of low-density lipoprotein cholesterol were decreased significantly in the mushroom supplementation group. Small changes were observed in the concentration of total cholesterol, triglyceride, high-density lipoprotein cholesterol of those supplemented with mushroom, but these changes were not statistically significant. Activities of superoxide dismutase and catalase with mushroom supplementation were higher than in controls, but and glutathione peroxidase activity was not affected. The levels of thiobarbituric acid reactive substance of mushroom group were lower than control group, but were not significant. We conclude that addition of mushroom influences glycemic control and may be effective in lowering blood lipids and improving antioxidant enzyme activities. Accordingly, such effects may reduce risk factors for cardiovascular disease in patients with type 2 diabetes. However, to confirm these effects and to make dietary recommendations for patients with type 2 diabetes, further studies are necessary.
